Multimodal Pain Relief After Total Knee Replacement

Knee replacement can be extremely painful. Previously, opioids or narcotics were administered for pain relief. But excessive addition of opioids is not exactly effective for controlling pain.

Multimodal pain management has become an important part of the perioperative care of patients undergoing total joint replacement. The principle of multimodal therapy is to use interventions that target several different steps of the pain pathway, allowing more effective pain control with fewer side effects. Many different protocols have shown clinical benefit. The goal of this review is to provide a concise overview of the principles and results of multimodal pain management regimens as a practical guide for the management of joint arthroplasty patients.

Multimodal denotes administering two or more than two types of medications that work with different mechanisms. The following are the techniques used:

  1. Pre-operative Femoral Nerve Block: Prior to the surgery, a catheter is placed beside the femoral nerve for blocking it. This nerve is located in the upper thigh. Medication is delivered through the catheter for the nerve to be numbed for 24 hours. Thus, pain signals to the brain are blocked. This method reduces the use of narcotics and the consequent side effects.

  2. Patient Controlled Analgesia (PCA): This method is also known as ‘Pain Pump’. An intravenous pump is used to administer pain relief medications, such as oxymorphone or morphine, after the surgery. The control button of the machine could be pressed, by the patient for 6 to 10 times per hour. The machine is used for two post-operative days.

  3. Oral Medications: The oral medications include Non-Steroidal Anti-Inflammatory drugs or NSAID; such as Celebrex which is similar to aspirin, structurally. Alternatively, acetaminophen, such as Tylenol or its equivalent composition, can also be used.

  4. Acetaminophen: It acts on the Central Prostaglandin Synthesis and relieves the patient of pain through multiple mechanisms.

  5. Epidural Analgesia: It produces lower pain scores and involves less time for achieving physical therapy goals. However, this is subject to side effects such as dizziness, urinary retention and itchiness.

  6. Gabapentinoids: These medications include membrane stabilizers such as Gabapentin and Pregabalin.

The objective of multimodal treatments is to provide quick relief to the patient and immediately so. Earlier the rehabilitation, more successful will be the knee replacement surgery.

Back pain will settle soon. U need to do physiotherapy. Because of annulus tear, some episodes of back and leg pain will come because of the chemical reaction. If pain persist, you can take caudal epidural injection, which will help regarding the pain.

I am having pain in right shoulder my pharmacist gave me an injection of deca50 mg with diclofenac q. First of all you have to be assessed for any of the muscles or soft tissues of your shoulder. Then your movements have to be checked properly. And for this you need expert consultation. Then only reach to some decision else you would be prescribed 1 thing or another by different people no matter whether you get relief or not.

Many times all tests are negative and the patient still suffers. You are suffering from indigestion and due to which there is a toxin called AMA being formed in your stomach. This is accumulating in joints leading to swelling and stiffness. So digestion needs to be rectified first along with panchakarma treatment. Pls see me personally.

Disc bulge can be managed with Ayurveda Treatments, Life style modifications, Medicines and Yoga. If disc bulge untreated may lead to numbness, pain, stiffness and many other symptoms. You can wait upto a period of 6 weeks during which you have to undergo physiotherapy to strengthen your thigh and calf muscles and also to improve your knee movements. At the end of this regimen if you are able to carry out all your pre-injury activities comfortably you may not need surgery immediately. But if you have knee pain / instability at the end of 6 weeks, it is definitely advisable to undergo surgery. Thanks & Regards.

You have to go for traction and also you can do IFT. Please consult the near by therapist for quick relief. It is also suggestible to go for cervical muscle strengthening exercises.

As knee pain is very common that you get generally bilaterally. Ice therapy would definitely help to reduce the inflammation. We also advise you to use knee cap which would help to prevent the knee from damaging further and also to maintain the quadriceps muscle tone. You can take Ultrasonic therapy in one of the nearby physiotherapy clinics which would help to heal the damaged cartilages along with shortwave diathermy which would help to improve the blood circulation. Simple Knee Exercies Specific knee exercises will also help ie. Keeping ball underneath the knee and keep pressing it. That's the simple exercise which will help you to strengthen the knee Ice therapy would definitely help to reduce the inflammation. We also advise you to use knee cap which would help to prevent the knee from damaging further and also to maintain the quadriceps muscle tone. If you have problem in your legs then it might be due to ligament laxity where your legs would become very weak due to the old ligament injury and that should be treated immediately. Wear knee cap so that you will feel firm while walking or climbing stairs.

Consult physiotherapist for full assesment and treatment - check vit d, uric acid, b12 - avoid lifting heavy weight - avoid any jerky movement - mai tain erect posture of spine - do contrast bath for 2- 3 times daily for 5 days, specially at night.

At your age the most common cause of back pain is mechanical i.e. Due to poor tone of back muscles and is usually lifestyle and posture related. You can start off with some Core strengthening and Spinal Extension exercises and this will surely improve the symptoms. Thanks & Regards.
