It's minor abnormality, not very serious. Take medicine for it only if you are planning to conceive or are already pregnant. Otherwise repeat TSH after 3 to 6 months. But get yourself evaluated for metabolic complications of PCOS I.e. Diabetes, hypertension, dyslipidemia.

Hello my dad is 49 years old recently past sunday we did coronary Angiogram + PTCA + RCA. It is common for sugar levels to be uncontrolled after such procedures. If he is on insulin, you need to discuss with his doctor regarding dose adjustments at home according to his sugar levels. Also, a common reason is that oral medications are changed/stopped during such procedures. You can ask his doctor regarding this and whether any tablets can be restarted.

Take same dose of 100 mcg daily on 6 days per week, don't take it on Sundays. Repeat TSH after 3 months.

I am a known case of thyroiditis detected in the year 2015 but wasn't on any medication as TSH Profile was WNL, though I took thyrovol supplement for 3 months when my TSH was fluctuating sometimes high and sometimes low. I had spontaneous conception. During pregnancy I had low TSH and was started on tab eltroxin 50. My post pregnancy TSH aftr 2 months of delivery was 0.01 and after 4 months it has com to 0.18 (2_4. 2) with T3 4.09 (0.6-1.7) and T4 0.77 (0.34-4.25) though I had continued tab eltroxin 25 till now. What should I do now as my TSH is still below normal range. M having severe hair fall, lack of appetite, and irritability, I feel sleepless sometimes.

lybrate-user, Thanks for the query. You are being treated with thyroxine means the condition is hypothyroidism. When a person is on treatment for that with levothyroxine TSH should be maintained <3 mU/L, to be free from all the symptoms. But 0.18 mU/L is bit too low. So you need to reduce the dose to 12.5 mcg per day. Then check TSH again after 6 weeks, to see if desired level is achieved. Thanks.

lybrate-user, Thanks for the query. Fasting blood glucose level 147 mg & PP 273 mg, both confirm that you have diabetes (T2). Diet control and exercise do help in controlling blood glucose levels, but once diabetes is confirmed, then there is absolutely no option but to start medication with out any further delay. Lifestyle modification withexercise, diet control and weight management are useful in pre-diabetes, to prevent progression to diabetes. But once it is confirmed, unfortunately there is no other option. So please start proper medication and also take restricted diet and exercise daily for 1 hour. With this you will be able to live an almost normal life. Thanks.

You nearly don't have Diabetes. You can eat rice. Have small portions of food more frequently. Make sure protien content is more in your food. You can eat strawberries, blueberries, watermelon, Musk melon, papaya regularly or daily without worrying much about blood sugar.

Diabetic Retinopathy is a complication of the eye that occurs because of diabetes. It is a condition that damages the light-sensitive tissue of the retina. It can start with a medium to low level of problems in vision and can advance to complete blindness. It is mostly witnessed in patients who are suffering from type 1 or type 2 diabetes. An uncontrolled blood sugar and diabetes levels in a patient pose a greater threat of diabetic retinopathy.

What are the symptoms?
Diabetic retinopathy could be devoid of any symptoms in its early stages. However, as the condition progresses, a person might experience the following symptoms:

  • Empty or dark areas in the vision
  • Blurred vision
  • Spots floating in the vision
  • Fluctuating vision
  • Complete loss of color vision
  • Complete vision loss

Possible causes of Diabetic Retinopathy
Too much sugar in the blood vessels can block the blood supply in the retina. In the meanwhile, new blood vessels can form in the eye that does not fully matures and leak easily. Typically, there are two types of diabetic retinopathy:

  1. Early diabetic retinopathy: This is the type of diabetic retinopathy where the new blood vessels haven’t started growing in the eye yet. This condition is also known as NPDR. In this phase, the walls of the retina blood vessels weaken and numerous tiny bulges extend from the vessel walls. This condition could even leak blood into the retina. The nerve fibers and the macula tend to swell and requires immediate treatment.
  2. Advanced diabetic retinopathy: This is the most severe and advanced type of diabetic retinopathy and is known as proliferative diabetic retinopathy. This is a condition where the damaged blood cells shut down resulting in the growth of new and abnormal blood vessels. It leaks a jelly like substance and fills the vitreous. Eventually, the scar tissue can detach the eye from the retina. It can also result in a condition known as glaucoma.

What are the risk factors involved?
Patients suffering from diabetes can have diabetic retinopathy. The risk factors further increase in the following condition persists:

Tests and diagnosis:
An ophthalmologist tests the vision, look for cataracts and measure the eye pressure. In addition to this, he looks out for symptoms of abnormal blood vessels, retinal detachment, the growth of scar tissue swelling of the retina etc. A doctor might also prescribe tests such as optical coherence tomography and fluorescein angiography.

Treatment:
For early diabetic retinopathy, a doctor might not take any action but closely monitor the condition. If it aggravates, a doctor starts treatment when he deems fit. For advanced diabetic retinopathy, the doctor might go for focal laser treatment, scatter laser treatment and vitrectomy.

Sleeping 14 hours doesn't mean you are resting. Sleep can be tiresome if it's not deep one. Do pranayam every morning. Before sleeping, do shavasan. Keep sleep time same every day. Same with food. In couple of weeks you should get a good sound sleep, and energy after you wake up.

Check your fasting, PP and pre-dinner levels if possible. Alternatively, there is something called continuous monitoring of glucose which can provide good idea about your fluctuations. The medicines will need to be changed and insulin must be adjusted. Follow a healthy lifestyle, exercise regularly and avoid sugar in any form

Mr. lybrate-user, Thanks for the query. Insulin dose decided on the basis of blood glucose level both fasting and after food. Plus there are different types of insulins, it is not which one you are using. There are rapid acting and long acting insulins plus combination of both. It is given in units per dose, not as ml. Taking excess insulin is not advised. It causes hypoglycemia. Thanks.

I had hypothyroidism since few years so was taking 100 mg tablet since my pregnancy and did not get tested after delivery. So got my thyroid test done. T3=1.18 T4=10.4 and TSH is 0.03. I recently lost 13 kilos thru diet and exercise. Also my tsh levels were 6.4 before my pregnancy. Do I need to adjust my dosage?

If you are clinically doing well and there is no increase in resting heart rate, don't be in a hurry to decrease dose of Thyroid meds. Check your free T3 levels and discuss the same with your treating doctor. He/she will decide about dose modulation.

Mr. lybrate-user, Thanks for the query. Blood glucose fasting 98 mg is normal. However, PP 176 mg, shows that you have pre-diabetes state. If proper care is not taken like food restriction, regular exercise and weight control, then you will progress to diabetes. Presently no medication is needed. Only adapting a healthy lifestyle. Thanks.
