Sir, I am a diabetic patient for the last 14 years and it is under control. Recently it was recognized that I have fatty lever and prostate stage 3. My age is 58 and weight is 84 and my height is 5.6. I am taking daily two egg omelette and two egg whites in the morning and chicken and mutton twice in a week. Can I have them. And what is your advise.

Mr. lybrate-user, Thanks for the query. It would have been better if you had given actual fasting, PP glucose values and HbA1c%, instead of saying "diabetes is in control" Because I am not aware what according to you is good control. Ideally fasting glucose should be < 100 mg, PP should be 150 to 160 mg & HbA1c% < 6.8. In many persons Non Alcoholic Fatty Liver Disease (NAFLD) is a condition observed alog with diabetes and being overweight. You are close to being in grade 1 obesity with a BMI of 29 kgs/M2 (normal < 23 kgs/M2). In such a situation it is necessary to give a closer look at your lipid profile, liver function tests and dietary intake. Items like egg yolk, red meat, chicken need to be reduced. Plus one has to look at total dietary intake and extent of exercise. Also let me know if you indulge in alcohol intake and smoking. Plus let me know what are the drugs being used for diabetes with dosage. As regards prostate enlargement you will have to go by the surgeons opinion and treatment advised. Thanks.

Hi mam. Already I'm diagnosed with hypothyroidism. But every month I never missed my cycle. I'm planning for a baby. But still we never had a success. Tsh level 2.75/ml T3--- 0.91/ml T5----- 11.67/ml Daily morning I'm taking 75 mg of thyronorm. Kindly advise me.

lybrate-user ji, Thanks for the query. Hypothyroidism if not well controlled does affect chances of getting pregnant. Based on your TSH levels, it seems to be fairly well controlled. For conception to occur, some times TSH needs to be maintained < 2 mU/L. So please increase the dosage by 12.5 mcg and check the TSH afeetr 6 weeks. That may help in getting conception. Plus yu will have to maintain it around 2 to 2.5 mU/L through the gestation period. If all other parameters are normal, then this should help. It is also necessary to get sperm count and quality checked once. Thanks.

Thyroid problem is taken care by the Thyroid medicine that you take. It does not effect your weight once you take the medicine. 1. Aim for long term weight loss @ 2-3 Kg per month = 12-15 kgs in 6 months. So on. 2. Add 2-3 Fruits to your daily diet before meals. Double your water intake. 3. Cut down Sweets/Biscuits/ Colas/Juices from your diet. 4. Completely eliminate Puris/Parathas/Pizzas/Burgers/Fried Rice/Biryani etc 5. Eat slowly and mindfully. Reduce portion size. 6. Exercise daily - 'Intense' Cardio for 40-60 mins. This will keep you toned and trim. 7. Love yourself and respect your body. 8. For a customised plan, you can consult me privately.

Both issues are different. Your life style should be good for healthy body. You can check with gynecologist for family planning, and consult a dietitian for weight reduction diet plans, do more exercises and eat healthy like low calorie and fat free products.

Sir/Madam, My wife is 28+years old. After marriage of 16 months she is not getting pregnant. As we visit Doctor also 2 months before and doctor given CCQ-50 and Consevel for this also some tests. Her ovulation period is 20-22 each and every month. Her health is well but doctors found high bp earlier at visit now she taking tablets fr bp also as per doctors concern. Also we r together in the ovulation times. Now my question is that what to do next. Please suggest something and give your valuable advise for it. Thanks.

A lifestyle disease such as diabetes is never nice to have, isn’t it? Also, when a person has diabetes, there are some things of which more should be had and then, there are some whose consumption should be scaled back. So what exactly does a diet for a person who has diabetes look like?

  • Breakfast is said to be the most important meal of the day as it kicks off the metabolism for the day. Having a large breakfast is quite important so as to take care of the body, but this only makes sense if the food that is being eaten for breakfast is healthy enough. It is quite a good idea to have some porridge. While it is true that porridge is a carbohydrate-rich food item, what it also has is a great amount of soluble fibre. In fact, this sort of fibre takes some time to digest, so it reduces the pangs of hunger, which would have normally made a person eat unhealthy junk and fast food.
  • In a world of food that is processed, packaged and picked up from the supermarket, people are just not eating the correct amount of fruits and vegetables as they should. What is important to keep in mind is that vegetables are to be had in multiple portions on a daily basis, but these vegetables must be low in starch content. This means that for a person who has diabetes, vegetables which should not be eaten include potatoes, sweet potatoes, peas as well as corn.
  • On the other hand, the sort of vegetables which are to be eaten include broccoli and spinach. Spinach is also very good for increasing the level of haemoglobin in the blood. Also, making use of cinnamon extract is good as it has the potential to cut a person’s sugar levels by up to a tenth.
  • While it is not true that potatoes and other starchy foods should be completely avoided, a diabetic person should watch the amount of them that is being consumed by him or her. The reason for this is that starch-filled vegetables have high amounts of carbohydrates, which are generally responsible for raising the level of blood sugar. This is surely the last thing a diabetic person would want or need.
  • If you notice, strawberries have some sugar content, but they are a lot better than biscuits and other snacks. So in case a diabetic person wants to nibble on something, sweet fruits are always a better option.

Male factor infertility is a common problem these days and it describes those couples that have failed to be pregnant owing to health issues of the male partner. While in most cases, people have a general misconception that infertility is related to female reproductive issues only, recent statistics say otherwise.

Various reasons can be attributed to male infertility which may range from low production of sperm to poor quality sperms, to abnormal morphology. Male factor infertility is also related to men who have normal sperm production, but it does not get transported to the vagina during intercourse.

Evaluating male factor infertility-

It is crucial to evaluate men who are suspected to be infertile through comprehensive checkups, physical tests, and numerous semen analysis. In certain conditions, additional tests and evaluations may also be required. For men with a severely low production of sperms, and poor sperm motility, it is important to consider karyotypic abnormalities. Male factor infertility was previously regarded as a contraindication to in-vitro fertilization since the presence of abnormal sperms may not be able to fertilize female eggs. But recent statistics show that there is an indication of IVF allowing fertilization and eventual live births in couples where the man has impaired sperms. IVF has shown commendable success in the treatment of such men.

When to consider IVF?
It goes without saying that IVF has benefitted many couples suffering from male factor infertility over a decade. Some of the most common problems treated by IVF include:

  1. Men having poor quality semen which may involve poor motility, low concentration and even poor morphology dealing with the shape of the sperm.
  2. Couples who have failed to conceive through normal intercourse and conventional infertility treatments.
  3. Men with non-obstructive and poor quality sperms can also benefit from the IVF process. Generally, such men produce sperm in a reduced quantity and also have an absence of sperm in their ejaculate. Some men who have undergone chemotherapy may also have compromised sperm quality as well as quantity.
  4. Men suffering from the congenital bilateral absence of the vas deferens or CBAVD are able to produce sperm but they do not have the vas deferens which is the outflow tract.
  5. Lastly, men who have undergone vasectomies and did not wish to undergo a reversal can obtain sperm that can be used in the IVF process.

Regardless of what’s causing infertility, it is important to consult an IVF expert to get through the process. IVF with the help of intracytoplasmic sperm injection is an advanced laboratory process in which a healthy sperm is injected into an egg for creating an embryo which is subsequently implanted into the uterus of the women. Besides the IVF treatment, medications and hormone therapy may also be used for altering high or low hormone level affecting fertility.
