A kidney transplant is a surgical procedure where your kidneys are cleansed and rebuild using the kidneys of a deceased person, in order to flush out the excessive waste fluids from the kidneys. It is of paramount importance and your dietary patterns and lifestyle should be altered after you go through a kidney transplant. Your dietician will help you out by preparing your diet chart to clearly list out the foods that you should consume and those which you should avoid after the surgery. The diet chart that would be prepared for you would be highly subjective and it would totally depend  on your lifestyle, medical history, family health records, genetically transmitted conditions and such others. When do kidneys need dialysis?

Kidneys are a pair of organs that are located on either side of the spine, each about a size of a fist. The kidneys help in purifying blood by removing toxins, waste materials and excess fluids from the human body. Disorders and dysfunctions of the kidney can lead to severe and often fatal consequences. When the kidneys stop functioning as they are supposed to, dialysis is performed to resume normal functioning in the body.

Dialysis is a type of treatment, which filters and purifies the blood with the aid of a machine. It is an artificial way of purifying blood. There are two types of dialysis, namely hemodialysis and peritoneal dialysis. Hemodialysis, the most common form of dialysis, which involves using a manmade kidney called a hemodialyzer, which removes toxins and waste materials from blood.

Peritoneal Dialysis, on the other hand, is a type of treatment which involves implanting a catheter in the stomach. During the procedure, a fluid called dialysate flows into the abdomen that absorbs all the waste material, which is consequently drained out of the body.

You may need a dialysis if:

- Your kidneys are dysfunctional
- When waste materials and toxins start to accumulate in the body
- In an event of an injury or accident to the kidney such as internal lacerations (wound caused by the tearing of a tissue)
- If the creatinine (a type of a chemical waste product) level falls to 10-12 cc/minute
- In an event where the kidneys aren't able to work properly leading to accumulation of toxins, irregular amounts of chemicals in the body and other dysfunctions

Weakening of the abdominal muscles and weight gain are some of the risks involved in dialysis. Dialysis is a temporary treatment and serves to function till the time the actual kidneys get repaired. In chronic cases of kidney disease, kidney transplant may perhaps be the last resort.



When Do Kidneys Need Dialysis?

Glomerulonephritis is a disease that is caused due to inflammation of the small filters that are present within the kidneys or glomeruli. Glomeruli eliminate the excess waste, electrolytes and fluids from the blood, discharged through urine. A glomerular disease can be either acute or chronic. If the condition arises without a combination of any other disease, it is termed as primary glomerulonephritis. Secondary glomerulonephritis is characterized by diabetes or lupus (an auto-immune disorder) being at the root of the disease. Prolonged or severe inflammation can take a toll on the kidneys.


Causes-


Nephrotic syndrome can be primary, being a disease specific to the kidneys, or it can be secondary, being a renal manifestation of a systemic general illness. In all cases, injury to glomeruli is an essential feature. Kidney diseases that affect tubules and interstitium, such as interstitial nephritis, will not cause nephrotic syndrome.


Primary causes of nephrotic syndrome include the following, in approximate order of frequency:
Minimal -change nephropathy
Focal glomerulosclerosis
Membranous nephropathy
Hereditary nephropathies


Secondary causes include the following, again in order of approximate frequency:
Diabetes mellitus
Lupus erythematosus
Viral infections (e.g., hepatitis B, hepatitis C, human immunodeficiency virus [HIV] )
Amyloidosis and paraproteinemias
Preeclampsia
Allo-antibodies from enzyme replacement therapy


Nephrotic- range proteinuria may occur in other kidney diseases, such as IgA nephropathy. In that common glomerular disease, one third of patients may have nephrotic- range proteinuria.


Nephrotic syndrome may occur in persons with sickle cell disease and evolve to renal failure. From a therapeutic perspective, nephrotic syndrome may be classified as steroid sensitive, steroid resistant, steroid dependent, or frequently relapsing.


Treatment-
Whether one is suffering from acute or chronic nephrotic glomerular , with symptoms moderate or severe, the treatment lies in treating high blood pressure and other underlying conditions of the disease.
Water pills help control sweating, thus increasing the fluid flushed from the kidneys.
Statins reduce cholesterol level.
Blood thinners such as anticoagulants lower the risk of blood clots.
Corticosteroids regulate the immune system and ease the inflammation that results from kidney disorders.
Incorporating certain lifestyle changes such as opting for lean proteins, cutting on the intake of fat in the diet and consuming lesser amounts of salt can treat inflammation and swelling.
For kidney failure, dialysis can come to the aid in eliminating excess fluids and regulating hypertension.


Nephrotic Syndrome: Causes and Treatment

The kidneys may be a pair of small organs but they perform a very important function. Without the kidneys, it is impossible to filter blood and remove toxins from the body. One of the common problems associated with kidneys is the development of a kidney stone. This can be described as a hard, pebble like substance that is formed when urine contains high levels of certain minerals. Kidney stones vary in size and shape. Small stones may pass through the ureter into the bladder and out of the body with minimal discomfort. However, in some cases, a kidney stone can be as large as a golf ball. In such cases, it can block the urine flow and cause extreme amounts of pain as well as bleeding. In such cases, a doctor needs to be consulted as early as possible.

There are four different types of kidney stones.

Calcium Stones: This is the most common type of kidney stones. It is caused by excess calcium that is not flushed out with the urine.
Uric acid stones: Highly acidic urine can trigger the development of such stones. This may be caused by excessive consumption of meat, fish and shell fish.
Struvite stones: This is often formed as a side effect of Urinary tract infections. These stones may develop and grow rapidly.
Cystine stones: These stones are formed as a result of a genetic disorder known as cystinuria. This disorder causes an amino acid known as cysteine to leak into the urine.
Kidney stones are a fairly common occurrence. It affects both men and women but men have a higher risk of developing kidney stones as compared to women. Other risk factors that can influence kidney stones are:

Family history of kidney stones
Not drinking enough water
Obesity
Digestive problems
Recurrent UTIs
Gout
Bowel inflammation
Disease that causes blockage of the urinary tract
Certain medications such as diuretics or calcium based antacids
Once a person has been diagnosed with a kidney stone, this issue can recur. Treatment for kidney stones depends on the type of stones, size and location. In most cases, medication is prescribed to help break the stone down into smaller parts so that it can pass through urine. In extreme cases, surgery may be required. If not treated in time, it can cause severe pain, bloody urine, UTIs that can further lead to kidney failure and reduced kidney functioning.
Kidney stones can be prevented by drinking plenty of fluids and making a few dietary changes. Ideally, a person should have a minimum of 8-10 glasses of water a day. If you have a high risk of developing any type of kidney stones, reduce your sodium consumption and the amount of meat eaten.

Diabetic nephropathy (Diabetic Kidney Disease) is defined as the progressive damage to the kidneys caused by diabetes. It is characterised by the scarring of the glomeruli in the kidneys due to prolonged diabetes mellitus.

What Causes Diabetic Nephropathy?

Diabetes is a condition characterized by high concentration of blood sugar. Although the cause of Diabetes Nephropathy is not yet well defined, it is likely that the high sugar levels in the blood damage the blood vessels present in the kidneys that help to filter waste products, thus resulting in decreased kidney function and ultimately, kidney failure.

Not everyone with diabetes suffers from diabetic nephropathy. Although, factors that can increase your chances of getting diabetic neuropathy are-

Smoking

Poor control of blood glucose

High blood pressure

What are the symptoms of diabetic nephropathy?

In the early stages, there may be no symptoms at all, but as the disease progresses, you may notice swelling in the extremities like your feet. Thus it is recommended to get your blood albumin levels tested on a regular basis. If it is diagnosed in early stages, the damage can be reversed. Other symptoms that can indicate damage to the kidneys are-

Severe malaise

Headaches

Nausea

Lack of appetite

Frequent voiding

Itchiness of skin

General feeling of illness

Diet: Reduce the dietary intake of salt, phosphorous and potassium in the advanced stages.

Glycemic Control: Monitor your blood glucose levels regularly

Management of Hypertension: It is important to manage your blood pressure as it can have a direct effect on the coronary blood vessels which in turn pump blood to the various blood vessels of the body

Renal Replacement Therapy: In chronically ill patients, renal replacement therapy is the best option. This can include haemodialysis (a procedure to flush out toxins from the blood, such as urea), peritoneal dialysis or ultimately renal transplantation.

The 4 types of kidney stones and their common symptoms

Defined as hard and small mineral deposits that develop inside your kidney, kidney stones are made up of acid salts and minerals. The causes of this disease are not well-defined, although risk factors include drinking very little amounts of water and having a diet rich in sodium, protein and oxalate (chocolate and green leafy foods for example). However, knowing the type of kidney stone you suffer from can help ascertain the cause.

Here are the common types of kidney stones.

Struvite stones

This type of kidney stone is usually a response to certain kind of infection, such as a urinary tract infection for example. Characterised by rapid growth and a large size, struvite stones can develop without any warning signs.

Calcium stones

These are the most common type and are caused by a diet rich in oxalate. Cystine stones

Those individuals with a hereditary disorder that causes their kidneys to produce certain amino acids (cystinuria to be precise) in excess are most likely to get cystine stones.

Uric acid stones

These are common in those whose diets are lacking in water or those who suffer from the excess fluid loss. People with a high protein diet and those who suffer from gout are also at risk. Genetic factors play a major role as well in increasing your risk of getting uric acid stones.

Apart from these, there are other rarer types of kidney stones that can also arise depending on a combination of an individual's lifestyle and genetic make-up.

  excruciating pain in your sides, and back, especially below your ribs

  feelings of pain in the groin and the lower parts of your abdomen

  intense and fluctuating pain that comes and goes in waves

  vomiting and nausea

  constant urge to urinate

  increased frequency of urination

  urinating in small amounts
