Should Kidney Stones Be Removed?

A kidney stone may not be as big as the stones in your garden but can be quite a pain. Kidney stones are actually mineral crystals formed that are usually a combination of calcium and phosphates. The size of a kidney stone ranges from the size of a sugar crystal to a ping pong ball. While some kidney stones pass out of the body with urine, others can block the urethra and become painful. 

In addition to being painful, a kidney stone can cause permanent damage to your kidneys. Since large kidney stones are usually painful, they rarely go undiagnosed. However, if a kidney stone is left untreated, it could cause the kidney to atrophy and lower the functionality of the kidney. Kidney stones that are related to an infection can also lead to chronic urinary tract infections and damage the kidney through scarring and inflammation. This could eventually lead to kidney failure.
 
Not all kidney stones need to be treated with surgery. Of the kidney stone is very small, your doctor may prescribe plenty of water and medication to treat the pain. With plenty of water, you should be able to pass the stone in your urine. Ideally, you should take plenty of rest until the stone is passed. 

 Larger kidney stones may need you to be hospitalized for treatment. These are:

-   Extracorporeal shock wave lithotripsy (ESWL)
To begin this form of treatment, a painkiller is administered. Ultrasonic waves are used to determine the location of the kidney stone. Shock waves are then passed though the kidney stone to break it into smaller pieces. These can then be passed out of the body through urine.

-   Ureteroscopy
This is also called retrograde intrarenal surgery and is performed when the kidney stone is stuck in the ureter. A ureteroscope is passed through the urethra and bladder into the ureter. Laser energy may then be used to break the stone into smaller pieces to unblock the ureter. 

-   Percutaneous nephrolithotomy (PCNL)
This surgery is performed under general anesthesia.  It involves a small incision being made in the back and a nephroscope passed into the kidney through it. Laser or pneumatic energy is then used to break up the stone into smaller pieces and pull them out. 

-   Open surgery
Open surgery is performed only in the case of an abnormally large stone or abnormal anatomy of the person. An incision is made in the back that allows the doctor to access the kidney and manually remove the stone.

If your USG pvrv is ok then you can start tab solifenacin 5 mg twice daily along with urimax d. Take it for 15 days and see how is the response.

We’re you conscious at the time you fell. Did you have vertigo. Did you slip. What I mean to ask is why did you fell. Reason is important. If you slipped, then you need to change your slippers or keep bathroom dry etc. But if reason was somoelse or you lost consciousness etc. You need to do thorough checkup. Then it should not be ignored.

I am 30 male I have fissures since 10 days I have used lidocaine pilorute sucral ano creams taken cefixime bid and zerodol od bid. I do not have bleeding. But the uncontrollable pain during passing the stools and after the stools is extremely painful and unbearable. I have also been taking hot baths for 15 mins twice daily. But unable to get rid of the pain. Kindly suggest way so that I can get even 50% relief from the pain till my fissures heal.

I am 23 year old female, and since a couple of weeks I am facing the problem of frequent urination with a little vaginal discharge. The frequency has reached to 6-7 times a day, as compared to twice or thrice earlier in an entire day. Also, the fluid intake throughout the day is low, i.e. 2-3 glasses a day. Can you please guide me in the right direction as to what might be the cause and treatment preferably without medications.

You could be having uti please get urine routine and culture and sensitivity done. Start tab nitrofurantoin 100 mg thrice daily for 1 week and see how is the response. You can change antibiotics once your urine culture report is ready.

If you don't have any symptoms then no need to treat it. If you are symptomatic then get urine routine and culture and sensitivity done and take antibiotics according to the sensitivity report for 15 days.

I got the anal fissure. It's been 3-4 weeks I am suffering from that. There is little bit pain sometimes not always as of now I am using diltiazem gel 2% ointment but still it's not working .please do let me know how to cure this. Also let me know why this problem occurs?

You should have to change your diatery habbits nd use some laxative with sitz bath its help you if chronic fissure than go for surgery.

At office I have to pee more at least every 30 minutes, Even have to hesitate in a meeting so have to control my urination. But when at home I don't feel get that much urgency to urinate. Am non diabetic. And now even my head starts paining because of tension.

Please get your urine Routine/microscopic and culture sensitivity test to r/o UTI. If your blood sugar is normal in middle aged male another cause is prostate enlargement. If there is no cause found it may be psychogenic also as you informed that at home you remain normal.
