This is quite a common condition Rule out diabetes & vit. D deficiency. Sleep on a hard bed with a soft bedding on it. Use no pillow under the head. Any way Take CALDIKIND PLUS 1 tab daily for 10days Paracetamol 250mg TDS x 3 days Contact me after that. Make sure you are not allergic to any of the medicines you are going to take. It may have to be further investigated.

Back stretching- lie flat, pull one of the knee forward to chest, hold for 3 seconds, then bring the other knee forward to the chest and again hold for 3 seconds. Then pull both knees towards the chest and hold for 3 seconds. Repeat 10 times each exercise twice a day.

PHYSIOTHERAPY TREATMENT OF HEAD INJURY

The treatment may comprise of the following measures:

IMPROVES ALERTNESS OR AROUSAL THROUGH SENSORY STIMULATION:

The patient who is drowsy or confused need to be stimulated by makes them more alert and awake. The therapist should encourage the patient’s cooperation during the treatment. The main aim is to stimulate the reticular activating system by making the patient sit or even stand in the tilt table.

The therapist should provide tactile, visual, auditory and Proprioceptive stimulation to the patient that will send facilitatory signals to the brain and will enable the alert response to be provoked. Auditory stimulation can be given by speaking to the patient during the course of treatment. Visual stimulation is given by showing familiar faces, objects or movement in the visual field of the patient. 

Proprioceptive stimulation by giving traction and approximation at joint structures is very helpful in stimulating the arousal response in the patient.

PREVENTION OF SPASTICITY:

As hyper tonicity generally sets in almost all head injury cases various measures need to be taken to keep them under control. Gentle passive movement, gradual rhythmic sustained stretch, prolonged icing for 20 minutes over the muscles, biofeedback, proper positioning are certain measures that needs to be employed for controlling spasticity.

MAXIMISE THE PATIENT’S FUNCTIONAL CAPACITY:

The main aim of this management is to improve the ROM, improve the control of voluntary movement, strengthening paretic muscles, improve the coordination, balance and teach various safety measures.

The treatment should be wide spread over the periods of time as the patient’s attention span and endurance is very less.

NEUROMUSCULAR TRAINING can be given through the development sequence by inhibiting abnormal movement pattern and by facilitating normal movement pattern.

The patient may give activities like bridging, prone on elbow, on all fours, side lying to sitting, sitting, kneeling, half kneeling, standing and walking.

PROPER DOCUMENTATION is necessary of the entire event through- out the day. Infact the routine of the patient should be maintained in the register and the patient need to be reminded of various activities especially if the patient has memory problems. The patient may be given register with photo and names of various health professional visiting him so that each day’s program can be entered. This will benefit both the patient and his acquaintance to know regarding the activities given to the patient.

USE OF VESTIBULAR BALL while training the patient for crawling, bridging, sitting, balance helps in building the Proprioceptive stimulation and teaches proper control to the patient.

Each task has various subtasks which need to be mastered by the patient so that he learns the actual activity using normal movement combination and performs it with precision. Like for training the patient to get up from bed, he may be taught to do asymmetrical push up with the trunk in partial rotation, then lower leg patterns are incorporated and finally the whole task of get up from sidelying is practiced.

REPETITION ACTIVITIES is key like any other neurological disorders. Ambulation training should always be done in upright position training the patient in each and every phase of the gait cycle. If the patient’s balance is poor then assistance may be used.

FUNCTIONAL ELECTRICAL STIMULATION has been shown more effective than kinetic joint training in certain types of cases. The upper extremity also appeared to use specific synergies for hand use in different positions. Clients often can opens hand in out stretched arm position but will be unable to perform the same action when the elbow is flexed. Some patient with minimal functional deficit in th upeer limb may be given some assistive devices or support for the hand so that they can perform some basic activity like eating, combing, writing, etc. this technique helps the shoulder and other proximal structures to produce appropriate movement sequences for hand use but does not facilitate hand function. The treatment however does provide whole task practice even though some basic component of the function is substituted by other means.

REVERSING TASKS in some patients helps in developing increased control by modifying a task or synergy as well as making the muscle work both eccentrically and concentrically. For instance lowering a glass of water on the table may help the patient in getting th glass close to the mouth by improving motor control of biceps during eccentric contraction.

You should take acupressure treatment and take biochemic kali phos 3x + nat sulph3x + nat mur 3x + silica 12x. And 4 tab each thrice a day with warm water and take it 5 day's

Caldikind plus 1tab od x10. Do neck back knee & general exercises. It may have to be further investigated. Make sure you are not allergic to any of the medicines you are going to take. For emergency treatment contact your nearest hospital or family doctor. If it does not give relief in 4-5days, contact me again.

Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. In initial stages, physical therapy and supportive treatment is management of choice. You need to take following precautions: - to maintain proper posture while working and sleeping both for neck and back region. - take short breaks in between your work to relax your muscles. - if pain is more then you can take a short course of an anti-inflammatory medication which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc in case you are having back pain. Hello. Try to maintain errect posture while sitting 2. Take a break every 1hr and have a slight walk 3. Do some asanas like dhanurasana with instructors. 4. Apply some tailas like ksheerabala taila before bath to strength en your back bones and muscles.

I am 62 Yrs old. ,height 5' 10" ,Wt 93 Kg, usually taking very light diet and do brisk walking in morning for abt 1 Hr. All my clinical tests are normal. At one spot on right heel , I feel pain for last 6 months in morning but subside to large extend during day. At present I am not taking any treatment except weekly foot massage. Hi The main reason behind weight gain is eating more of the processed food, less of exercise and drinking less water. You observe the habit of yours, you will find that these three reasons. Hence to lose weight cutting on processed food, sugary food, drinking 3-4 liter of water daily and exercise for min of 30 min per day is essential. Balanced food is the key to long term weight loss. Dear lybrateuser, - follow some tips to reduce pain: - wear shoe inserts for proper fitting of shoes - apply medicated bunion cap fro m chemist - take tablet brufen, 1 tab twice a day after meals - apply ice pack on it 2-3 times in a day - wear low heel shoe with wide toe area.

My wife is diebetic since 40 years, bp/sugar is under controle. But she has too much pain and swelling in both knees. She walks within the house only. Knees can not be replaced as she has multiple blocks & heart does not permit that.

It depends on d set up. In our set up I have replaced both knee with multiple valvular cardiac disease n also in patients wit optd for bypass surgery also. Think positive . If she will not walk. Then cardiac function will also compromised. Even newer drugs n anesthesia permits any high risk pt. Optd comfortably.

5 Things to Keep in Mind After Fractures

When you get a fracture, a lot of pain and a lot of suffering is all you can imagine. But you have to take care of these 5 things to get a good healing in place.

1. Give it some rest. A lot of rest. Give yourself a break from all the household chores, from your work life and take a nice month off. Sleep well and just let your body experience healing holistically. 

2. Apply ice immediately if you suspect an injury. As soon as you sustain an injury and you think it could be a fracture, apply cold pack. This limits the inflammation around the bone and allows skin and structures to permit healing process. Also it prevents nerve compression damage.

3. Avoid weight bearing or any pressure on the broken bone. This is a non-negotiable part of healing that your bone doctor will also advice you surely. This will only increase swelling and hamper the development process. So take care to avoid any stress to your injured part and body.

4. Eat healthy foods rich in calcium. The bones need calcium and vitamin D to aid in the repair process. Using up all the calcium and vitamin D stored up, makes the body's stores depleted. Hence make sure to check your Calcium & Vitamin D levels to make sure that wasn't the cause of the fraccture in the first place. A few examples of calcium rich foods are eggs, milk and milk products, lentils and beans, legumes like spinach and salads. Include a good amount of Vitamin C also such as in citrus fruits. 

5. Take your pills. If your orthopedic has prescribed pills to you for the fracture, you should take them. Very often I have seen patients avoiding pain killers to make the side effects less, but what it does is only harm. You should know that pain killers or NSAIDs(non-steroidal anti-inflammatroy drugs) that are most commonly prescribed after fractures are to reduce your inflammatory response. This means that the pills help your body fight off too much bleeding and allow a fast recovery, so don't shy away from them.

Hi For back pain you can take 8 sessions of S.W.D and I.F.T followed by flexion exercise of back and for ankle do streching of calf muscles followed by contrast bath, consult physiotherapist for same.

You can start light exercise and walking After 6 months you can do your daily day to day work with abdominal exercise.

I am a male 56 years old, work (only desk job) for 12 hours a day, mon to sat. Suffering from back pain for the last 2 or more years. I suffer more when I forward bend for a longer time. let's say for 3-5 minutes. It takes time to make myself stand straight immediately after if I do some work in forward bend position.

Hello lybrate user, back pain that is associated with day time sedentary jobs like computer professional s are associated with paraspinal muscle spasms. The best way to tackle this sort of pain is to do well spaced stretching exercises of the back, back extension exercises, using hot water bags to provide relief and avoiding stooping posture while sitting. Posture also plays a very important part in back pain, always maintain a straight posture while sitting. While getting up from the bed try to first go on your left or right side of the body and get up side ways. Avoid cruntches or sit-ups: they may increase the pressure on your back and may predispose to slip disc. Always maintain a healthy lifestyle in terms of regular brisk walking, meditation, eating healthy. P. S: try to avoid pain killers (analgesics) as much as possible as many are associated with side effects that you may not be aware of. Stay healthy. Stay happy god bless you.

It's only traction and exercises those help. If it aggravates, you may not avoid surgery which should be avoided. Pl take full precautions for that. Ayurveda Panchakarma is important and effective tool.
