Mr. lybrate-user, Thanks for the query. To treat hypothyroidism only available drug is Levothyroxine tablets. Hence, if you have that condition, then it may be necessary to take Thyroxine tablets. But I can only give a proper yes or know based on actual TSH value, BP reading plus some details on the heart condition. Since nothing is mentioned, it is difficult to give any opinion. Thanks.

Hi sir I am 45 years old my weight is 74 kgs height is 172 cm I am suffering from thyroid problem, I went to doctor he suggested thyrox 75, I used six months, after one week I checked thyroid it comedown TSH level 0.06,and went to doctor he suggested thyronorm 50 mcg .Now I am using that medicine .Even though I am feeling nervousness inactive left chest is uneasy please suggest me what to do sir.

Mr. lybrate-user, Thanks for the query. When one is on treatment with adequate dose of Levothyroxine, TSH is usually maintained between 3 to 4 mU/L. At that level, most of the symptoms are in check. Usually after starting treatment initially TSH should be checked after 6 o 8 weeks to see if the desired level is reached. Then accordingly dosage of thyroxine should be adjusted in the range of 12.5 to 25 mcg per day. Drastic change in dose is not advisable. If TSH goes below 0.1 mU/L there may be symptoms of excess thyroxine. Now you may check TSH after 8 weeks of starting the new dosage. Thanks.

I need to know following things: Fasting, PP blood glucose levels, HbA1c%, height, weight, dietary pattern, extent of daily exercise, drugs being taken with dosage. I have done a blood sugar test 15 days back. And the fasting blood sugar was 98 and HBA1C was 6.2. Test was done in pathcare. Then I am so worried and did a test again and now fasting is 84 and post lunch is 141. And I did not done HBA1C now. Is the test appears normal? Please help me out on this.

Mr. lybrate-user, Thanks for the query. The blood glucose levels (fasting) are very much normal. However, PP 141 mg and HbA1c% 6.2, show that you may be in a pre-diabetes state. It is a warning signal. What it means is if you take adequate care like avoiding too much of sweets, deep fried foods, junk foods, pastries, bakery items, foods made of maida and aerated sweetened drinks, then there will be no reason to worry. Plus there has to be a proper balanced diet. Regular exercise for at least 1 hour a day and keeping normal weight are other aspects to be kept in mind. Thanks.

Aap ka information bahut is general hai, usame blood glucose level, HbA1c%, unaki dava aur dosage, diet details, roj ki exercise ke bare me koi jikra nahi, in sab cheejonke bina mai koi bhi salah de nahi sakata hun. It is better if you give following information: Fasting, PP glucose levels, HbA1c%, weight, heigh, daily exercise, dietary pattern, dosage of drugs being taken, BP reading, Lipid profile, Hemogram and creatinine level. Mr. lybrate-user, Thanks for the query. The test is to measure level of insulin secretion and resistance to its action. Not really needed in routine treatment. Most important is your fasting, PP glucose levels & HbA1c% are more relevant for initiating the treatment. Plus diet control plus exercise which play equally importnat role in controlling blood glucose levels. Thanks.

Mr. lybrate-user, Thanks for the query. Some times foreskin when stretched dose get cut. This could be due to inflammation in the skin due to some sort of infection like fungal. If the problem is occurring repeatedly then you may have to look at getting the circumcision so that there is a permanent cure to it. Thanks.

lybrate-user, Thanks for the query. During pregnacy drugs to treat diabetes like Insulin and some oral anti-diabetics are quite safe. Thyroxine for hypothyroidism is also absolutely safe. Only issue is with certain anti - psychotic drugs which may not be good during pregnancy. If there is absolute need then your psychiatrist can advise better. Thanks.

lybrate-user, Thanks for the query. The blood glucose levels mentioned are too high, which show that present medical treatment is not at all helping him. There has to be an up gradation of treatment. If you want me to guide in relation to treatment, diet and exercise, I I need to know following details: Fasting, PP blood glucose levels, HbA1c%, current drugs with dosage, dietary pattern, extent of daily exercise, lipid profile and BP readings. Mr. lybrate-user, Thanks for the query. Fasting 168 mg (very high) & PP 238 mg both are suggestive of presence of uncontrolled diabetes. In such a situation drug treatment is essential plus diet control and regular exercise to control weight and burn extra glucose also is needed. Once diabetes is confirmed, then it is rather impossible to control it with only exercise and dist. That is possible in pre-diabetic state. My suggestion is please initiate appropriate treatment without any delay. If you are looking for online guidance, then come with following details: Fasting, Pp (current), HbA1c%, dietary pattern, extent of daily exercise, weight & height, then I will be able to give specific guidance. lybrate-user, Thanks for the query. Risk of getting T1 diabetes does increase if parents have it. If father has T1 DM then child developing T1 diabetes is 1 in 17. Where as If a woman with type 1 diabetes and your child was born before you were 25, your child's risk is 1 in 25; if the child was born after the woman turns 25 years, the risk is 1 in 100. These are the statistics. It is very difficult to predict what may happen. Thanks.

Mr. lybrate-user, Thanks for the query. First and foremost important thing is to achieve a strict blood glucose control with fasting glucose < 100 mg, PP 150 to 160 mg & HbA1c% < 6.8. Then you can think of vigorous workout and taking extra proteins needed for building the muscles. Best protein is egg albumen. Chicken can be good, but best is egg protein. Thanks.

Since last 6 months my TSH is above 5 and below 10. T3/T4 being normal. Ultrasound of thyroid also normal. Last 15 days having chronic constipation, fatigue and cold feeling. Are the above due to hypothyroidism. Male 46, Weight 90 kgs, Height, 177 cms. BP 150/100 Medicines taking: cilnidipine 10 mg twice daily. Do I need further medical evaluation.

Mr. lybrate-user, thanks for the query. Usually with TSH < 10 mU/L, no treatment is advocated. However, if there are troublesome symptoms are present then treatment is given. In your case symptoms like constipation, fatigue and cold intolerance as well as weight increase are indicating that there is a need for treatment. Your BP is also not in good control (ideally with treatment it should be 120 to 130 / 70 to 80 mmHg). Thanks.

Mr. lybrate-user, Thanks for the query. By taking Atorvastatin 2 tablets a day (20 mg per day), is unlikely to cause any such problem of increase in blood glucose. It is possible that you were earlier in a pre-diabetes state and that condition has now changed to overt diabetes. Please tell me the earlier Fasting, PP levels if you remember. In any case if present level is showing 300 mg, then it needs to be treated. So please check fasting, PP and HbA1c% and come back. Also inform details like your height, dietary pattern, extent of daily exercise. Also check the lipid profile. 300 mg (whether fasting or after food) is a very high glucose level, demanding urgent correction. Thanks.

Hello, Thanks for the query. For treating diabetes there are many different medications both oral and injectable. The drug selection is done on the basis of individual patients needs. One has to look at blood glucose levels, HbA1c%, weight, height, dietary pattern and extent of daily exercise. Without those details drug/s can not be decided. Hence, my suggestion is please come in a private conversation (paid consultation) with all the above mentioned details, then I will be able to give the specific prescriptions and much needed instructions to be followed for stricter glucose control. Or else consult a close by diabetologist and start treatment early. Neglecting the condition is detrimental to your health. Thanks.

I was diagnosed with hypothyroidism on October 2016 with TSH level 5.12 I consulted with endocrinologists. They suggested me not to start medicine and keep on checking TSH, T3, T4 level once in 2 months. I did so. My TSH level in the mean time came as 2.49, 2.21, 3.09 and also normal T3, T4 levels. But now 10 days ago my TSH level came as 6.45 I am extremely worried. I am preparing for civil services exam (IAS) which is the toughest exam. If I am diagnosed with hypo thyroid, my brain power will surely get diminished. Then is it possible for me to crack the exam? Many of my friends and family members suggested me to forget the IAS exam as it may not be possible anymore. I am worried. Please suggest.

Mr. lybrate-user, Thanks for the query. The TSH levels earlier and current both are in sub-clinical hypothyroidism level. Usually at this level no treatment is advised. When level crosses 10 mU/L, hypothyroidism is confirmed and treatment is initiated. However, occasionally in some patients even at the sub-clinical level, if there are troublesome symptoms of hypothyroidism present then treatment is initiated with a minimal dose. Please tell me why was the test done? Was there any specific reason for getting the test done? If there is no specific problem, then there is absolutely no reason to worry. But if you have any specific issue, then treatment can be started. As regards your preparation for civil service exam and clearing it, if you are confident of your preparation and capabilities, then nothing will stop you from clearing it with flying colours. Low thyroid levels affect clear thinking and intelligence etc only when it is uncontrolled hypothyroidism. Still if you have any doubts please come back with answers to my few queries mentioned above, then I will be able to guide. Thanks.

Hello, Thanks for the query. TSH 57.08 mU/L, shows it is confirmed hypothyroidism. Hence the treatment is also correct. Please ensure that TSH level is maintained between 3 to 4 mU/L, with adequate dosage of thyroxine. So please check TSH after 6 to 8 weeks, if the level is still above the recommended range, then there is a need to increase the dose further. Plus it is necessary to take the drug early morning on empty stomach, then nothing to be taken for at least 1 to 1 and half hour. Plus she should avoid soy beans, soy containing products and consume more of green leafy vegetables, salads and fruits. Avoid bakery items and foods made of maida completely. Thanks.
