Cholesterol is a waxy, fat-like substance that’s found in all cells of the body. The term cholesterol is often directly related to a heart condition. But the truth is that not all types of cholesterols are harmful for your body. It is important that one knows about the different types of cholesterol, having healthy levels of both types of lipoproteins is important, deal with them appropriately and lower the chances of having a stroke or getting a heart disease. Bad cholesterol levels can be high in people irrespective of them being thin, if it is genetic or if they are smokers.

Types of Cholesterol.

The Bad One:

LDL cholesterol sometimes is called “bad” cholesterol. A high LDL level leads to a buildup of cholesterol in your arteries. Lowering your cholesterol may slow down, reduce, or even stop the buildup of plaque in your arteries. It also may reduce the risk of plaque rupturing and causing dangerous blood clots.

The Good One:

HDL cholesterol sometimes is called “good” cholesterol. This is because it carries cholesterol from other parts of your body back to your liver. Your liver removes the cholesterol from your body and  is important for the body as it reduces the risk of heart attack, disease and strokes.

Cholesterol count of the body is counted by adding the two types of cholesterols (LDL and  HDL) along with body's triglyceride level. Triglycerides are a type of fat found in your blood. Too much of this type of fat may raise the risk of coronary artery disease.

The level of your cholesterol shows the level of risk your body is in, to have a stroke or a heart condition. Other factors which determine the risk are whether the person is a smoker, diabetic, has high blood pressure along with their age, sex and race.

Cholesterol levels are affected by:

1. Genetics: Genetics have a direct effect on your cholesterol level. If the levels have been high in the previous generations, you are automatically at higher risk of this condition.An inherited condition called familial hypercholesterolemia causes very high LDL cholesterol.

2. Age and Weight: Once we cross the age of 20, our cholesterol level starts to rise. Being overweight however is also a high risk factor of increased levels of bad cholesterol in the body as well as developing heart disease.

3. Lifestyle: Less or no exercise is another reason which reduces the good cholesterol in the body, as does being overweight. Regular exercise however reduces LDL and increases HDL in the body. Also a diet rich in saturated fats makes your LDL levels shoot up and puts you to higher risk.

 Know Your Cholesterol: Good Vs Bad

The staging is as follows:

Stage 1: Mild emphysema (FEV1 greater than 80% of normal)
Stage 2: Moderate emphysema (FEV less than 80%, but more than 50% normal)
Stage 3: Severe emphysema (FEV lesser than 50%, but more than 30% normal)
Stage 4: Extremely severe emphysema. (FEV lesser than 30% of normal or lesser than 50% normal, with low levels of oxygen in the blood.
The BODE emphysema staging system
The condition affects several abilities. The BODE index, an acronym for BMI (body mass index), obstruction, dyspnea (breathlessness), exercise capacity (health index) is another staging system for emphysema in which the emphysema s impact on several areas of life are measured. They include the following:

Body mass index (B)
Airflow limitation or obstruction (O), which is measured by pulmonary function tests
Breathless or dyspnea (D), which is assessed thoroughly via a questionnaire
Exercise capacity (E), which is measured to check how long a person with emphysema can walk in a span of six minutes
Life expectancy
The prognosis of emphysema is impossible to be determined in any person individually. The staging system helps to identify the severity of the condition, but the future cannot be predicted. Not many studies have been undertaken for the determination of the effects of emphysema on life expectancy. The best research that has been carried out included around 100 people, which is very less. The condition varies largely among people, even if they are in the same stage.

The statistics available on emphysema are not reliable enough for individuals who wish to know their life expectancy. It is considered that if a patient falls in a higher stage, his/her long-term life expectancy is less.

Peripheral Artery Disease (PAD) is caused by atherosclerosis or plaque build-up in the leg arteries. Mostly, individuals with PAD have mild or no symptoms. But with the extension of the blockages, individuals may experience disability and pain that restricts free walking. In more severe cases, individuals may develop a risk of losing their limb if the circulation is not improved.

Stenting and Angioplasty are invasive treatment methods. The traditional method includes bypass surgery. The best available treatment method for an individual depends upon various factors such as severity, location of the artery blockage in the leg, general health of the patient, history of any vascular problems, and previous treatments.

Surgical bypass in the leg or Lower Extremity Bypass Surgery is usually the last option for the patients with PAD. This method is used when all the other alternative treatment methods fail or are not suitable for the individual. During the Leg Bypass Surgery, the surgeon forms an alternative channel for blood flow to bypass or avoid the blocked area and restore the direct flow of blood to the lower leg and foot. 

The risks and benefits of the surgery are calculated prior to the surgery. For the best result of the surgery, surgeons try to use the patient s own saphenous vein, from inside the leg, to execute these grafts. Surgeons may also use veins from other leg or arms or use prosthetic or artificial graft, in case the required length of the saphenous vein cannot be achieved. 

The results of artificial and saphenous vein from other limbs are not as effective as the saphenous vein from the same leg. Thus, the availability of an appropriate vein to execute this method is an important consideration that is taken into account before the surgery.

Arrhythmia is a condition which causes abnormal rhythm of heartbeat in the patients. It is a life-threatening condition. The device is located in the chest to correct the rhythm of a heartbeat.

AICD device works in two ways:

Firstly, it tries to correct the abnormal heartbeat rhythm and release high- speed heartbeat pace. This is called cardioversion. Patients with AICD generally do not feel the process.

However, if the effect of cardioversion is null then the device will give a mild electric shock to the patient's heart to correct arrhythmia and reset the rhythm.

Preparation for AICD Implantation

The patient is required to fast six hours before undergoing the surgery. On admission, prescribed medicine is given to the patient and the latest heart condition is measured by an ECG test. If the patient is on blood-thinning medication, then it will be stopped 24 hours before the surgery.

The procedure will be carried out by a cardiologist. The patient will remain under observation in the hospital for at least two days.

Procedure

The entire process of setting up the device in the chest of the patient takes about 2 hours. The patient is sedated, and the area below the collarbone is numbed for the operation. A small incision is made in the chest area. With the help of imaging techniques, cardiologist sets up the device in the chest. The device is then connected to the veins and the heart. After the surgery, the patient is kept under observation. Heart rate, blood pressure, and overall health condition are observed by the doctors to determine successful implantation of Automatic Implantable Cardioverter Defibrillator (AICD). Post Procedure After the successful recovery of the patient, doctors usually discharge the patient with AICD attached. The doctors give complete care instructions of the device along with health.
