Are Osteoporosis and Osteopenia fast turning into diseases of the young?

International Arthroscopy , Shoulder Surgery Fellowship, M Ch. Ortho, Fellowship in Joint Replacement, Fellowship Tr.Sports surgery/arthroscopy, Fellowship in Orthopaedic Rehabilitation (FOR), MS - Orthopaedics, MBBS
Orthopedist, Delhi
Are Osteoporosis and Osteopenia fast turning into diseases of the young?

Why Osteoporosis and Osteopenia are fast turning into diseases of the young.

Did you know that osteoporosis causes more than 8.9 million fractures worldwide every year? What is frightening is many  of these fractures are now taking place among the youth due to deficit  diet and poor lifestyle coupled with genetic predisposition. Both osteoporosis and osteopenia are two forms of bone loss conditions, with the latter being a precursor to the former. To distinguish between the two a bone mineral density test is usually performed.

What is difference between Osteoporosis and Osteopenia?

A BMD (bone mineral density) T-score between -1 SD and -2.5 SD indicates osteopenia with a high chance of osteoporosis. On the other hand, a T-score of -2.5 SD and below stands for osteoporosis. A higher bone density (above -1SD) is usually indicative of stronger bones. Fragility index is more accurate way to assess risk of fractures.

In osteoporosis the bone loss or weakening of bones take place over a long period of time, and results in unexpected fractures and severe bone problems. Disfigurement, fractures in the spine, loss of mobility and independence are usual characteristics of this bone disease once it progresses further.

Prevention is better than cure, ALWAYS!

To keep bones strong and healthy, calcium and phosphate is continuously absorbed by the body. But after 30, the body’s ability to absorb calcium slows down and there’s a decrease in the production of new bones. In contrast, bone desorption (the process of transferring calcium from the bone to the blood) continues unabated.

When the body starts absorbing the store of calcium in the bones to meets its needs, it leads to calcium depletion and is one of the causes of weakening of the bone.

At present, the age limit (50-60 years) for osteoporosis has seen a drop with many younger people increasingly suffering from the ailment. Factors like unhealthy lifestyle, late-night shifts, smoking and alcohol consumption have been found to be the culprits behind the rise in osteoporosis cases among the young.  Recent studies have identified vitamin D deficiency as a major factor for poor bone health and bone mass density in the young.

How to keep your bones strong and healthy? And manage Osteo - penia/porosis complications:

Bone loss can be prevented by adopting these very simple but effective ways:

- Exercising can help in building strong bones and muscles, while preventing bone loss.

- Maintaining a calcium intake of 1000 mg for pre-menopausal women and 1200 mg for women post menopause.

- Staying out in the sun for 20 minutes to produce requisite amount of Vitamin D for calcium absorption and using supplements of vitamin D.

- there are many safe medicines available to prevent and treat osteoporosis like alendronates, calcitonin, Parathyroid , teriparatide preparation etc.

- In osteoporosis induced /associated fractures we have to to modify implants to negate weak purchase of implants in bone and improve mobility.

We have been using these modifications in established osteopenia/porosis case in fractures treatment/joint replacement/ arthroscopic fixations and MIS spine fracture management.

I am 22 years old guy. My weight is only 50 kg and height is 175 cm.
How can I increase my weight?
I am a registered dietitian and a doctor who will help you in managing your weight. You need to provide me your daily food intake details. There are many lifestyle and dietary guidelines that can be followed: take high protein prescribed diet, have paneer, eggs and potatoe daily. Incorporate dry fruits in your diet on a daily basis. Eat a balanced diet as prescribed by a registered dietitian, I being a registered dietitian and doctor have been successfully helping patients with their problem. I will also prescribe medicines that will speed up the time to achieve your weight management goals. Gradual weight management is always advisable. An increase in 500 kcal per day can result in increasing the body weight by 0.5 kgs per week. Objective should be to restore a desirable weight/ ideal body weight according to one's age, gender and height. To rebuild body tissues, to maintain a good nutritional status and to maintain ideal body weight. I will need to carry out detailed assessment in your case, after assessing your personal challenges to weight management, I will recommend a strategy to gradually change habits and attitudes that have sabotaged your past efforts. I being a both a registered dietitian (rd) and doctor have been successfully helping patients with their problems through a holistic approach using customized therapeutic diet and medications. I will also suggest home remedies. Do reply back for private consultation for rx.

You need some lifestyle modifications. Better avoid sitting or standing for a long time. Take a small walk every 30 minutes. Selected yoga postures by a good and experienced yoga therapist can help. Ayurvedic massage will help a lot for such problems. Above all, physical and mental relaxation is most important than any other treatment.

My weight is decreasing very rapidly what should I do for this. Am I suffering from any disorder?
1.Don’t drink water before meals. This can fill your stomach and make it harder to get in enough calories. 2.Eat more often. Squeeze in an additional meal or snack whenever you can, such as before bed. 3.Drink milk. Drinking whole milk to quench thirst is a simple way to get in more high-quality protein and calories. 4.Try weight gainer shakes. If you are really struggling then you can try weight gainer shakes. These are very high in protein, carbs and calories. 5.Use bigger plates. Definitely use large plates if you are trying to get in more calories, as smaller plates cause people to automatically eat less. 6.Add cream to your coffee. This is a simple way to add in more calories. 7.Take creatine. The muscle building supplement creatine monohydrate can help you gain a few pounds in muscle weight. 8.Get quality sleep. Sleeping properly is very important for muscle growth.

Dexona is a steroid and should not be used carelessly for weight gain weight which is only temporary .If you feel you are grossly underweight and not gaining weight, first of all you have to check if you have any disease like anaemia or thyroid disease by a blood tests CBC, PBS and TSH. If the tests are normal, you have to do this for gaining weight and follow these steps .1) Estimate your calorie needs for weight gain. First, you need to determine your total energy expenditure (TEE). For thin men, multiply your weight in pounds by 11, and for thin women, multiply your weight in pounds by 10. Then multiply this number by an activity factor. If you get very little activity, use a factor of 1.3; if you get a little exercise or have a job that requires standing, use 1.6 if you're a man and 1.5 if you're a woman; or if you are moderately active, such as with a light labour job, use an activity factor of 1.7 if you're male and 1.6 if you're female. Once you've determined your TEE, add 500 to this number for your total calorie needs for weight gain. 2) Eat five to six small meals a day. Eating smaller meals more often helps prevent you from getting too full, which might help you get in more calories and help you reach your weight-gain goals. 3) Balance your intake. Even though you need to eat more calories to help you fill out, the Academy of Nutrition and Dietetics says it's still important that you include a variety of foods from all the food groups, including fruits, vegetables, whole grains, protein foods and dairy products. 4) Add calories to your usual foods. For example, at breakfast make your oatmeal with milk instead of water; add cheese and avocado to your sandwich at lunch; and instead of steaming your veggies at dinner, sauté them in olive oil. 5) Snack on high-calorie foods. Fruit and nut mixes, peanut butter on whole-wheat toast, cheese and crackers are all nutrient-rich, high-calorie snack options that can help you gain weight. 6) Drink your calories -- but in a nutritious way. While water is good for hydration, when you want to gain weight it's better to drink beverages that supply calories. Milk and 100 percent fruit juice make the healthiest drink choices. The Academy of Nutrition and Dietetics also suggests that you drink your fluids in between meals so that it doesn't affect your appetite during mealtimes. 7) The most important gist of the weight gain programme is to Keep a food diary or use an online calorie tracker to help make sure you are meeting your calorie needs. Add non-fat dried milk powder to your milk, soup, yogurt, mashed potatoes and hot cereal. A 1/4-cup serving of the powder adds 109 calories and 11 grams of protein. To help make sure you gain muscle and not fat, add strength training to your exercise routine.

start doing surya namskar everday, sun is the source of energy for every living being and so do humans ,secondly, Make sure you have your head in the south or east direction while sleeping. In this position your body is taking force of attraction. Sleeping in north is very unhealthy and will make you tired and lethargic because your body is taking force of repulsion. Attraction bring elasticity, ease, good blood pressure, sound sleep, relaxation, increase in height while North brings high BP, quick heart beats, muscle and internal body part contraction, your breathing will affect. In Indian culture head of the Dead body during Antim sanskar is kept in North direction. In west direction researches are going on, so don’t sleep till researches come.
