The human ear is divided into three parts, namely the outer, middle and inner ear. All these parts function in coherence with each other and help us in converting vibrations into sound and information. The middle ear consists of the air space between the inner ear and the outer ear and contains the bones that transmit the vibrations of the outer ear and translate them into information. This area is prone to infections, especially among children and thus may cause discomfort or pain. 

Symptoms of middle ear infections
Middle ear infections primarily tend to manifest themselves as inflammation of the tissues and buildup of fluids. Some of the symptoms are mentioned below.

1. Infections in children - middle ear infections primarily occur in children although it can occur in adults as well. Some of the common symptoms could be:
A. Ear pain either sitting or standing up and especially while lying down

Causes of middle ear infections 
The primary cause of middle ear infection is due to the presence of bacteria or virus in the area. The eustachian tube, which runs from the middle ear to the back of the throat, is connected to nasal passages as well. Any infection that affects the throat or the nose may also affect the middle ear through this tube. Primary causes for the infections could be
A. Cold or flu like symptoms, which then infect the middle ear as well
B. Otitis media, which is the buildup of fluid and inflammation either due to the presence of bacteria and viruses or even without it
C. Seasonal infections, which are common during late autumn or winters
D. Air pollution is also known to be a factor
E. Infection from others, especially in the case of children when they are part of a group care
F. Children from 6 months to 2 years are also susceptible to middle ear infections as their immune systems are much less developed.

Treatment of middle ear infections
Usually, middle ear infections resolve on their own within a day or so. You can try warm compress with a soft piece of cloth to ease the pain. However, if pain persists for too long then antibiotic medications may be required to fight the infection with pain relief medications to lessen the pain and discomfort.

What is a foreign body (FB)?
Foreign body or FB is an object or piece of extraneous matter that has entered the body by accident or design. It is common to see certain items getting stuck in the ear, nose or throat especially, in young children. While most of the time they do not raise an alarm, however, sometimes they can lead to severe consequences like rupture of the eardrum or even choking.

How do FBs affect ear, nose and throat?

Children over 9 months develop an ability to pick up objects. As they explore the environment around them, they tend to put items such as peas, beans, marbles, beads, cotton, paper, etc, in their mouth, nose or ears. Similar trend may be seen in adults with mental disorders.
Trauma/ fall/accident can force FBs like rock, pebbles into the ear, nose or throat.
Adults while riding a bike or sleeping on the ground (campsites) can have insects: flies, ants, roaches fly into the nose or ear.
Choking occurs when FBs block the airway, common reasons are: not chewing the food properly, diseases like Parkinsonism, drinking alcohol or certain drugs that slowdowns the process of swallowing.
Managing FB in the ear, nose and throat

Foreign bodies in the ear
Symptoms: Pain in the ear, discharge or bleeding from the ear, feeling of fullness or pressure in the ear and dizziness and nausea in extreme cases.

Management: 

Make sure the patient is sitting in an upright position, pull the pinna or outer semicircle of the ear towards the back. This can help you see the object clearly and also loosen it to wiggle out. If you can see the object clearly, use a tweezer to grasp the object and gently remove.
In case a live insect gets trapped inside the ear lie down and place few drops of warm vegetable or baby oil. Use oil only if symptoms of eardrum perforation like pain, bleeding/discharge from the ear is absent.
Tilting the head to the affected side can facilitate gravity to dislodge the object.
Foreign bodies in the nose:
Symptoms: Pain and difficulty in breathing from that side of the nose.
Management: Keep the uninvolved nostril closed by pressing a finger against the side of the nose and gently ask the person to blow from the affected nostril.

Foreign bodies in the throat:
Symptoms: Choking, inability to breathe and speak 
Management: Perform Heimlich maneuver: abdominal thrusts, if trained otherwise, it is advised to call for medical help immediately.

How can you prevent foreign bodies from entering your ear, nose, throat?

Discourage talking, playing, laughing when food is in the mouth.
Do not give whole grapes, popcorn, nuts to children below 3 years.
Keep small objects out of reach of small children.
Teach children to avoid placing objects into their nose, ears.

Central vertigo is when there is an issue in the brain that is causing vertigo. Middle ear problems leading to vertigo is known as peripheral vertigo.

Central vertigo:

Paroxysmal positional vertigo (BPPV): Also known as, Benign paroxysmal positional vertigo (BPPV). These episodes are benign and occur in paroxysms or spurts. The person will feel the attack with simple actions like turning over in bed. They do not last long, often about 20 to 30 seconds.
Migraine: Known for the severe headaches, migraine can also cause vertigo.
Head trauma: This type of vertigo usually subsides with time as the person recovers from trauma.
Neck injuries: Injury to nerves and blood vessels in the neck can also lead to vertigo.
Health conditions: General health issues like arteriosclerosis, multiple sclerosis, and diabetes lead to vertigo.
Stroke or blood flow blockage: When blood does not reach the base of the brain, it can result in vertigo.
Peripheral vertigo:

Middle ear infections: Labyrinthitis (either bacterial or viral) a middle ear infection can cause vertigo. Common viruses include herpes, influenza, measles, rubella, hepatitis, and EBV.
Meniere s disease: Another ear problem causing vertigo is Meniere s disease. This is known for its 3 symptoms   vertigo, ringing in the ears (tinnitus) and hearing loss. This can be caused by various factors, some of which can include ear infections, head injuries, or heredity. The exact cause of the condition is not established clearly.
Acoustic neuroma: A tumor of the nerve tissue in the inner ear may cause vertigo. The person can also have hearing loss and ringing sensation.
Other causes of vertigo include:

Pregnancy: During the first trimester, in particular, the various hormonal changes can cause vertigo. The expanding uterus puts pressure on the blood vessels, leading to reduced blood supply to the brain, which can also induce vertigo. As the baby continues to grow, lying on the back can put pressure on the blood vessels to the heart, causing vertigo.
Stress: It is known to worsen vertigo, which a person may already have. Anxiety and panic attacks are known to worsen vertigo.
Travelers: Be it ship, air, or train, some people experience a spinning sensation with any form of travel.


Vertigo In Adults - Why It Happens?

Nosebleeds can be frightening and dramatic. Luckily, most cases of nosebleeds can be dealt with fair ease. Nosebleeds can be classified into two types depending upon the origin of the nose bleed, namely anterior and posterior.

Anterior bleed is the most common, and the bleeding is from a blood vessel which is present in the front part of the nose. These bleeds can be easily controlled by simple measures taken by doctors or at home.
Posterior nose bleeds are usually indicative of something more complicated and occur mostly in adults or elderly. The bleed can be caused by a rupture in arteries in the posterior part of the nose. These nosebleeds can get severe and may require admission to a hospital for treatment and management. They are the most common in kids in the range of 2 to 10 years and adults older than 50 years.
What can cause nosebleeds?

The most common cause of a nosebleed is probably trauma or an injury to the nose. External trauma includes a sharp blow to the nose, whereas internal injury comprises of injury from picking your nose continuously or irritation from cold.
Less frequently, an underlying condition can also result in nosebleeds. These conditions include diseases that make clotting difficult. The inability of blood to properly coagulate and clot can be contributed by blood-thinning agents like aspirin and warfarin. 
Disease in the liver can also cause problems in the clotting of blood. Rarely, cancer in the nose or abnormal blood vessels present in the nose can contribute to bleeding. Another cause of nosebleeds can be high blood pressure, but it is rarely the only reason.
What are the symptoms of nosebleeds?

Nosebleeds usually occur from one nostril. Sometimes, if the bleeding is excessive, the nostril can get filled with blood and it can cross the nasopharynx and overflow into the other nostril and result in bleeding from both nostrils. The blood can also go into the throat and one may spit out blood from the mouth or vomit out blood (hematemesis).
Excessive blood loss can cause dizziness, confusion, lightheadedness, and can also cause fainting. Excessive blood loss from nosebleeds is not very common.
Although, nosebleeds are rarely a complication, it is advised to consult a doctor if the occurrences are too frequent.

Nose Bleeding - Things That Can Cause It!

The infection of the bone of the ear is Mastoiditis. This may be an acute infection needing hospitalisation and injectable drugs or may be in the form of slow bone eroding processes like cholesteatoma.

Understanding the mastoid bone: The mastoid bone is made up of air sacs and it looks a lot like a sponge. In this sense, it is different from the other bones that can be found in the human body. In order to function properly, this bone requires air from the other parts of the ear, including the Eustachain Tube which connects the middle part of the ear with the portion that lies at the back of the throat. If this tube gets infected due to any reason, the infection and bacteria can also travel to the Mastoid bone, leading to an infection in this area. Another form of the infection starts from the ear drum and a slowly progressive bone eroding process starts which spreads into the air sacs and can potentially spread to the brain.

Symptoms: Acute mastoiditis will present with earache, ear discharge and onset of fever and chills, as well as redness and inflammation behind the ear. Pain in the ear may be experienced along with headaches and swelling that may be experienced behind the eyes. At the very earliest sign, these symptoms must be reported to an ENT or ear throat nose specialist so that the treatment can happen at the earliest. Chronic mastoiditis may be due to repeated infections and bone eroding cholesteatoma which presents with repeated episodes of ear discharge which may be foul smelling and may even have blood.

Diagnosis: The diagnosis is mostly made on ear examination and may involve imaging tests like a CT scan as well as an MRI scan of the head and the ear. A simple X-ray of the mastoids can help in detecting the severity of the infection, and it spread.

Treatment: After diagnosing the problem with imaging tests as well as a detailed ear examination, the doctor will usually start the treatment by admitting the patient in the hospital. Antibiotic medication will be administered through intravenous methods, after which oral antibiotics will be prescribed for many days after the patient has been discharged from the hospital.
