My husband age 59, known case of congenital heart disease, coarctation of aorta operated 20 years back, now have asthma, now facing problem of pulmonary hypertension any new researches in this cases.

Pulmonary hypertension occurs, when the pressure of lungs increase. Shortness of breath with exertion is the main symptom. This happens because the right ventricle can't force enough blood through the lungs to furnish enough oxygen. Chest pain often occurs. Fainting or loss of consciousness happens when the brain doesn't get enough oxygen. If you exert yourself, the need for oxygen may cause you to faint. Symptoms may become worse over time. How is it diagnosed? at your physical exam your doctor will ask you to describe your symptoms and other things about your health, especially what medications and drugs you have been taking. The symptoms already listed, especially when present in a young woman, make pph one possibility. If your doctor suspects pph, you will have an electrocardiogram, a procedure that records the electrical activity of your heart. Your electrocardiogram may show thickening of the muscle of the right ventricle. A chest x- ray may show evidence of decreased blood flow in the lungs. An echocardiogram, a test that uses ultrasound waves to create a picture of the heart, shows thickening of the right ventricle when pph is a cause of symptoms. The ultrasound waves can also make a reasonable measurement of the blood pressure in the arteries in the lungs. Sometimes a heart catheterization is needed to measure the pressure in the blood vessels in the lungs directly. Your health care provider will also need to test for other causes of high blood pressure in the lungs, such as blood clots in the lungs, diseases of the lung tissue, and liver diseases. How is it treated? pph used to be untreatable and always fatal, but current treatments seem to prolong survival. Blood thinners (anticoagulants) are usually used to prevent small blood clots that might further block blood flow through the lungs. The response to treatment varies from person to person. Some people benefit from the use of drugs called calcium antagonists. These drugs improve symptoms but aren't a cure. They expand (dilate) blood vessels in the lungs and improve blood flow. Prostacyclin, a normally occurring body hormone, may be given through a vein with the help of a special pump. Prostacyclin and a combination mixture of air and nitric oxide both may improve symptoms temporarily. In unusual cases people may be offered a heart-lung transplant.

Diabetes - Why Portion Control Is So Important?

Diabetes mellitus is a serious medical condition in which your body does not produce enough insulin, the hormone that breaks-down sugar, or is not able to use it properly. So, what happens is that your blood sugar levels shoot up damaging your kidneys, heart, brain, nerves, eyes, and blood vessels. It is diagnosed through a blood test after you have fasted overnight.

Type 1 diabetes is different from type 2 diabetes. Type 1 is predominantly a genetic disorder and is found in even babies but type 2 diabetes is the one we develop as adults mostly because of our eating habits, lifestyle and diet.

If you have diabetes, your doctor will counsel you about making serious changes to your diet to reduce your risk of developing complications. Your doctor will also mandate losing 5-10 percent of your body weight if you are overweight and exercising more to get rid of blood sugar by burning it. 

Portion size control for diabetics

  1. Portion control which essentially means how much you eat at every meal—is key in helping you manage type 2 diabetes.
  2. Portion control also reduces your calorie intake and leads to weight loss which in turn improves your body’s insulin sensitivity and leads to manageable blood glucose levels.
  3. Truth be told, if you have type 2 diabetes, you need to lose weight even if you are not obese. Carrying even a slight bit of extra weight can increase insulin resistance which means that your body can no longer properly use insulin.

This is why portion control is critical to managing diabetes. So downsize your portions and get rid of carbs, especially the processed carbs like pasta, cookies, dessert and sodas, to stay healthy.

What goes on your plate?
According to the American Diabetes Association, your plate at each meal must have lots of non-starchy vegetables and smaller portions of starchy foods like potatoes. Proteins and fats especially the healthy fats are good for you as they don’t trigger insulin.

Here are some tips for portion control at mealtimes

  1. Draw an imaginary line in the centre of your plate.
  2. Divide one side of your plate yet again so your plate is split into 3 sections.
  3. Half of your plate should be filled with non-starchy vegetables like carrots or cauliflower.
  4. One fourth of your plate should be filled with protein or lean meat like egg, turkey or tuna.
  5. The other one-fourth of your plate should be filled with a good carb like whole grain bread.
  6. This method can be followed for breakfast as well.

For the first meal of the day, eat small portions of healthy foods like a bowl of oatmeal with a banana and 2 tablespoons of peanut butter for protein. Following these portion control basic guidelines will lead to guaranteed results in your weight as well as your blood glucose levels. But you need to be patient. Early ejaculation or Premature Ejaculation is a common problem and most Indians have it. It happens because you are not able to control your excitement when you go for sex. By the time you start to sex with your partner, your peak is near and you get discharged soon. Males usually have 3 minutes from start of arousal to ultimate discharge. Females take 10-15 minutes to get aroused. So how to match both. Learn to desensitize or control your emotions a little by thinking something non sexual else while you do foreplay on your partner. You can increase the time of foreplay, sex play time increases and because of that you will be able to satisfy your wife. You are spending more time in bed. When about 10-15 minutes have passed then think of yourself and you will charge immediately in a second. Then do sex. If you have proper desires then erection will too be good. If negative thoughts are there then your erection will not be good. For insertion take help of some lubrication but if you have good desires and foreplay is good, lubrication comes automatically. Ask your wife/ GF to help in getting insertion. Changing of positions also helps in proper insertion and lasting & satisfying sex. Your Diet should consist of is fresh fruits what ever available and similarly fresh vegetables and one hour of jogging or brisk walking every morning. This will keep you sexually active and fit. To take medicine without and problem will land you in Problem. Rest you deciede.

Since last 2-3 weeks , I feel ear block type , although there is no problem in hearing . Sometimes throat also gets jam type. Please advise . regards

Ear blockage can be due a problem in the external ear ( like wax, fungal infection etc) or in the middle ear ( fluid behind the eardrum due to cold,Eustachian tube dysfunction etc)You need to be examined by an ENT surgeon for diagnosis.

For your erectile dysfunction you need to build up your self confidence and also I would suggest you to take Tablet Folvite 5 mg once a day for three months.

I am 21 years old. I have like ringworm in many places in my body last 2 months. I take itraconazole it reduce it but not remove entirely. What should I do?

Along with applying suitable cream, you take care of your personal hygiene. Please keep the infected area clean and dry as moisture helps the ringworm grow. Take care of your clothes etc as the infection may recur. Keep the body dry from sweat. This will solve your problem.

If no partner has HIV, you cannot contract it even if you dont use a condom. But condom has other benefits too (eg. contraception, time prolongation etc). in any case you cant be absolutely sure of your partner's status, so condom is always recommended. If you use a condom, even if a partner has HIV the chances of contracting it becomes very less, although is not completely eliminated. Use condom, stay safe.

Mr. Lybrate-user, this could be a simple coincidence, because usually no pain killer can cause such a serious problem, unless it is an anaphylactic (very severe acute allergy). Plus there is no mention of her other ailments. She may have had some other ailments or conditions related to heart which is not mentioned. So no explaination can be given.

Hi lybrate-user, as you are suffering from psoriasis. Psoriasis is a manageable disease, generally patient use steroidal application on patches which give temporary relief and disease reappear after 2/3 days. In severe cases methotrexate is used which is not a safer option and is liver toxic. Ayurvedic external application give very quick results with sun exposure in clearing the scales and internal treatment does not allow the disease to re appear. You should use 3 gms of haldi with milk helps in psoriasis patients.
