Dear Lybrate user. In many of the cases Stammering has two important reasons. Vocal chord misalignment or other problems are the primary reason. For this vocal chord complaint, you should consult an ENT specialist and get it corrected. Second reason is purely psychological. Stammer therapy can help you in psychological reasons. Take care.

Vocal Cord Lesions - Causes + Treatment

Vocal Cord Lesions - Causes + Treatment

When the vocal cords suffer from the accumulation of non-cancerous and abnormal growth, these are known as vocal cord lesions. These lesions usually cause a variety of voice and speech related problems. These are also known as singer’s nodules and nodes, which may be caused by pressure, stress and stretching of the vocal cords. Women between the age of 20 to 50 are more prone to this condition. Here is everything you need to know about the types, causes and treatment of these lesions.

Types: There are three kinds of vocal cord lesions, that is nodules, polyps and cysts. A vocal cord polyps may be caused on one side of the vocal cords and may also lead to many disturbances in one’s voice. Another type of vocal cord lesions includes a mass of tissue that may manifest in the sac or membranes around the vocal cord. This cyst can be on the surface of the vocal cord or in the ligaments of the vocal cord. This may require surgical removal as well as voice therapy thereafter. Nodules are the third type of lesion that may form in the vocal folds. These are basically calluses that may disappear once the pressure and overuse of the vocal cords diminish.

Causes: These lesions are usually benign or non-cancerous growths that may be caused due to a variety of reasons. Screaming and overuse of vocal cords are few of the most common reasons for the eruption of such lesions. The heavy use of one’s voice can cause quite a bit of trauma to the vocal cords and may lead to problems including lesions and disturbances in one’s voice. Conditions like reflux and allergic reactions may also cause vocal cord lesions as a side effect.

Symptoms: From hoarseness to pain, there are a number of symptoms caused by this condition. A low pitch and difficulty while speaking are also some of the common symptoms of this condition. Patients suffering from the growth of such lesions will also find themselves making an effort in order to speak normally and to be heard. Constant throat clearing may be required as well.

Treatment: Besides giving your voice a rest and gargling warm saline water with salt, one can also have the condition checked by the doctor so as to ascertain whether or not it is a serious lesion that may require surgical removal and voice therapy. The doctor will usually prescribe medicines in order to deal with any underlying conditions that may have caused the lesions, including allergic reactions, reflux, sinusitis and other such ailments. One will also have to stop smoking so that there is less chemical pressure on the vocal cord. Finally, one will have to go through voice therapy for more serious lesions.

People also make fun of my voice when they cannot understand what I am speaking. Also my 2 upper front teeth are outwards and my voice is a bit Raucous. MBBS

One of the main reasons for stammering is fear and anxiety. Reduce your speed of speech for now, thus the stammering would decrease and fluency of your speech would become better. When fluency of speech reaches normal, then you gradually increase speed of speech, thus the overall speech can become as original. Because stress and anxiety increases stammering , you should also do transcendental meditation. How meditation affects health? It has been scientifically proved that meditation not only helps the mind but also body because controlled mind and purification of consciousness leads to better lifestyle and improved habits of food, sleep etc which in turn leads to better overall health. Most diseases have their roots in uncontrolled mind and impure consciousness. So do transcendental meditation or Bhakti Yoga

Yes, do not delay as early therapy - Speech therapy is important but first it has to be seen if it is just expressive language problem or a pervasive developmental disorder or a hearing problem. A child like this also needs 2 -3 hours of patient one on one interaction with the mother or father who can - 1. Speak short phrases with the child- will have to learn these tips from someone. Instead of getting the child what they want, show two things and ask if they want this or that- If the child attempts to say the name of the object, give it. However do not frustrate the child. The parent will need to learn these from a trained therapist. 2. Model- Eg on a Slide- Say SSSSlide. Or while pushing the swing - say Push. Bury objects in sand and as you dig them out, name them. Use single words or short phrases. Eg Mom is cooking. Or Mom is eating. Yum. Break words into shorter ones- eg toffee - to Taw and Fee. Talk about the child- eg Karan wants the red car. Etc.

Will stammering fade away with age? My age is 19? What a stammer does usually to cure himself?

MD - Psychiatry
Psychiatrist, Chennai
Stammering is typically recognised by a tense struggle to get words out. This makes it different from the normal non-fluency we all experience which includes hesitations and repetitions. Commonly it involves repeating or prolonging sounds or words, or getting stuck without any sound (silent blocking). Sometimes people put in extra sounds or words. Often people lose eye contact. Some people who stammer talk their way round difficult words so that you may not realise they stammer at all. This avoidance of words, and avoidance of speaking in some or many situations, is an important aspect of stammering. Stammering varies tremendously from person to person and is highly variable for the person who stammers who may be fluent one minute and struggling to speak the next. Get an mri brain and eeg with a psychiatrist evaluation.

You cannot change your voice from deep to high. In fact deep voices are much looked up to in society fro many reasons. You could cultivate your voice for this pitch and get into another genre of music. So, go for good voice training to groom your deep voice gift and make the most of an envious voice really.

Stress and Stuttering - How to Deal with It?

Psychiatrist, Delhi
Stress and Stuttering - How to Deal with It?

Stuttering is a type of speech disorder wherein, a person speaks slower than normal. It is quite  common in the age group of 3 to 8. The relationship between stress and stuttering has been pondered over for decades now. A lot of people seem to believe that stress and stuttering are co-related. On the contrary, recent studies show that stress is more of a consequence rather than a cause of stuttering. If you take the first theory into consideration, then according to that, children would be the greatest victims of anxiety and stress. But it is seen that the level of stress increases proportionately with the increase in age.

Social anxiety in teenagers increases due to stuttering, when they come across individuals who can speak in a normal pace. This in turn can crush their self confidence and lead them to isolate themselves from social groups and people in general. Hence, it is important to deal with stuttering in the early ages or as and when it occurs.

Sometimes stuttering can develop due to physical or mental trauma.  Most people with stuttering do not seek therapy for it because they might not think of it to be too severe. Little do they realise that a simple process of speech therapy can help them overcome this disability and rejuvenate their level of self-confidence.

It has also come to light, that people with relapse in stuttering post therapy has shown a three-fold increase in anxiety levels than normal. Hence we cannot ignore the strong relationship between stress and stuttering. So from the above evidence, it is safe to conclude that social anxiety or stress develops as a result of chronic stuttering.

How to deal with stress related to stuttering?

Seeking help is a great start to tackle your disability. If your stuttering is prominent enough to warrant therapy, you must seek professional help for the same. You must also learn to control your anxiety by dismissing any anxious thought or make sincere efforts in controlling physical anxiety.

You can also try to increase your social skills by participating in events, such as public speaking that can reduce your stress levels and make yourself more confident about yourself. One must also remember that a person is defined by what he does rather than what he says. So never let a minor disability come in the way of your self worth!

Yoga is excellent. You have a problem with fear. The fear has to do with people and more so if they are people in authority. If you deal with that fear, then a lot of your stammering will stop. Talk to a counselor and sort that out. There will still be a residue affect left because of pure habit. This can be resolved by some exercises, which should begin with real earnest right away. Practice speaking aloud in a closed room, but this time, choose words for which your stammer is most. Also practice singing sentences but ending the last word as a statement that is read. Next time end with two words stated, then three words etc. The lip exercise: with tight pressure press and roll the tip of your tongue against the edge of your lips, clockwise first and then anticlockwise, as many times as you possibly can. Purse your lips into an ‘O’ and withdraw it back stretching it like when you say an ‘E’, several times a day. Drop your lower jaw and flip it left to right and right to left, as many times as you can to loosen it. Practice making a resonating sound by increasing and decreasing the volume within the same breath. Sing the vowels A, E, I, O, U stretching each vowel as much as you can.

Other people talking is not yoru problem: you say that people's observation is that you speak loudly. It is definitely coming from your own hearing loss whereby you have not accurate feedback mechanism to know your volume and to make necessary corrections. Now that you know about this is it possible to speak in a volume that is less audible to you but that you know it is sufficient to the hearing level of the other people. With time and practice you will be able to master this technique.
