Celiac disease is a serious genetic autoimmune disease with over 300 symptoms, and most people manifest with a combination of them. It is caused by intolerance to gluten, which is a protein found in wheat, rye, barley, and other cereals. Gluten is usually present in the small intestine, and in patients with celiac disease, the lining of the small intestine gets attacked. Consequently, a number of organs and/or body systems are affected, making it extremely difficult to pointedly say that a person has celiac disease.

These symptoms cover almost all systems of the body including reproductive to digestive, cardiovascular to endocrine, respiratory to neuromuscular, and even affect the bones, and the teeth. Another major variation also is the age at which celiac disease becomes symptomatic. While some show symptoms quite early in their childhood, some others show symptoms later in their life, and some may not manifest with any symptoms all through their life.

Talking of the oral/dental symptoms, there are two main causes for concern one involving the soft tissues and the other involving the teeth. Let us look at the two in a little more detail.

Similar to the canker sores or the usual mouth ulcers, celiac disease also cause whitish sores. Unlike the canker sores which appear on the lips, the celiac mouth sores are common on the insides of the cheek, gums, lips, palate, and sometimes even the tongue. The appearance is very similar to that of aphthous ulcers, which have a whitish or grayish margin. They appear at random with no specific reason. The soft tissue lining of the digestive system is present from the stomach or intestine all the way to the mouth, and therefore these ulcers are seen. These can be quite discomforting and interfere with nutrition. Topical creams and/or gels can be applied to treat them. If celiac disease is diagnosed by then, abstaining from a gluten-free diet will help relieve the symptoms. Else, they may continue to recur and symptomatic treatment would be required.

The second oral symptom of celiac disease is the presence of visible defects of discoloration in the enamel. There are usually white, brown or yellow spots, usually discolored on the incisors and molars (front and back teeth). The bad news with the tooth issue is that these do not go away once a gluten-free diet is adopted. Depending on the severity, these may require tooth whitening treatments ranging from bleaching to veneers to full crowns.

Celiac Disease: How to Recognize and Manage Them

Most women attain menopause between the ages of late 40s and early 60s, the average age being about 51. This is an important milestone in a women's gynecological history. One major change is altered female hormone levels, and this leads to a lot of physiological changes. From hot flashes to mood swings, there is also increased predisposition to osteoporosis and uterine cancer.

If you have not had your menstrual cycles for close to 12 months, chances are you are into menopause. So, that means absolutely no vaginal bleeding anymore whatsoever. However, if you experience bleeding, even spotting, be on the alert. It is not normal and needs to be examined, and if required, diagnosed and treated.

Postmenopausal bleeding or PMB, as it is popularly called, can be due to a variety of reasons. While it could be something as trivial as inflammation of the uterine or vaginal lining, it could also be an indication of more severe issues like cancer.

Atrophic vaginitis: Decreasing hormonal levels lead to increased dryness and therefore inflammation of the vaginal and uterine tissue. This is one of the common causes of bleeding after menopause.

Endometrial atrophy: Also caused by lower hormone levels, the lining of the body of the uterus gradually thins down and can get inflamed.
Polyps: Noncancerous growths in the uterus, cervix, vulva, or vagina can also lead to bleeding.

Infections: General infection of any area along the uterine tract could lead to occasional bleeding.

Cancers: Though only 1 in 10 PMB cases turn out to be cancers, the prognosis improves with early diagnosis and intervention.

Diagnosis: As repeated above, reach out to your doctor if you notice postmenopausal bleeding.  

Diagnostic methods could include the following:
1. Physical exam
2. Transvaginal ultrasound
3. Endometrial biopsy
4. Hysteroscopy
5. Dilatation and Curettage

Treatment: Needless to say, this would depend on the diagnosis. For very minor cases with diagnosis like altered hormone levels, no treatment may be required other than modification of the hormone replacement therapy. For endometrial atrophy and atrophic vaginitis, use of estrogen creams and pessaries would be sufficient. Polyps would require removal followed by cauterization (application of slight heat) to stop the bleeding.

Cancer: This would depend on the type and location and require a combination of chemotherapy and surgery.  Removal of the uterus also may be required in some cases. So, if you have had bleeding of any sort after a year of menopause, do not ignore it.

1. Ovulation problems - One reason for female infertility is when eggs don't mature in the ovaries. This is known as ovulation problems. Possible symptoms of this are absent or irregular periods and the possible solution is keeping a check on body weight, taking fertility drugs and/or having IVF.

2. Uterus problem - There are tissues, known as endometrial tissues which form the lining of the uterus. When they grow outside the uterus, it is called endometriosis and can be a reason for infertility. While some women have no symptoms of this, some have painful periods. Fertility drugs, ivf and surgery to remove the tissue are its possible solutions.

3. Pcos - Pcos or Polycystic Ovarian Syndrome is of late one of the most common reasons of female infertility. It is characterised by unpredictable ovarian patterns and hormonal imbalances. Possible symptoms are acne, obesity, irregular periods, and excessive hair growth. Weight loss, changes in lifestyle, injected fertility drugs, ivf and ovarian drilling are its possible solutions.

4. Damaged fallopian tubes - Some women have blocked or damaged fallopian tubes which prevent the sperm from getting to the egg. This situation leads to stds and pelvic inflammatory diseases. There are no symptoms as such and the methods of solution are surgery to open the tubes and if that is not possible, to remove the tubes and try ivf.

5. Unidentified reasons - There are many unexplained fertility problems where none of the above mentioned problems exist. In such situations, fertility drugs - with or without artificial insemination or ivf are advised.
