Seasonal changes can be very exciting for most people. Each season brings with it a new set of colours and nature takes a new turn. However, for people with seasonal allergies, a seasonal change also comes with a set of allergies. From skin rashes to wheezing to breathing difficulties, the list of symptoms can be quite long. Asthma is the most common problem, and the attacks can be quite bothersome with wheezing attacks and breathing troubles.

Asthma is an allergic reaction caused by narrowing of the airway with thick mucus, which makes breathing difficult. During an attack, the person can have a dry cough, face shortness of breath and wheezing.

Asthma is often triggered or worsened by some factors, and being aware of these can help prevent or manage an attack:

Common allergens like pollen, mites, ticks, furs of animals, bird feathers, specific foods (peanuts, milk products, eggs, etc.) and mold spores appear during change of season.
Environmental chemicals like cigarette smoke, car fumes, dust, etc., can also trigger an attack.
Attacks of cold or flu can lead to an asthma attack.
Workout during cold weather can be problematic, but exercise can also help control asthma attacks. Lung function improves sufficiently, but an exercise regime should be drawn up for the patient based on his condition.
Stressful emotions like anger and anxiety can also lead to an attack or make it worse.
Some medications like aspirin, beta blockers, glaucoma drops can aggravate attacks of asthma.
Once you know the triggers, here are some common measures that can help prevent and relieve the wheezing.

The preventers reduce the inflammation in the airway tube and the swelling. While they do not provide immediate relief during an attack, using these in the long term helps avoid attacks. These are low-dose inhaled corticosteroids such as beclomethasone, fluticasone, and budesonide. Doctors would advise their usage even when there are no symptoms, as asthma attacks can be triggered when they are not taking these medications for a while. Newer drugs include leukotriene antagonists like montelukast and zafirlukast. Relievers are used for symptom relief and include Salbutamol (short acting) and Salmeterol/Formoterol (long acting). Peak flow meter may be useful in acute attacks, wherein the peak flow rates can be reduced.
A good strategy is to use preventers regularly and rely on relievers during an attack. The first one helps build resistance and so reduces the incidence of attacks. Reach out to a doctor if a severe attack ensues (lasts more than 3 hours).

Tuberculosis is a respiratory disease that is very infectious and usually affects the lungs. It is one of the deadliest diseases that have killed millions of people worldwide. According to WHO, 9 million people get infected with tuberculosis in a year. It is one of the top reasons for death among women aged between 15 to 44 years. Symptoms can range from cough, fever, weight loss and night sweats. Moreover, it is an airborne disease meaning that people having tuberculosis can infect many others around them through coughing, sneezing or sharing a glass or cup.

The main cause of tuberculosis is a bacterium called Mycobacterium tuberculosis. However, there are two types of tuberculosis-

Latent TB- In this condition, the bacteria causing TB is present in the body and is inactive. It is not contagious or results in any symptoms.
Active TB- The bacteria causing TB is active and the person carrying it can infect others as well.
Diagnosis
The most common diagnostic test for tuberculosis is a skin test where an injection of PPD tuberculin which is an extract of the TB bacterium is injected into the skin. After 2 to 3 days, the doctor checks the injected site and if there is red swelling, it means that person has TB. Along with this the doctor also uses a stethoscope to gauge the condition of lungs. However, the skin test is not very accurate to know whether a person has TB or not.

Therefore, doctors also use blood tests, x-rays and sputum tests to find out whether a person has TB or not.

Treatments
Tuberculosis can be cured with medications. However, the correct medicines will depend on the type of TB you have and the period of treatment. If you have latent TB, then taking a single TB drug may be enough. But if you have active TB then you need to have proper medicines prescribed by a doctor for at least 6 months. Some common medicines used to treat TB (active) are:

Isoniazid
Rifampin (Rifadin, Rimactane)
Ethambutol (Myambutol)
Pyrazinamide
Also, antibiotics and injections are used to treat drug-resistant TB that may take longer to cure.
When taking medications or any sort of drugs for TB, one needs to be careful and look out for any side effects that can occur due to TB medications. As medicines taken for TB can cause:

Dark Urine
Loss of Appetite
Nausea or Vomiting
Jaundice

Treatments For Tuberculosis - What All Should You Know

When blood clot blocks one or more arteries in your lungs the condition is known as pulmonary embolism. Pulmonary embolism requires various lab tests and generally requires short time to resolve.

Symptoms:
The symptoms of pulmonary embolism mainly depend upon many factors pertaining to your previous medical history. Pulmonary embolism varies with the size of your lungs, the size of the clots formed in your blood, and any other underlying history of the heart or lungs. That said, pulmonary embolism has symptoms which are not similar to any two persons and will vary accordingly.

2. CT plumonary angiography is gold standard for diagnosing pulmonary embolism.
3. Chest x ray, which is a common and non invasive test to check your heart and lungs as well as the problems with any bones surrounding them.
4. ECHO can detect secondary changes in heart due to pulmonary embolism.

Treatment of pulmonary embolism can be done with medicines if it is detected at an early stage or also if the clot is small. However, if the size of your clot is problematic and blocks the blood flow to your lungs or heart then surgery may be necessary. Open surgery and clot removal surgery are common in the treatment of pulmonary embolism and will require good regular follow up check ups. Regular leg exercises will also reduce the chances of any underlying disease and you should hence consult your doctor.

Lung Cancer: This is a fatal disease which can be found in any part of the lungs. It has many forms and the type as well as the location usually determines the kind of treatment option that will be followed.
Acute Respiratory Distress Syndrome: When the lungs suffer a sudden injury which is usually caused by a bout of serious illness, it is called ARDS. Treatment involves the use of life support ventilation.

Why is Cancer the biggest Threat to the YOUNG in India?

There are fewer Indians who have successfully battled Cancer and have emerged successful. Yuvraj Singh, the legendary cricketer, who was diagnosed with a rare cancer at 29, survived due to an early detection. Cancer cases in India are not as many as they are in the developed world. However, due to late detection the survival rate in the country is far worse. Particularly, we are losing our youth to Cancer. Most tobacco (pan masala) related cancers and child cancers go undetected until very late.

Few cancers like cervical cancer and breast cancer which affect women of any age, are curable, however, due to lack of awareness and timely screening, many Indian women succumb to these cancers. The most common sites for cancer among men are lung, prostate, colon, rectum, stomach and liver while that among women are breast, colon, rectum, lung, cervix and stomach.

In proportion to our population, we lack the right infrastructure nationwide. esp in the northern and north eastern region, there is a dearth of good cancer facilities, where radiation, care or treatment can be given. Not even one third of cancer patients are alive beyond 5 years after their diagnosis in India, and about 71% of cancer deaths occur in patients in their prime productive years   between the ages of 30 to 69 years.

International Childhood Cancer Day - Why Cancer Is A Threat For Young?

Chronic obstructive pulmonary disease is a long-term condition of the lungs where the flow of air to the lungs is restricted (obstructed). Symptoms include cough and breathlessness. The condition is most often caused by smoking and the most important treatment is to stop smoking. Inhalers are commonly used to ease symptoms. Other treatments such as steroids, antibiotics, oxygen, and mucus-thinning (mucolytic) medicines are sometimes prescribed in more severe cases, or during a flare-up (exacerbation) of symptoms.

What causes COPD?
Smoking is by far the biggest risk factor for COPD. The lining of the airways becomes inflamed and damaged by smoking. Cigarette smoking is the biggest risk factor.

Air pollution may cause some cases of COPD, or make the disease worse. Air pollution can be indoor air pollution or outdoor air pollution. Indoor air pollution seems to be involved in more cases of COPD than outdoor air pollution. Indoor air pollution includes fumes from indoor fires or stoves, and dust, fumes or chemicals present in some workplaces. The combined effect of exposure at work to air pollutants and smoking increases the chances of developing COPD.

A small number of people have a hereditary (genetic) risk of COPD due to very rare protein deficiencies that can lead to lung, liver and blood disorders. (The condition is called alpha 1 antitrypsin deficiency. Less than 1 in 100 cases of COPD are due to this.


COPD symptoms-


Cough is usually the first symptom to develop. It is produced with phlegm (sputum). It tends to come and go at first, and then gradually becomes more persistent (chronic). You may think of your cough as a 'smokers cough' in the early stages of the disease. It is when the breathlessness begins that people often become concerned.
Breathlessness (shortness of breath) and wheeze may occur only when you exert yourself at first. For example, when you climb the stairs. These symptoms tend to become gradually worse over the years if you continue to smoke. Difficulty with breathing may eventually become quite distressing.

Sputum - the damaged airways make a lot more mucus than normal. This forms sputum. You tend to cough up a lot of sputum each day.

Chest infections are more common if you have COPD. A sudden worsening of symptoms (such as when you have an infection) is called an exacerbation. Wheezing with cough and breathlessness may become worse than usual if you have a chest infection and you may cough more sputum. Sputum usually turns yellow or green during a chest infection. Chest infections can be caused by germs called bacteria or viruses. 
Other symptoms of COPD can be vaguer. Examples are weight loss, tiredness and ankle swelling.

What is the difference between COPD and asthma?


COPD and asthma cause similar symptoms - in particular difficulty breathing (breathlessness). However, they are completely different diseases. Briefly:

In COPD there is permanent damage to the airways. The narrowed airways are fixed, and so symptoms are persistent (chronic). Treatment to open up the airways is therefore limited.

In asthma, there is inflammation in the airways which makes the muscles in the airways narrow (constrict). This is temporary so the narrowing comes and goes. So, the symptoms tend to come and go, and vary in severity from time to time. Treatment to reduce inflammation and to open up the airways usually works well.

COPD is more likely than asthma to cause an ongoing cough with phlegm (sputum).

Waking at night with breathlessness or wheeze is common in asthma and uncommon in COPD.

COPD is rare before the age of 35, whilst asthma is common in under-35s.

There is more likely to be a history of asthma, allergies, eczema and hay fever (so-called atopy) in people with asthma.

Do I need any tests?

The most common test used in helping to diagnose the condition is called spirometry. This is a breathing test. You will be asked to breathe or blow into a small machine called a spirometer which will measure how well your lungs are working. This test can help make a diagnosis of COPD.


A chest X-Ray show signs of COPD and can be used to help exclude other serious conditions (including lung cancer). Occasionally, a special CT scan of the chest - high-resolution CT - is needed. A blood test to make sure you are not anemic is often helpful.

A pulse oximeter is a device which can be clipped on to your finger. It measures your heart rate (pulse) and the amount of oxygen in your circulation (oxygen saturation). Lower levels than normal tend to be found in people who have COPD, especially if you have a flare-up (exacerbation) of your symptoms.

COPD treatment-

Smoking cessation is the most important step towards treatment.
Inhalers

The main treatments are medications given in devices called inhalers. The medicine within the inhaler is in a powdered form which you breathe in (inhale). The medicines in standard inhalers reach the lungs better if used with a spacer device. Inhalers include one, two or even three types of medicines. They may include:


A bronchodilator agent - this is a medicine which opens up (dilates) the airways. Some work quickly but don't last very long; others are longer-acting.
A steroid - these reduce the inflammation in the airway, thereby reducing the swelling. This, in turn, adds to the space available for air to pass.
A medicine which helps to dry up the secretions produced by the airways.
Home oxygen
This may help some people with severe symptoms or end-stage COPD. It does not help in all cases. Unfortunately, just because you feel breathless with COPD it does not mean that oxygen will help you. Great care has to be taken with oxygen therapy. Too much oxygen can actually be harmful if you have COPD.
Regular follow up in OPD is an essential part of treatment success.
