Ulcerative colitis (UC) is an inflammatory disease of the colon and rectum limited to the mucosa, and may vary in severity from a mild intermittent disease to an acute fulminant and potentially fatal disease requiring urgent surgery. Management of ulcerative colitis depends on severity, extent, and duration of the disease, response and tolerance to medication, patient age and comorbidity as well as patient preference.

Surgery plays an important role in the management of UC both because of the premalignant nature of the disease, and because of the periodic failure of medical management. The underlying rationale for surgical treatment of the disease is that the disease is confined to the colon and rectum, and therefore proctocolectomy is curative. The goal of surgical therapy for ulcerative colitis is to remove the disease with as little alteration of normal physiological functions and lifestyle as possible.

Four surgical options exist for patients with ulcerative colitis and each has its own advantages and disadvantages.

The surgical choices are:

Proctocolectomy and Brooke ileostomy.
Abdominal colectomy and ilcorectal anastomosis.
Proctocolectomy and Kock pouch.
Restorative proctocolectomy with ileal pouch-anal anastomosis (IPAA).
The choice of operation requires consideration of the advantages and disadvantages of each option and must be tailored to an individual patient's needs and circumstances. Important factors to be considered in the choice of operation include the indication for surgery, age, associated medical conditions, body habitues, and quality of the anal sphincter. Extensive preoperative education is required which should include discussion with a specialized Gastroenterologist.

Ways To Deal With Ulcerative Colitis!

Eczema is not a disease per se, but is a name given collectively to a group of medical conditions, which can cause the skin to become inflamed and irritated. Atopic dermatitis is one of the most common types of eczema.

Causes

While the exact cause of eczema is not known, the most commonly attributed reason is an increasingly sensitive immune system. The overactive immune system produces antibodies, which cause these skin reactions. It is also noted that people with allergies including hay fever and asthma are more likely to develop eczema.

Other conditions which can lead to eczema include allergic dermatitis, irritant dermatitis, stasis dermatitis, scabies, fungal infections, xerotic dermatitis, lichen planus and scalp infections.

There is also a genetic predisposition, and children whose parents have had eczema are likely to develop eczema.

Symptoms

In most people, eczema is present in the form of small blisters, which are filled with fluid and can ooze. Gradually, there could be dryness, crusting, scaling and thickening of the skin with plaque formation. The most common symptom would be itching. The different types of eczema as noted above would have diagnostic features. For instance, when testing for irritant or contact dermatitis, there would be an irritant that is causing this. Stasis dermatitis would have circulatory issues and often affects the lower legs.

In fair-skinned people, these lesions begin as red and then turn brown. There also could be lightening of the affected skin area in people with slightly darker skin.

Diagnosis

For a trained medical eye, diagnosis of eczema is not a tough task. A complete physical examination may be done to assess the extent of eczema spread. Blood tests may be done to test for allergies. In very rare cases, a biopsy may also be required.

Treatment

The goal of treatment is to reduce itching and improve moisturization. Eczema, unless associated with an infection, is not contagious.

Using copious amounts of creams and lotions help in improving hydration and moisturization, as they are allowed to soak into the skin.

Topical steroids may be required if allergy is suspected. In most cases, hydrocortisone and corticosteroid creams are very effective.

Associated infections like scalp infections and fungal infections should also be treated with suitable products.

Immunomodulator creams like Elidel are useful by suppressing the immune system and preventing flare up of the allergic reactions.

Cold compresses may be used to relieve itching.

Prevention

Since there is a strong genetic component related to eczema, prevention is helpful.

Constantly keep the skin moist and hydrated by using moisturizers, especially during winters.

Avoid clothes and metals which can cause allergy- like wool, nickel, harsh soaps, detergents, etc.

Avoid or be careful about food items which are known to cause allergies.

Eczema - Causes, Symptoms and Treatments 

A cough and cold are caused by infections, sinusitis, bronchitis, and exposure to dust. Ayurveda recommends not just medications to treat a cough and cold. It also insists on incorporating certain dietary and lifestyle changes for complete cure. A cough is a way that the human body uses to blow off foreign bodies from the respiratory airways. However, when someone coughs for long, it can lead to dizziness, hoarseness of voice, and wheezing. In case of a chronic cough, fatigue and aches in the joints are experienced.

In Ayurveda, cold is also known as Vata-Kaphaja Pratishaya. According to Ayurveda, Virudh ahar (incorrect combination of foods), Ama (toxins), and Asatmya (change in environment) lead to allergic reactions, and cold is one such example. Toxins may accumulate in the body of an individual due to impaired metabolism and digestion. On the other hand, intolerance towards specific food items may lead to an allergy.

Vamana Panchakarma, followed by Virechana can be administered for the treatment of a chronic cough and cold. This helps correct the balance between the three doshas and to eliminate toxins. In some individuals, nasal drops therapy or Nasya treatment can be administered to clear congestion. This is done by pouring herbal oils such as Shadbindu or Anu taila in the nostrils. Ayurvedic medications can also be administered to strengthen the immune system and the respiratory tract.

The following sections explain some of the other Ayurvedic remedies that a person can try out to relieve cough and cold.

Home remedies: Ayurveda points the kitchen to get relief from a cough and cold. Ingredients from the kitchen shelf can help a lot, along with medications. Ginger juice is an effective way to treat a cold and cough. This helps in ejecting mucus. To try out this remedy, an individual can prepare fresh ginger juice and add turmeric (one-fourth teaspoon), tulsi leaves juice and honey (half teaspoon each) and a pinch of black pepper powder to it. Mix everything together. If you want to use it multiple times, you can increase the quantities in the same ratio and refrigerate it. Consume one and a half teaspoon of the mixture at least three to four times a day to get relief from a cough. For children, give half the quantity thrice a day. This is an excellent remedy to relieve a cough and cold without leading to any side-effects.

Gargling also helps. Mix a spoon of salt to warm water. Gargle twice a day with this. Chewing cardamom buds and cloves also help in eliminating cough. You can also take four black raisins and add a pinch of rock salt and black pepper powder to them. Dry roast and take this three times a day. You need to eat this for at least for 15 days.

Otherwise, take a glass of water. Boil with licorice powder and black pepper powder (half teaspoon each). Grate fresh ginger and add one spoon of it to the mixture. Add 10 basil leaves, and make a decoction of everything. The water should turn half the quantity. Palm candy can be added to sweeten the decoction. Try this instead of coffee or tea in the morning and evening to relieve cold and cough.

Dietary changes: Take lots of fresh pulses with grains. Taking pulses with grains help in gaining all the goodness of the pulses. Green gram, yellow lentils, and red lentils can offer nutrients that are required and are easy to digest. Vegetables such as pumpkin, marrow, squashes, ivy gourd, ridge gourd, snake gourd, smooth gourd, onions, beetroot, celery, carrots, and parsnips can be consumed as well. However, consume cooked vegetables only. Grains such as rice, oats, and millets are good choices. You should refrain from taking wheat and processed flour. Almonds, apricots, figs, dates, and hazel nuts are beneficial. Limit your consumption of fish, chicken, and turkey. Red meat can be taken twice a week. Avoid dairy milk. You can take soy milk or oat milk as a replacement.

Lifestyle changes: Quit smoking. The health of the lungs can be damaged to the core with smoking and this can also lead to cancer. When you suffer from chronic cold and cough, immediately consult a doctor, if you smoke. Avoid exposure to pollens, dust, and pollutants. Use a mask while driving, and cover ears and nose exposed to cold.

Talk less: This gives enough rest to vocal cords. Taking warm water instead of plain water is a good option. Take tea made of herbs. Tulsi, mint leaves, and ginger can be added to the herbal tea. Do not mix milk with tea. Take a balanced diet and keep yourself away from processed and junk foods.

Chronic Cough And Cold - How Ayurveda Remedies Can Help? 

Many children are affected by constipation; for them, it usually implies pain while passing stool as the stools tend to be hard. In some cases, parents often tend to confuse soiling as diarrhea when it may actually indicate symptoms of impacted constipation (obstruction of faeces). 2. Diet: A diet devoid of fiber may be the cause behind your child's constipation. Also, not drinking enough water throughout the day may cause the stools to harden, thus causing difficulty while passing them. The fiber content in food adds roughage to the diet and thus, helps in facilitating smooth bowel movements.

3. Psychological problems: Constipation may also be caused due to certain emotional problems and drawbacks, such as certain fears or phobias. An example would be when the child feels the need to go to the toilet at night, but withholds it if he/she has a fear of ghosts. A change in the surroundings such as moving to a new place may also cause bowel problems in your child.

Causes of Constipation in Children

Lotion and creams, both will do the trick, but it will depend on what you need and when you use it. Give a good thought to your skin. Is it always dry or only during certain times of the year. If your skin dries easily and you can see a white line if you scratch with your nail, you'll need a cream that absorbs more slowly and has benefits that stay longer.

Applying cream then creates a layer of oil that locks the moisture in your skin. The water does not evaporate, and your skin stays hydrated. Lotions are easy to spread on the skin. If your skin is already soft and supple and you simply need a layer to keep the dryness at bay, a lotion is a good choice for you.

Use both lotion and cream:
Morning, before bed, after baths or repeatedly throughout the day? The time you plan on using a moisturizing product will play a part in determining which type you choose. Creams are great to use right after a bath when the skin is soft after absorbing moisture. The cream then creates a protective layer of oil, which allows the skin to stay hydrated longer.

Creams are also great to use during bedtime when you don't have to worry about getting an oily look on your face or body. Lotions are best if you need to reapply during the day, for instance,  you wash your hands and face and need to dab on some quick moisture.
