In some cases, pain may also be induced by damage to the brain and spinal cord, which happens after a stroke.
A constant barrage of pain signals may cause the cells at the end of nerve fibres to become over-sensitized. This is known as 'wind-up' and is one of the most common causes of chronic pain that occurs even though the root of the problem has been identified and treated.

The causes and effects of pain differ from case to case depending on the signals received by the brain and its interpretation. It affects the individual's day to day activities and if persistent, can also have a harmful impact on mental health and psychosis. Cubital Tunnel Syndrome is also known as Ulnar Neuropathy, a nerve compression syndrome where the Ulnar nerve, also known as the funny bone nerve, gets compressed due to heightened pressure or stretching. It can lead to numbness or a tingling sensation in little and ring fingers, sometimes pain in forearm and an overall weakness in the hand. Ulnar nerve is one of the three main nerves in the arm. It runs in a groove on the inner side of the elbow. Since it travels through a narrow space and has very little protective tissue, the nerve is quite vulnerable to compression.

Symptoms
The symptoms of this condition may range from mild to severe. Some of the mild symptoms include:

Numbness in little and ring finger as the fingers fall asleep
A tingling sensation, usually like the pinch of pins and needles in ring and little finger
Pain in forearm
Weakness in the hand

Wasting of muscles of the hand
Bending elbow over a long period of time like while using cell phone or during sleep can cause ulnar nerve compression. Resting the elbow for a long period over a hard surface can also cause an irritation of the nerve, leading to such symptoms. In some cases, the nerve snaps back and forth over a bony bump, resulting in an irritated nerve. People who undertake intense physical activity, especially using their arms, are more likely to develop this problem. Eg. baseball pitchers. Apart from this, people who have suffered from a dislocated elbow or have arthritis are also at risk.

Generally, doctors diagnose this condition through the symptoms. However, nerve tests are also conducted to check the level of nerve compression. Electromyography is a procedure in which electrodes are placed on the skin and muscles to measure muscle health. Determining muscle health and level of compression helps decide the mode of treatment. Generally, the symptoms of cubital tunnel syndrome are managed through a conservative treatment. However, in cases of severe compression, surgery can be considered as an option to relieve pressure, moving nerve to the front of the elbow or removing a part of the bone.

For mild cases, a towel or a protective cover for elbow is recommended. The towel should be wrapped around the elbow loosely. An elbow splint can be worn at night to protect the elbow from being bent for long time.

How can a physical therapist help?
A physiotherapist has an essential role to play in treatment of this syndrome. A therapist can help the patient to learn ways of avoiding pressure to the nerve. After surgery, with restrictions of movement, a therapist can help achieve smooth recovery and movement of the elbow. Your physical therapist will determine the activities that bring on your symptoms. The recommendations at this point will be to avoid those activities for a time. Remember, the nerve is irritated and at times swollen. If the irritation and swelling are reduced, the symptoms should resolve.

This position should be maintained for 4-5 seconds.
Strengthening the hips: The body should rest on the stomach. The buttocks should be tightened and one leg should be slowly lifted. This position should be maintained for a stretch of 3-5 seconds. The same should be repeated for both the legs.
Strengthening the hip and the stomach muscle: The body should rest on the back with a leg bent. The stomach muscle should be tightened and the bent leg should be lifted carefully while ensuring that the bend does not supersede more than 90 degrees. While keeping the stomach muscle tight, the lifted leg should slowly come down to touch the surface. This act should be repeated for 5-6 times.
Strengthening the thigh muscle: While sitting in a steady chair, an elastic loop should be wrapped around the ankles. The elastic loop should be stretched with one leg while keeping the other foot firmly rooted to the ground. The lifted foot should slowly come down to the normal position. The same act should be repeated for 4-5 times.
Improving balance: An elastic loop should be tied to one leg of a table and the other end of the loop should be tied to one ankle. The table should be held on for maintaining balance. Maintaining an erect position, the tied up ankle should be stretched sideways. The position should be maintained for 3-5 seconds to return back to normal standing position.

Most of the arthritic conditions don t have any cures. Ankylosing Spondylitis is also a type of arthritis that does not have a cure. It is a kind of arthritis of the spine; it affects the vertebrae. The cause of this disorder is not yet known, yet a genetic link cannot be ruled out. It has been observed that most of the people affected with Ankylosing Spondylitis carry a specific gene in them. People carrying this gene are susceptible to spondylitis. However, there are about 10% people who carry this gene, but don t have any sign of Ankylosing Spondylitis. So, a sure determinant of cause of this disease is not completely known. 

Diagnosis of Ankylosing Spondylitis 

To diagnosis Ankylosing Spondylitis, the physician has to look at several factors. 

These include: 

  Symptoms exhibited by the patient 

  Physical Findings 

  X-Rays of pelvis and back regions 

  Taking chest measurements while taking deep breaths 

  Laboratory tests Based upon the complete diagnosis, physician can positively determine if the condition is indeed Ankylosing Spondylitis. 

Treatment 

While a cure for Ankylosing Spondylitis is still not on the horizon, there are some treatments which can reduce pain and discomfort and thereby improve daily activities. Goals of treatment Since the disease can t be cured, the goal is to reduce pain and stiffness, prevent deformity, maintain a good posture, and help perform daily activities. In fact, if treatment is proper, the patient may lead to a semblance of a normal life. However, it calls for a team approach. The team must include a physical therapist and an occupational therapist, apart from a rheumatologist. If the patient is suffering from bony growth, the doctor may go for an osteotomy.
