The condition of enlarged prostate occurs due to the enlargement of a man s prostate gland, with the passage in time. Also known as benign prostatic hyperplasia (BPH), it is more common in men over the age of 60. Some cases might have symptoms and others may be symptomless. Although the causes are relatively unknown, it is evident that BPH is not a form of cancer, neither does it cause cancer. The prostate is located below the bladder and is responsible for producing the fluid needed by semen. The growth of the prostate tissue that is associated with benign prostatic hyperplasia begins near the inner prostate which is a tissue ring around the urethra. Its growth is generally inward.

Causes
It is of common knowledge that in males, the urine originates from the bladder and flows through the urethra. BPH is a condition where the prostate experiences a benign i.e. non cancerous enlargement which leads to blockage of urine flow through the urethra (the urinary duct). The resultant enlargement, caused due to the gradual multiplication of cells, subjects the urethra to extra pressure. Further narrowing of the urethra causes more contraction of the bladder, resulting in the urine being forcefully pushed out of the body.

With time, the condition leads to the bladder muscles gradually becoming thicker, stronger and oversensitive. Contraction occurs even due to the presence of small amounts of urine, giving rise to frequent needs of urination. At one point, the bladder muscle is unable to overcome the effects of the narrowed urethra. Due to this, urine does not pass properly and the urethra is not emptied.

Some of the common symptoms of enlarged prostate include:
1. Frequent urination
2. Urgency to urinate
3. Difficulty during urination
4. A slow or weak urinary stream
5. Requirement of extra effort to urinate
6. Interrupted sleep due to need of urination

Further risks
Sometimes, when the bladder is not emptied completely, a risk of urinary tract infections develops. Also, make sure that you wash your private parts with warm water infused with a few drops of normal liquid antibacterial, so that you can be sure that the bacteria do not get a chance to thrive and affect your system. - Diet: Your diet should have plenty of fluids and lemons or other citrus food that are rich in vitamin c, as this is a good vitamin to keep bacteria at bay. Also, you should eliminate sweet and refined products to ensure that you do not contract UTI. Other foods to avoid include caffeine and alcohol.

- Clothing: Wear airy and cotton based undergarments that are breathable and have a soft texture. This non abrasive type clothing can also help in keeping the area dry, so that bacteria does not get a chance to develop, as they typically need a damp environment for growth. Change your undergarments twice a day, if need be, and avoid wearing trousers and shorts that are too tight.

The urethra is a thin tube that is responsible for carrying urine from the bladder. The urethra is wide enough to facilitate a free flow of urine. However, in some cases, the tube can narrow down and it can restrict urinary flow. This condition is known as a urethral stricture. It is a condition that affects mostly men.

Causes:
The inflammation of the tubular tissues or the presence of scar tissue is the number one cause for urethral stricture. In some cases, an accident or the injury to the groin area can also lead to the formation of such thin tubes. The other factors can include pelvic fractures, radiation, urinary infection and in some cases even STDs(Sexually Transmitted Disease).

Symptoms:
Most of the symptoms of Urethral stricture are mild. Only in some rare cases, the symptoms can be extreme. These can include weak or restricted urine flow, frequent urges to urinate, and feeling of heaviness in the bladder, a burning sensation during urination, swelling and pain while urinating. Sometimes your urine may discharge volts of blood and the color of urine may change.

Diagnosis:
Diagnosis is by a physical examination and by analyzing the urine samples. In most of the cases, the thickness of the walls is measured by cystoscopy. The rate of the urine passed may also be checked for identifying the symptoms.

Treatment:
Treatment depends on the severity of the condition. Nonsurgical methods aim to improve the width of the urethra. This is done by using an instrument called a dilator. This is done as an outpatient procedure. This procedure is the most common method done and is done for most of the milder cases. Surgery is another option. The surgery aims to remove the heaviness of the urethra and enlarges the size of the flow.

Urine Flow Diversion:
This is done in extremely rare cases if the bladder has to be completely removed. In those cases, hole in the abdomen is done and urine is bypassed to that hole. However, this is an extremely rare scenario.

Prevention:
Try to avoid injuries in the groin area. If you are playing sports or a fitness enthusiast, use proper protection for your abdominal and groin regions. Try to always have safe sex and if you feel that there is some discomfort or some uneasiness while passing urine, do consult with a doctor immediately. In many cases, the condition can be avoided if proper precautions are taken.

What Is Urethral Stricture?

A dialysis diet is a special diet that is customized for patients who are or will be starting dialysis within a short period of time. What can you eat?
Eat foods which are rich in high-quality proteins (poultry, fish, lean meat and egg whites).

What can t you eat?
Eat less of foods which are rich in sodium and phosphorus.

Foods high in Sodium
1. Use minimum salt in preparing the dishes; this helps control BP and probable weight gain that might happen between the dialysis sessions.
2. Make use of spices, herbs and other salt-flavored enhancers.

Meat
1. Patients on dialysis need to include more of high-quality protein foods in their diet. This will help regulate blood protein levels. Eat about 10 ounces of foods rich in high-quality protein every day.
2. 1 egg,   cup of ricotta cheese,   cup of tuna.

Salads/Vegetables

The potassium content of vegetables can be alarming for a patient on dialysis. Eat 3 servings of vegetables that are low in potassium every day. These include broccoli, cauliflower, cabbage, carrots, garlic, cucumber, eggplant, celery, radishes, etc.
Avoid vegetables such as beets, potatoes (in any form- wafers or wedges), winter squash, cooked spinach, pumpkin, asparagus and avocado.
Note:

Although seeds, nuts, peanut butter, lentils, dried beans and peas are rich in protein, they are generally struck off from the diet plan because of their high phosphorus and potassium content.
You can have moderate amounts of grains, bread and cereals.
Try and not have brown bread and other whole grains as they are loaded with phosphorus.
Usually, 5-10 servings of these are considered sufficient for patients on dialysis.
Also, limit intake of milk and other dairy products.
  cup yoghurt or   a cup of milk is considered safe. Also, remember that all varieties of milk- skim, whole and low fat, have the same amount of phosphorus.
All fruits contain some amount of potassium. However, some of them contain more potassium than the others. Limit your intake of fruit juices.
Avoid or limit fruits like: 
Orange juice and oranges
Kiwis
Prune juice and prunes
Dried fruit and raisins
Melons
Bananas

Hematuria is the medical name of what is commonly known as bloody urine. Treatment of stones depends on their size and severity. For smaller stones, doctors recommend drinking 2-3 litres of water per day that help in flushing out the stone through urine. Doctors also prescribe medicines which helps break down the stone and reduce pain.

If the stones are large in size and exhibit severe complications, then the urologist will resort to the following treatment procedures-

  Shock Wave Lithotripsy (SWL)   In this process, a machine called lithotripter is used which passes ultrasonic waves through the skin to break up the stone into smaller components. The shattered pieces then pass out of the body through the urinary tract.

  Percutaneous Nephrolithotomy (PNL)   This is an endoscopic method used to remove large stones, which are not possible to eliminate through SWL. This is an invasive procedure and general anaesthesia is done prior to surgery.

  Ureteroscopy   In this process, a ureteroscope is inserted into the urethra to locate the stone and a tool is used to break it into smaller pieces or is taken out as a whole.

Prevention of Kidney and Ureteral Stones

Certain lifestyle changes can go a long way in preventing stones in the body. Some of them include drinking lots of healthy fluids, reducing consumption of oxalate-rich foods and animal protein, reducing sodium intake and controlling one s body weight.
