Ruchi Gupta: We've launched this Center for Piles & Fistula since 10yrs and this is a very specific center mainly for anorectal disorder like piles, fissures and fistula.
Rakesh Gupta: I would like to tell about the disease which we're treating. The most common diseases of anorectal disorders are piles, fissure and fistula. First of all I would like to tell about piles. Piles is basically disease of the anal region. It's main symptoms are bleeding from the anal region during passing of stool.

Piles are of four stages:

  • First degree
  • Second degree
  • Third degree
  • Fourth degree

Proper pile can be analysed only by digital examination we do at the center. The first degree and second degree are best treated succesfully by infrared therapy and Ayurvedic therapy. Third degree and fourth degree piles are best treated succesfully by Ayurvedic therapies.

Ruchi Gupta: Fissure is unhealed wound occuring in the anal region. There are a lot of reasons that can cause fission like constipation, irregular bowel movements, junk food eating habits. LAdies generally suffer from fissure during pregnancy or post delivery. It is best treated succesfully by ayurvedic therapy and our therapy has magical results in fissure.

Rakesh Gupta: Fistula is a chronic waste discharge from the anal region. Patient can analyse easily, saying discharge is coming from anal region. Fistula is basically identified by two openings, one is external opening and another one is internal openings. External opening is something that the patient can analyse easily but internal opening can only be analyzed by a doctor by digital recognition or proctoscopic examination. It is successfully treated only by Ayurvedic therapy. And then we've Pilonidal Sinus. It is a mid and post pilonidal sinus commonly seen in young adults usually without any communication with the rectum. This is also a very common disease in teenagers. The main reason is that a tuft of hair comes out from the pilonidal sinus region. It is best treated succesfully only by Ayurvedic therapy.

Fistula - Ways It Can Be Treated!

The anal area is a highly vascular area, and if not maintained hygienically, it can lead to health issues including infection. The anal canal can get infected quite easily and if these infected parts begin to discharge pus, it could leak through the anal canal. This is known as anal fistula, and can be caused due to:

  1. Unhygienic toilet habits leading to infected anal region, which can pass up the anal canal
  2. Can be caused due to excessive straining, which can be due to constipation
  3. Post-surgical effect, where an existing anal abscess was drained

Symptoms:

  1. The fistula could be constantly draining pus or blood, producing a constant moist feeling in the anal region
  2. In some, it could be intermittent draining with full dry spells in between
  3. There could also be pain, which is constant and throbbing and worse with sitting
  4. Pus discharge can produce a foul smell
  5. The skin around the anus can be itchy and irritated
  6. Tenderness and swelling of the area
  7. Constipation and painful bowel movements
  8. Fever and chills, due to generic infection

Management: Prevention and treatment are both essential in managing anal fistula.

  1. Prevention is mainly achieved through healthy toilet hygiene habits, as this is the main cause for filstula formation
  2. Treatment would include para-surgical therapy ( non-invasive ), which should be followed by healthy toilet habits and good diet habits including fibrous meals to avoid constipation. 

The following are two well-established surgical corrective measures.

Khsarsutra:

  1. Ayurveda has a well-established technique known as ksharsutra wherein a medicated thread is used to drain the fistula and to prevent further fistula formation.
  2. A linen thread is smeared with latex, curcuma longa, and Apamarg Kshar which are known for their multiple healthy benefits – anti-hermorrhoidal, anti-inflammatory, and coagulatory properties.
  3. The linen thread is smeared with these herbs in layers and takes a few days to prepare.
  4. This thread is then allowed to stay in the fistula tract, which allows for healing and prevents pus accumulation and/or closure
  5. This is followed by a complete liquid diet, which is gradually moved to a semi-solid diet and if required sitz baths

Ayurvedic gugglu ksharsutra therapy:

Ayurveda is a thousands of years old system of holistic healing through herbs. It is not only based on drugs but a healthy diet, good lifestyle, staying happy and different Ayurvedic treatments which have no side effects and are absolutely safe to use. According to Ayurveda a body suffers from a disorder because of disparity in the 3 Doshas. Ayurveda deals with balancing the Doshas and healing the body. Synchronizing the body, lifestyle with nature and its paradigm.

The Doshas have to be balanced, one dominant or other ignored causes disorders, as a result the person falls sick. Here, we will talk about fistula in Ano and Ayurvedic treatments for fistula in Ano.

Anal Fistula is a two-sided canal. One end of tube opens up inside the anal canal and the other opening is near the anus on the skin. This leads to leakage of stool from the other opening, oozing of pus leading to discomfort, pain, and sometimes fever as well.

Ayurvedic herbs are especially known for keeping the digestive system in good shape by maintaining the right balance of the Pitta element that maintains the right balance of the digestive fire. According to Ayurvedic principles, any disorder in the body is a result of a faulty digestive fire that manages its functions. Stress - A Silent Initiator Of Piles!

Stress affects the entire body functioning and leaves a negative effect in its wake. If you are prone to piles (hereditary) and are stressed out, watch out as it can only accelerate the occurrence of piles or haemorrhoids. It is a bunch of blood vessels in and around the anal region. These can get irritated due to constipation and excessive strain during bowel movement. The result is pain and bleeding. These could be internal or external, depending on whether it is protruding to the internal or external environment.

If you are wondering about the correlation between the two, read on:

  1. Some of the bowel disorders including irritable bowel syndrome and peptic ulcer are related to stress
  2. Stress leads to more consumption of junk and feel good foods, which all lead to constipation and haemorrhoids. The extra pressure during training directly leads to pressure on the anal region and haemorrhoid formation
  3. Stress prevents people from exercising and promotes prolonged sitting, which again leads to haemorrhoids
  4. Stress also leads to increased blood pressure, which can aggravate piles
  5. It has been observed that the severity of piles is more when the person is stressed than in normal period
  6. Alcohol again is consumed more during stress, and this can lead to dehydration, which in turn worsens piles

Causes:

  1. Chronic constipation
  2. Smokers
  3. Genetic reasons
  4. Stress

Though they are initially painless, as they progress, they can lead to pain and bleeding.

Management:

Haemorrhoids can be managed through a multi-pronged approach. In genetically prone people, prevention through a fiber-rich diet is very useful and people who have developed it, treatment is required. Two techniques are widely used for treating piles.

  1. Kshar Sutra: This is an ancient Ayurvedic technique, wherein a linen thread is used to ligate the irritated and bleeding vessels to stop further bleeding. The linen thread is coated with herbs which have medicinal values (coagulating properties, anti-inflammatory, and anti-bacterial). This is left in place for a while to ensure the piles is cured. It is a minimally invasive procedure but should only be done by experts. It might require multiple sessions too, in people who have severe haemorrhoids and/or recurrence.
  2. Infrared rays: With light rays being used for multiple medicinal uses, clotting and ligating blood vessels is one of them. In piles, a beam of infrared rays of a specific wavelength are directed at haemorrhoid. This coagulates the blood vessels, and as there is no further blood supply to haemorrhoid, further growth is arrested. There is a scar tissue left behind which ensures further bleeding does not happen. This is done only for internal hemorrhoid, and each hemorrhoid is treated separately.

Ayurvedic Treatment of Piles:

Often Nagakesara is recommended by Ayurvedic doctors, especially when this is associated with bleeding. One teaspoon-full powder of Nagakesara is given to the patients three times a day. Haritaki is a useful herb for both dry and bleeding piles. One teaspoon full powder of Haritaki is given to the patients two to three times a day.

Ayurvedic drug Abhayarishta is a widely recommended medicine for this condition. 30 ml of Abhayarishta is given to the patient twice daily after food with an equal amount of water. Kasiaditaila can be used externally, which helps in shrinking piles and cures itching in the anal region. It helps in relieving pain and checks bleeding.

Diet:

Avoid any fast food and spicy items. Take foods which are rich in fiber. Barley, wheat, old rice, goat’s milk, brinjal, etc. are recommended food for the patient. Take sufficient water. Little warm water is sometimes recommended.

The last part of the digestive system including the rectum and the anus is a highly vascular area. It receives a rich blood supply, and there could be swelling or tearing of these blood vessels resulting in anorectal conditions, piles, fistula, or fissures. Seeing blood in the toilet bowl can be an alarming thing. Read on to know how these occur and how to manage them effectively.

Piles: The swollen blood vessels in the anal region are referred to as piles. These can be internal or external, based on the distance from the anal canal. The ones which are farther inside the anal canal are called internal piles.

Why they occur:

  1. Genetic
  2. Smoking
  3. Chronic constipation

Symptoms:

At the outset, piles present as painless bleeding with stools. This can be painless, and if left untreated, they can continue to grow and develop swelling and clotting.

Treatment: This would be two pronged with medicines to treat and food habits to prevent further growth of piles

  1. For pain relief, topical medicines can be used
  2. Dietary habits should be modified to include fiber-rich diet, with loads of fruits, whole grains, and vegetables
  3. Processed and refined foods should be avoided
  4. Hydration is very important in treatment and prevention

Fistula: An infected anal gland bursts into the anal canal, discharging pus constantly into the anal canal and becoming a fistula. This is mainly caused by unhygienic toilet habits and healthy toilet habits can prevent fistulas.

Symptoms:

  1. There is a constant discharge of pus, giving a constant wet feeling. It could also be intermittent, with completely dry spells in between
  2. There may or may not be pain, but when there is pus discharge, there could be a foul smell
  3. Prevention is very easy and should be followed. Healthy toilet habits can help avoid fistula formation

Diagnosis and Treatment:

  1. Prevention is the key in anal fistula
  2. In severe cases, since this is internal, surgical correction may be required

Fissures: Cracks in any point along the length of the anal canal are called fissues and are very common. These are very painful and the person may dread going to the toilet.

Symptoms

  1. Fissures are very painful, and a person with fissures usually refuses or avoids going to the toilet
  2. In severe cases, there could be bleeding too, though in small quantities

Treatment:

  1. It is often self-limiting
  2. Lifestyle changes like switching to a fibrous diet and adequate hydration
  3. Topical ointments for pain relief

Ayurvedic Treatment for Anorectal Diseases:

Ayurvedic Treatment: According to ayurveda, a disease occurs due to an impairment of the body energies. Ayurveda believes that the imparement of digestive system with the predominance of apana vata is the cause for fissures, and if bleeding occurs, it is due to Ranjaka Pitta. Ayurvedic Treatment for Anal Fissure improves digestion and metabolism. The treatment modalities includes Kshara Karma, panchakarma, external therapies, internal medications, Activities, Advice of food and life style changes.

Infrared Coagulation(IRC): IRC has been used for many years to treat internal hemorrhoids. It is less painful than surgery and can be preformed in the physician’s office. IRC involves the application of infrared light to the base of the hemorrhoid, which clots the hemorrhoid. Although you will occasionally feel the warmth, there generally is minimal pain or discomfort.You may return to your normal lifestyle immediately after treatment.Usually 3 to 5 treatments are required at 2 to 3 week intervals to obtain the desired result.

KSHARA SUTRA THERAPY (MEDICATED CAUSTIC THREAD): Kshara Sutra therapy is a minimal invasive Ayurvedic Parasurgical procedure and time-tested Ayurvedic technique in the management of Anorectal disorders. The digestive system is a highly complex tubular structure, with different parts of it performing distinct functions. The anal canal, which is the last part, is about 5 centimeters long. After digestion, stools move from the rectum through the anus to the outside. This area is highly vascular and has a rich supply of blood vessles, elastic tissue, and multiple muscles. Due to various reasons, these vessels can become dilated, and this condition is known as piles or hemorrhoids.

Causes:

  1. Food habits are constantly changing to include more processed and refined foods. There is reduced dairy and fiber, leading to reduced roughage and increased constipation.
  2. Food habits are also irregular, so no set pattern
  3. Physical activity is reduced with prolonged sitting, putting pressure on these vessels
  4. All these lead to strain when passing motion, which can cause these to rupture, causing blood to appear in the stool

Types:

  1. When these blood vessels are protruding into the external environment, they are known as external piles or hemorrhoids.
  2. When they are protruding into the anal canal, they are known as internal piles or haemorrhoids.

Treatment:

Managing piles is a combination of prevention through lifestyle changes and treatment to get rid of the piles which are formed.

  1. Fibrous diet: Switching to a diet rich in fiber with fresh fruits, vegetables, whole grains, nuts and seeds is very important. It adds to roughage and eases passage of stools.
  2. Avoid/reduce junk: The fried foods, pizzas, and sugary drinks taste great but are bad for health, including weight gain and piles formation
  3. Exercise: People who are required to sit for prolonged periods are more prone and should consciously take short breaks to prevent piles formation
  4. Water: One of the multiple benefits of drinking up to 8 to 10 glasses of water is easy movement of stools and reduced formation of piles
  5. Stooling habits: Keeping the anal area clean is very important to prevent piles.
  6. Ayurvedic gugglu ksharsutra therapy: Ayurvedic medicine are also helpful in case of piles. A linen thread coated with Ayurvedic herbs is used to ligate the piles, which is a minimally invasive procedure. These herbs have antibacterial, anti-inflammatory, and antihermorrhoidal properties, thereby curing piles. 
  7. Infrared rays: Another advanced technique for treating piles, a beam of infrared rays is directed at internal piles (not used in external cases) which acts on the blood vessels and causes coagulation. The blood supply to the haemorrhoid is cut off, and the formed scar tissue prevents further growth of blood vessels. If there are multiple haemorrhoids, they need to be treated at intervals. Infrared rays therapy is indicated only in 1st & 2nd degree piles . Infrared rays therapy never helpful for the treatment of fissure & 3rd & 4th degree piles .

Bleeding Piles Treated by Agni Karma and Infra Red Coagulation Therapy

Piles are Haemorrhoids which become inflamed and are accompanied with bleeding. Haemorrhoids can be defined as masses or clumps of tissues developing in the anal canal. Haemorrhoids comprise of blood vessels, support tissues, muscles and elastic fibres.

Bleeding piles can be treated using several methods or forms of surgery. The Ayurvedic form of treatment called agni karma is an effective, natural cure for bleeding piles. On the other hand, a medical procedure known as infra red coagulation therapy can also be executed for the efficient treatment of bleeding piles or haemorrhoids.

Agni karma an Ayurvedic Remedy for Treating Piles
In Sanskrit, the word 'Agni' refers to 'Fire'. Agni karma is an ayurvedic therapy, which involves the use of agni or fire. It is also called cauterization. Agni karma is popularly called Dahan Karma. Agni karma is used in the treatment of many diseases of different type. It is a good method for the treatment of bleeding piles.

In this process, any ailing surface of the body or a tissue, such as haemorrhoids, is destroyed and burnt by using a heated instrument using electric current and by freezing. An acidic agent for cutting like kshar karma is also used in the process of agni karma for the treatment of bleeding piles.

The different types of agni karma include:

  1. uchchh dagdha /scarce agni karma
  2. Durdaghda/ inappropriate burning surgery
  3. Aati dagdha /powerful or excess cauterization
  4. Samyak dagdha / appropriate agni karma

Infra red Coagulation Therapy

Infra red coagulation therapy or infra red photo coagulation is a medical process which is used to treat bleeding piles or haemorrhoids, and can only cure internal haemorrhoids.

During the process, a device is utilized, which produces a beam of intense, powerful infrared light. The heat released by the infrared light causes scar tissue and blood supply to the haemorrhoids to be cut off. This results in the death of haemorrhoid or piles, and a scar forms on the anal canal wall. The surrounding veins are held together by the scar tissue which avoids them bulging into the anal canal.

Agni karma, the Ayurvedic treatment, is considered to an effective medical treatment procedure of infra red coagulation therapy. This is because:

  • Agni karma is a much more natural method of treatment for bleeding piles, and no unnatural substance like infra red light beam is used.

Ayurveda and the Digestive System

The Ayurvedic body of knowledge relies on the rhythms and functions of the body to find the root cause of ailments. The digestive tract or system is one of the most important organs of the body. The entry and accumulation of Ama or toxins can lead to an imbalance in the way the small and large intestines functions along with various other participating organs like the liver and more. As per Ayurveda, the digestive fires or Agni also need to be produced in appropriate amounts.

So how can you understand and take care of your digestive system, the Ayurvedic way? Here's our take on the matter:

  1. Types of Agni: The digestive system has four kinds of Agni or fire that keeps it in working condition. The Jathar Agni ensures that hydrochloric acid is being produced in the stomach which directly affects the thyroid gland. The Bhuta Agni takes care of the bile acid formation in the liver. The Kloma Agni manages the pancreatic enzymes that help in digesting and absorbing sugar properly. And finally, the Dhatu Agni takes care of the tissue transformation in this entire process and region. A weak state of any of the four Agnis will lead an imbalance and food will not be digested properly.
  2. Doshas and Agni: As per Ayurvedic science, the state of your Agni and its strength depends on the four Doshas and the one that afflicts your being. In Vata and Kapha Dosha patients, the Agni will be weak which will lead to a 'cold' digestive system where the food will not be digested properly. This can lead to constipation in the long run as well as loose motions and gas. In case there is a Pitta Dosha, the raging fire of Agni will be unbearable, which can lead to acid reflux disease and chronic acidity as well as heartburn.
  3. Herbs and Spices: The use of herbs and spices while cooking can greatly cool or revive the Angi, depending on your Dosha. Ingesting a sliver of ginger along with lemon juice can help in warming the cold fire of the Agni, along with other aroma releasing spices like cardamom, pepper and licorice. Further, saliva and enzyme secretion can happen with these spices too. Also, these herbs can ensure that your digestion does not work overtime or create excess heat.
  4. Ama: Using hing to season your meals can also help in better digestion of food and less accumulation of Ama or toxins.

One needs to know the state of the Agni in the body and regulate the diet and routine with proper sleep and food intake patterns, for a well-functioning digestive system.

Constipation and Junk Food

Constipation and Junk Food

With the super busy lifestyle that everybody is gravitating to, there is less and less time for everything, including cooking and eating. Thus, more and more processed foods are finding their way into our refrigerators, as they can just be picked up and eaten anytime hunger pangs are felt. And, we do not immediately realize that processed foods are devoid of all the essential ingredients. What we end up eating has less fiber, more salt, and more preservatives (generally not very safe for all), all leading to an unhealthy lifestyle.

Refined flour, white sugar, cheese, canned juices, frozen foods, cheese laden pizzas and breadsare all yummy, but not something that is readily accepted and digested by your stomach. Try eating pizza with loads of cheese for two days and experience the bloated feeling for yourself. Not to scare you, but imagine this; you would have to eat an entire medium pizza to get 2 g of fiber. Where as, your body requires about 25 to 40 g of fiber per day for regular health maintenance.

Biologically, our digestive system requires a lot of fiber, which adds to the roughage. This is very important as it helps in digestion and aid in cleansing the bowel later. The function of the large intestine is to remove water and essential ingredients when food passes through it. Junk food passes through quite quickly, thereby, leading to loss of water and ingredients. A lot of digestive issues are also related to lack of fiber in the food, like polyps, colon cancer, irritable bowel syndrome, high cholesterol, and obesity

Most of us are aware, but tend to ignore the fact that junk food is as the name suggests, junk. There is hardly anything of nutritious value in it. On the contrary, it contains lots of salt, fat and artificial chemicals, which though, make it very tasty, but has zero nutrition value. 

Try to eat only junk food for about a week regularly, and you will surely feel:

  1. Bloated (a lot of gas due to indigestion)
  2. Dehydrated (excessive salt needs more water, which we don't realize)
  3. Constipated (lack of fiber leading to reduced bowel movement)
  4. Weak and lousy (due to lack of nutrition)

This of course does not include the harmful effects on skin, hair, looks, etc. It does not mean you have to stop eating junk completely and eat only healthy food all the time. The human body is adaptive and can adjust to a lot of variations. It is not wise to try the example above (eating pizza for a week), however, binging on junk food once a week would definitely not hurt. Use your discretion as nobody knows your body as well as you do.

What is Kshar-sutra (Alkaline thread)

A surgical thread is coated with 21 layers of ayurvedic medicines such as achyranthus aspera, turmeric, euphorbia etc. It may also be prepared by using guggul, turmeric and some alkaline material.
