Ulcerative Colitis: Symptoms and Treatment

Ulcerative Colitis: Symptoms and Treatment

This is a disease that affects the large intestine and the rectum. The Ulcerative Colitis refers to the inflammation in the innermost lining of the large intestine. The main function of the large intestine is to absorb water from indigestible food matter in the body before throwing away any waste. Hence, the large intestine is a vital part of our body. If left untreated, the disease increases risk of colon cancer.

Symptoms:

  1. Diarrhea with blood or pus: A person suffering from Ulcerative Colitis is likely to suffer from loose stools accompanied with blood or pus.
  2. Stomach pain and Cramping: In many cases, patients complain of severe stomach pain and cramping.
  3. Rectal pain: At times, many patients feel pain while sitting or even after a bowel movement.
  4. Bleeding from the rectum: Bleeding can be observed while passing stools.

The other complications leading to this disease are kidney stones, swelling of the colon, thickening of intestinal walls and blood infection.

Diagnosis:
This disease can be diagnosed by several methods. A simple stool test may be done to check out for bacteria and parasites. A blood test to check the level of C-reactive protein helps determine the inflation rate of the body. Endoscopy, colonoscopy, biopsy are some other methods of diagnosis.

Treatment

  1. Oral medication: Treatment involves drug therapy or surgery. The first step in treating Ulcerative Colitis will be an intake of anti-inflammatory drugs, but these may have a side effect. Another option is immune system suppressors, which help to bring down the inflammation by suppressing the immune system response. Antibiotics, anti-diarrheal medications, pain relievers are some of the additional drug supplements recommended by doctors.
  2. Surgery: Surgery plays an important role because this disease is pre-malignant in nature. Depending on the severity of the condition, the medical practitioner may advice surgery. The common methods of surgery are:
    1. Proctocolectomy and Ileostomy: Proctocolectomy involves removal of the colon in part or whole. Ileostomy is carried out by placing a special bag in the small intestine to collect waste from the body.
    2. Proctocolectomy and Ileo-anal: Ileo-anal pouch is a bag directly created the small intestine and connected to the anus, for diffusing the stools. As colon is removed Ulcerative Colitis cannot re-occur.

Precautions after surgery
It is very important that the patient takes healthy, sufficient and nutritious food so that bowel movement can be carried out with ease and zero strain. The patient has to restrict lifting of heavy grocery, mowing the lawn, any physical activity that can strain the abdomen and related areas.

Bloating - How You Can manage It At Home?

Bloating - How You Can manage It At Home?

Bloating isn't a medical condition. It doesn't necessarily mean that a person has eaten too much. It can be a case of eating wrong food which has not been digested by the body. Bloating occurs when air and water get trapped inside the stomach or intestine. As a result, the stomach gets swollen well past its normal level.

Culprits of bloating:Bloating can result from a number of cases. Fatty or junk foods are one of the biggest culprits of bloating. It hinders the digestion process and doesn't let the stomach empty on time. Carbohydrates too can cause bloating. Even carbonated drinks can play spoilsport. It creates bubbles in the stomach leading to indigestion and bloating. Some food items related to gas include cauliflower, lettuce, hard candy, beans, Brussels sprouts, broccoli etc. Apart from this, consumption of excessive junk food and soft drinks can lead to bloating.

Steps to avoid bloating: The best way to avoid bloating is to refrain from bloat causing foods as mentioned earlier. If you wear dentures, it should be ensured that it fits properly. There are instances when excess air gets inside the stomach due to an improper fixture of denture leading to bloating. Eating or drinking slowly is another good way of staying away from bloating. When you eat slowly, there are fewer chances of extra air getting inside the stomach. In the case of beverage consumption, avoiding the straw is a good idea. This way the unwanted air cannot make their way inside the tummy and play foul.

Taking a small walk after every meal is another good way of staying away from bloating. This way the gas inside the tummy cannot rise above the surface. The practice of calming technique such as yoga, deep breathing and meditation also goes a long way in giving relief from bloating.

Some home remedies to get rid of bloating include:

  • A cup of ginger, lemon and honey in a cup of hot water once everyday
  • 1 cup of herbal peppermint tea freshly prepared in boiled water
  • In the case of regular stomach bloating, intake of one caraway seed on a daily basis fetches immediate results
  • Consumption of a cup of baked pumpkin twice a week
  • Consumption of warm lemon water before you start your day is another good way of keeping bloating at bay.

Bloating can also be a symptom of other intestinal related problems as well. If the general method of control and measured diet doesn't suffice, it is time to visit a doctor to find the underlying cause of your problem. Doctors perform a series of tests to understand the problem and prescribe the required medicine to control or cure the problem. If you wish to discuss about any specific problem, you can consult a Gastroenterologist.

MBBS, MS - General Surgery

Colonoscopy is relatively safe in absence of medical/surgical contradications when done by trained doctor. The only issue you need to know about is about gut perforation the chances of which is very low about 1%.

What was the status of Physical activity of your child before he got sick. Is he quite sedentary and indoors?

Hard stool often causes a fissure in the anal canal & this may bleed & is also painful. If you are complaining of painless passage of blood on defaecation then you may perhaps have piles. So please consult a surgeon for the necessary assessment for an appropriate diagnosis, & management.

I was suffering from fissure for 3-4 months. It was painful only during the initial phase of passing the stool. Due to changes in food and after taking homeo medicines it got cured in 3 weeks but then after a week due to hard stool, I found lot of blood in the stool. I realized that it is because the piles got broken. I started using pilex ointment (used for 5 days) and stopped using that when I felt that I got cured. I started taking non veg and suddenly still got hardened and found blood stains on the stool. I started using pilex again. I pain is not severe. I would like to know if this can be completely cured using non surgical treatments. Pls suggest me the medicines with daily dosage. My pain is not severe but I want to get cured completely. Pls help.

Who Needs A Liver Transplant?

MBBS, DNB ( General Surgery)
Liver Transplant Surgeon, Pune
Who Needs A Liver Transplant?

The liver is responsible for filtering blood from the digestive system, metabolizing drugs and detoxifying the body. It also produces a number of proteins that help blood clot properly. Some diseases such as hepatitis, cirrhosis, cancer or gallstones can restrict the liver’s ability to function properly. In such cases, a liver transplant may be needed. This involves replacing the patient’s own liver with a part of a healthy liver or an entire liver from a donor.

A liver transplant can help save a person’s life. Without the liver, toxins will accumulate in the body and cause damage to other internal organs as well. It can also lead to bacterial peritonitis or a bacterial infection in the abdomen or cause blood to flow back in the stomach or accumulate in the spleen. Hence when a person suffers from a chronic liver disorder, a liver transplant is suggested. Some conditions under which such a procedure may be advised include:

  1. Cirrhosis or scarring of the liver caused by hepatitis C, biliary disease, excessive alcohol consumption
  2. Biliary atresia or the incomplete development of bile ducts
  3. Acute liver failure caused by viral infections or an overdose of certain drugs

However, not everyone suffering from these conditions is eligible for a liver transplant. A person’s eligibility for a liver transplant is determined by any other health problems the patient might be suffering from, age and the results of laboratory tests such as blood clotting tests, blood tests, liver and kidney function tests and electrolyte tests. In cases where an infection is suspected, blood, urine, sputum and bile cultures may also be grown to check for bacteria, viruses, fungi and other such organisms.

Donor livers are not always readily available and hence if a liver transplant is recommended, further tests will be needed to see where the patient will be placed on the liver transplant waiting list. This includes:

  1. Live Ultrasound- This test checks how well blood flows to and from the liver by using soundwaves to create a picture of the liver.
  2. Electrocardiogram or ECG- This tests the electrical activity of the heart and determines whether the heart will be able to take the stress of an operation.
  3. Dental Tests- A dental clearance is also required before undergoing a liver transplant as many types of Immunosuppressive drugs may increase chances of infections in cavities and periodontal disease.
  4. Purified Protein Derivative (PPD) Skin Test- This test is performed on the arm to check for tuberculosis exposure. If you wish to discuss about any specific problem, you can ask a free question.
