How to use heat and cold therapy?
The sooner heat and cold therapy is given to a person, the faster they will recover from their pains. Heat therapy can be given to a person for extended periods of time but when giving cold therapy, a person should not be exposed to a cold pack or ice bath for more than 10-15 minutes. Care should also be taken no to apply ice or a frozen pack directly on the skin as it can damage the skin and nerves.

When not to use Heat Therapy?
If the injury site is swollen or bruised, cold therapy is better than heat therapy. Heat therapy should also not be applied to an open wound. People suffering from conditions such as diabetes, dermatitis, deep vein thrombosis, vascular diseases and multiple sclerosis should also avoid heat therapy as these conditions can increase the risk of burns and other such complications. As per the data by American Academy of Orthopaedic Surgeons (AAOS), the most effective way to treat muscle sprains is by following the Rice method. The word rice stands for- rest, ice, compression, and elevation. The patient must take adequate amount of rest and not undertake physical activities. He or she must give proper time for healing of the injury or wound, prior to resuming back regular activities. If the sprain is chronic, take total bed rest. It is often advised to apply ice compression to the affected area for about 8 to 10 times a day. Try to compress your muscle by putting mild pressure to avoid swelling or any kind of inflammation. For this, it is best to tie up a bandage around the affected part. It is generally advised to elevate the injured part above the heart to avoid or reduce the swelling of the area. Sometimes, physical therapy is also needed to treat the sprains.

Risks of strain treatment
There are certain risk factors associated with muscle strain treatment. It is bad to take too many painkillers as they can affect other organs like kidneys, gut and heart. You should also avoid applying the ice pack directly on the skin for a prolonged period. Also, if you undertake a surgery to fix muscle strain, you are at the risk of suffering bleeding, developing blood clots, infection, pain and stiffness.

Causes

There are three types of deltoid strains

Grade 2 deltoid strain: This strain is a result of a major injury to the deltoid muscle; there may be visible swelling and pain while carrying out activities that require movement of the shoulder. Lifting your arm to the front or the sides becomes difficult and painful.
Grade 3 deltoid strain: This form of deltoid strain is the most severe of the three; it may cause the shoulders to become immobile. You usually will experience excessive pain and severe swelling. You might not be able to move your arm and carry out any activity that requires the shoulders.
Diagnosis
The diagnosis of a deltoid strain starts with a physical exam where the doctor will understand the symptoms of the deltoid strain. Various imaging tests such as x-rays may be required for the diagnosis. The basic treatment for shoulder strain comprises of the RICE protocol (rest, ice, compression and elevation) which is used to treat grade 1 and grade 2 strains. Grade 3 strains may require surgery in addition to the above treatment methods. In case of excess pain, the doctor may prescribe non-steroidal anti-inflammatory drugs to ease the pain.
