After avoiding it for many years due to our societal taboos, I finally reached out to the doctor. And now I realize how relieving it is to get your disorder treated rather than facing its consequences. He not only treats the disorder of the patients, but also tries to nurture all major and minor aspects of their personalities, that leads them to a healthy and productive life. And such tireless effort on each patient is commendable. A certain psychiatric condition which causes a person to be functionally impaired in social and occupational settings is known as Impulse Control Disorder. Most of you are granted with the ability to think before you act but it isn't the case for people suffering with such a condition. People diagnosed with this condition are not able to withstand the urge to do something detrimental to themselves or other people.
This condition is similar to other disorders such as kleptomania, compulsive gambling and pyromania to name a few. Although people suffering from this disorder do not plan their acts, the acts they do perform generally fulfill their conscious wishes. Other disorders of such similar nature like Kleptomania and ADHD, which are associated with difficulty in controlling impulses but that is not their chief feature. Patients of impulse control disorder find it highly distressful and are hence often seen to lose control of their actions and consequently their lives.
What causes this disorder?
Some medical practitioners are of the opinion that this disorder is a subgroup of other conditions such as stress, anxiety, OCD and other such. Scientists and researchers are not particularly sure what causes this condition but have determined that it can be caused due to a combination of physiological, emotional and psychological factors pertaining to cultural and societal aspects. Also, certain brain structures linked to emotions and memory functions in and around the frontal lobe are linked to planning functions and impulses.
Some studies show that certain hormones such as testosterone which are associated with aggression and violence may also play a role in forming such disorder. Aggressive and violent behaviors are apparent in people suffering from this disorder. Certain studies have shown a connection between certain forms of seizure disorders and aggressive impulsive behaviors.
People with such disorders are also more likely to contract addiction and mood disorders. Antidepressants are usually prescribed in treating such disorders.
Nicotine addiction or tobacco addiction refers to an acute dependence upon tobacco products primarily due to the presence of the nicotine drug. In other words, nicotine addiction refers to an inability to stop using tobacco products despite the knowledge and awareness that it is harmful to you. While nicotine only induces addiction, it is the presence of other substances in tobacco which makes it damaging and injurious to health.
Nicotine has the tendency to produce effects in the brain, both physical as well as mind-altering, that give temporary satisfaction. This is precisely why people get hooked into the habit of tobacco consumption which eventually leads to addiction.
Unfortunately, stopping tobacco use immediately can also lead to several withdrawal symptoms, like agitation and restlessness.
The main reason why tobacco is so addictive is due to the fact that nicotine releases neurotransmitters such as dopamine in the reward centre; of the brain, which in turn improves your mood and gives you temporary pleasure.
Causes of nicotine addiction can either be physical or behavioral, for instance situations or feelings which make you crave for tobacco. These include:
1. Specific times during the day, say after every meal or during breaks at work
2. While defecating
3. While drinking alcohol
4. Stressful or depressive situations
5. At the sight or smell of a burning cigarette
6. Peer pressure
7. While driving
Symptoms of nicotine addiction include:
1. You are unable to quit smoking despite frequent attempts.
2. You experience withdrawal symptoms after attempting to quit.
3. You continue to smoke despite the awareness that it is injurious to your health.
4. You forego social as well as recreational activities because of your need to smoke.
Psychiatric disorders like bipolar disorder or schizophrenia do not appear all of a sudden. There are small changes that happen to an individual over a long period of time until the disease fully develops. These signs and symptoms might be too subtle in the early stage for other people to notice until they become obvious. But an early detection of any irregularity proves to be more beneficial for the patient in the long run.
Everyone needs some quiet time to cool off and relax but if a person continuously withdraws him/herself from social occasions and avoids meeting or speaking to friends, it could be a sign that something is wrong. Losing interest in life, work and people are symptoms of depression and psychotic disorder.
Having trouble concentrating, remembering, understanding and explaining oneself in a coherent manner are some common signs of mental disorder.
Excessive stress and anxious behavior for a prolonged period are not normal. Frequent heart palpitations, headache, shortness of breath, racing mind and restlessness are cries of help that need to be heard.
Sudden dramatic outbursts for no apparent reason, frequent mood swings and feeling distressed are potent signs of mental illnesses. These kinds of behavior are seen as part of a person's nature and so they are often unnoticed.
-Changes in appetite and sleeping patterns
Oversleeping may be a sign of depression and insomnia could be a sign of anxiety. Changes in appetite and not caring about oneself or the world are indicators of a mental problem.
An eating disorder, mostly appears when your food habits reach levels of extremity.
Anorexia nervosa and Bulimia are two main and the most common eating disorders found in men and women. One is the lower extreme while the other is the higher extreme in terms of food intake.
People who are affected with anorexia think of themselves to be very overweight in spite of actually being very slim and thin. Anorexia inflicted people consume less than a thousand calories per day. They exercise rigorously and try to find more ways to lose weight. They vomit a lot due to the insufficient diet and suffer more.
The treatment for anorexia nervosa generally includes a mix of mental treatment, and regulated weight gain program. An anorexia patient should seek treatment as early as possible. The best ways to deal with anorexia are:
- Psychological Treatment
Various psychological treatments can be used to cure anorexia of different stages. They are- Cognitive analytic therapy, cognitive-behavioral therapy, interpersonal therapy, focal psychodynamic therapy. A family's response is also an important psychological factor. Proper encouragement and effort should be given.
- Weight gain program
This measure will guide an anorexic patient to improve his diet habits and slowly gain weight. A patient is monitored carefully, traits are recognized and a likewise diet pattern is given.
Bulimia nervosa is another category of eating disorder. The main characteristic of this disorder is overeating in the highest level. Binge eating is observed, followed by attempts by the affected individual to lose weight. A bulimia patient is constantly trapped in the cycle of overeating and then thinking about losing weight.
Maybe you do not like the idea of binge eating and fear getting obese, but once affected by bulimia a patient cannot follow this and binge eats. Later, they adapt extreme measures for losing weight.
Psychotherapy, or talk therapy includes examining your bulimia and related issues with an expert. It is proven that these sorts of psychotherapies restrict the symptoms of bulimia. The three modes of psychotherapy include- Cognitive-behavioral therapy, Family-based therapy and Interpersonal psychotherapy.
Medicines such as anti-depressants can be taken by bulimia patients along with psychotherapy. Prozac is a recommended and effective medicine.
- Proper diet plan
For bulimia affected patients, having a proper balanced diet is very essential. A diet expert should be consulted, who will make the most essential food pattern for you. Undertaking a weight loss program is also recommended.
Eating disorders such as anorexia and bulimia should be avoided, and you should know the best ways to deal with them.
Schizophrenia is a debilitating mental and emotional condition that is characterised by a heavily distorted thought process as well as hallucinations and even extreme fear that borders on paranoia. Let us find out more about this debilitating disease:
- Genetics: Genetic factors play an important role in the development of schizophrenia.
- Environment: Possible environmental factors include obstetric complications, the mother's exposure to influenza during pregnancy or starvation. It has also been suggested that stress, trauma even migration can lead to the emergence of schizophrenia.
- Neurodevelopmental Factors: Schizophrenia appears to be a neurodevelopmental disorder. That is changes that cause the illness have been occurring from the earliest stages of development even in utero, and may continue to influence the development of the brain over the first 25 years of life.
This disease has a variety of symptoms include cognitive and emotional ones that can vary in the degree and severity. One of the most common symptoms included hallucinations and delusions where the patient may end up imagining things that do not really exist. The patient may also be gripped by sudden and crippling fear like paranoia, which can be debilitating as far as everyday functioning of the person goes. The symptoms can also interfere with the way a person deals with situations and life skills on a cognitive basis. Withdrawal and hopelessness are also a part of this disease.
The earliest signs and symptoms must be referred to a psychiatrist. The treatment involves the management of the case of an individual basis along with psychological rehabilitation programs and inclusion of the patient in self-help groups. Housing and employment programs must also be availed so as to keep the patient busy. Also, behaviour therapy will be required in such cases. The mode of counselling should usually be on a one on one basis, although many doctors also suggest that group therapy can help in showing examples of progress so as to encourage the patient to change and get a new lease of life.
The one condition that is highly underdiagnosed and undertreated is depression as symptoms rarely go away without an expert’s assistance. The ICD-10 or the International Classification of Diseases (Tenth Revision) is an internationally recognized diagnostic system that classifies several known psychological disorders based on the number of criteria symptoms specified by the system. It is commonly used by physicians to diagnose and treat depression in patients.
Primary Symptoms of Depression as per ICD-10
Physicians look for three vital signs of depression before evaluating other symptoms to identify the severity of the depression correctly. These three symptoms include persistent sadness or a bad mood, low energy or constant fatigue and loss of interest and pleasure in daily activities. Depressed mood or sadness is identified through self-subjective reports from the patient or through observations from others living around the patient. Loss of interest or pleasure or experiencing low energy/fatigue in any, in most or all activities is also evaluated through subjective accounts from the patient or through observations by others.
Even if at least one of the above three symptoms have been present in the patient for most days for at least two weeks, the physician then proceeds to evaluate if the patient has symptoms from the remaining seven out of ten signs included in the ICD-10 system.
Associated Symptoms of Depression as per ICD-10
The seven associated symptoms are evaluated only if the patient has one or more of the three above mentioned symptoms. These seven symptoms include the following:
- Disturbed Sleep: The patient being unable to sleep in almost all or most days including cases of insomnia or hypersomnia.
- Low concentration: The patient is shown to lack concentration or is indecisive during daily activities and shows signs of vacillation.
- Lack of self-confidence: The patient is known to show low levels of self-esteem and a general feeling of self-worthlessness and guilt in during everyday interactions.
- Slowed movement: Slowed movement is considered as a symptom only if others observe it and not through self-evaluation. This includes agitation and retardation in the day to day activities and being unable to perform motor functions efficiently.
- Excessive guilt and self-blame: The patient is feeling unreasonable levels of guilt and self-blame and shows signs of self-reproach.
- A sudden change in appetite levels: The patient shows sudden increase or decrease in appetite. This also includes cases where the patient loses or gains more than 5% weight in a month or less with no changes to diet and nutrition. Varying levels of appetite on a day to day basis is also a symptom under this category.
- Suicidal Tendencies: The patient expresses repeated thoughts about his/her death with no clear plan, or attempts to commit suicide or expresses a plan to commit suicide.
The number of symptoms from the three primary and seven associated symptoms may vary patient to patient and physicians use these ten symptoms under the ICD-10 to evaluate the severity of the mental health condition and proceed with treatment.
OCD or obsessive compulsive disorder is a mental condition, which traps people in a cycle of repetitive thoughts that push them to do repetitive behaviors. As the name states, the obsession over certain things compels them to do it over and over again. These actions will only help you feel relieved for a short time and soon the cycle starts back again.
Symptoms of OCD
The symptoms of OCD are classified into two groups, one for the obsessive behavior and the other for compulsive behavior. Some of the common symptoms for these are mentioned below:
As the name states this means obsessing over certain things, which varies from person to person. Some of the things could be:
1. Being scared about making mistakes
2. The fear of being embarrassed in public situations
3. Constantly being scared about contamination by germs, microbes or dirt
4. Doubts about sure and simple situations and the need to be constantly reassured about them
The obsessions usually results in compulsive behaviors, which you may need to perform to reassure and relieve yourself of the anxiety temporarily. Some of these may include:
1. Trying to constantly arrange things in a certain way and still not being satisfied
2. Thoughts, words and images being stuck in your head throughout the day that may keep you disturbed and cause to lose sleep
3. You may even repeat specific words or actions multiple times as a measure of self reassurance.
4. Hoarding of items, which have no value
Causes of OCD
Biological factors: Certain chemical imbalances in the brain may lead to OCD, which may happen due to hormonal or biological changes. In fact certain infections are also known to trigger OCD in people.
Environmental factors: Certain types of mental trauma have been known to trigger OCD as well, such as
1. Office, school, college or other work related stresses over a long period of time
2. The passing away of a near and dear one
3. Relationship problems
4. Repressed abuses from earlier in life
Most treatments of OCD are categorized into two types:
1. Cognitive behavioral therapy: This is a counseling based therapy where you may be slowly asked to face your fears and overcome them over a period of time.
2. Medications: Antidepressants, inhibitors, or antipsychotic drugs or relaxants in various combinations may be used to treat OCD.
3. Electroconvulsive therapy: Only in the rarest of cases would doctors consider electroconvulsive therapy, if the patient is a danger to themselves or others around them.

- Taking care of oneself: Often after the death of a loved one, an individual may become wary of his or her own condition and fail to look after himself or herself. This should be avoided. Normal diet, exercise and activities are required.
- Channeling grief into productive activity: Constantly focusing on the loss of the loved one can increase the trauma and stress. Sometimes it helps to focus on helping others, e.g. Volunteering at a charity or assisting others in some way. Even taking up artistic pursuits or any productive hobby proves to be beneficial.

- Cooking: Learning to cook can help in activating various parts of the brain such as sight, smell and taste. This is related to different parts of the brain resulting in an overall development.
Eating disorders can be defined as conditions that influence your eating behaviour significantly and which may give rise to serious health issues. Eating disorders are associated with behaviours like compulsively counting calories, puking after meals, living on a rigid diet etc. which often leads to a feeling of self-loathing and obsessive thoughts about food and weight.
Here are the three most common eating disorders and the symptoms by which they can be identified.
1. Anorexia nervosa - This disorder is considered to be a mortally dangerous condition in which the body weight reduces drastically. An individual is terrified about gaining weight and can take extreme steps to maintain their weight and shape, often leading to starvation, vomiting and many other health problems.
Common symptoms include:
- Reducing the amount of food to be consumed to cut down the calories
- Exercising beyond capacity to burn calories
- Recurrently monitoring your shape, weight and appearance
- Skipping meals
- Avoiding social gatherings where you will be offered food
- Irregular menstrual cycles in women
- Erectile dysfunction in men
2. Bulimia nervosa - Mostly observed among people in their mid-teens, Bulimia nervosa is a serious type of eating disorder associated with episodes of bingeing and purging with the individual being unable to regulate their eating habits. Moreover, the person suffering from this disease often feels guilty and horror-struck about weight gain and forces himself/herself to vomit.
Normally, people suffering from this condition exhibit the following habits:
- Binge eating
- Use of laxatives or throw up forcibly to get rid of calories
- Feeling exhausted and guilty
- Experiencing irregularity in their menstrual cycles
- The inability to maintain normal weight, in spite of trying to eliminate calories
3. Binge-eating disorder - This disorder is characterised by overeating, eating even when you don't feel hungry and continuing to eat even after feeling full. People suffering from this disorder cultivate a feeling of guilt inside them for overeating but do not try to nullify its effect by vomiting or exercising intensely. Some of the most prominent symptoms of this disorder include:
- Eating alone to hide the embarrassment caused due to overeating
- Eating quicker than normal people
- Being overweight
- Binge eating at minimum one instance every week
