Pulmonary Function Tests or PFTs are a set of tests that are used to determine, measure or observe the functioning of the lungs. The most common pulmonary function tests used are spirometry, gas diffusion, and lung plethysmography. 

Highlights of pulmonary function tests

A spirometry test is used to measure the amount of air inhaled and exhaled. It also measures the amount of time taken to complete a respiration cycle.
A lung plethysmography measures the amount of air in the lungs after the patient has taken a deep breath. It also measures the amount of air that remains in the lungs after the patient has exhaled as much air as possible.
What does a Pulmonary Function Test measure?
Pulmonary function tests would not only check how well the person is breathing but also how effectively the lungs are able to conduct oxygen to the rest of the body. In addition to air flow, these tests would measure the size and volume of the lung and lung diffusion.

These tests can also be used to measure

Total Lung Capacity   the amount of air present in the lungs after breathing in deeply
Forced Vital Capacity   the amount of air that can be exhaled after breathing in deeply
Minute Volume   the amount of air exhaled per minute
Vital Capacity   the total volume of air that can be breathed out after inhaling as much as possible
Tidal Volume   the amount of air inhaled or exhaled when breathing normally
Maximum voluntary ventilation   the amount of air that can be inhaled and exhaled in one minute
Functional Residual Capacity   the amount of air that remains in the lungs when breathing normally
Forced Expiratory Flow   the flow or volume of air during an exhalation
Peak Expiratory Flow Rate   the rate at which a person can exhale
When are Pulmonary Function Tests recommended?
A physician might recommend a patient undergo a PFT as part of a routine physical check-up. This test would be advised if the patient is experiencing any symptoms associated with lung problems or disorders such as asthma, respiratory infections, chronic obstructive pulmonary disease, COPD. A doctor would also use these tests to monitor and check whether a certain treatment prescribed for a lung disease or condition is working effectively.

Pulmonary function tests are safe as they are non-invasive but it is important to follow the instructions of the doctor or technician before, after and during the test. It is important to make your doctor aware of any surgeries, respiratory infections, chest pains or heart attacks that you might have experienced before the test. If you are taking any medications you might need to follow the advice of the doctor as to whether you can take them before the test.

Pulmonary Function Tests - Why Is It Required?

The sugar found in milk and other dairy products such as yoghurt and cheese is called lactose. An enzyme in the small intestine called 'lactase' breaks lactose into galactose and glucose. When the lactose is not digested properly by the small intestine, it passes unbroken into the colon. The bacteria in the colon break the lactose down into hydrogen and carbon dioxide. People who fail to digest lactose in their small intestines are diagnosed with lactose intolerance.

Usually lactose intolerance only affects adults, but sometimes children do suffer from it. For children, their symptoms can be mild or very severe (for example, cramps, diarrhoea, etc.)
Normally, parents can confuse milk allergy with lactose intolerance. They do share the same symptoms, but they are completely different medical conditions. Milk allergy is caused by the immune system while lactose intolerance is a problem with the digestive system.

The causes of lactose intolerance include:
1. Absence or deficiency of the lactase enzyme
2. Infections in the gastrointestinal tract that damage the lining inside the small intestine
3. Gluten intolerance (It affects lactase production)
4. Genetics (Lactose intolerance can run in the family)

When the lactose is broken down in the colon, certain gases are produced. These gases can cause symptoms such as:
1. Abdominal inflammation and pain
2. Excessive burping
3. Loud sounds in the bowel
4. Excessive diarrhoea and gas
5. Explosive and watery bowel movements
6. A feeling of urgency when it comes to bowel movements (In children, they might feel like they need to get to the bathroom as soon as possible or they might embarrass themselves)

How best to treat it?
Living with lactose intolerance usually involves dietary modifications and taking supplements such as over-the-counter lactase to aid in digestion. For instance, you can replace milk with soy milk, or take lactase before ingesting any dairy product. Or alternatively, you can consume lactose-reduced or lactose-free milk and dairy products.
Calcium deficiency is a serious side effect of lactose intolerance. So incorporate calcium-rich foods such as broccoli, kale, tofu, almond, dried fruits, soybeans, turnip greens, collard greens and fortified orange juice in your diet.

How to Treat Lactose Intolerance in Children

The inflammation of the cavities around nasal passage is known as sinusitis. When the cavities are full of fluid, germs attach themselves to the fluids, which give rise to infections. Some of the causes of sinusitis are common colds, allergic rhinitis, nasal polyps and deviated septum.

There are 4 different types of sinusitis:

    Acute sinusitis: It remains for about 4 weeks or less. It is usually characterized by a runny nose and pain in the face which will stay for at least 10 to 14 days.
    Sub acute sinusitis: It is almost like the acute sinusitis but stays longer for about 4 to 8 weeks.
    Chronic sinusitis: The inflammation in this case lasts about 8 weeks, sometimes longer than that.

Chronic obstructive pulmonary disease is a long-term condition of the lungs where the flow of air to the lungs is restricted (obstructed). Symptoms include cough and breathlessness. What causes COPD?

COPD symptoms-


Cough is usually the first symptom to develop. It is produced with phlegm (sputum). It tends to come and go at first, and then gradually becomes more persistent (chronic). You may think of your cough as a 'smokers cough' in the early stages of the disease. It is when the breathlessness begins that people often become concerned.
Breathlessness (shortness of breath) and wheeze may occur only when you exert yourself at first. For example, when you climb the stairs. These symptoms tend to become gradually worse over the years if you continue to smoke. Difficulty with breathing may eventually become quite distressing.

Sputum - the damaged airways make a lot more mucus than normal. This forms sputum. You tend to cough up a lot of sputum each day.

Chest infections are more common if you have COPD. A sudden worsening of symptoms (such as when you have an infection) is called an exacerbation. Wheezing with cough and breathlessness may become worse than usual if you have a chest infection and you may cough more sputum. Sputum usually turns yellow or green during a chest infection. Chest infections can be caused by germs called bacteria or viruses. 
Other symptoms of COPD can be vaguer. Examples are weight loss, tiredness and ankle swelling.

What is the difference between COPD and asthma?


COPD and asthma cause similar symptoms - in particular difficulty breathing (breathlessness). However, they are completely different diseases. Briefly:

In COPD there is permanent damage to the airways. The narrowed airways are fixed, and so symptoms are persistent (chronic). Treatment to open up the airways is therefore limited.

In asthma, there is inflammation in the airways which makes the muscles in the airways narrow (constrict). This is temporary so the narrowing comes and goes. So, the symptoms tend to come and go, and vary in severity from time to time. Treatment to reduce inflammation and to open up the airways usually works well.

COPD is more likely than asthma to cause an ongoing cough with phlegm (sputum).

Waking at night with breathlessness or wheeze is common in asthma and uncommon in COPD.

COPD is rare before the age of 35, whilst asthma is common in under-35s.

There is more likely to be a history of asthma, allergies, eczema and hay fever (so-called atopy) in people with asthma.

Do I need any tests?

The most common test used in helping to diagnose the condition is called spirometry. This is a breathing test. You will be asked to breathe or blow into a small machine called a spirometer which will measure how well your lungs are working. This test can help make a diagnosis of COPD.


A chest X-Ray show signs of COPD and can be used to help exclude other serious conditions (including lung cancer). Occasionally, a special CT scan of the chest - high-resolution CT - is needed. A blood test to make sure you are not anemic is often helpful.

A pulse oximeter is a device which can be clipped on to your finger. It measures your heart rate (pulse) and the amount of oxygen in your circulation (oxygen saturation). Lower levels than normal tend to be found in people who have COPD, especially if you have a flare-up (exacerbation) of your symptoms.

COPD treatment-

Smoking cessation is the most important step towards treatment.
Inhalers

The main treatments are medications given in devices called inhalers. The medicine within the inhaler is in a powdered form which you breathe in (inhale). The medicines in standard inhalers reach the lungs better if used with a spacer device. Inhalers include one, two or even three types of medicines. They may include:


A bronchodilator agent - this is a medicine which opens up (dilates) the airways. Some work quickly but don't last very long; others are longer-acting.
A steroid - these reduce the inflammation in the airway, thereby reducing the swelling. This, in turn, adds to the space available for air to pass.
A medicine which helps to dry up the secretions produced by the airways.
Home oxygen
This may help some people with severe symptoms or end-stage COPD. It does not help in all cases. Unfortunately, just because you feel breathless with COPD it does not mean that oxygen will help you. Great care has to be taken with oxygen therapy. Too much oxygen can actually be harmful if you have COPD.
Regular follow up in OPD is an essential part of treatment success.
