Fertility in women can be affected because of various external or internal reasons. It is good to understand the problems since this fast becoming a very common problem. 

The causing factors usually include - 

1. Ovulation problems - One reason for female infertility is when eggs don't mature in the ovaries. This is known as ovulation problems. Possible symptoms of this are absent or irregular periods and the possible solution is keeping a check on body weight, taking fertility drugs and/or having IVF.

2. Uterus problem - There are tissues, known as endometrial tissues which form the lining of the uterus. When they grow outside the uterus, it is called endometriosis and can be a reason for infertility. While some women have no symptoms of this, some have painful periods. Fertility drugs, ivf and surgery to remove the tissue are its possible solutions.

3. Pcos - Pcos or Polycystic Ovarian Syndrome is of late one of the most common reasons of female infertility. It is characterised by unpredictable ovarian patterns and hormonal imbalances. Possible symptoms are acne, obesity, irregular periods, and excessive hair growth. Weight loss, changes in lifestyle, injected fertility drugs, ivf and ovarian drilling are its possible solutions.

4. Damaged fallopian tubes - Some women have blocked or damaged fallopian tubes which prevent the sperm from getting to the egg. This situation leads to stds and pelvic inflammatory diseases. There are no symptoms as such and the methods of solution are surgery to open the tubes and if that is not possible, to remove the tubes and try ivf.

5. Unidentified reasons - There are many unexplained fertility problems where none of the above mentioned problems exist. In such situations, fertility drugs - with or without artificial insemination or ivf are advised.

Hysterosalpingogram: This is an X ray that can identify if a blockage or any other issue in the fallopian tubes are the cause for infertility.
Pelvic ultrasound: This is used to have a look at the anatomy of the female reproductive system and to check for fibroids.
Laparoscopy / hysteroscopy: This is used to get a clearer picture of the ovaries, uterus and other parts of the reproductive system.
Tracking cycle: Ovulation and the thickness of the uterus lining may also affect infertility, This can be diagnosed by tracking and assessing the menstrual cycle.
Tests for men
Semen analysis:This test is used to get an idea of sperm concentration, motility and the appearance of sperm in a man s semen. In some cases, the semen may not include any sperm cells. In such cases, a follow up investigation may be required to assess if sperm cells are being created in the testicles. This is known as testicular sperm aspiration or a testicular biopsy.
Sperm DNA: The DNA in sperm also plays an important role in fertility. This test is used to assess the sperm DNA integrity and to see how it may affect embryo development and chances of an on-going pregnancy.
Infertility treatment depends on the cause identified by the above tests and hence it is important for both partners to undergo complete testing.

4 Things that Cause Vaginal Dryness

Vaginal dryness is an uncomfortable and painful condition that usually occurs during or after menopause. The estrogen level in your body gets reduced during this time which eventually leads to vaginal dryness. However, there are other factors that can trigger this condition including certain medications and immune disorders. This is a problem that can affect women at any age but it is visible more frequently in older women.

Here are some common factors that may lead to vaginal dryness:

1. Hormonal changes - A decrease in estrogen level that occurs during menopause is one of the most common causes of vaginal dryness. The decrease in estrogen may also occur during perimenopause, during breastfeeding or after childbirth. Radiation and chemotherapy for cancer treatment may also result in a decrease in vaginal lubrication.

2. Irritants - Chemicals present in soaps, dyes, hygiene products and perfumes may cause allergy in some women. Irritants present in underwear or towels and other allergens like objects and lubricants that are placed in the vagina may also cause vaginal dryness.

3. Insufficient arousal - Low libido or other sexual problems may also lead to vaginal dryness. Poor sexual performance and early ejaculation may also contribute to it.

4. Anxiety - Sexual desire can be affected by emotional and psychological factors such as anxiety and stress that may eventually lead to vaginal dryness. High anxiety level in women results in insufficient flow of blood, causing vaginal dryness.

How can vaginal dryness be treated?

Topical estrogen therapy is a common method of treating vaginal dryness caused by low estrogen level. Topical estrogen helps in inserting some hormone that your body is no longer producing. This controls the vaginal symptoms to a certain extent but it does not replace as much estrogen as oral hormone therapy, in your blood stream. Lubricants and moisturizers may also be applied to reduce vaginal dryness and make sex less painful and uncomfortable.

4 Things that Cause Vaginal Dryness

HIV is a systemic disease which affects all parts of the body. The oral cavity also has some tell-tale symptoms which indicate HIV / AIDS. A careful examination and detailed history of symptoms is essential. In some cases, the oral manifestations could be the area where HIV is suspected. This can help in reducing morbidity and improves prognosis. The oral lesions that occur in HIV patients can vary and differ significantly in children and adults. While there are a variety of oral lesions in HIV-infected individuals, listed below are some common infections seen in HIV patients. These are a combination of fungal, viral and bacterial infections.

Candidiasis: Candida is an opportunistic fungus that is normally present in the oral cavity and with reduced immunity of HIV, recurrent bouts of the infection begins to show up. It can be in the form of regular thrush which is whitish and cannot be scraped off (pseudomembranous candidiasis), hyperplastic candidiasis (white patches which can be scraped off) or erythematous (reddish patches). Candida can involve any part of the oral mucosa including the pharynx and the palate.
Herpes Simplex: This is the most common viral infection seen in patients with HIV/AIDS. There could be primary or secondary infection of herpes virus, especially inside the mouth and the vermillion border of the lips.
Herpes zoster: This virus, when already present in the body, can be reactivated with HIV/AIDS and with oral herpes. The distinction with herpes simplex is from their distribution. These are unilateral, along the distribution of the maxillary or mandibular nerve. The lesions appear both on the facial skin and the oral mucosa. While the facial ones break open and form crusts, the mucosal ones coalesce to form larger lesions.
Hairy Leukoplakia: This is present in about 20% of asymptomatic HIV patients. Onset of hairy leukoplakia is an indication of rapid progression of HIV with increased CD4 counts. The typical lesion is a non-movable, hairy lesion along the side of the tongue and can spread to the top and the undersurface of the tongue. There are large amounts of Epstein-Barr virus (EBV) identified from biopsies of hairy leukoplakia.
Cytomegalovirus: If the ulcers have a necrotic base with a halo surrounding it, it is CMV infection, usually seen on any oral mucosal surface.
Periodontal disease: This is one of the bacterial infections that manifests itself in HIV patients. It can take two forms such as Linear Gingival Erythema (LGE) which can subsequently lead to Necrotizing Ulcerative Periodontitis (NUP). The oral hygiene is generally good with minimal plaque and there is rapid bone loss and soft tissue reddening and swelling. The, mouth, therefore is certainly a window to one s health.
Diagnosing HIV with Western Blot Test-

It is a series of blood screenings are performed to test for HIV. The enzyme-linked immunosorbent assay (ELISA), also known as an enzyme immunoassay (EIA), is the first test that your healthcare provider will order to screen for HIV. ELISA, like the Western blot test, detects HIV antibodies in your blood. Antibodies are proteins your immune system produces in response to the presence of foreign substances, such as viruses. If you test positive for HIV on the ELISA test, your provider will order the Western blot test to confirm HIV infection. 

Can HIV Be Diagnosed Through Oral Manifestation?

What Are the Types of Female Hypogonadism?

Primary Hypogonadism

Central Hypogonadism or Secondary Hypogonadism
In central hypogonadism, the problem lies in your brain. The hypothalamus and pituitary gland in your brain, which control your gonads, aren't working properly.

Causes of Primary Hypogonadism
1. Autoimmune disorders, such as Addison's disease and hypoparathyroidism
2. Genetic disorders, such as Turner syndrome and Klinefelter syndrome
3. Severe infections
4. Liver and kidney diseases
5. Hemochromatosis, which happens when your body absorbs too much iron
6. Radiation exposure
7. Surgery of your sex organs

Causes of Central or Secondary Hypogonadism include
1. Genetic disorders, such as Kallmann syndrome (abnormal hypothalamic development)
2 Infections, including HIV and AIDS
3. Pituitary disorders
4. Inflammatory diseases including sarcoidosis, tuberculosis, and histiocytosis
5. Obesity
6. Rapid weight loss
7. Nutritional deficiencies
8. Use of steroids or opiates (especially long-term usage)
9. Brain surgery
10. Radiation exposure
11. Injury to your pituitary gland or hypothalamus
12. A tumor in or near your pituitary gland

Symptoms that May Affect Females Include:
1. Lack of menstruation
2. Slow or absent breast growth
3. Hot flashes
4. Loss of body hair
5. Low or absent sex drive
6. Milky discharge from your breasts

Treatment for Hypogonadism in Women
Your treatment will involve increasing the amount of female sex hormones in your body. Your first line of treatment will probably be estrogen therapy if you've had a hysterectomy. Either a patch or pill can administer the supplemental estrogen hormone. Estrogen must be balanced with progesterone, because taking estrogen alone leads to a dangerous build up of the lining of the uterus (endometrial hyperplasia) that can lead to cancer of the uterus. Low-dose testosterone may be added for women with hypogonadism who have a low sex drive.
