How Brazilian Buttock Lift Surgery Can Benefit You?

I am doctor Ajay kashayp medical director of the casmedical centre and American board certified in plastic surgery today we’ll be talking about a somewhat newer procedure, it's called a Brazilian buttock lift this procedure involves augmentation of the buttock lift. This procedure involves augmentation of the buttocks not with the implants but with the person’s own fat or autologous fat. The fat is taken from the abdomen the sides and pretty much anywhere we can find it on the body and this fat is purified and used for augmenting the buttocks. Now this concepts started once autologous fat was something that was the technique of putting autologous fat was perfected to a point where large amounts could be put into a person's body and they would survive the augmentation of the buttocks was earlier done through implants which were found to be not as desirable both in terms of comfort as well as having significant amount of complications the word brazillian came because brazil is probably the most cosmetically conscious country and the curve in the back where you had a narrow think waist and big buttocks was became very much in fashion there. It spread all over the U.S to all over now to India where people want that nice small waistline and large buttocks some people have taken it to a ridiculous extent like Nicki Minaj where the waistline is very small and buttocks are enormous. Most people, however, want aesthetically pleasing figures and not ridiculously big buttocks or extremely narrow waist that certain ratio is required and when we do the procedure we kind of take into consideration the patients bone structure and height and the weight and where the fat can be harvested from.

The main technique is to remove the fat from areas where it is an excess and where removal of the fat would accentuate in the improvement of the buttocks. Those areas are the abdomen the sides and just above where the buttocks start the hollow of the back the technique of how you take out the fat is quite important the way you do it is you take you do a marking of the buttocks where you want to take the fat from and where you want to insert it. The fat is removed either through laser liposuctions through micro canalicular liposuction or probably the best technique today is water jet or body jet liposuction. The purification of the fat is done either through sedimentation which is just letting it settle down with gravity the second would be through straining it taking a sip like a strainer and pouring it the fat through it then the third would be purifying it through centrifugation. By centrifugation, you get probably the most purified fat all that is takes a little longer than other techniques. Now once the fat has been purified it is injected through a very small cannula which is blunt needles through tiny needle holes into the buttocks and reshape it the buttocks is done. Now reshaping of the buttocks is done from so-called Brazillian buttock lift mostly in the back but there are some people mostly come from the African continent who want more bulk on the sides and I like to call it an Australian or African butt lift where people from largely from African continent want to increase on the size of buttocks and have a curve in that direction. So we have to S’s one is the back S and the side S which is African buttock lift and this is the back lift which is the S which is the Brazilian buttock lift. You can see how nicely the buttocks can be shaped and the advantage of today's techniques is that this improvement lasts as long as the diet and exercise adhere to within normal limits nothing excessively needs to be done. The cosmetic surgery of the breast is basically breast reduction for very large breast which can be symptomatic. Breast lift or mastopexy for lifting up breast that has become saggy and loose but has adequate volume and thirdly breast augmentation on enhancing the size of the breast. Now going over one by one with these three surgeries lets first talk about breast augmentation. Now breast augmentation can be done through a variety of approaches. One can be through a small incision just below the breast or in the fold of the breast the second incision could be in the axila or the armpit and the third incision could be around the nipple area. An implant is put in place a different place it can be behind the muscle or in front of the muscle. It gives an impression of the enhanced volume of the breast and the surgery is carried out in general anaesthesia. Takes more than an hour or so and the patient goes home the same day usually. It’s a fairly comfortable procedure and the results are very satisfactory.

The risks are minimal and the results are very predictable as well. the implants can be placed be behind or in front of the muscle. And it results in something like this. This is through autologous or through fat which can be the other approach instead of the implants you can use fats from patients own body and reshape the breast the incisions are usually very hidden and the implant should last for many many years. Although for a young person they may need to be changed in 20 or 25 years depending on the kind of implants the second procedure I’ll talk about is a breast reduction. This is a very common problem in ladies have very heavy breast with accompanying shoulder and back pain and a poor posture. This can only be improved by a procedure of breast reduction and the approaches to breast reduction can be either liposuction which can give a very small amount breast reduction or there can be a surgical breast reduction. Now breast reduction involves different techniques.

One can be through an incision just around the nipple the second can be through what we call a lollipop or a vertical scar technique and the third can be anchor-shaped incision. All three of them have different advantages and disadvantages however the most important thing is that eventual result should be safe and ladies should beautiful looking breast with a decrease or increased elevation of shoulder and back pain. So going through the different incisions something like this. There are two different incisions this is the vertical scar technique this is the circumareolar technique and this is the vice pattern where very large reductions can be done. The results look something like this the breast is lifted up they are shaped up and they become smaller some of the relief is from lifting up itself and the rest of it is from size reduction they can be done at any age we usually prefer to do them once the person has achiever adulthood but I have done it at patients as old as 78 years. So it depends on how bad the situation is how much the hypertrophia and decrease in sizes and how symptomatic the person is. Now, this is a lady in her 70’s and you can see the nice look you can get. The third procedure which involves similar incision is called a breast lift or mastopexy this is usually done when the breast becomes saggy and they kind of fall down.

This happens because of major weight losses and weight gains and also after pregnancy ageing is also a significant contributor to the sagginess of the breast. The procedures are done in a similar manner as a reduction but without loss of volume, the breast is lifted up with the same kind of incision around the nipple alone or with a vertical scar or with a vice pattern which is like anchor shaped scar. Having said this there is one kind of reduction which is specifically for males which is a gynecomastia reduction. A significant number of males develop a breast and they can be very conscious about it especially in the locker rooms or the swimming pools around there friends. Or especially when they wear tight tee shirts or do not have a tee shirt on. For them, a very simple procedure is gynecomastia reduction which can be done through a virtually invisible scar. Here we go this is what it looks like before and after you don’t even see a scar here so it’s a very satisfying procedure for gentlemen because they can come in the morning get it done and by afternoon they are home. there are a couple of things that I have to say in addition to what we just talked about one thing is one kind of augmentation is becoming increasingly popular is autologous fat which is from the persons own body which I've briefly touched upon. It can sometimes be improved further by adding what we call platelet-rich plasma from the persons own blood the amount of augmentation you get with this is less than with the implants but usually, it is quite satisfying for the person who is seeking average improvement in the size of the breast and firmness of the breast. This has to be done very carefully it takes a little longer than implant augmentation but it’s a very satisfying procedure. This can happen due to periodic surgeries, or the people own effort when they lose a large amount of weight in access of 20, 25 kilos. What happens after this is that a lot of the skin start sagging and for that there are different treatments the skin can sag in the face, the neck, the breast, the body, the abdomen, the sides and the legs, pretty much everywhere because of envelope becoming looser because of fat being gone the skin start hanging down, it can happen at any age I have as young as 15 to 16 years old and as old as 75 80 years old. My experience has started with people who had the massive weight loss about 20 25 years back and techniques are still evolving at this time, now we have techniques are pretty evolved and most of the reductions of skin are surgical.

Let’s first talk about the abdomen and torso which is most often the biggest sufferer for weight loss when there is a lot of losing in the abdomen and the sides what we do is called a lower body lift or belt clap it so the large number of tissues are taken down , the skin is made titer and the incision is put below the bikini line of the underwear line, this is all that happens if you look at the lady on the left on your left on her right you see that the skin has loose pretty much all over from the breast the abdomen to the back to the love handles to the thighs everywhere.


All kind of surgeries that are done lifting off the breast the lower body lift or a belt clip it and we talked about the arms can also be taken care off, as the skin of thighs. For the thighs what we do is called the inner thigh lift surgery which is done through vertical incision in the inside of the thigh, the other part of the thigh the outer thigh can be lifted up with the buttocks and the incision goes all around like a belt or the lower body lift goes all around and can easily be hidden in the underwear , this is incision for the inner thigh lift can’t be hidden very but it is put in the position that when you look at the person if the person has shots you don’t see it right away its kind off on the back side.

This is a gentleman who had a lower body lift elsewhere and we did an inner thigh lift that you can see the redundant skin is all gone.
Now, these are incisions for completion of the buttock and outer thigh lift or completion of the belt-like or lower body lift, that’s what people look like body lifted make titer looks more astatically pleasing.it feels a lot of cloths often times when they are out of cloths also although some incisions can be seen you can see how the shape has become much better much fuller and much more youthful, this is a lady only in her late 30s, if you look at this picture it looks much older now she looks very handsome, once the skin has been tightening the body looks younger.

The other part is the arms, arms also become sagging redundant and sometimes some fat is left over the arms when everything else is shrunk so we remove the fat and tighten the arms something like this, you can see the loose hanging arms some people call it the back wing deformity but this can be tightened with this technique, sometimes we can go for small incision which is very well hidden and the rest of it we can take off with a technique called the tight,
Again upper body the arms, the breast, the back is also been tighten up. Often times breast lift is required and there is just isolated redundancy without loss of falling we can do it without putting implants, however often times some implants are also necessary, after massive weight loss even in a young person you see a lot of skin redundancy especially in the neck and this can be tightened up with the procedure called a neck lift and platysmaplasty,

All these procedures are surgical procedures that require a lot of experience and expertise because the incision is long, however, the eventual results are very very grated both for surgeon and the patient

Otoplasty: Is It A Good Idea?

Otoplasty is a Cosmetic Surgery which corrects the deformities or absence of the external ear. Otoplasty originated in ancient India during 800 BC. 

When should you opt for an Otoplasty? 

  1. Cagot Ear: This is a defect where there is no ear lobe attached to the ear. 
  2. Cat’s Ear: In this condition the outer side of the ear is pulled forward. It gives the person a more feline appearance. 
  3. Cauliflower Ear: This generally occurs when your ear looks like a cauliflower. This is also caused due to hematoma that occurs because of severe trauma. Cleft Earlobe: In this condition, there is a notch on the fleshy part of the earlobe and can be corrected by otoplasty. 
  4. Constricted Ear: In this condition, some of the cartilage is discarded by surgery and a new one is reconstructed. 
  5. Cryptotia: An incision is made and the upper part of the ear is exposed by contraction. 
  6. Darwinian Ear: This occurs when the rim cartilage of the conchal bone (hollow next to the ear canal) is flat and not folded outwards. The term ‘Darwinian Ear’ is derived from the British biologist Charles Darwin. 
  7. Lop Ear: People who have lop ear generally have a protruding ear with a central depression, producing a more ‘cup ear’ appearance. The helix is enlarged after surgery. 
  8. Macrotia: In this situation the ear of the patient is very big as compared to his head. So the surgeon makes incisions on the lateral surface of the pinna (outer ear). 
  9. Microtia: In this condition the pinna is extremely small and the whole ear seems much smaller. 
  10. Question Mark Ear: This is a rare defect. There is an indentation between the cartilage and the ear lobe rim. This is also known as Cosman ear. 
  11. Scroll Ear: The outer edge of the ear protrudes forward in such a condition. Stahl’s ear deformity: In this condition the outer skin and cartilage folds abnormally giving an ‘elf’ kind of appearance. Otoplasty is usually done to improve the appearance of the ear in severe cases and it is important to get a recommendation from more than one doctor or more than one ENT specialist before going forward with this procedure.

Things To Consider, Before You Get Breast Implants!

One of the leading types of Cosmetic Surgery in the world is breast augmentation through implants. These implants are prosthetic inserts which are put into your breasts surgically to enhance the look, shape, feel, size, and balance in your body. Although these are primarily used for cosmetic purposes, they may also be used for reconstructive surgery after an accident or after mastectomy (breast removal surgery). 

Types of Breast Implants 
There are primarily two types of breast implants - Silicone and Saline. The sacs that contain both forms of implants remain the same. The sac holding the material in both the cases is made out of silicone. The two primary types are explained below: 

  1. Saline Implants: As the name states, saline implants are sacs filled with saline water. In the case of saline implants, the sacs are inserted into the breast through an incision. After the sacs are put inside the breast, they are filled with saline water and once the desired shape, contour, and size are achieved, the implants are sealed off and the incision is closed. 
  2. Silicone Implants: In this type of implant, the sac is pre-filled with silicone gel. This option is considered to have a more natural look and feel as it resembles human fat. The size of the implants however has to be decided beforehand. The doctor will make an incision on the underside of the breast, where the crease lies and then insert the implant into the breast. The incision is then closed and after healing, the scar will be hidden under the crease of the breast. 

Considerations before surgery 

Complications 
This form of Cosmetic Surgery is one of the most commonly performed around the world. However, there could be a few complications that can arise after the surgery – Infection After Surgery After surgery, scar tissue formation may cause the shape of the tissue to change. The implants may leak or rupture. Pain in the breasts Change in the breast sensations and the nipples (although in most cases this temporary)

Post-Surgery 

Some of the other considerations that you might have to consider before surgery would be – 

  1. Breast Feeding: While some women have stated that they were able to breast feed successfully after an implant, others have reported problems. Thus you may need to talk to your doctor about this especially if you want to have children post surgery. 
  2. Complications in getting a Mammogram: Mammograms to check for breast cancer and growths in the tissue may become complicated as the implant will block a straight view. However, pictures from other angles can be taken. 
  3. Getting breast implants won’t stop your breasts from sagging: If you have sagging breasts, then you will need to opt for a breast lift surgery instead of breast implants as implants won’t correct sagging.

HI, I recently had a mammogram and it came back with this finding "There is a 6-7 mm oval shaped mass projecting over the pectoralis muscle on the left MLO view in the superior left breast. Additional imaging is recommended. I had a breast reduction 14 years ago, could this be scar tissue or does scar tissue not show on a mammogram?

The difference between these two terms depends upon who you ask for the definition. Most people define liposuction as the technique for removing a considerable amount of fat whereas liposculpture is a process for removing a lesser amount of fat in a way that it reflects the underlying muscles. Liposculpture is great for creating a toned and athletic appearance rather than the bulk of shapeless volume. 

Liposuction or Liposculpture: When you are spoilt for choice, which one would you opt for? The reply is obvious; most people would love to be sculpted rather than sucked. The term liposculpture rose into prominence primarily to connote a more refined overall impression of sculpture than mere suction. I do not know what it is but its been 4-5 years, I have got puffy nipples. I do nots know what it is but after searching on net I got to know that it is gynecomastia, I anyone could tell me the possible treatment for it and the cost of treatment. I would prefer non surgical remedies.

Dear Mr. lybrate-user, The best treatment to treat Gynecomastia is surgery, where excess fat is removed which is followed by removal of breast tissue or glands. For best results VASER liposuction can be used to remove the excess fat. This can be done on a daycare basis.
