She diabetic from last 10-12 years and last week she felt very weak and her sugar level reached 450-500 so we consulted a doctor and he suggested to take insulin and today after 3 days of taking insulin her sugar empty stomach was 70, so I want to should we continue with insulin or what should we do.

Hi Managing diabetes with insulin needs some education regarding how to adjust the insulin dose, about the type of insulin your taking, how to manage when the blood sugars are low or high and diet as well. You need a consultation to discuss the same. As of now it looks like her insulin dose is high. Please do not stop the insulin as the blood sugars are settling and if stopped can shoot up the blood sugars. The dose has to be adjusted gradually and may be stopped if she has repeated low blood sugars.

Yes, this is called shirodkar stitch, given to close mouth of uterus which remains open - due to repeated abortions/ instrumentations. As a rule the mouth is tightly shut, being elastic tissue it opens only at delivery or periodically during periods to let the collected blood out, closes again, once elasticity is lost, mouth of uterus called cervix cannot regain its normal tight self again, stitch is opened just before delivery to let the baby out. During your 2nd pregnancy, same process will be followed to save you from any mishap like miscarriage / infection to growing baby due to open mouth of uterus.

diet plan has to be one which will not make you feel hungry all the time and gives you feeling of 1. satisfaction after eating and 2. energy inbetween meals. diet should not create battle between mind and metabolism. Because metabolism wins and will power fails in most cases.

Hi , dark circles can come because of many reasons eg: lack of sleep, not wearing spectacles inspite of having it, vitamin deficiency, lot of tension, stress or anxiety, straining the eye, etc. Evaluate your problem yourself and start working on it. Don’t worry. You can lose weight easily if you are patient and determined to shed those extra kilos. You just have to alter your daily routine, watch what you eat and be physically active. This requires you to be on your weight loss commitment for quiet long. As soon as you start seeing the results, you will be surprised as well as motivated to go beyond your weight loss and adopt and a lifelong healthy attitude. For a start please follow: 1.Watch what you eat – Are you eating too much junk, drink excessive alcohol and eating too much white food and sugar? Please cut it down to as low as you can from tomorrow. Cut down tea/coffee if you are too addicted to it. It is creating havoc inside your body. 2.Lifestyle – Are you a late sleeper and late riser? You can’t imagine what this schedule of yours is doing to your metabolism. It is degrading your digestive system from inside and your looks from outside. 3.Physically active – If you are a couch potato who loves to watch tv whole day then throw that couch of yours in the street and get yourself a treadmill tomorrow. Yes it is as simple as that. Who is stopping you? You have to be physically active all day long if you want to burn those extra calories. Do daily brisk walk, Pranayam, atleast 20 surayanamaskar everyday to start seeing the results. Take break between office work to walk a little or rather just climb some stairs to get that food you ate in lunch have space and heat to digest and absorb. What’s the harm in using your washroom to do a little middle of the day squats. Do that – No one is watching you. The key is to stay physically active all day long, no matter what. 4.Eat healthy and eat on time: Not only eating healthy is important but eating at right time is equally imp too. Citrus fruits are best taken during this hour and absorb well in the body. Have oranges, amla, pineapple etc during this hour. D.Lunch (between 2-3 p. M) – Daal, roti, mix veg, buttermilk and salad. You can also take brown rice with daal in lunch. Try to get your food cooked in olive oil. Lace your salad with apple cider vinegar, black pepper and seeds like flaxseeds or chia seeds to make it nutrition packed lunch. E.Between 4 p. M – 5 p. M – Cup of Green Tea F.Evening (4-6 p. M) – Have some nice sprouts with tomatoes and cucumber. G.Dinner (2 hours before sleeping, preferably before 9 p. M) – Paneer & veggies salad with olive oil, Roti or brown rice with Daal. H.Have a glass of warm milk. Weight loss is 80% about what you eat and 20% about exercises. That said, both the things go hand in hand. If you want to lose weight in a more healthy way than be patient and just follow the regimen everyday. Slow and steady wins the race.

I have internal bleeding (piles) since from last 5 yrs. Always on the month of December end, blood comes out non stop at the time of stool. I don't get any pain. Who to overcome with the sunstroke and what are the symptoms of sunstroke and suggested the measures to protect from it.

Hye. Thankyou for your query heat stroke is the most serious form of heat injury. Heat stroke can kill or cause damage to the brain and other internal organs. Heat stroke often occurs as a progression from milder heat-related illnesses such as heat cramps, heat syncope (fainting), and heat exhaustion. But it can strike even if you have no previous signs of heat injury. Heat stroke results from prolonged exposure to high temperatures -- usually in combination with dehydration -- which leads to failure of the body's temperature control system. Other common symptoms include nausea, seizures, confusion, disorientation, and sometimes loss of consciousness or coma. The symptom of heat stroke is a core body temperature above 105 degrees fahrenheit. But fainting may be the first sign. Other symptoms may include: throbbing headache dizziness and light-headedness lack of sweating despite the heat red, hot, and dry skin muscle weakness or cramps nausea and vomiting rapid heartbeat, which may be either strong or weak rapid, shallow breathing behavioral changes such as confusion, disorientation, or staggering seizures unconsciousness if you suspect a heat stroke, immediately transport the person to a hospital. Till then give first aid. Move the person to an air-conditioned environment -- or at least a cool, shady area -- and remove any unnecessary clothing. Fan air over the patient while wetting his or her skin with water. Apply ice packs to the patient's armpits, groin, neck, and back. Because these areas are rich with blood vessels close to the skin, cooling them may reduce body temperature. Immerse the patient in a shower or tub of cool water, or an ice bath. When the heat index is high, it's best to stay in an air-conditioned environment. If you must go outdoors, you can prevent heat stroke by taking these steps: wear lightweight, light-colored, loose-fitting clothing, and a wide-brimmed hat. Use a sunscreen with a sun protection factor (spf) of 30 or more. Drink extra fluids. To prevent dehydration, it's generally recommended to drink at least eight glasses of water, fruit juice, or vegetable juice per day. Because heat-related illness also can result from salt depletion, it may be advisable to substitute an electrolyte-rich drink like lime juice, kokam sarbat, for water during periods of extreme heat and humidity. Take additional precautions when exercising or working outdoors. The general recommendation is to drink 24 ounces of fluid two hours before exercise, and consider adding another 8 ounces of water or sports drink right before exercise. During exercise, you should consume another 8 ounces of water every 20 minutes, even if you don't feel thirsty. Reschedule or cancel outdoor activity. If possible, shift your time outdoors to the coolest times of the day, either early morning or after sunset. Monitoring the color of your urine. Darker urine is a sign of dehydration. Be sure to drink enough fluids to maintain very light-colored urine. Avoid fluids containing caffeine or alcohol, because both substances can make you lose more fluids and worsen heat-related illness. The easiest and safest way to replace salt and other electrolytes during heat waves is to drink lime juice or fruit juice. Check with your doctor before increasing liquid intake if you have epilepsy or heart, kidney, or liver disease; are on fluid-restricted diets; or have a problem with fluid retention. If you live in an apartment or house without fans or air conditioning, try to spend at least two hours each day -- preferably during the hottest part of the day -- in an air-conditioned environment. At home, draw your curtains, shades, or blinds during the hottest part of the day, and open windows at night on two sides of your building to create cross-ventilation.

Since women experience different symptoms prior to menses. It's called pre menstrual syndrome. Bloating, breast tenderness are few signs. It will resolve on its own. Avoid excess salt. Wait for your periods as usual.

I'm 33 year old. Since last 1-2 years neither i'm able to sit or stand for even few minutes. I feel my legs and back have become too weak. If I walk, my legs start paining & when I sit on the ground, it takes some time to get up and straighten my back. I'm unable to sit upright on the chair as well. Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Because of your previous history and lifestyle I will advise you conservative management initially failing which we will have to investigate you in a greater detail. For back pain: if you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. For leg pain: actually pain at calf/leg region can be due to many reasons, commonly it is muscular in nature but it can also be due to tendons, nerves, blood vessels etc. My advise to you for initial 2-3 weeks will be: - ice packs application intensively with elevation of your limbs at night - take a short course of anti-inflammatory medication as per your body's suitability - stop your physical activity till pain completely subsides - intensive physical therapy, training and proper muscle conditioning once pain has subsided - slow resumption of physical activities with proper pre activity warm up and stretching of muscles and short increments in duration of physical activity each day as per pain tolerance. - we will get some blood tests for you - serum vitamin d and serum potassium levels, esr and crp once you have all these reports then you can contact me as I can advise you for proper supplementation do not hesitate to contact me if you need any further assistance.
