High fever is common in kids and a common worry for parents as well. If you are a parent, you need to know when the fever is serious, and when you should worry about it. Fever is a defence mechanism of the body. Body temperature rises if any foreign body (or antigen) attacks your body. As a defence, body temperature rises making it an unfavourable condition for the antigen. Fever means 100.4 degree Fahrenheit. Normally for kids, thermometer is placed under the armpit to record the temperature. You need to add an extra degree for accuracy. Paediatricians recommend treatment for fever if it's more than 101 degree F.

When to Call a Doctor

If the kid is less than 4 months old and has a fever of 100.4 degree F with other mentioned symptoms as highlighted, one must see the doctor as an emergency situation.
If a kid has 104 degree F fever, rush to the doctor. Such a high fever in children could lead to seizers.

You must call a doctor if the kid has a fever along with the following symptoms:

    Looks very dizzy
    Has a deteriorated immune system or even any other medical condition
    Has a seizure
    Has symptoms such as rashes, painful throat, annoyance, rigid neck or ear pain

Medication Side Effects:

At times, if you treat your children with some medication that does not suit them, it may lead to high fever. In case fever in a kid aged less than 2 years lasts for more than one day, it's time to see a doctor. And lastly, remember that normal body temperature is 98.6 degree F.

High Fever: When It Should Raise Alarm for Parents

Insulin is the most common treatment prescribed for people affected with diabetes. Diabetes is condition where high amounts of glucose prevail in the blood for an extended period of time. However, there are some drugs that offer an alternative to insulin in treating diabetes, which are:

1. Liraglutide: Liraglutide is a glucagon-like peptide-1 (GLP-1) drug that causes the body to release greater amounts of insulin so as to facilitate the movement of glucose from the blood to the cells. People with type 2 diabetes generally use this treatment. It slows the digestion process and can cause symptoms of headache and nausea. 

2. Pramlintide: It's an artificially produced version of amylin. It is taken by both type 1 and type 2 diabetic patients, facilitates slower digestion of food and therefore, controls release of sugar in the blood. It can cause tiredness and nausea.

3. Dulaglutide:This treatment is for people affected with type 2 diabetes and it is administered once a week. It causes more insulin to be released and pass the glucose to the cells. It can cause loss of appetite, nausea and abdominal cramps as side effects.

4. Albiglutide: This treatment facilitates pancreas to release insulin and limits the glucagon hormone production. It is used by type 2 diabetes patients who have not taken well to other treatments. The side effects are skin reaction, respiratory tract infection and nausea.

5. Exenatide: It is a drug that causes pancreas to release insulin that facilitates movement of glucose to cells. It is a treatment for type 2 diabetes and it restricts release of glucagon in the body. The possible side effects are acidity, constipation and vomiting.

Common Insulin Alternatives

The appendix is a small, finger-shaped pouch attached to the large intestine in the right belly area. It is a vestigial organ as it has no specific role to play in humans, but the organ is still seen, though in a very small size compared to the earlier living beings in the evolutionary chain. Acutely inflamed appendix is the most common cause leading to it removal, often seen in the ages of 10 to 19.

Causes: the appendix gets infected by two main reasons - general infection in the abdomen that reaches the appendix or blockage of the appendix leading to inflammation and swelling within it. The appendix is a blind pouch, and there is a good chance for its blockage from food particles, lymphatic tissue, or even sometimes feces. Some of the potential risk factors for appendicitis include a diet low in fiber, high in sugar, gut flora, and family history.

Symptoms/diagnosis: in adults, the appendicitis has very characteristic symptoms including acute pain in the right upper part of the belly associated with fever and vomiting. However, in children, the pain may not be as tell-tale a sign but is still quite diagnostic of appendicitis. However, presence of the following symptoms together is surely indicative of appendicitis.
1. Right abdominal pain, especially rebound tenderness, where pressure placed in the right upper part of the belly and released leads to excruciating pain.
2. Fever, nausea, and vomiting
3. Abdominal fullness or bloating
4. Elevated white blood count (as with most infections)
Additionally, the younger the child, the symptoms are not very clear, but ultrasound will confirm the diagnosis. The inflamed, enlarged appendix will be visible on the images and could be surrounded by free fluid. Ct scan also can be considered if required to confirm the diagnosis.

Treatment: as noted above, children present with symptoms that do not pinpoint to appendicitis. Treatment usually takes two routes:

If diagnosed as appendicitis before rupture, then surgical removal is the best method to contain its symptoms. Other symptoms like fever and nausea and vomiting usually subside a couple of days after the surgery.
If the appendicitis goes unnoticed and ruptures, then the intestinal cavity can get infected, which is called peritonitis. Earlier, the preferred approach was to control the infection and then go for removal. However, lately, removing the appendix followed by antibiotics to control the infection is the preferred approach.
The prognosis and recovery from appendicitis is very good. Deaths have occurred only in very small infants, where they are not able to pinpoint the area and therefore it can go undiagnosed, leading to rupture and subsequent death.

Early identification is the key to proper identification, immediate treatment, and complete recovery from appendicitis.

How to diagnose asthma in children

Asthma in children under the age of 5 is a matter of concern. It is not a rare disease and early diagnosis is important. Common signs of asthma in children are:

1. Recurring bronchitis
2. Wheezing
3. Discomfort in the chest
4. Coughing
5. Trouble breathing

While some children have persistent yet mild symptoms, some children have severe attacks at times. Seasonal allergies and smoke make the symptoms worse.

What to notice?
While feeding an infant, if you notice that your child is facing shortness of breath, it might be a symptom of asthma. In a toddler, if your notice a decreased desire to run and play, or easy fatigue, then this could be a symptom of asthma. Asthma attacks for many children under the age of 5 are worsened when they catch cold or have any other respiratory infections. All these, if noticed should not be ignored and your child's pediatrician should be consulted.

The above mentioned symptoms were mild. Some children have it worse and the attacks can be life-threatening, needing immediate treatment. If your child is under the age of 5 and you see them doing one of the following, contact emergency services immediately:

1. Trouble in speaking because they have restricted breathing
2. When they are breathing in, the effort is so much that the abdomen is getting sucked in
3. Gasping for air.

Diagnosing asthma in a child can be tricky as symptoms like coughing, wheezing, etc can be for other conditions also. Once a child is a little older and if the symptoms persist, breathing and lung function tests are taken to gauge the situation.

  Iyavazhi Eraippu Noi - Both Vata and Kapha accumulate to trigger this kind of asthma. Its symptoms include breathlessness, bloating, indigestion, disorientation, etc.

  Mukkuttra Eraippu Noi - An imbalance of all three humours- Vata, Pitta and Kapha are the culprits behind mukkuttra Eraippu Noi. A person who has contracted it might experience severe chest pain, constipation and even loss of consciousness.

  Mel nokku Eraippu Noi - This is the gravest form of bronchial asthma and requires extensive treatment.

Treatment of Asthma according to Siddha -

Siddha will first try to understand what kind of imbalance has caused your asthma. The medication depends on the type of asthma. Siddha medicines are made from particular parts of herbs that will induce you to either vomit or defecate to get rid of the excess humour that has accumulated in your body.
