Hi I have a problem in my right shoulder. The pain is persistiall along. I find difficulty in lifting my hand above the shoulder level. A sharp pain in the joint of my arm and shoulder. I cannot sleep on my right side cause of the pain.

Hi thanks for your query. Shoulder pain at this age can be due to shoulder pathology or can come from degenerative discs in neck. You also have a restricted shoulder movement, can be due to adhesive capsulitis or frozen shoulder, degenerative rotator cuff etc. My advice, start intensive physical therapy in presence of a physiotherapist and see if there is any improvement, if not then we will have to look at getting some investigations done for you. If you have localized back pain with - no neurological symptoms like numbness, paresthesia, weakness in limbs etc, my advice is: - intensive physical therapy initially under supervision of a physiotherapist and then at home. - avoid forward bending, lifting heavy weights etc - take an analgesic like diclofenac or something which suits your body for 5-7 days - take muscle relaxant for same time as analgesic to relieve muscle spasm - ice packs at the site of pain, muscle spasm - local analgesic spray can be used but no massage is required let me know how you are feeling. I will actually ask you to get x ray of involved region of spine (ap & lateral) and send reports across to me so that I can advise further in a better manner. Psychological disturbances may cause the lack of sleep (insomnia), if it is so then you need to come out of this condition, stay away from anxiety, depression, or some time it could be non specific may be age related, try some hot milk prior to bed, try some exercise, yoga, relaxation therapy and no need to worry about this.

After writing some notes my hand getting pain don't know the reason I wrote regularly but now a days I feel pain I used bams & pain relief but non of them gives me relief. What to do now.

I understand your pain. Take some alum and mix well in hot water. Immerse your hand and take hot fermentation for 15 min thrice a day. When your hand is in water do some exercises like making fist open and then individual finger movements. This will give you relief from your pain and discomfort. You can also take laser and ift at your nearby physiotherapy center. For any doubt please contact. Make a habit not to write in countinous. 8 Risk Associated With Heel Bone Spurs!

Heel bone spur is a form of calcium deposit that causes a bony protrusion under the heel bone. An X-ray can reveal up to a half inch elongation under the hill. Without image report, this condition is commonly known as heel spur syndrome. Heel spurs are mostly painless but reports of pain in not uncommon. They are often related to plantar fasciitis. The latter is an inflammation of the connective tissue that stretches through the foot bottom connecting the heel bone and the football.

What causes heel spurs?
Heel spurs are a result of prolonged calcium deposit. This condition can result from the heavy strain on the muscle of the foot and ligament, stretching of fascia and wear and tear of the heel bone membrane. These injuries are frequently observed among athletes who are involved with activities such as jumping and running. 

What are the risk factors?
1. Walking abnormalities that involve putting more than normal stress on the bone, nerve and ligament in and around the heel.
2. Running on surfaces that are hard in nature
3. Shoes lacking arch support
4. More than normal body weight 
5. Spending too much time on the feet
6. Too flat or too high arches
7. A person suffering from diabetes
8. In case the protective pad of the heel is fading away due to old age or other bone disorder


Non-Surgical treatment
Unlike common belief, only rest may not be the best way to treat heel bone spurs. On the contrary, a patient might feel sharp pain immediately after sleep. This happens when he tries to walk and the plantar fascia elongates all of a sudden. The pain decreases with more walking. Some treatment methods that work for 90 percent of the sufferers includes wearing the right shoe, stretching exercises, wearing orthotic devices inside the shoes and physical therapy. Over the counter medicine such as Aleve, Tylenol and Advil can be consumed to reduce the pain and for improving the overall condition. Corticosteroid injection also tends to give relief from the inflammation.

Surgical treatment
If heel spurs persist for more than 8-9 months, surgical options should be explored by the patient. There are two angles on which a doctor works, either removing the spur or release the plantar fascia. Pre-surgical exams are necessary to ensure that a person is eligible for surgery and all non-surgical avenues are explored. Post-surgical activities are equally important for the process of healing. Usage of bandages, crutches, splints and surgical shoes is a mandate to avoid complications such as infection, numbness, and scarring. Possible side effects should be discussed with the surgeon well before the surgery. The estimated healing time from this procedure is close to 8-12 weeks.

I have been suffering from sleep apnea since last 7-8 years when I used to wake up early in morning 3-4 am with bad dream and huge headache along with weight loss . One of the top doctor at Sir Gangaram Hospital advised me Rivotril 0.5 mg for faster relief which did give me sleep also allowed me to regain weight over 5-6 years. But I wanted to get rid of my dependence on sleep medicine and went to a sleep specialist who advised me sleep test. Report indicted I am having excessive snoring with oxygen deprivation which is disturbing my sleep. My AHI is 11.6 /hr with minimum oxygen saturation at 91%. He advised use of CPAP m/c on trial for 7 days. I could use CPAP only for 3-4 days as it puts me into inconvenience specially my upper jaw becomes painful and tight in the morning besides other inconvenience of using this mask. While morning walk and meditation has allowed me to get rid of medicine for last 10 days. I still get restless sleep after 3-4 am and headache in morning and not completely cured. I had front upper 3 teeth crowned (middle teeth has to be removed and 2 side teeth crowned in order to have front look intact) around same time (8 years back) and feel the less gap in 3 crowned front upper teeth and raised portion could be troubling me for lack of oxygen or when I wear mask for CPAP. Can you please advise what should I do as this problem is spoiling my health and I am determined to get rid of this.

Never use this medicine Some tips for sound sleep and early awaking in morning follow advises given below: 1. Strict to sleep time schedule 2. Do regular physical activity, you can go for daily morning walk 3. Regular meditation/ yoga 4. Avoid tea coffee after 7 pm 5. Take bath at bedtime 6. Avoid daytime sleep 7. Use alarm and gradually reduce your waking time by 15 minutes 8. Avoid watching TV and reading news paper at bedtime, you can listen some soft music
