I am a diabetic since last 2 years. I am too fat. I tried many medication but nothing worked. Prescribe some medicine for normal daily life ?

MBBS
General Physician, Delhi
I am not sure why you have asked for medcines, whether it is because of diabetes or obesity? in any case, prescribing medicines is not possible without examining you properly and reviewing your medical records. For obesity however, if you are morbidly obese, there is this thing called bariatric surgery. It helps obese people get back to normal. You may contact your nearest qualified bariatric surgeon for more details on the surgery and if you can have it. Thanks.

Eat small frequent meals. Choose meals which are little calorically dense so that your body has some extra reserves of energy after the body burns of the energy. Calorically dense food does not include just fats and oils. combination of all the food groups i.e cereal, pulse, milk, fats etc. Include thyroid-suppressing food in the diet. Broccoli, soya products, cabbage, cauliflower, rajmah, bajra etc. When you choose food make sure that it nourishes your body and not just give you empty calories. So eat fruit, fruit juices, nuts, eggs, meats etc Do not fill up your stomach with low calorie foods like salads and veggies. Eat foods which are stomach filling. For that include protein and 5-10gms of fats in each major meals. Eat food containing paneer. It gives satiety and and stomach fullness for a longer time. Eat home made sweets like sheera, puddings, custards etc to give you additional calories which will avoid weight loss. Eat food at regular interval. Do not increases gap between 2 meals. meal gap should be lesser than 3 hrs. Do not keep habit of having soups before meals. They tend to give feeling of fullness and satiety. Do not drink water immediately after meals. It is also important to keep replenishing the body with vital minerals and vitamins. So add multivitamin supplements to your diet. Bugleweed, green tea, and motherwort are a few herbs found to be helpful for hyperthyroidism.

Hello doctor, this is about my 62 year old mother. She is a known patient of type 2 diabetes mellitus from past 24 years and simultaneously she is hypertensive and went through angioplasty (standing) just six month before. Her uterus and gall bladder is in post operative status. She is also suffering from acute gastritis with recurrent bile vomits. She is on insulin Tresiba, and huminsulin R. Drugs-coversyl 4 mg, stamlo 10, atorvastatin 10 mg, clopivas Ap 75, surprise plus 5. Now the problem from last 2 months she is experiencing tremendous cramps and spasm in her stomach all day long and getting bit Rlfd by evening. When endoscopy was done no such findings had been found rather doctors told due to high diabetic the blood flow in intestine had decreased and as a result of that she is facing cramps on eating. Doctor suggested mebeverine 200 mg twice daily before food. But as of now there is no satisfying result. Sir, please suggest any other remedy if possible. She is suffering a lot.

Mr. lybrate-user, Thanks for the query. I have gone through the details given. As I can see she is on multiple medications. Some times drugs like statins can cause muscle cramps. Plus if she is on any othe acidity reducing drugs, then those may reduce B12 absorption. This can also contribute to cramps. Your over all information is very general, there is no mention of actual blood glucose values (fasting, PP)& HbA1c%, so there is no way to know about blood glucose control. Primary need is to achieve a strict blood glucose control. Then also provide B12, Calcium and Vit D3 supplements, which should help. Thanks.

Hello, Thanks for the query. The question is a bit unclear, as blood glucose level < 250 mg does not give any idea. Plus there is no mention whether that is fasting or after food and most importantly HbA1c% is not mentioned. Any ways, usual precautions to be taken are: avoid sweets, avoid deep fried foods, pastries, bakery items, junk foods, foods made from maida and colas. Refrain from alcohol and smoking. Increase the intake of green leafy vegetables, salads and fruits. Use whole grain flour for roti and chapati. Exercise daily for at least 1 hour. Use fenugreek seeds (methi seeds) either in powder form or roasted seeds about 10 grams before each meal. Thanks.
