Will a person with Type 2 diabetes under control end up with the need for insulin?

MBBS, CCEBDM

Will a person with Type 2 diabetes under control end up with the need for insulin?
Hi Type 2 Diabetes is a progressive disease. So over a period of time treatment plans change from one oral medicine to two and then three to keep the HbA1C within the target. Once multiple oral medications do not help in managing diabetes insulin has to be started, As most of the insulin producing cells in pancreas are gradually lost by then.

I am 48 year female, am having diabetes for the past two years. Now am taking ziten 500 mg in the morning and glycomet sr in the night. Sugar levels are under control its 100 and 140 in fasting and after food. I had earlier the problem of a shaky and tired feeling when the dosage was half (i was taking only 500 mg glycomet SR in the night). From january I started taking ziten in the morning and I had no shaky feeling for two months. But now again, though my sugar is almost normal, I am still getting the shaky feeling and fatigue often. Can you advise me why its like this? I find it very difficult to manage my daily routine with this issue. I am also taking Budamate 400 inhaler at times as I have wheezing prob.

Your risk for hand tremors increases with age and can also be influenced by diseases you may have, including diabetes. A sudden onset of a hand tremor may be a sign that blood glucose is too low. However, chronically high blood glucose can also cause a condition known as diabetic neuropathy, which may lead to hand tremors as well. Your SHAKY FEELING can also be due to certain medication for asthma-like salbutamol, theophylline. In case you are taking then you can visit your physician for alternate medications.

I AM constantly gaining weight. Is it due to thyroid.
It may not be so. Do consult a doctor for medical check up and other routine tests. Please recheck your daily routine and bring in some fresh changes in your lifestyle. Keep checking you weight every 15 days and monitor you calories. You can take the help of a dietician who can create diet chart taking into account your calorie need. Make exercise (and yoga) an integral part of your daily routine. Drink enough water (at least 2 litres every day during summers ). Sleep well (6-8 hrs daily ). You can also start Homeopathy later which can boost your metabolism naturally. You can contact me for further details.

Recently I got thyroid tested for myself and my T3 AND T4 level were found normal./ In range. However, my TSH level is beyond the max. Level. I.e normal range is (0 to 4.5) and my TSH is 9.2 My doctor has prescribed me thyronorm (50 mg) for 3 months, and asked me to get retested after 3 months. My query is - Should I start taking medicines as I have read at many places that 1) immediately medicines should not be taken. 2) once medicine is started, It has to be continued lifelong. Please suggest whether I should start medication or not?

TSH 9.2 mU/L, is high but considered as subclinical hypothyroidism, usually no treatment is advocated at this stage. However, if a person has troublesome symptoms related to hypothyroidism then treatment is initiated in order to control the symptoms. If no symptoms are present then person is asked to repeat the thyroid test after 6 months and if the TSH goes beyond 10 mU/L, then treatment is advocated. Thanks.
