Gallstones are small stones that form in the gallbladder as a result of the deposits caused by the bile, a type of digestive fluid. Actually, the gallbladder stores bile and releases it into the duodenum (part of the small intestine) for the digestion of fats. However, when the gallbladder is unable to release the bile properly, the bile deposits start building up and manifest in the form of stones. These stones vary in size, ranging from tiny sand-like grains to big golf ball-like stones.

Symptoms of gallstones: Many times, individuals have stones in their gallbladder and yet they do not experience any discomfort. These stones are very small in size and therefore, do not cause trouble. This is why they are called silent stones and for such individuals, treatment is not needed. But if an individual experiences discomfort, it is a signal for treatment. The signals come in the form of specific symptoms such as pain in the upper right side of the abdomen, pain between the shoulder blades, nausea, vomiting, fever, and even jaundice.

Causes of gallstones: Gallstones may result due to common. Excess bilirubin produced by the liver causes hard, dark brown stones called pigment gallstones. Even when the gallbladder cannot empty the bile, stones are formed due to the concentration of bile.

Treatment: Once gallstones are detected through a CT scan or ultrasound, the doctor begins the course of treatment. Gallstones can be removed only through surgery. In surgery, the gallbladder is removed to help the bile flow directly into the duodenum from the liver. This surgery is called cholecystectomy and it is a safe procedure. Such individuals include women over 40, those who lead a sedentary lifestyle, obese/overweight individuals, and those with a family history of gallstones. Therefore, such people should be aware of the risk they face. If such individuals report any symptoms related to gallbladder stones, they should get it diagnosed immediately. Apart from this, one should also keep a check on the diet. A high-fat or high-cholesterol diet should be replaced with a high-fiber diet and one should also take meals on time. Not eating anything for long hours and fasting increases the risk of gallstones. Regular exercise also helps in this matter. Last but not the least, one should drink plenty of water as it regulates the functioning of the liver and prevents the formation of stones. Gallstones - What All Should You Know About It?

Hemorrhoids are masses, clumps, cushions of tissue in the anal canal - they are full of blood vessels, support tissue, muscle and elastic fibers.

Internal hemorrhoids -

They are classified into four grades:

Grade 1 - there are small inflammations, usually inside the lining of the anus. They are not visible.
Grade 2 - larger than Grade 1 hemorrhoids, and also inside the anus. When passing a stool they may get pushed out, but soon return.
Grade 3 - often called 'prolapsed hemorrhoids'; these appear outside the anus. The patient may feel them hanging out. They can be pushed back in if the patient presses with his/her finger.
Grade 4 - these cannot be pushed back in and need to be treated by a doctor. They are large and stay outside the anus all the time.
External hemorrhoids -

called perianal hematoma. These are small lumps that are located on the outside edge of the anus. They are extremely itchy and can be painful if a blood clot forms inside (thrombosed external hemorrhoid). Thrombosed external hemorrhoid requires medical treatment straight away.

What are the signs and symptoms of piles?

A symptom is something the patient feels and describes, such as a pain, while a sign is something everybody can see, such as a rash.

A hard lump may be felt around the anus. It consists of coagulated blood, called a thrombosed external hemorrhoid. This can be extremely painful.
After going to the toilet, a feeling that the bowels are still full.
Bright red blood after a bowel movement.
Itchiness in the anus area.
Mucus discharge when emptying the bowels.
Pain while defecating.
The anus area may be red and sore.

Why do piles occur?

Anal fistula, a challenge for modern surgery, but can be cured with grade ksharsutra in Ayurveda (Indian system of medicine).  Anal fistula is a disease for which different methods of treatment have been advocated time to time, even today when we are talking about robotic surgery, this disease has no satisfactory method of treatment in modern surgery. The latest method of video assisted anal fistula treatment (VAAFT) is not satisfactory due to high incidences of recurrence after the surgery. The patient is routinely investigated with special reference to any other associated disorder like hypertension, diabetes mellitus, tuberculosis etc. The next step lies in the proper identification& evaluation of the fistulous tract. It is mandatory to define the fistula tract wrt its depth, direction & branching pattern. This may be achieved by careful probing under expert hands using malleable probes& if required help may be taken from other investigations like x-ray fistulogram or MRI fistulogram. 

Once the tract has been clearly defined the Ksharsutra is placed in it with the help of certain instruments, under local anesthesia. This entire procedure is usually completed in ten to fifteen mins. As there is no cutting of tissues hence no bleeding, no pain & patients can go home after half an hr rest in the daycare room. The medicine-coated on the thread is gradually released in the tract. The cumulative effect of the coated drugs exerts a powerful debridement effect on the fistula tract & induces healing by the fresh & healthy granulation tissues. Thus Ksharsutra is the mechanism of drug delivery precisely at the tissues involved. 

Every seventh or tenth day the old sutra is replaced with a new Ksharsutra till there is a final cut through. It has been observed that the tract heals @ 0.5-1 cm per week. The post-operative complications like recurrence of the disease, fecal incontinence, bleeding is not seen with Ksharsutra treatment. Further, this therapy is performed as an OPD procedure. No general anesthesia & no hospitalization is required. Patient maintains his normal routine activities as usual during the treatment period.
