Infertility is a condition where a couple, trying to conceive in a natural way is not able to do so even after one year of trying. Causes:

There are many causes of infertility. It may be down to a condition present in one or both partners. As per a medical study, 20% of the cases of infertility are due to males, while 40% to 50% cases are due to problems in the female partner. 30% of the cases suffer from infertility due to problems in both males and females. 

Ovulation and fertilisation are the main elements of the conception process and any condition in the female or male can upset either function, which can make it difficult to conceive. While abnormal sperm production and transport may be a common condition that leads to infertility in males, other factors like side effects of cancer treatments and too much exposure to toxins and chemicals can also play an important role in this regard. 

For females, the causes of infertility may range from ovulation disorders, uterine and fibroid tumours, uterine and cervical anomalies, damage to or blockage of the fallopian tubes, endometriosis, primary ovarian insufficiency and pelvic adhesions. Also, factors like thyroid, cancer and medication leading to temporary infertility may be at play. 

Risks: 

There are a number of factors that put a male or a female at risk of infertility. Being in your mid 30s usually has an adverse effect on the ovulation and production of well functioning sperm. Further, smoking and tobacco are among the main causes of infertility in either partner. Excessive alcohol consumption may also lead to this problem. Being overweight or underweight can also make a person infertile. Further, not getting enough exercise and routine activities out of your day, can lead to this condition as well. 

Treatment: 

Male patients can go through a treatment for any existing infections, hormonal injections and medication, surgical options like vasectomy reversal and ART or assistive reproductive technology. Female patients can turn to medication that will stimulate ovulation, Intrauterine Insemination, and surgical processes to correct a uterine septum and remove endometrial polyps, or even a hysteroscopy surgery

If these methods fail, the patient or the couple can always turn to methods like In Vitro Fertilisation. For other mild reasons, lifestyle changes along with a stress free environment can help in increasing the chances of conception.

Hypertensions - What Are The Types Of It In Pregnancy?

Hypertension or high blood pressure is common in pregnant women, even in those who have no previous history of high blood pressure. This leads to complications in about 6% to 10% of all pregnancies around the world. High blood pressure may develop before or after conception and as such needs special medical attention.
Hypertension prevents sufficient flow of blood to the placenta and this inhibits the normal growth of the fetus. This could result in low birth weight of the child. However, if diagnosed in time and treated properly, hypertension does not affect the child's health too much. There are various types of hypertension during pregnancies, such as:

1.Gestational hypertension
This type of hypertension is developed about 20 weeks after conception. There is no abnormality in urine or any signs of other organ damage (as is common with hypertension during pregnancy) but the condition can worsen and complicate very quickly. Pregnant women below the age of 20 and above the age of 40 are often diagnosed with high levels of blood pressure. Women who have heart or kidney conditions before pregnancy and women carrying more than one child are also likely to develop gestational hypertension.

2.Chronic hypertension
Chronic hypertension is high blood pressure which develops around 20 weeks before conception and does not normalize within 12 weeks after childbirth. The patient may also have been suffering from high blood pressure for a long time but the complications appear only during pregnancy because high blood pressure rarely exhibits symptoms without an associated condition.

3.Chronic hypertension superimposed with Preeclampsia
Women who have hypertension before pregnancy may develop even higher blood pressure levels during pregnancy. This leads to several health disorders like frequent headaches, fatigue and depression. Sometimes, protein is found in the urine.

4.Preeclampsia
Preeclampsia is often a serious complication of gestational pregnancy and affects about 5% to 7% of all pregnancies globally. Gestational pregnancy does not always develop into preeclampsia but it needs to be diagnosed and treated in time to avoid the complication. Women who have conceived for the first time or have a history of hypertension in the family are at a greater risk of preeclampsia. The symptoms of the condition are throbbing headaches, blurred vision, nausea and vomiting, pain in the upper abdominal region and shortness of breath.

Sprains happen when there is a pull or a twist of the ankle or the joint. This is very common in athletes and can be caused by repetitive motions in the knees, wrists, elbows, etc.

The most common symptom is pain, swelling, limited movement, and sometimes bruising. The degree of these injuries depends on its severity. The most common ways to treat such injuries are as follows.

  1. RICE therapy: RICE is an acronym for rest, ice, compress, and elevate. Rest the affected area immediately by stopping any further activity. Ice the area using an ice wrap, with the ice cubes wrapped in a towel or a cotton cloth. Repeat this exercise if possible for every 20 minutes. This will reduce the swelling and pain. Compress the area using a sling or a wrap. This will help in reducing the swelling. Elevate the affected area if possible above the heart. This will help control the blood flow and thereby reduce the swelling and pain.
  2. Pain control: If there is pain that needs medication, go for the non-steroidal drugs like ibuprofen or acetaminophen. If it is a child that is injured, it is advisable to give non-aspirin ones.
  3. Seek professional help: Most sprains can be self-limiting over a couple of days. However, it is advisable to seek medical help if there is significant pain, swelling, or numbness, open wounds or bruises, or inability to move the joint.

The doctor may decide to do the following.

  1. Scanning or imaging to rule out any internal hard/soft tissue injury like fracture or tear
  2. Immobilise the joint with a splint or a cast
  3. Start physical therapy if there is severe pain and immobility
  4. Surgical repair if there is a ligament injury

A word of caution to athletes or people who are very active physically is to go easy on the joint. In an eagerness to quickly get back to their routine, more and severe damage may be caused. This can be avoided by allowing time for complete healing.
