Dear Dr. My age 32 and I have a thyroid problems from last some years and this month I checked thyroid test. Free T3 :- 3.58, T4:- 1.40, TSH :- 0.080 and last month thyroid test (this test is before period) free T3:- 1.79, T4:- 0.65, TSH :- 13.348 already already underweight like 40 kg and currently my weight is continuously loss like my today's weight is 39 kg. I am afraid all this things happens with me. My diet is okay. For menstrual problems I eat Dr. prescribe me tablets for 5 days. I have so many problems like body heck, joint pains, skin diseases, too much hair loss fever n cold. Also my sonography report comes :- the endometrial thickness is increased 12.1 mm Both ovaries are bulky in nature. Multiple follicles are arranged in the periphery of the ovary with echogenic stroma is seen in the centre. Finding suggestive of polycystic ovarian diseases. Dr. Gave me throxin 50 mg tablet am empty stomach. I eat regular but my above problem yet not solve even my weight is continuously loss. Before 3 months my weight is 46 kg and today 39. Kindly suggest me am so afraid.

Treatment of Infertility With IVF

IVF or In vitro fertilization is a form of fertility treatment where the sperm and egg is combined, in a laboratory. IVF is one of the most common fertility treatment procedures, which involves high technology and are undertaken by many couples all over the world. If you have ovulation problems or problems with the quality of egg and blocked fallopian tubes or in case your male partner has low sperm count or high mortality, you can undertake IVF to become pregnant.

The process of IVF involves the following steps and methods:

  1. Ovarian stimulation: Before your menstrual cycle begins, you should take a gonadotrophin. This is a fertility drug which will stimulate your ovaries for development of multiple, mature eggs for fertilization process. Taking synthetic hormones like cetrorelix and leuprolide also help in ovaries stimulation.
  2. Development of follicle: You must visit your doctor frequently while having the medicines for a checkup of your blood hormone levels. Getting an ultrasound done for regularly measuring your ovaries is also very important. This will allow your doctor to monitor the development of follicles.
  3. Trigger shot: You will be given a trigger shot injection after the follicles are ready. This injection causes full maturation of the eggs and makes them ready to get fertilized. The eggs can be triggered after 36 hours of trigger shots.
  4. Gathering of the eggs: You will be given an anesthetic by the doctor and an ultrasound probe will be inserted through your vagina for observing the ovaries and follicles. A thin needle is inserted through the vaginal wall and the eggs are removed from the follicles.
  5. Fertilization: Your eggs will be observed once more before being combined with your partner's sperm and being incubated overnight. Fertilization occurs during this time and abnormal eggs are not fertilized. Three days after the eggs are retrieved, some of the successfully fertilized eggs turn into embryos made up of six to ten cells. After five days, some of these embryos become blastocysts made of fluid-filled cavities and tissue, which begin to develop and separate into the baby and the placenta.
  6. The most viable embryos are selected to be placed in your uterus. The doctor places one to five embryos in your uterus by the insertion of a thin tube or catheter through the cervix. The embryo implants in the walls of the uterus and slowly develops into a baby. The chances of pregnancy are higher in case of multiple embryos.

IVF is a successful and quite a common fertilization process. It is an old procedure used and has been in use for a very long time. lybrate-user, Thanks for the query. The values mentioned are fairly normal. TGL 200 mg is not really a problem at all. If the levels go above 300 to 400 mg or more, then you need to worry about. Your weight appears to be a bit more than what it should be considering the age. About 50 kgs is fine. So please work towards reducing it. That will improve TGL levels. There is no mention about blood glucose levels or HbA1c%, so no idea how well is diabetes controlled. Thanks.

Dear lybrate-user. Thyroid definitely causes troubles for conceiving as it produces hormonal fluctuations. Do take proper homoeopathic treatment as in a constitutional medicine will only work for complete cure of d disease. Pls also do yoga. GD luck Tnkyu.

Hi, i'm 30, single. Woman from 8 months back when I checked my thyroid profile tsh level got increase like 8.82 after having thyronorm tab 25 mcg tablet 3 months it got decrease to 4.3 so doctor asked me to same dose continue, again I checked after 3 months I got my tsh level 6.82mcIU/ml. T3 level is 1.33 ng/ml and t4 level 9.17 mcg/dl. I just want to know which supplement i'm taking thyronorm 25 mcg is the right one r should I consult endocrinologist please guide me and help me.

lybrate-userji, Thanks for the query. When on treatment with levothyroxine for hypothyroidism, it is necessary to maintain TSH <3 mU/L. Then only one is free from all the symptoms of that condition. Therefore, present level of 1.33 mU/L is really good. There is no need to reduce the dose of thyroxine that you are taking. You should check TSh once in 3 to 6 months to see if it in the desired range. If level goes below 0.5 mU/l then dose may have to be reduced to 12.5 mcg per day. Thanks.

Hi doctor my mother is diabetic from last 18 years, she is 58 year old now. She is taking insulin consulted by doctors 24 /16. And taking Medicine glimmer 3 mg and gluconorm 500 mg. She is facing pain in her right leg from last few month. Doctor saying there is some problem from disc to vain in leg. Any medicine you can advice for reducing the leg pain. A Medicine doctor consult a year before named as neurosis capsule. But now it's not available in chemist shop. Please consult any alternate medicine. Or some thing good for her.

Mr. lybrate-user, Thanks for the query In a patient with long standing diabetes one of the common long term complication is peripheral neuropathy. This develops even when the blood glucose control is reasonably good. In case of badly controlled diabetes the problem becomes more severe. Since there is no mention of her blood glucose levels and HbA1c%, it is difficult to know if the condition is well controlled at all. There are some drugs used to treat neuropathic pain like pragabalin, gabapentine and some vitamin combinations. I did not understand what you mean by" Doctor sayi ng there is some problem from disc to vain in leg" Because whether the problem is due to neuropathy or blood supply affection leading to pain. This needs to be clarified. Has there been ultra sound or Doppler study done to see if the blood supply to the lower limbs is proper? Thanks.

Diet control and regular exerise are very important for diabetes control. You should avoid sugar, jaggery, honey,sweets, sweet fruits like mangoe, banana,cheeku, grapes etc. If you are oerweight please correct the weight. Consult a physician who after clinial ealuation will adise you mediines to control it.

Do you believe in all the facts about infertility which you hear around you, and which a lot of people believe in? Infertility is the inability of a woman to conceive after having unprotected sex. There are various assumptions and perceptions of people regarding fertility, and many of them are not true. It is important for you to clear such myths and get the facts right.

Here is a list of the most common myths and facts associated with infertility in females:
1. Myth: A woman’s menstrual cycle lasts for 28 days.
Fact: A normal menstrual cycle lasts between 21 to 36 days. 28 days is an approximate estimate.

2. Myth: A woman can become pregnant only on day one of her cycle.
Fact: After the egg gets released, it is available for around 12 to 24 hours. However, a woman can get pregnant five days prior to and two days after the ovulation by having sexual intercourse.

3. Myth: Stress is a factor that leads to infertility.
Fact: Stress has nothing to do with causing infertility. Stress can only lead to some delay in ovulation by suppressing hormones.

3. Myth: Sperm can live for a few hours only.
Fact: Sperm can live for 5 to 6 days in fertile cervical liquid.

4. Myth: A woman must wait for three months for conceiving after stopping the use of birth control pills.
Fact: Birth control pills reduce hormone levels and cause suppressed ovulation. After having the pills, the hormone levels become normal instantly and ovulation is ought to begin during the next menstrual cycle.

5. Myth: Infertility is caused by females primarily.
Fact: According to studies, infertility problems occur in both men and women at an equal rate. 20% of infertility problems occur because of issues faced by both.

6. Myth: Ovulation takes place on the 14th day of the menstrual cycle.
Fact: This is not true, and for counting ovulation you should count 14 days backwards from the last day of your last menstrual cycle.

7. Myth: Having daily sex increases the chance of conception.
Fact: Having sex every other day during ovulation, especially between the 12th and the 16th day of a woman’s menstrual cycle is sufficient as sperm lives more than two days.

8. Myth: Fertility problems start showing at 35.
Fact: A woman reaches her peak fertility stage during her 20’s. Fertility keeps on changing all throughout her life. While some women remain fertile until their late 30’s, some experience problems at an early age. The chances of conceiving decline after attaining the age of 35.

Another common belief is that single embryo transfer lowers the success rate of pregnancy. This is baseless, as women have limited eggs. When they run out of eggs, menopause occurs. If you wish to discuss about any specific problem, you can consult a Gynaecologist.

भारत में डायबिटीज के मरीजों की संख्या हर रोज बढ़ रही हैं। लगभग हर 5 भारतियों में से 2 भारतीय को डायबिटीज की समस्या हैं। देखा जाए तो डायबिटीज खुद कोई भयानक बीमारी नही है बल्कि यह आपने बाद धीरे धीरे विभिन्न बीमारियों को न्योता देकर शरीर के अलग अलग अंगों नुकसान पहुंचाती है। जैसे हम आसानी से देख सकते हैं कि शुगर के रोगी को आंखों व किडनी के रोग, सुन्नपन आना जैसी समस्याएं हो सकती हैं।

डायबिटीज को कण्ट्रोल करने के लिए जितना महत्त्व दवा और व्यायाम का है उतना ही महत्व आहार या डाइट का हैं। माना जाता है कि डायबिटीज के मरीज नॉर्मल रुटीन लाइफ नहीं जी सकते इसलिए उन्हें एकदम स्ट्रिक्ट डाइट लेना चाहिए।  
इन्सुलिन हार्मोन के स्राव में कमी से डायबिटीज रोग होता है। डायबिटीज आनुवांशिक या उम्र बढ़ने पर या मोटाप, स्ट्रेस जैसी कई वजह हो सकती है। डायबिटीज ऐसा रोग है जिसमें व्यक्ति को काफी परहेज से रहना होता है। मधमेह रोगी अगर अपनी दिनचर्या का पालन नहीं करते हैं तो इसके बढ़ने की संभावना ज्यादा होती है। इसलिए डाइट चार्ट फॉलो करने जरूरी हो जाता है। 

डाइट चार्ट
शुगर एक बार होने के बाद जीवनभर रहता है। इसे कंट्रोल करने के लिए नियमित दिनचर्या और पोषण युक्त आहार की जरूरत होती है। शुगर होने का कारण असंयमित खानपान, मानसिक तनाव, मोटापा, व्यायाम की कमी आदि है। जिसके कारण दिन-प्रतिदिन इसके मरीजों की संख्या बढ़ रही है। ब्लड शुगर के मरीजों के लिए सबसे ज्यादा जरूरी है संतुलित और नियमित भोजन। योग्य डाइट चार्ट अपनाकर डायबिटीज के मरीज न सिर्फ अपनी शुगर नियंत्रण में रख सकते है बल्कि उनकी दवा और इन्सुलिन का डोज़ भी कम हो सकता हैं।
डायबिटिक को अपने आहार में कुल कैलोरी का 40 प्रतिशत कार्बोहाइड्रेटयुक्त पदार्थों से, 40 प्रतिशत वसायुक्त पदार्थों से व 20 प्रतिशत प्रोटीनयुक्त पदार्थों से लेना चाहिए। यदि शुगर मरीज का वजन ज्यादा है तो उसे कुल कैलोरी का 60 प्रतिशत कार्बोहाइड्रेट से, 20 प्रतिशत फैट से व 20 प्रतिशत प्रोटीन से लेना चाहिए।

आइए हम जानते हैं डायबिटीज के रोगी आइडियल डाइट चार्ट किस प्रकार होनी चाहिए और साथ ही कुछ खास हिदायतें किसी पर्टिकुलर स्थिति पर डायबिटीज को कंट्रोल करने के निर्देश।

  • सुबह 6 बजे - एक ग्लास पानी में आधा चम्मच मेथी पावडर डालकर पीजिए।
  • सुबह 7 बजे - एक कप शुगर फ्री चाय, साथ में 1-2  हलके शक्कर वाली बिस्कुट ले सकते हैं।
  • नाश्ता / ब्रेकफास्ट - साथ आधी कटोरी अंकुरित अनाज और एक गिलास बिना क्रीम वाला दूध।
  • सुबह 10 बजे के बाद - एक छोटा फल या फिर नींबू पानी।
  • दोपहर 1 बजे यानी लंच - मिक्स आटे की 2 रोटी, एक कटोरी चावल, एक कटोरी दाल, एक कटोरी दही, आधी कटोरी सोया या पनीर की सब्जी, आधी कटोरी हरी सब्जी और साथ में एक प्लेट सलाद।
  • शाम 4 बजे - बिना शक्कर या शुगर फ्री के साथ एक कप चाय और बिना चीनी वाला बिस्किट
  • या टोस्ट या 1 सेब।
  • शाम 6 बजे - एक कप सूप पिएँ
  • डिनर - दो रोटियां, एक कटोरी चावल (ब्राउन राइस हफ्ते में 2 बार) और एक कटोरी दाल, आधी कटोरी हरी सब्जी और एक प्लेट सलाद।
  • बिना क्रीम और चीनी के एक गिलास दूध पिएँ। ऐसा करने से अचानक रात में शुगर कम होने का खतरा नहीं होता। 
  • एक खास हिदायत मधुमेह के मरीजों को उपवास करने से बचना चाहिए। इसके अलावा भोजन के बीच लंबा गैप भी नही करना चाहिए और रात के डिनर में हल्का भोजन करना चाहिए। इसके अलावा नियमित रूप से योगा और व्यायाम करने से भी ब्लड शुगर नियंत्रित रहता है। 
  • रोजाना इस डाइट चार्ट को फॉलो करने के साथ ही बताई गई कुछ एक चीजें और इस्तेमाल करें।
  • दरारा पिसा हुआ मैथीदाना एक या आधा चम्मच खाना खाने के 15-20 मिनट पहले लेने से शुगर कंट्रोल में रहती है और इससे और भी कई अंगों को फायदा होता है।
  •  रोटी के आटे को बिना चोकर निकाले यूज़ में लाएं हर चाहें तो इसकी गुणवत्ता बढ़ाने के लिए इसमें सोयाबीन मिला लें।
  • घी और तेल का दिनभर में कम से कम इस्तेमाल करें। 
  • सभी सब्जियों को कम से कम तेल का प्रयोग करके नॉनस्टिक कुकवेयर में पकाएं। हरी पत्तेदार सब्जियां ज्यादा से ज्यादा खाएं।
  • शुगर रोगी को खाने से लगभग 1 घंटा पहले अच्छी स्पीड पैदल चलना चाहिए और साथ ही व्यायाम और योगा भी करें। सही समय पर इंसुलिन व दवाइयां लेते रहें। नियमित रूप से चिकित्सक के पास जांच कराएं।
  • इसके साथ शुगर के मरीज को प्रोटीन अच्छी मात्रा में व उच्च गुणवत्ता वाला लेना चाहिए। इसके लिए दूध, दही, पनीर, अंडा, मछली, सोयाबीन आदि का सेवन ज्यादा करना चाहिए। इंसुलिन ले रहे डायबिटिक व्यक्ति एवं गोलियां ले रहे डायबिटिक व्यक्ति को खाना सही समय पर लेना चाहिए। ऐसा न करने पर हायपोग्लाइसीमिया हो सकता है। इसके कारण कमजोरी, अत्यधिक भूख लगना, पसीना आना, नजर से धुंधला या डबल दिखना, हृदयगति तेज होना, झटके आना एवं गंभीर स्थिति होने पर कोमा में जाने जैसी विपत्ति का भी सामना करना पड़ सकता है। 
  • डायबिटिक व्यक्ति को हमेशा अपने साथ कोई मीठी चीज जैसे ग्लूकोज, शक्कर, चॉकलेट, मीठे बिस्किट रखना चाहिए। यदि हायपोग्लाइसीमिया के लक्षण दिखें तो तुरंत इनका सेवन करना चाहिए। एक सामान्य डायबिटिक व्यक्ति को ध्यान रखना चाहिए कि वे थोड़ी-थोड़ी देर में कुछ खाते रहें। दो या ढाई घंटे में कुछ खाएं। एक समय पर बहुत सारा खाना न खाएं।
  • डायबिटीज के मरीज हमेशा डबल टोन्ड दूध का प्रयोग करें। कम कैलोरीयुक्त खाद्य पदार्थों का सेवन करें जैसे - छिलके वाला भुना चना, परमल, अंकुरित अनाज, सूप, सलाद आदि का ज्यादा सेवन करें। दही और छाछ का सेवन करने से ग्लूकोज का स्तर कम होता है और डायबिटीज नियंत्रण में रहता है।

lybrate-user, Thanks for the query. Hypothyroidism is a hormone deficiency disorder. Here thyroid gland is not producing enough or almost nil hormone. This hormone (Thyroxine) is required every day for all metabolic functions in the body. If not supplemented then all the symptoms of hypothyroidism are seen troubling the person. Therefore, only treatment to reduce TSH level is taking Thyroxine tablets in adequate quantity. There is no other medicine in any alternative medical branches. Plus you are a young lady, post marriage conception or fertility depends on maintaining TSH <3 mU/L. So my suggestion is take adequate dose of levothyroxine tablet. Mr. lybrate-user, Thanks for the query. The blood glucose values given are abnormally high. He needs urgent treatment that too with Insulin injection. Tablets (oral anti-diabetics) are not going to be beneficial. First he has to be started on Insulin for at least 3 months along with an oral drug, once a desired control is achieved, he can be shifted on to only oral drugs later. So if you are keen on getting treatment initiated online, can come in a private conversation with following details: HbA1c%, dietary pattern, extent of daily exercise and details of medications that he may be taking. Either get treatment from me or consult a local doctor preferably a diabetologist. Do not delay. Diabetes leads to damage on the nerves of the body and due to damage to nerves the sensations for which these nerves are responsible which could be touch, pain, cold, hot etc would get compromised. Pain in legs is one such thing for which diabetics get prone to due to diabetic neuropathy.

Burning palms and feet is due to peripheral neuritis which is more common in diabetes but you need not worry as your mother is non diabetic avoid spicy food and proper hydration cap palsineron 2 twice a day 7 day reduce later 1 cap twice 15 days apply shtodhat ghrit (medicated ghee) on the surface and gentalt rub with bronze utensil like plate or glass at night surly the problem will subside.

lybrate-user, Thanks for the query. In uncontrolled diabetes weight loss and tiredness are very common. It appears that your blood glucose levels are not in control. My suggestion is please come back with following details: FBG, PP glucose levels, HbA1c%, present treatment with dosage, dietary pattern & extent of daily exercise, in a private conversation where I can give specific advise with proper prescriptions. As in this communication we are supposed to give only general guidelines. Weight gain is possible once strict glucose control is achieved. Thanks.

My father is 70 years old with diabetes and single bypass. His average sugar level is within 200. But of late, he is having 350 sugar level with severe constipation. Whatever he eats, he is vomiting and very weak. He is on insulin once daily at night, can the insulin dosage be given twice for couple of days to reduce sugar level. Two, is there any medicine which would immediately give relief from gas and constipation. For the weakness, what kind of ORS can be given to him. Please respond ASAP, this is very urgent. All the solutions and the problems that you are perceiving are completely incorrect. You cannot have a proper solution if you interpreted the problem in the wrong way in the first place. If average blood sugar is 350 mg/dl, then in itself it may cause vomiting due to a very critical problem called Diabetic ketoacidosis Or DKA. Moreover, not everything is due to gas/acidity/constipation - they are one of the problems - not the only problem. Moreover, ORS is glucose solution - never ever think about giving it to a diabetic patient without recommendation. Plus, You have stated just Insulin- please be aware that there are many variants of insulin which vary according to type, time of action, duration of action etc. These parameters decide when and in which frequency insulin would be advised.

Mr. lybrate-user, Thanks for the query. With urea 66 mg and creatinine 1.7 mg, it appears he has developed problem with his kidneys. Plus high WBC count also indicates that there is presence of systemic some infection. Your information regarding his diabetes is very inadequate. Please let me know FBG, Pp & HbA 1c%, present medications with dosage. Plus following details: Lipid profile, height, extent of daily exercise, dietary pattern and BP readings. His hemoglobin also seems to be fairly low, showing he has anemia. To me it is necessary to control is diabetes very strictly, similarly is BP too should be well controlled. Unless I get all the details, it is difficult to give any specific opinion or advise. Thanks.

High uric acid produces gout, to prevnt you have to mantain diet and proper medication.in diet you need to limit the intake of alcohol, sugary foods, pourine rich foods like meat, sea foods, pulses, vegetables like cualiflower, asparagus, mashroom. For vit d deficiency you need to take morning sunrays.
