Can HIV Be Diagnosed Through Oral Manifestation?

HIV is a systemic disease which affects all parts of the body. The oral cavity also has some tell-tale symptoms which indicate HIV / AIDS. A careful examination and detailed history of symptoms is essential. In some cases, the oral manifestations could be the area where HIV is suspected. This can help in reducing morbidity and improves prognosis. The oral lesions that occur in HIV patients can vary and differ significantly in children and adults. While there are a variety of oral lesions in HIV-infected individuals, listed below are some common infections seen in HIV patients. These are a combination of fungal, viral and bacterial infections.

  1. Candidiasis: Candida is an opportunistic fungus that is normally present in the oral cavity and with reduced immunity of HIV, recurrent bouts of the infection begins to show up. It can be in the form of regular thrush which is whitish and cannot be scraped off (pseudomembranous candidiasis), hyperplastic candidiasis (white patches which can be scraped off) or erythematous (reddish patches). Candida can involve any part of the oral mucosa including the pharynx and the palate.
  2. Herpes Simplex: This is the most common viral infection seen in patients with HIV/AIDS. There could be primary or secondary infection of herpes virus, especially inside the mouth and the vermillion border of the lips.
  3. Herpes zoster: This virus, when already present in the body, can be reactivated with HIV/AIDS and with oral herpes. The distinction with herpes simplex is from their distribution. These are unilateral, along the distribution of the maxillary or mandibular nerve. The lesions appear both on the facial skin and the oral mucosa. While the facial ones break open and form crusts, the mucosal ones coalesce to form larger lesions.
  4. Hairy Leukoplakia: This is present in about 20% of asymptomatic HIV patients. Onset of hairy leukoplakia is an indication of rapid progression of HIV with increased CD4 counts. The typical lesion is a non-movable, hairy lesion along the side of the tongue and can spread to the top and the undersurface of the tongue. There are large amounts of Epstein-Barr virus (EBV) identified from biopsies of hairy leukoplakia.
  5. Cytomegalovirus: If the ulcers have a necrotic base with a halo surrounding it, it is CMV infection, usually seen on any oral mucosal surface.
  6. Periodontal disease: This is one of the bacterial infections that manifests itself in HIV patients. It can take two forms such as Linear Gingival Erythema (LGE) which can subsequently lead to Necrotizing Ulcerative Periodontitis (NUP). The oral hygiene is generally good with minimal plaque and there is rapid bone loss and soft tissue reddening and swelling. The, mouth, therefore is certainly a window to one’s health.

Diagnosing HIV with Western Blot Test-

It is a series of blood screenings are performed to test for HIV. The enzyme-linked immunosorbent assay (ELISA), also known as an enzyme immunoassay (EIA), is the first test that your healthcare provider will order to screen for HIV. ELISA, like the Western blot test, detects HIV antibodies in your blood. Antibodies are proteins your immune system produces in response to the presence of foreign substances, such as viruses. What Are Antibiotics.The word antibiotic comes from the Greek anti meaning ‘against’ and bios meaning ‘life’ (a bacterium is a life form).’ Antibiotics are also known as antibacterials, and they are drugs used to treat infections caused by bacteria. Bacteria are tiny organisms that can sometimes cause illness to humans and animals.

Antibiotics, first used in the 1940s,

An antibacterial is a compound or substance that kills or slows down the growth of bacteria.

Before bacteria can multiply and cause symptoms our immune system can usually destroy them. We have special white blood cells that attack harmful bacteria. Even if symptoms do occur, our immune system can usually cope and fight off the infection.

The first antibiotic was penicillin.


 

How do antibiotics work?

Although there are a number of different types of antibiotic they all work in one of two ways:


 

bactericidal antibiotic kills the bacteria. Penicillin is a bactericidal. A bactericidal usually either interferes with the formation of the bacterium’s cell wall or its cell contents.


 

bacteriostatic stops bacteria from multiplying.


 

What are antibiotics for? Do antibiotics work against all infections?

An antibiotic is given for the treatment of an infection caused by bacteria. Antibiotics target microorganisms such as bacteria, fungi and parasites. However, they are not effective against viruses. If you have an infection it is important to know whether it is caused by bacteria or a virus. Most upper respiratory tract infections, such as the common cold and sore throats are generally caused by viruses – antibiotics do not work against these viruses.


 

What are the side-effects of antibiotics?

Some patients may develop an allergic reaction to antibiotics – especially penicillins. Side effects might include a rash, swelling of the tongue and face, and difficulty breathing. If you have ever had an allergic reaction to an antibiotic you must tell your doctor and/or pharmacist. Reactions to antibiotics can be very serious, and sometimes fatal – they are called anaphylactic reactions.

Interactions between alcohol and certain antibacterials may occur and may cause side-effects


 

Use antibiotics with extreme caution and ensure you inform your doctor/pharmacist if:

  1. You have reduced liver or kidney function.

  2. You are pregnant

  3. You are breastfeeding.


 

Antibiotic Resistance:

One of the foremost concerns in modern medicine is antibiotic resistance. Simply put, if an antibiotic is used long enough, bacteria will emerge that cannot be killed by that antibiotic. This is known as antibiotic resistance.But overprescribing them has resulted in the development of resistant bacteria, which are bacteria that don’t respond to antibiotics that may have worked in the past.  Infections exist today that are caused by bacteria resistant to some antibiotics. The existence of antibiotic-resistant bacteria creates the danger of life-threatening infections that don’t respond to antibiotics.


 

Bacteria become resistant more quickly when antibiotics are used too often or are not used correctly (such as not taking a full course of antibiotics as prescribed by your doctor).

Guideline Steps for Breast Examine by Patient

Step 1: Begin by looking at your breasts in the mirror with your shoulders straight and your arms on your hips.


Here's what you should look for:

Breasts that are their usual size, shape, and color

Breasts that are evenly shaped without visible distortion or swelling

If you see any of the following changes, bring them to your doctor's attention:

Dimpling, puckering, or bulging of the skin

A nipple that has changed position or an inverted nipple (pushed inward instead of sticking out)

Redness, soreness, rash, or swelling


Step 2: now, raise your arms and look for the same changes.

 

Step 3: while you're at the mirror, look for any signs of fluid coming out of one or both nipples (this could be a watery, milky, or yellow fluid or blood).


Step 4: next, feel your breasts while lying down, using your right hand to feel your left breast and then your left hand to feel your right breast. Use a firm, smooth touch with the first few finger pads of your hand, keeping the fingers flat and together. Use a circular motion, about the size of a quarter.

 

Cover the entire breast from top to bottom, side to side — from your collarbone to the top of your abdomen, and from your armpit to your cleavage.

 

Follow a pattern to be sure that you cover the whole breast. You can begin at the nipple, moving in larger and larger circles until you reach the outer edge of the breast. You can also move your fingers up and down vertically, in rows, as if you were mowing a lawn. This up-and-down approach seems to work best for most women. Be sure to feel all the tissue from the front to the back of your breasts: for the skin and tissue just beneath, use light pressure; use medium pressure for tissue in the middle of your breasts; use firm pressure for the deep tissue in the back. When you've reached the deep tissue, you should be able to feel down to your ribcage.
 

Step 5: finally, feel your breasts while you are standing or sitting. Many women find that the easiest way to feel their breasts is when their skin is wet and slippery, so they like to do this step in the shower. Cover your entire breast, using the same hand movements described in step 4.

Homeopathy is based on the law of similars, according to which, a substance capable of producing certain symptoms in a healthy individual is capable of treating similar symptoms in a diseased person. Thus, the logic is to evoke a healing response from the body itself rather than trying to suppress the disease manifestation.


 

Myths About Homeopathy

Here are some of the common myths (about homeopathy) that have clearly fallen apart:


 

Myth 1: Homeopathic medicines are placeboes

Our answer to this: the world health organization (who) has recognized homeopathy as an accepted system of medicine and the world's second largest system of medicine. There have been many double blind trials conducted where the efficacy of homeopathic medicines have been proved. Studies have shown the ability of homeopathic medicines to induce physiological changes in the body temperature. Myth 4: Homeopathy cannot help surgical cases at all

However, every medical science has its set of advantages and limitations. Thus, in certain cases where the disease has reached an advanced stage or in emergency cases, surgery is essential and cannot be avoided. In fact, many surgeons and dentists use homeopathy pre and post operatively. Myth 5: There are too many dietary restrictions to be followed during homeopathic treatment

Our answer to this: the only medical restrictions that a homeopath may impose depend upon the disease that the patient suffers from; for example, diabetics should avoid sweets and a high carbohydrate diet. Since homeopathic medicines act through nerve endings, traditional homeopaths do not prefer the use or consumption of any substance with strong odors like toothpaste, garlic, onions, etc, as this could affect the action of homeopathic medicines. Modern homeopaths recommend having the medicines after rinsing the mouth half an hour before and after meals and also advocate placing the medicine below the tongue for best results.


 

Myth 6: Homeopathy and allopathy cannot be taken together

Our answer to this: in patients suffering from drug-dependant diseases such as diabetes and high blood pressure, allopathic drugs cannot be suddenly withdrawn; hence, homeopathic medicines may be taken with allopathic medicines. However, allopathic medicine is often palliative or suppressive. For example, if a patient with headache takes a painkiller, his symptoms would get masked and therefore a good prescription by a homeopath could become difficult. Similarly, some medicines in allopathy tend to suppress rather than cure while homeopathic medicines may expel toxins. In such cases, it is better to take homeopathy alone.

Homeopathic remedies are easy to take with no special dietary or lifestyle restrictions, unless a particular disease calls for them. Additionally, homeopathy is against animal testing as this tends to harm them irrevocably or kills them. Another benefit is that while animal testing only provides information about structural pathologies, homeopathic drug proving enables recording of even mental and emotional symptoms.
