Causes of bladder control problems in women

Urinary incontinence is the term used to describe bladder control problems that affect several people. Many think it only occurs among older, menopausal women but it actually isn't uncommon amid young and active women.

Inability to control the bladder accompanied by pain can be symptomatic of various disorders ranging from a minor infection to cancer. Fortunately, bladder cancer is rare, and bladder pain is usually not serious.

6) Nerve Damage and Neurological Disorders - Strokes and spinal cord injuries have a heavy impact on bladder control and amount to urinary incontinence. Diseases such as Alzheimer's disease, Parkinson's disease and multiple sclerosis also lead to the same.

7) Health Complications - Disorders such as diabetes and obesity directly affect the bladder and the ability to exercise control over it.

8) Disability and Impaired Mobility - For women who are physically impaired and suffer from problems of arthritis, urinary incontinence is a major problem as they are unable to reach the toilet easily.

9) Bladder Cancer - Even though it is extremely rare, bladder cancer affects a number of women every year and is treated through surgery and chemotherapy.



What Causes Bladder Control Problems in Women?

The urethra is a thin tube that is responsible for carrying urine from the bladder. The urethra is wide enough to facilitate a free flow of urine. However, in some cases, the tube can narrow down and it can restrict urinary flow. This condition is known as a urethral stricture. It is a condition that affects mostly men.

Causes:
The inflammation of the tubular tissues or the presence of scar tissue is the number one cause for urethral stricture. In some cases, an accident or the injury to the groin area can also lead to the formation of such thin tubes. The other factors can include pelvic fractures, radiation, urinary infection and in some cases even STDs(Sexually Transmitted Disease).

Symptoms:
Most of the symptoms of Urethral stricture are mild. Only in some rare cases, the symptoms can be extreme. These can include weak or restricted urine flow, frequent urges to urinate, and feeling of heaviness in the bladder, a burning sensation during urination, swelling and pain while urinating. Sometimes your urine may discharge volts of blood and the color of urine may change.

Diagnosis:
Diagnosis is by a physical examination and by analyzing the urine samples. In most of the cases, the thickness of the walls is measured by cystoscopy. The rate of the urine passed may also be checked for identifying the symptoms.

Treatment:
Treatment depends on the severity of the condition. Nonsurgical methods aim to improve the width of the urethra. This is done by using an instrument called a dilator. This is done as an outpatient procedure. This procedure is the most common method done and is done for most of the milder cases. Surgery is another option. The surgery aims to remove the heaviness of the urethra and enlarges the size of the flow.

Urine Flow Diversion:
This is done in extremely rare cases if the bladder has to be completely removed. In those cases, hole in the abdomen is done and urine is bypassed to that hole. However, this is an extremely rare scenario.

Prevention:
Try to avoid injuries in the groin area. If you are playing sports or a fitness enthusiast, use proper protection for your abdominal and groin regions. What Is Urethral Stricture?

The prostate gland in a man begins to grow in size on reaching 25 years of age quite naturally, and this is known as benign prostatic hyperplasia abbreviated as BPH. Now the problem arises when the enlargement of the prostate is more than normal.

In men, the urine flows through the urethra from the bladder. An abnormally enlarged prostate blocks the flow of urine through the urethra. Transurethral Microwave Thermotherapy or TUMT is used to treat mild blockages that reduce the frequency of urination, the urgency, and straining. However, it is not able to correct the bladder emptying issues. Transurethral Radio Frequency Needle Ablation or TUNA helps in the destruction of the prostate tissues resulting in an improvement in the flow of urine.
Prostatic stents are also sometimes used for the management of the condition of an enlarged prostate. In this treatment procedure, a small metal coil is inserted into the urethra for widening it and keeping it open. In most severe case of prostate enlargement, only surgical methods remain as the opted method of treatment. Laser surgeries are performed to destroy the tissues of the prostate for easing of the condition. Ultimately, an open prostate surgery is performed when the transurethral procedure cannot be used. In this surgery, the tissues of the prostate are removed by the surgeon in case of great enlargement.

Enlargement starts usually after 50 years. Surgery 'Transurethral resection of prostate (TURP)' either by monopolar or bipolar electrocautery. Enucleation of prostate by laser is also an option.

Enlarged Prostate - How Can It Be Managed?

The condition of enlarged prostate occurs due to the enlargement of a man s prostate gland, with the passage in time. Also known as benign prostatic hyperplasia (BPH), it is more common in men over the age of 60. Some cases might have symptoms and others may be symptomless. The prostate is located below the bladder and is responsible for producing the fluid needed by semen. Causes
It is of common knowledge that in males, the urine originates from the bladder and flows through the urethra. The resultant enlargement, caused due to the gradual multiplication of cells, subjects the urethra to extra pressure. Further narrowing of the urethra causes more contraction of the bladder, resulting in the urine being forcefully pushed out of the body.

With time, the condition leads to the bladder muscles gradually becoming thicker, stronger and oversensitive. Contraction occurs even due to the presence of small amounts of urine, giving rise to frequent needs of urination. At one point, the bladder muscle is unable to overcome the effects of the narrowed urethra. Due to this, urine does not pass properly and the urethra is not emptied.

Some of the common symptoms of enlarged prostate include:
1. Frequent urination
2. Urgency to urinate
3. Difficulty during urination
4. A slow or weak urinary stream
5. Requirement of extra effort to urinate
6. Interrupted sleep due to need of urination

Further risks
Sometimes, when the bladder is not emptied completely, a risk of urinary tract infections develops. Some other serious problems which can be a result of enlarged prostate include blood in urine, bladder stones as well as acute urinary retention (inability to urinate). In some rare cases, kidney and/or bladder damage might also result from such a condition.

1. Using sound waves to break up stones 
In this technique, a procedure called Extracorporeal Shock Wave Lithotripsy (ESWL), is employed. ESWL uses the help of sound waves to create vibrations which breaks the stones in smaller pieces. This procedure takes about an hour and can cause mild pain and discomfort. Your doctor may administer sedatives to reduce your sensitivity. ESWL has side effects like blood in urine or bruising in the abdomen. 

2. Surgical removal of kidney stones 
Surgical removal of stones is done with the help of a procedure called nephrolithotomy. It uses small telescopes and other instruments which are inserted in your back through a small incision. It is generally done in case of large stones and when ESWL fails. 

3. Ureteric Calculi (Using a scope to remove stones) 
A thin illuminated tube called ureteroscope is used to remove a comparatively smaller stone in the kidney or ureter. The ureteroscope is equipped with a tiny camera which determines the location of the stone which is then broken into pieces with the help of other instruments. It may require general or regional anesthesia.

Interstitial cystitis is another condition which gives rise to severe bladder pain. This condition is more commonly seen in women. The bladder becomes inflamed and irritated and the pain gets worse when the bladder is full or during menstruation. It is a chronic condition.
Urethral stricture is another cause of bladder pain. Men are likely to be more affected by this condition. In this case, the urethra becomes inflamed and narrow creating difficulties to pass urine.
Yeast infection also known as, Torulopsis is another cause of bladder pain.
