Gastroparesis is a condition when your stomach feels full and heavy even after it has been more than a couple of hours after meal. Generally, it takes maximum 1 &   hours for the stomach to feel empty, but if the time taken for it is more, then this is a condition of gastroparesis. It means there is some change in your normal digestion process.

In this circumstance, the food in your stomach doesn t digest easily and creates a lump. As a result, you have hard time digesting your food. It also means the nerves connected to the stomach are not functioning well. There are a number of reasons for which you can get gastroparesis.

Diabetes is one of the most common causes of making you feel fuller.
Disorders in the nervous system, mainly Parkinson s disease or cerebral attack can create this problem.
Some medicines, especially antidepressants or anti-inflammatory in nature and certain painkillers can cause this problem.
Gastroparesis shows a number of symptoms:

Occasional vomiting as you feel unnecessarily fuller. When you vomit, it gives out chunks of undigested food
A feeling of nausea
You feel full after you have eaten very little
Bloating and pain in your abdomen
There is alteration in the blood sugar level
You don t feel like eating due to loss of appetite
You lose considerable weight as you don t eat much
A possibility of suffering from malnutrition, again due to lack of appetite
How it can be treated?
When you identify the reason for gastroparesis, then treating it becomes easier. If diabetes or nerve disorder is creating the problem, leave it to your doctor to treat. For other causes like lack of appetite, there are some natural ways of treating it.

Have frequent smaller meals. This way you don t push yourself to eat much and at the same time, the body doesn t lack nutrition and energy
Avoid fibrous fruits
Avoid high carbohydrate and high fat foods, which takes time to digest
Intake foods which you can swallow, like soups or stew. They are healthy and are also easy to digest
If the problem still persists, consult your doctor who may prescribe some medicines to stimulate the stomach muscles. They generally have no side-effects. There are some medicines which can control your vomiting and nausea. There are some surgical treatments for gastroparesis, where the doctors place a feeding tube in your small intestine. Consuming too much alcohol can cause people a lot of problems like nausea and headache. Additionally, it can also mess up with your bowel movements and affects your motions by giving you loose motions or constipation. Alcohol hinders digestion, causes dehydration and drying out, discourages glucose metabolism and compromises the working of the central and peripheral nervous systems. Accordingly, stools become dry, hard and difficult to pass.

Inability to move your bowel when you want, expands, impacts and solidifies the already dried out stools until it gets too difficult to go out without straining and subjecting yourself to anorectal harm. These are the two issues that usually precede chronic constipation.
Loss of electrolytes: Alcohol causes loss of electrolytes that are vital to hold dampness in the poop. Eg. Sodium and potassium. Water maintenance in the body and stools requires sodium and potassium, the minerals known as electrolytes. B.  Polyps: Polyps are soft tissue growths seen in the large intestine. They are usually silent but can present with bleeding or anaemia. Polyps usually grow slowly in size and large polyps can turn into cancer of large intestine also known as colorectal carcinoma. Colonoscopy is required for the diagnosis. The gastroenterologists can diagnose the presence of polyps by colonoscopy. Smaller polyps can be removed by polypectomy by gastroenterologists without the need for open surgery. However large polyps or those polyps with suspicion of cancer require surgery.

C. Large intestine Ulcer: They can occur due to early cancer, SRUS, Infections or drugs. They require colonoscopy for the diagnosis. The gastroenterologists usually take Biopsy from the ulcer to know the cause of the ulcer. The mere taking of biopsy does not mean cancer and biopsy samples are seen in the microscope for better diagnosis.

D. Ulcerative Colitis and Crohn s disease: Ulcerative colitis is one form of Inflammatory bowel disease (IBD). The other IBD is known as Crohn s disease. Whereas ulcerative colitis involves only large intestine, Crohn s disease can involve any part of the gastrointestinal tract including small intestine. Ulcerative colitis and Crohn s disease present with Loose stools(diarrhea), bleeding , abdominal pain, weight loss, Anemia and Perianal fissures and  fistula. The diagnosis of IBD is challenging and involves blood tests, CT Scan abdomen, Colonoscopy with biopsy and clinical history. Management includes Medicines which control the inflammation such as Pentasa, Rowasa, Mesacol etc. Severe disease requires additional drugs such as Azathioprine and Anti TNF sich as Infliximab and Adalimumab. Occasional cases require surgery such as those with tight stricture of the intestine, severe dilatation of Large intestine (also known as Toxic megacolon), excessive bleeding, the disease not responding to medical management, or presence of cancer with IBD.

E. Anal fissures: Cut in the anal skin (fissure) causes severe pain in the anal area during passing motions. It is usually due to constipation or straining. It is usually treated by Gastroenterologists initially by medicines and ointments. If the pain continues Surgery is required named as Lateral internal sphincterotomy (LIS).

2. Loose motions or Diarrhea: Excessive passing of liquid motions is known as diarrhea or commonly lose motions. It is usually due to Infections caused by contaminated food and water. It is usually self-limiting. Severe cases with dehydration require admission for IV fluids and antibiotics for the infection. Loose motions for more than 6 to 8 weeks is known as chronic diarrhea. It is evaluated by blood tests and colonoscopy. Usual causes include tuberculosis, celiac disease, ulcerative colitis, microscopic colitis, Malabsorption such as chronic pancreatitis and small bowel bacterial overgrowth.

3. Irritable bowel syndrome (IBS): IBS is very common and at least 20 to 30 percent of Indian patients suffer from it. Symptoms include altered bowel habits such as diarrhoea or constipation, abdominal pain which is usually relieved or increased by defecation. Weight loss or bleeding is not the feature of IBS. Few simple blood tests are required and good medicines are now available for IBS. Few cases may require colonoscopy to rule out alternative causes of the symptoms.

Understanding colonoscopy: Colonoscope is a flexible tube with a camera at its tip to see the inside of the large and small intestine. Bowel preparation is required at least 8 hours before the procedure so that the inside of the large intestine can be seen clearly without the interference of stools.colonoscopy is an OPD based procedure where the patients are discharged the same day. A colonoscopy helps in evaluation symptoms of the large intestine such as loose stools, bleeding and abdominal pain.

Diarrhea is a disorder that affects people of all age groups. Medically speaking, Diarrhea is a mechanism that the body uses to eliminate germs from the body. Usually this disorder lasts for a few days to a week. It is usually accompanied by nausea, fever and dehydration.

Children can be affected by diarrhea in a number of ways, some of them are:
1. Infections: Various infections, such as the ones caused by rotavirus and salmonella bacteria can lead to diarrhea. Usually diarrhea in children is caused by viruses; if it is viral in nature, then it can also cause stomach cramps, headache and vomiting. It also leads to loss of fluid from the body. Hence, the child's fluid intake should be compensated accordingly. Usually oral rehydration solution that contains nutrient such as potassium and sodium are preferred.

2. Medications: Intake of certain medications, such as antibiotics and laxatives can cause diarrhea in children. If the cause is due to antibiotic intake, then you should consult a doctor. The doctor may alter the dosage or ask to increase the child's water intake. Yoghurt may also help ease the symptoms of diarrhea if it is caused by antibiotics.

3. Food poisoning: Diarrhea may also result from food poisoning; symptoms of this type usually disappear quickly. Along with loose and watery stools, the child may vomit frequently.
The major complication that results from diarrhea is dehydration. Severe dehydration may lead to severe brain damage and seizures. It may also cause symptoms of dry mouth, fatigue, dizziness and dark yellow urine.

Certain preventive measures are:
1. Use your good judgment while eating outside. Do not consume food from places which use unhygienic modes of preparation.

2. Make sure your child does not drink contaminated water and there is access to clean and filtered water all the time.

3. Your child should be eating a balanced diet that primarily consists of vegetable and fruits. They also help in adding fiber to the diet that keeps your bowels healthy.

All About Diarrhea in Children

In healthy individuals, the liver contains little or no fat. In overweight or obese people, gradual fat accumulation leads to significant liver disease. Interestingly, these individuals may be consuming minimal to no alcohol. So, alcohol, which is the main cause for liver damage, does not have a significant role to play.

The non-alcoholic fatty liver disease (NAFLD) has 4 significant stages as outlined below. It is a chronically progressive disease and may take years to reach the final stages of cirrhosis and fibrosis.

1. Simple fatty liver (steatosis): This is usually identified when diagnostic tests are done for some other suspected conditions. There are usually no symptoms obvious in this stage, other than the buildup of fat in the liver.

2. Non-alcoholic steatohepatitis (NASH): The second stage where the liver is inflamed to a greater extent due to fat accumulation.

3. Fibrosis: The next degree of inflammation where blood vessels may be narrowed leading to scarring in the liver with impaired liver functions.

4. Cirrhosis: This is the most severe stage that occurs due to years of cumulative inflammation. The liver shrinks in size, is scarred, and liver functions are markedly impaired and can also result in liver cancer.

Risk factors for NAFLD: The exact reason why a person develops NAFLD is not established, but some of the risk factors include

1. Obesity, with more weight concentrated around the abdomen

2. Type 2 diabetes

3. High blood pressure

4. High cholesterol levels

5. Age greater than 50

6. Smoking

Symptoms: This will depend on the stage in which it is identified. While there are no symptoms in the early stages, in some people there could be a dull, aching below the ribs, unexplained weight loss, weakness, and extreme tiredness. As it progresses to cirrhosis, there could be jaundice, fluid accumulation in the abdomen and feet, and itching of the skin.

Management: While there is no treatment aimed at curing the disease per se, there are ways to manage the symptoms, as noted below.

1. Weight loss: Reducing excess accumulated fat will help reverse symptoms and prevent further progression of the disease. A BMI of 18 to 26 is considered optimal.

2. Dietary changes: Modify your diet to a carbohydrates and protein rich and reduced fats and sugars. Increasing fiber through fruits and vegetables is highly recommended.

3. Exercise: Whatever your choice of workout, it will do wonders for NAFLD. Keep a target of an hour or two of moderate to intense exercise per day to reduce weight.

4. Smoking: This is another risk factor and can also help prevent other effects of NAFLD such as diabetes and heart disease.

NAFLD is highly controllable with these changes and other damages can be reversed too.
