Hello, my first question to you would be what is the major concern in Indian society which is associated with diabetes? a

As you all know in this modern world, digital world India is having a big threat of diabetes. India is having the largest diabetic population in the world in 2015 WHO and Indian diabetic Federation found out that 6.91 crore people are there in India who are diabetic. In next 2-3 years there will be 12 crores diabetic, every 9 person is diabetic in India. That’s an alarming figure.

Why is that this disease called diabetes is not easily managed?

This can be divided into 2 factors: patient factor and doctor factor.

Doctor factor is that we have very good doctors and physicians in India but good doctors are very busy they are having 20-30-40-60 patients a day so they are not being able to give sufficient time to patients, there is a study in USA that biggest issue amongst doctors patient relationship is that poor communication poor time given by the doctor to the patient that is lacking so doctor could not provide information, pura jankari nahi de pata patient ko usko kya kya chaiye because diabetes bohot badi issue hai. Usme lifestyle changes chaiye, diet ka role chaiye, exercise ka role chaiye aur manage ho nahi paati hai so 80-90 % patients ka blood sugar manage nhi ho pata. Ye bohot bada issue hai toh isko control karna bohot zada zaruri hai , toh patient mein kya hota hai patient ek toh darai mein rehta hai mujhe bimari kese hogayi no 2 ignorence ki keise hogayi kyun hogayi keise thik karu toh knowledge hota nhi hai aur doctor k pass time nehi hai samjhane ka teesra fear k abb kya hoga thik hoga nehi hoga aur isike wajah se complies kharab ho jata hai means wo doctor ka prescription follow hi karta test thik se nhi karta manage hota nhi hai aur wo door to door doctor prescription change karna dawai change karna toh pareshan rehta hai patient aur kabhibi diabetes sugar manage nhi ho paati hai toh patient ko ek regime follow karna chaiye ek strict regime aur doctors k sath bethna chaiye aur discuss karna chaiye ki mein apni diabetes ko keise thik kar sakta hoon.

Ok we have also heard that diabetes never come alone so what are the other complications associated with diabetes? diabetes agar kaafi saal uncontrolled rahe toh cholesterol ki abnormalities ho jati hai , lipids kharab ho jatein hai cholesterol jamm jate hai apke blood vassals mein aur cardiovascular diseases , hyper tension long term mein kidney failure , erecting mein problem pet mein azma nhi rehta kabzi hoti hai aur aankhon ka parda aur aankhon ki roshni jaa sakti hai so ye haar major aang ko kharab kar sakti hai , bohot bada issue hai .

And how do you compare Ayurveda with modern medical practices? My wife is an Ayurvedic consultant and I am practicing modern medicine so we try to combine modern medicine with Ayurvedic. So the evicasy of modern medicine is maintained and patients can tolerate them very well so Ayurveda mein bohot classical drugs aati hai jo prescription mein milti hai jaise triphala hai lasan adibati hai jo sugar level ko niche rakh sakta hai , aur neem aur jamun aur karela ka sevan se bhi kamm rehta hai particular matra mein lena chaiye doctor k prescription k upar lena chaiye toh inse jo modern medicine hai uske side effect bhi kamm rehta hai aur control bhi badiya rehta hai . aur hum ye yahan pe practice karte hai ki modern medicine aur Ayurveda ka mishran patients k liye kaafi suitable rehta hai .

What is Erectile Dysfunction (ED)?

A person having erectile dysfunction (ED) is not able to achieve and/or maintain an erection of the penis sufficient for sexual activity. So, penis do not get hard at all or not enough for doing sex. 

ED is responsible for great deal of stress and anxiety as well as low self-confidence in men. Mostly its occasional, when you cannot get an erection and it is only temporary. Normally after emotional or physical stimulation there is an increased filling of blood in sponge like tissue of penis and decreased blood flow out of penis, so blood remain trapped in penis and it becomes hard and erect.

Your ability to have erection is dependent of multiple factors like our emotions, hormones, nerves, blood vessels and cells of penis itself. A problem with any of these elements can cause ed. It can occur at any age, but becomes more common with increasing age. In the older adult male, it is a major problem affecting more than 75% of men.

Common Causes of ED

Physical causes indicate you have problems with penile structure itself or any bodily disease affecting it. Diseases include less blood flow to the penis or injury of the nerves going to the penis (for example, multiple sclerosis, a stroke, parkinson's disease, spinal injury etc.).

Lifestyle disorders like high blood pressure (bp), obesity (high body weight than normal for our height and age), diabetes (high blood sugar).

Also lack of male sexual hormone called testosterone, alcohol and drug abuse, clogged blood vessels (atherosclerosis) and side effects of some medications causes ed. Sometimes psychological issues like anxiety, stress due to work or home environment, depression, relationship difficulties leads to ed.

Myths Associated with ED

Myth 1: ED is part and parcel of your body's aging process

Contrary to popular belief, ED is not a normal part of the aging process. If you're having difficulty in getting an erection, the underlying cause can be medical in nature such as undiagnosed or diagnosed cardiovascular disease, high blood pressure or diabetes.

Myth 2: ED can't be reversed

Through the right treatment and certain lifestyle changes, most of the cases of ED can be reversed to almost normal. Everyone can understand that men feel anxious and upset when they are diagnosed with erectile dysfunction. However, not many know that if these feelings are not dealt with then you may suffer from anxiety, depression as well as erectile dysfunction. The statistics agree with this statement that 82% of men who suffer from erectile dysfunction also suffer from depression. As you all know in this modern world, digital world India is having a big threat of diabetes. India is having the largest diabetic population in the world in 2015 WHO and Indian diabetic Federation found out that 6.91 crore people are there in India who are diabetic. In next 2-3 years there will be 12 crores diabetic, every 9 person is diabetic in India. That’s an alarming figure.

Why is that this disease called diabetes is not easily managed?

This can be divided into 2 factors: patient factor and doctor factor.

Doctor factor is that we have very good doctors and physicians in India but good doctors are very busy they are having 20-30-40-60 patients a day so they are not being able to give sufficient time to patients, there is a study in USA that biggest issue amongst doctors patient relationship is that poor communication poor time given by the doctor to the patient that is lacking so doctor could not provide information, pura jankari nahi de pata patient ko usko kya kya chaiye because diabetes bohot badi issue hai. Usme lifestyle changes chaiye, diet ka role chaiye, exercise ka role chaiye aur manage ho nahi paati hai so 80-90 % patients ka blood sugar manage nhi ho pata. Ye bohot bada issue hai toh isko control karna bohot zada zaruri hai , toh patient mein kya hota hai patient ek toh darai mein rehta hai mujhe bimari kese hogayi no 2 ignorence ki keise hogayi kyun hogayi keise thik karu toh knowledge hota nhi hai aur doctor k pass time nehi hai samjhane ka teesra fear k abb kya hoga thik hoga nehi hoga aur isike wajah se complies kharab ho jata hai means wo doctor ka prescription follow hi karta test thik se nhi karta manage hota nhi hai aur wo door to door doctor prescription change karna dawai change karna toh pareshan rehta hai patient aur kabhibi diabetes sugar manage nhi ho paati hai toh patient ko ek regime follow karna chaiye ek strict regime aur doctors k sath bethna chaiye aur discuss karna chaiye ki mein apni diabetes ko keise thik kar sakta hoon.

Ok we have also heard that diabetes never come alone so what are the other complications associated with diabetes? diabetes agar kaafi saal uncontrolled rahe toh cholesterol ki abnormalities ho jati hai , lipids kharab ho jatein hai cholesterol jamm jate hai apke blood vassals mein aur cardiovascular diseases , hyper tension long term mein kidney failure , erecting mein problem pet mein azma nhi rehta kabzi hoti hai aur aankhon ka parda aur aankhon ki roshni jaa sakti hai so ye haar major aang ko kharab kar sakti hai , bohot bada issue hai .

And how do you compare Ayurveda with modern medical practices? My wife is an Ayurvedic consultant and I am practicing modern medicine so we try to combine modern medicine with Ayurvedic. While science might suggest that with age, one’s libido takes a nose dive, it does not necessarily have to be this way all the time. Here are a bunch of reasons why sexual intimacy and pleasuring your partner can be phenominal in your thirtirs:

  1. You only get better with age:  Sex is like wine, it just gets better with age. When you compare the sex in your teenage years to the sex in your 20s, you will have your answer. You were probably more nervous in those teen years, but in the 20s when you libido was at the optimum level, your confidence increased by scores.

  2. Asking for what you want gets easier: This comes from maturity. The older you get, your approach towards sex becomes more mature. You get more comfortable to ask for what you want and you have a better chance of really knowing what you want out of a person.

  3. More confidence: You are 30. You are probably settled and know exactly what you are doing with your life. And it is a well known fact that you look your best in your 30s. This ought to give you the boost you need.

  4. You can laugh it off: Sex can get awkward sometimes. It is not the most pleasant of things. And funny things can happen. With experience you get to know it. And you are in a better position to laugh off things, because you are simply more mature about it and you find the humour in it.

  5. Experience: At certain jobs, people will probably give a passing glance to your qualifications and concentrate more on your experience in that field. The same applies to sex. Your fantasies might have taken a backseat for the time being or might have remained silenced for more than a while. The other processes of your usual life go on perfectly except for a few changes that you either consider as minute or you attribute them to any cause other than lowered libido (interest in sex). 

Loss of libido can create hurdles in a man as well as a woman's life and thus should be corrected at the earliest possible. Even if you don't care much about an impaired sex life, libido loss can affect your overall health that can prove to be harmful in the long run.

Some may say that it’s all in the mind. Yes, but to a certain extent only.  Considering the following causes, you would agree that it is not always in the mind that one loses interest in sex.

The common causes of decreased libido can be listed as below:

  1. While in males, testosterone levels may sharply drop due to aging, in females the estrogen levels may drop after surgeries pertaining to the vaginal tract, after menopause or after childbirth and subsequent breast- feeding.

  2. Exhaustion from prolonged office work or extensive travelling.

  3. An instance of sexual abuse in the past can result in lowered libido.

  4. A lousy lifestyle where you either smoke or drink regularly can prevent you from experiencing sexual desires.

  5. Irregular sleep patterns can be a contributing factor as well.

  6. Illnesses like cancer, diabetes and neurological disorders can hamper your sex drive.

Ways to deal with your libido loss:

  1. Make significant efforts to change your lifestyle. If you have an erratic lifestyle, sexual dysfunction is almost inevitable. You need to follow a routine to prohibit your libido from deterioration.

  2. Meditate and/or workout daily.  Indulge in brisk walk, jogging, yoga, light exercises, swimming, play games like, badminton, table tennis, squash, etc.  Over a period of time, it can restore your interest in sexual activities.

  3. Try and reduce the stress level from your life.  Spend quality time together and with family. Take time out for a movie, an event, a long drive, or a vacation.  Share your day’s activities with each other.

  4. Eat lots of vegetables and fruits. Cut down on drinking and smoking and gradually give them up completely.

  5. Take at least 8 hours of sleep so as to wake up latest by 6:00 in the morning. 

  6. A counseling by a good sexologist can help to a great extent in overcoming this problem. A major part of my treatment for libido loss consists of Counselling. Sexual intercourse for humans is not only a reproductive act, but is also considered to be an emotional and pleasurable act. However, statistics report that almost 30% of the women experience pain during intercourse also known as vaginismus. This not only hampers the pleasure, but is also an unhealthy sign since it is indicative of a problem.

Vaginismus / Painful intercourse could be a regular occurrence, or a sporadic one. There could be numerous reasons that cause the discomfort and pain. Though it might be an early manifestation of grave diseases such as endometriosis or fibroids, but more often, it is triggered due to conditions such as vaginal dryness, yeast infection, bacterial vaginiosis, which can be easily treated. It is pertinent to understand that it is not the physical infirmity of the entire female anatomy that causes such pain/ discomfort, but the existence of certain conditions that can be treated by a medical practitioner. 

Certain such causal factors associated with painful intercourse and the efficient ways to deal with them are discussed below:

  1. Vaginal dryness- Vaginal dryness is one of the most commonly cited causes for painful intercourse. There are various causal factors of vaginal dryness. These include, but are not limited to, consumption of certain drugs, bathing with hot water, health conditions or even lack of foreplay. Vaginal dryness due to the aforementioned causes is a short-lived condition that can be treated well with the help of a medical practitioner. Usage of lubricants that is suitable for you, as advised by a medical practitioner, could go a long way in aiding the ease of sexual intercourse. The lubricants act as an effective substitute, if the body is unable to naturally secrete vaginal fluids to ease intercourse, thereby greatly reducing the pain that is experienced.

  2. Improper vaginal expansion- The female anatomy is designed in such a way that it expands normally during intercourse. Proper foreplay enables the woman to get excited, which results in the release of vaginal fluids that aids in sexual intercourse. However, in the event that the female is not excited enough prior to the act, sexual intercourse can become a painful act, since the vagina is unable to expand in the absence of vaginal fluids. In such cases, take things slow and give your body time to comfortably go with the flow.

  3. Vaginal itching caused by infection- Vaginal infections can be caused by antibiotics, prior sexual activity that has resulted in contraction of Sexually Transmitted Diseases etc. This results in an itching or burning sensation while having sex. It is essential to rest your body instead of forcing things, and to consult your doctor as soon as possible. The doctor can correctly diagnose the causal factors behind the infection, and prescribe the necessary medicines for it.

  4. Emotional issues including fear of sex, marital issues/ disputes, etc are also leading cause of painful sex

Treatment:

However, symptoms vary from the common ones mentioned above, it is always advisable to consult a experienced doctor including a gynecologist, sexologist or sexual counselor.

Role of sexual counselor is very important, as he she can offer Sex therapy in the form of counseling, brief psychoanalysis, or behavioural therapy .This can help to address any underlying psychological issues, such as fear or anxiety, tackle any irrational beliefs that you have about sex, educate you about sex.

Also some special techniques can be taught eventually stop your vagina closing involuntarily, such as using vaginal trainers and pelvic floor exercises
