Skin cancer is a condition with abnormal and cancerous skin growth. This often develops due to the over exposure of skin to the sun rays. The three main types of skin cancer include basal cell carcinoma, squamous cell carcinoma and melanoma. Skin cancer affects those areas of skin which remains sun-exposed, including ears, neck, chest, scalp, lips, face, hands, arm and legs. It can also develop on those areas of skin that are not exposed to sunlight like beneath toenails or fingernails, on palms and on genital area.

Here are some of the factors which increase your risks of getting skin cancer:

1.Fair complexion: Low levels of the pigment melanin in skin causes fair complexion. Fair- skinned individuals who have a history of hazel or blue eyes, repeated sunburns and people who have red or blond hair are highly susceptible to developing this form of cancer. Less pigment in skin makes an individual more vulnerable to skin damage from harmful UV radiation.
 
2. Excessive sun exposure: Exposure to the sun may lead to the development of skin cancer, if you don't protect your skin by sunscreen and clothing. Tanning beds and lamps may also increase the risk of developing this type of skin cancer.
                                            
3. High-altitude places: The exposure to sunlight is more intense in areas of high altitude and near the equator. Living at higher elevation also makes you more vulnerable to radiation because the sunlight is strongest there.

4. Moles: People with abnormal moles are at a higher risk of developing skin cancer. These moles are irregular in shape and are larger than normal moles.

5. Precancerous skin lesions: Your risk of developing skin cancer increases if you have skin lesion. These are scaly and rough patches that range from brown to dark pink in color. The most commonly affected areas are head, hands and face of fair-skinned people.

6. Weak immune system: Weak immune system caused by HIV or AIDS and immunosuppressant drugs that you take after an organ transplant may increase your risk of developing skin cancer.

Know If You Are At Risk Of Skin Cancer

Fibroadenomas are benign non carcinogenic tumors that occur in the breasts of women. Although the condition may affect females of any age; spanning from pubescent girls to middle aged ladies, yet it is most commonly diagnosed in young women below 30 years of age. Fibroadenomas can be described as a stiff, smooth, supple marble under the skin of your breasts, which move when touched. These tumors which occur in varied sizes, may shrink or expand with time. Diagnosing through biopsies and treatment by surgeries are commonly employed for such a condition.

Types of fibroadenomas
Fibroadenomas are of two types: simple fibroadenomas and complex fibroadenomas. The simple tumors are usually harmless and almost look the same under the lens of a microscope. On the other hand, the complex tumors are comprised of macro cysts (large fluid-filled sacs) and calcifications (deposits of calcium) which can slightly increase the chance of breast cancer.

What is the primary cause of fibroadenomas?
Doctors aren't able to pinpoint the chief cause of fibroadenomas, but they have reasons to believe that the condition may pertain to the reproductive hormones. The condition may occur during pregnancy or during the use of hormone therapy, owing to which, the tumors may increase in size. The tumors are usually seen to shrink postmenopause, when the hormone levels start to dwindle.

Fibroadenomas are apparent and you may be able to detect them even without a doctor. The marble like tumor within your breasts are usually:
1) painless
2) firm
3) rubbery and elastic
4) easily movable

These tumors can range from being too small to as big as 3 inches or more in diameter. These lumps, which are usually harmless may be a cause of concern if they start to grow or change. Consulting a doctor is duly advised in such cases.

Risks
Fibroadenomas, only in the rarest cases, increase the likelihood of breast cancer. A complex fibroadenoma comprising of cysts or thick tissues called calcifications may aggravate your condition. Causes of concern could be if the tumor pains, a family history of cancer or an event of a questionable biopsy report.

Causes and Symptoms of Fibroadenomas

One of the most common problems faced by ageing men is benign prostatic hypertrophy. Prostate is the gland at the base of the urethra near the bladder and when it enlarges it can lead to symptoms, mostly related to urination.

1. Frequent urge to pass urine
2. Prolonged urination
3. Frequent nocturnal visits to the toilet
4. Intermittent urination
5. Difficulty to start urinating
6. Inability to completely empty the bladder
7. Urinary tract infections

There are medications available to manage this, but offer only temporary relief. Many men therefore prefer to have the surgery undergone to manage these bothersome symptoms. However, like any surgery, the risks and benefits need to be considered along with other conditions like age, overall health status, other comorbid conditions, etc.

Surgical removal of the enlarged prostate gland is a more definitive approach to manage these symptoms. In addition to providing a quick cure, it also is used in the following cases:

1. Patients who do not respond to medications
2. Presence of blood in the urine
3. Associated bladder stones
4. Frequent infections of the urinary tract
5. Associated damage to the kidneys

Procedure of Surgery
During the procedure, a tube is passed through the tip of the penis into the urethra towards the bladder neck. Once it is in the desired position, laser is passed through it to deliver energy that acts on the prostate to either completely or partially destroy it. There are two methods by which laser acts on the enlarged prostate and making way for free flow of the urine.

1. Ablation: Excess prostate tissue is melted away by the laser by using photosensitive vaporization of the prostate. This is also known as Greenlight laser therapy or KTP laser vaporization. Alternately, Holmium can be used as the source of laser energy to ablate the prostate tissue.

2. Enucleation: Excess prostate tissue is cut and teased out through the urethra. Holmium laser is used to resect the prostate into smaller pieces, which are then removed out through a resectoscope. Another technique uses a tissue morcellator which grinds the enlarged prostate into smaller pieces to enable easy retrieval.

More men now opt for laser prostate removal as it has the following advantages:

1. Reduced risk of bleeding: This becomes essentially important in patients who are on blood thinners.
2. No hospitalization: This can be done either as an outpatient or with minimal one day stay at the hospital
3. Immediate symptom relief: As compared to medications, the relief is felt almost immediately after the surgery
4. No catheter: With laser surgery, a catheter may be required for less than 24 hours unlike in open surgical cases.

As noted above, as with any surgery, once enlarged prostate symptoms set in, have a detailed discussion with your doctor to identify if you are a suitable candidate for laser surgery.

There are fewer Indians who have successfully battled Cancer and have emerged successful. Few cancers like cervical cancer and breast cancer which affect women of any age, are curable, however, due to lack of awareness and timely screening, many Indian women succumb to these cancers. The most common sites for cancer among men are lung, prostate, colon, rectum, stomach and liver while that among women are breast, colon, rectum, lung, cervix and stomach.

In proportion to our population, we lack the right infrastructure nationwide. esp in the northern and north eastern region, there is a dearth of good cancer facilities, where radiation, care or treatment can be given. Not even one third of cancer patients are alive beyond 5 years after their diagnosis in India, and about 71% of cancer deaths occur in patients in their prime productive years   between the ages of 30 to 69 years.

Lumpectomy is a surgery where a cancerous or benign tumour is removed from a breast along with some surrounding tissues. The amount of tissue removed varies according to the nature of the tumour. It is a form of breast-conserving surgery. It is also called a partial mastectomy.

Lumpectomy is the first option of treatment for women diagnosed with early-stage breast cancer.

What is the goal of lumpectomy?

Lumpectomy aims to remove the cancerous cells from the breast and at the same time maintaining the appearance of the breast. Studies have proved that for early stage breast cancer, lumpectomy is as effective as mastectomy (complete removal of the breast).

Doctors may recommend lumpectomy to remove precancerous cells as well.

Pre lumpectomy measures-

If you are under any other medication or if you suffer from allergies, inform your doctor. Other pre-surgery measures include-

  Stop taking aspirins or any pain relieving medication

  Your doctor will recommend you to not drink or eat anything 8-12 hours prior to the surgery.

The procedure:

A mammogram and a biopsy reveal the exact location of a tumour. Lumpectomy is performed under general anaesthesia. The surgeon makes a small incision over a tumour to extract the cancerous cells and the surrounding tissues. Sometimes doctors have to perform sentinel lymph node surgery or axillary node dissection.

The surgeon then closes the incisions with sutures. The sutures may dissolve over time or may have to be extracted later by a doctor.

If you have undergone lumpectomy and sentinel node biopsy, you will be discharged on the same day as the surgery as soon as your blood pressure stabilizes.

However, after lumpectomy with axillary node extraction, you may have to stay in the hospital for 2-3 days.

Therapy after lumpectomy

Your oncologist will recommend a follow-up radiation therapy to destroy the mutated cells that may have remained in the area where a tumour had developed. Radiation reduces the risk of breast cancer recurrence.

You might also have to undergo hormone therapy. Some types of breast cancer need female hormones like estrogen and progesterone to grow. Hormone therapy inhibits the development of cancerous cells by not allowing these hormones to reach the cells.

Your doctor will advise you to avoid strenuous physical activities for at least a month after the surgery.

Lumpectomy is a safe surgical procedure that can root out tumours or precancerous cells from the breast without deforming the shape of it.

Breast Lumpectomy - All You Should Know!

November is designated as the Lung Cancer Awareness Month, to ensure every individual is equipped to fight against lung cancer right from the start. Here are 4 vital facts you must know to win your fight against blood cancer.
Lung cancer is also known as pulmonary carcinoma or cancer of the lung. It occurs when DNA mutations develop in the cells/ tissues of the lungs leading up to uncontrolled growth in the tissues of the lung. By far, it is known to be the most common cancer in Asia.

1. Type: lung cancers can present as one of the following types: 

Non-Small Cell Lung Cancer (NSCLC): Most common type of lung cancer and constitutes nearly 85% of all lung cancers. The sub-types of NSCLC are as follows:
Large cell carcinoma
Squamous cell carcinoma (SqCC) (Epidermoid)
Adenocarcinoma   accounts for majority of the NSCLC.
Small Cell Lung Cancer (SCLC): It is also known as oat meal cancer and tends to spread (metastasize) quickly. This constitutes about 15% of the lung cancers.
Mesothelioma: It is a rare cancer that develops in the mesothelial cells of the pleural or peritoneal surfaces. It usually arises in the pleural membrane lining the lungs, known as pleural mesothelioma. One that arises from the peritoneum is called the peritoneal mesothelioma and the one that arises from the pericardium is known as pericardial mesothelioma. Lastly, it can also arise from the tunica vaginalis known as testicular mesothelioma. People working with or prior exposure to asbestos are mostly at risk of developing mesothelioma. The latency period between time of exposure and development of mesothelioma can be somewhere between 20 to 40 years. Maximum of the patients who develop mesothelioma are men.
2. Gender: It affects the male populace predominantly. It is more common in men than in women and in those of lower economic status. However, the incidence of lung cancer, in women too, is on the rise of late.

3. Etiology: Cigarette/ Tobacco smoking remains the most important cause of lung cancer accounting for 85   90% of the cases. Incidence/ risk is proportionate to the number of cigarettes smoked irrespective of the age. Also, environmental toxins including smoke from burning black tar, exhaust gases from automobiles etc. too contribute actively to various lung diseases including cancer. People working in asbestos manufacturing factories who are exposed to asbestos dust are also likely to get lung cancer. Tuberculosis too is an additive risk factor. Again, people who have received ionizing radiation especially to treat Hodgkin Lymphoma or other malignancies also run the risk of developing lung cancer. Above-mentioned causes apart, genetic factors may also play a role.

4. Features: Lung cancer clinically presents in the following ways mainly. Signs & symptoms of malignant mesothelioma are as enumerated below

Difficult/ labored breathing (Dyspnea)
Chest pain
Palpable chest wall mass
Discordant chest wall expansion
Weight loss (Cachexia)
Loss of appetite (Anorexia)
Night sweats
Signs & symptoms of non-small cell lung cancer (NSCLC) are as enumerated below. Majority of patients are symptomatic at diagnosis

Chronic cough
Difficult/ labored breathing (Dyspnea)
Chest pain
Blood stained sputum (Hemoptysis)
Hoarseness
Weight loss (Cachexia)
Loss of appetite (Anorexia)
Weakness
Bone pain
Pleural or pericardial effusion
Superior vena cava syndrome
Brachial plexopathy
Neurologic pain
Hypercalcemia of malignancy
Signs & symptoms of small cell lung cancer (SCLC) are as enumerated below

Symptoms as per the points 8   18 depicted above for NSCLC are applicable for SCLC as well. Additionally, patients suffering from SCLC may have:

Fever associated with chest infections like bronchitis/ pneumonia that refuse to go away or keep coming back
Horner syndrome
Paraneoplastic syndromes including encephalitis/ sub-acute sensory neuropathy, hyponatremia, acromegaly, cancer associated retinopathy, ectopic corticotropic syndrome etc all.
5. Screening: is generally recommended for asymptomatic/ symptomatic populations as surveillance for high-risk individuals   who are either current or former smokers (quit smoking within the last 15 years) have at least a 30 pack-year smoking history and those who do not have any prior history of lung cancer. The goal of screening, as usual, is to be able to detect & diagnose lung cancer at an early stage which is potentially curable. It is mostly radiologic with a low dose helical computed tomography (CT) scan being more effective in detecting early-stage lung cancer than a chest radiograph can.

6. Diagnosis: Abnormal blood test results may be indicative of malignancy, but a follow-up imaging/ biopsy is always the gold standard for an accurate diagnosis. Following are the diagnostics employed mainly:

Blood: Hb may be low, TLC, ESR and polymorphs increased. Adenocarcinomas are likely to express thyroid transcription factor 1 (TTF-1) or carcinoembryonic antigen (CEA). On the other hand, mesotheliomas are likely to express Wilms Tumor -1 (WT-1) protein and Calretinin.
Imaging: Thoracoscopy, Bronchoscopy, X-Ray, CT Scan etc all. Positron Emission Tomography (PET) / CT scan and Magnetic Resonance Imaging (MRI) scan help detect metastasis if any.
Biopsy: It clinches the histologic diagnosis and the nature of the disease.
7. Treatment: Conventional treatment includes surgery, chemotherapy and radiotherapy as contextually appropriate. Simultaneously, an adjunctive or integrative naturopathic treatment with suitable complementary & alternative medicines (CAM) too can help improve clinical outcomes and facilitate recovery as would be feasible contextually.

8. Prognosis: Preventive measures, earlier diagnosis and right early treatment are key for effective therapeutic management & better prognosis. Like most other cancers, the chances of cure for early-stage lung cancer are more. The cure/ recovery chances are influenced by the grade, stage of cancer, recurrence and the patient s general health & vitality etc all. The primary determinant of prognosis in NSCLC is the stage at which the cancer is diagnosed. For non-metastatic cancers, however, it is the nodal status that determines the stage and hence the prognosis.

9. Prevention: Rightly said, prevention is always a better choice. Cigarette smoking is to be avoided by all means as it is the single major cause of lung cancer. Even exposure to cigarette smoke (passive or secondhand) is also an established cause of lung cancer and is known to increase the risk of lung cancer in non-smokers as well. Preventing exposure to secondhand smoke can be helpful in decreasing the incidence and mortality of primary lung cancers. Above-mentioned apart, other important risk factors such as exposure to ionizing radiation, environmental carcinogens like smoke from burning black tar, exhaust gases from automobiles, outdoor air pollution, and occupational exposure to asbestos, arsenic, beryllium, nickel, cadmium, chromium etc all too are known to increase the risk for lung cancer and mortality thereof. In fact, cigarette smoking is known to potentiate the effect and hence the lung cancer risk of many of the above-mentioned carcinogens, multi-fold, in smokers. Thus, either eliminating or reducing the exposure to the above-mentioned carcinogens can lead to a decrease in the risk of lung cancer and incidences thereof. Furthermore, randomized clinical trials indicate that high-intensity smokers (only) who take supplementation of beta-carotene have an increased risk/ incidence of lung cancer. Vitamin E supplementation, on the other hand, does not affect the risk of lung cancer as indicated by the trials.
