Is the commonest type of work up in the entire infertility patient. Single puncture laparoscopy was replaced by double puncture laparoscopy in most of the centers of the world. We must record both laparoscopy & hysteroscopy for future important record in infertility patient, for second opinion & for deciding future treatment protocol. Observing free spill with methyline blue from both fallopian tubes is not enough/complete work up during laparoscopy. We must see on both ovarian fosses after lifting the tubes for the possibilities of endometriosis. We must see for tuberculosis, endometriosis & pid in all the cases. Hysteroscopy requires dilatation of the cervix and flushes both fallopian tubes with high pressure fluid helps in achieving very good fertility enhancing results following endoscopy in infertility patients. Just below umblicus small needle is introduced and co2 gas is insufflated inside abdomen. Rather than creating a large incision and opening up the body, tiny incisions are made and a laparoscope is inserted. This slim scope has a lighted end. It takes pictures – actually fiber optic images - and sends them to a monitor so the surgeon can see what is going on inside.

Performing laparoscopy usually only requires two tiny incisions less than one half inch, (about 5-10 millimeters) in length. One incision is made just below the navel, and another is usually made near the bikini line. The first incision allows a needle to be injected into the abdomen so carbon dioxide gas can be pumped inside the cavity of the abdomen which helps to keep intestines & omentum up and away from organs.

This allows the surgeon a better view and more working space to maneuver the laparoscope and surgical tools as needed. Using small incisions rather than opening the abdomen lessens recovery time as well as discomfort and makes surgical scars less noticeable. 
Many patients undergo laparoscopy as an outpatient procedure, returning home within 4 to 6 hours of surgery. For normal laparoscopy procedure takes about 5 to 10 minutes only. Most begin feeling much better within one day.

Can BPAs Damage a Healthy Penis?

Practicing regular penis care is one habit from which every man can benefit. After all, maintaining a healthy penis is a prerequisite for having a happy (and active) penis. In addition to taking typical steps like washing regularly and giving it plenty of exercise, guys need to look into other issues which could adversely affect their manhood. One which has arisen in recent years, surprisingly, is the effect BPAs can have on a healthy penis.

About BPAs

BPA is short for bisphenol A, an industrial chemical that has been used for years in the manufacture of some plastics. Many of these plastics are used with foods and beverages. For example, many water bottles are made of plastics that contain BPAs, as are a lot of plastic take-out food containers and some canned food containers.

BPAs came into the news when it was discovered that using products with BPAs in a microwave - as often happens when a person re-heats their leftover take-out food - may cause the BPAs to leach out of the plastic and into the food. Some studies indicate that this leakage can also occur even when these products are not exposed to significant amounts of heat.

In general, the FDA believes that the amount of BPAs that leaks into bottles or containers over normal usage is small enough to be safe; however, there is concern about the amount that might come from heating these products. In addition, there is some concern that even small amounts might be damaging to young children or fetuses.

The healthy penis issue

So how does all this affect the penis? Again, the jury is still out, but there are some areas of possible concern.

Most prominently, many scientists believe that exposure to PBAs could result in decreased fertility in men. The general rate of male fertility has been declining for several decades, and the reasons for this are still unclear. However, one theory is that chemicals which affect the endocrine gland may be playing a role - and BPAs fit that bill. BPAs can impact the endocrine glands, and in doing so it creates a product which is kind of a copy of estrogen.

In a study involving rats, part of the group was exposed to BPAs and part was not. Male rats in the exposed group had a significantly lower sperm count than those in the group which was not exposed to BPAs. And a study involving men found that those with high BPAs had 23% lower sperm counts.

A 2009 study also found that BPA exposure could result in a reduced sex drive and a much greater likelihood of erectile dysfunction. However, it's important to note that the men in this study all worked at plants that manufactured products with BPAs; they therefore were exposed to much higher concentrations of BPAs than the normal male would be.

While more studies are needed, it seems likely that long term exposure to BPAs could present a risk to a man's healthy penis. Limiting that exposure by using containers that are not made of plastic, or that are labeled "BPA free" if they are made of plastic, can be a big help, as can avoiding plastic containers for the purposes of reheating food.

Avoiding BPAs isn't all that a man must do to maintain a healthy penis. Regularly applying a top rank penis health crème (health professionals recommend Man 1 Man Oil, which is clinically proven mild and safe for skin) should also be a part of his care regimen. Autofellatio, the performing of oral sex upon oneself, is an act that only a select number of men can perform - but the idea of autofellatio is one that most men find intriguing and man have at least attempted. While autofellatio certainly wouldn't have a serious impact on a man's penis health, the pursuit of it can result in potential damage to other parts of the body; thus, the pleasures of self-administered oral sex should be weighed against the potential pain.

There's an old joke that asks why a dog licks its nether regions; the answer is an enthusiastic and envious "Because it can!" It's just a joke, but the fact remains that men, as they explore their sexual equipment, often fantasize about performing oral gratification on their own tools. This is understandable: oral sex with a partner feels amazing, but a partner is not always available. Besides, some men feel curious about what performing fellatio would feel like but are not interested in actually having sex with another man. Who can do it?

For understandable reasons, there are no reliable statistics on what percentage of men are capable of performing autofellatio; it is, after all, not the kind of question that is typically asked or readily answered. How do they do it?

There are several positions that are employed in performing autofellatio. These include:

Lying down on the back. This seems to be the most common method, perhaps because the position takes advantage of the force of gravity to bring the penis closer to the mouth. A man starts lying down on his back. He lifts his legs and hips up and moves them forward, bringing his legs down over behind his head. With the penis dangling above the head, he then inches it closer until it reaches the mouth.
Standing up. Seemingly a much more difficult position, some men are flexible enough to bend from a standing position so that their mouths reach their penises.
Sitting. Although some men perform autofellatio while seated in a chair, more seem to be capable of performing seated if they are seated on the floor, with their legs crossed in something resembling a yoga position or the "crisscross-applesauce" position favored by young children.
It should perhaps be noted that most men who are capable of autofellatio do not manage to get more than the head of the penis into their mouths. But that is reward enough for their efforts!

There can be consequences.

As mentioned before, flexibility and agility are essential to successful performance of autofellatio. It is VERY easy for a man to cause damage to his back, neck, torso, hips and other parts of the body in attempting to perform this difficult maneuver. This is especially true if a man has not properly warmed up and stretched out. In some cases, the damage can be severe, so it is advisable to think carefully before attempting autofellatio.

Self-performed oral sex (like all sexual acts, whether performed solo or with a partner) is better enjoyed when the penis is in good general health. What Is Premature Ejaculation?

There is no set time when a man should ejaculate during sex. But it’s probably too soon if you have an orgasm before intercourse or less than a minute after you start. It’s a problem because when you ejaculate you lose your erection and can’t continue having sex. You and your partner may feel there’s not enough time to enjoy it.

How to do Self-Breast Examination?

The best time to do your Breast self-examination is while having a shower. Soap your hands and body, then use the flat of your fingers and move them over the breasts step by step as indicated below:-

Step 1: Look at your breasts in the mirror with your shoulders straight and your arms on your hips. Observe the size, shape and colour of the breasts. Look for any deviation from normal – any visible distortion or swelling should be immediately brought to the doctor’s attention. Also, look for the following changes:-

  • Dimpling or small depression on the breast
  • Change in nipple position or inversion of nipple (nipple being inward instead of sticking out)
  • Redness, soreness, rash, or swelling

Step 2: Raise your arms and look for the same changes. Sometimes, the changes in size and shape are more evident in this position.

Step 3: Look for any signs of fluid coming out of one or both nipples (this could be a watery, milky, a yellow or bloody discharge). Gently squeeze the nipples to check for discharge and observe the colour of the discharge.

Step 4: Lie down and feel your breasts using your right hand to feel your left breast and then your left hand to feel your right breast.

  • Use a firm, smooth touch with the finger pads of your hand (ring finger, middle finger and index finger), keeping the fingers flat and together.
  • Use circular motion (each motion measuring about the size of a coin) to palpate* the breasts.
  • Cover the entire breast from top to bottom, side to side – from your collarbone to the top of your abdomen, and from your armpit to your cleavage.
  • Follow a pattern (either horizontal – side to side or vertical – up and down) to be sure that you cover the whole breast.
  • Squeeze the nipples and observe if there is any discharge.

Step 5: Repeat step 4 in standing and sitting position.
