Neurological disorders are an enigmatic bunch. There are various ongoing researches to identify the exact cause and how to treat them. While some argue that schizophrenia is a chronic brain disorder, others do not agree with it and debate that the origin is yet to be identified and the factors which produce it are yet to be identified.

Factors supporting it to be a brain disease are listed below.

Scan images (MRI and PET scans) of people who have had schizophrenia are different and have a reduced volume of grey matter, especially in the frontal and temporal lobes. In some people, this loss could be as high as 25%. Serial images have shown that the loss begins in the outer (parietal) region and gradually spreads to the rest of the brain. There is a strong correlation between the extent of this grey matter loss and the severity of the symptoms more the loss, worse the symptoms. Severe symptoms include hearing voices, psychotic thoughts, delusions, hallucinations and severe depression. This grey matter reduction does not depend on whether the person has undergone treatment or not for the condition. 

The counter argument is that grey matter loss is not seen in patients with schizophrenia alone. A number of neurological disorders, including normal ageing, manifest as grey matter loss on the scan images. Also, another consoling news is that this grey matter loss is completely reversible.

The way neurotransmitters respond to stimuli in people with schizophrenia is also slightly different. There are two variations to this debate: that schizophrenia is caused by excessive amounts of dopamine or increased sensitivity to dopamine. There are two proofs for this hypothesis first, dopamine suppressants are useful as antipsychotic drugs and secondly drugs which produce the same effect as dopamine can cause hallucinations similar to those seen in schizophrenics. There could be other neurotransmitters involved too, but the dopamine connection is more strongly established.

The electrical activity of the brain as seen in EEG is very different and abnormal in schizophrenic people. This reduced brain activity is another indication of the fact that schizophrenia is another brain disorder. There is also a strong genetic component to schizophrenia. Family history correlation is also very strong. The correlation extends to the point that where there is a neurologic disorder in the first-degree relatives, the chances of developing schizophrenia is increased.

There are specific genes, mutations of which are also attributed to schizophrenia. While the risk factors are in place (as mentioned above), the environmental conditions (brain infections, head trauma, family/social stressors and toxins can all lead to the manifestation of the symptoms. With medications and support therapy, both prevention and symptom management are possible.

Schizophrenia - Is It Really a Brain Disease?

Depression is recognized as a mood disorder characterized by the prolonged feeling of sadness and loss of interest in otherwise regular activities. The symptoms include loss of sleep and appetite, loss of interest, persistent feeling of sadness and lowered self-esteem. These symptoms should last for a minimum of 4 weeks for the individual to be diagnosed with depression. Sometimes, at its later stages, depression can also be accompanied by suicidal tendencies.

How it can be treated?

Role of anti-depressants: Depression has become rather common in the present day scenario. The treatment varies from prescription drugs to therapy. People who have depression are usually given mood enhancers and anti-depressants. Antidepressants are medicines used to help people who have depression. With the help of these depression medications, most people can achieve significant recovery from depression. Most antidepressants are believed to work by slowing the removal of certain chemicals from the brain. These chemicals are called neurotransmitters (such as serotonin and norepinephrine). Neurotransmitters are needed for normal brain function and are involved in the control of mood and in other responses and functions, such as eating, sleep, pain, and thinking. Antidepressants help people with depression by making these natural chemicals more available to the brain. By restoring the brain s chemical balance, antidepressants help relieve the symptoms of depression. Specifically, antidepressant drugs help reduce the extreme sadness, hopelessness, and lack of interest in life that are typical in people with depression. However, these are but short-term solutions to depression and don t necessarily cure the individual completely. For the treatment to be effective, the drug intake must be combined with therapeutic techniques to solve the problem at the root level.
Therapy: Different therapeutic techniques are used to treat depression depending majorly on the individual, the cause of depression and general history and state of mind. Research suggests that client centered therapy and cognitive behavioral therapy are a good start with an average case of depression. The therapists usually incorporate several other methods based on the client s need to get maximum effect. Adlerian techniques, where individuals are made to comprehend the thoughts and emotions which influence their lifestyles and adopt a more productive and positive way of life, are rather helpful with most clients who suffer from depression.

In severe cases, if there is extensive tissue damage, the damaged portion of the brain may be surgically removed. This creates space for the healthy brain tissue to heal. In the case of an open brain injury, surgery is the first course of treatment. In such cases, the doctor s first sim is usually to repair the fractured skull and remove damaged tissue. This minimizes pressure on the brain and also helps prevent secondary infections and injuries.

These surgeries are generally performed while the patient is under general anaesthesia. Recovering from a traumatic brain injury takes time and hospitalization is usually required for a few weeks. Once the injury has healed considerably, rehabilitation is then required to relearn basic skills. This may begin as an inpatient procedure and later be transferred to an outpatient exercise. The duration and type of rehabilitation required vary from case to case and depends on the type and severity of the injury.

Suicidal behavior: People suffering from this disorder may often be associated with behaviour that physically harms them. They are also capable of following through on suicidal threats.
Mood swings: Extreme fluctuations in emotions and moods are a common experience or people suffering from borderline personality disorder. These mood swings usually start only a few minutes of a few hours unlike depression and other anxiety disorders.
Suspicion about everyone around: Borderline personality disorder can make a person struggle with suspicious thoughts about everyone around them and make them paranoid. They often feel out of touch with reality and can feel spaced out when under stress.

Borderline Personality Disorder - Signs and Symptoms!

Non competitive exercises are better than competitive exercises as it does not create an opportunity for the person to feel stressed. Exercising and practicing breathing techniques can also help a person analyze himself and his response to stressful situations. By understanding their reactions, a person can slowly bring about changes to handle stress in a better way.

For a PTSD patient, exercise should not be seen as a group activity but as an individual effort to increase self control. Patience is key here both for the patient and the therapist. It is also necessary for the person to have realistic goals from his exercise regimen. An exercise program for PTSD patients also needs to be flexible. In order to meet the needs of a patient, it may need to be changed in accordance to the medication he is under. Small incidents like having a flat tire when cycling can trigger anxiety in a person suffering from PTSD. However, if dealt with correctly, it can bolster confidence in the long run.

Lastly, exercising also helps improve self-image and build confidence which helps a PTSD patient break out of his protective shell and socialize with others.
