Dear, low back pain are often referred for physical therapy for four weeks as an initial conservative (nonsurgical) treatment option before considering other more aggressive treatments, including back surgery. The goals of physical therapy are to decrease back pain, increase function, and teach the patient a maintenance program to prevent future back problems. Common forms of physical therapy include: 1.Passive physical therapy (modalities), which includes things done to the patient, such as heat application, ice packs and electrical stimulation. For example, a heating pad may be applied to warm up the muscles prior to doing exercising and stretching, and an ice pack may be used afterward to sooth the muscles and soft tissues. OSee Physical Therapy: Passive PT (Modalities) for Back Pain 2.Active physical therapy, which focuses on specific exercises and stretching. For most low back pain treatments, active exercise is the focus of the physical therapy program. This article focuses on active physical therapy and exercise as a means to help recover from back problems and prevent or minimize future flare-ups of low back pain Low back pain has lasted between two and six weeks, or if there are frequent recurrences of low back pain, physical therapy is often recommended. Some spine specialists consider physical therapy sooner, particularly if the pain is severe. •Read more: Exercise and Back Pain In general, the goals of physical therapy are to decrease pain, increase function, and provide education on a maintenance program to prevent further recurrences. It could be due to peripheral neuropathy, muscle strain, dehydration, over exertion, compression of nerve as in lumbar stenosis, mineral deficiency (calcium, Magnesium,Potassium), Treatment 1.Take good nourishing diet with plenty of green leafy vegetables and, fruits 2.Drink plenty of water with electral/ORS powder 3.Take multi vitamin supplement like one capsule of polybion daily 3. Massaging cramped muscle, Take pain killer like one tablet of crocin pain relief sos after food up to a maximum of three tablets per day 4.Hot fomentation by heating pad, warm water bath 5. Stretching exercise etc 6. Take adequate rest, avoid over exertion and dehydration 7.Consult physician for further advice and treatment.

My wife is having Parkinsons. No trembling but stiffness in muscle, difficulty in walking, speech, writing. Taking treatment with syndopa 900 mg/daily for the past six years. What is your suggestion to get relief / improvement . Siddha, Ayurvedic, homeopathy or any alternate treatment is ok ?

Syndopa is the best medicine available for Parkinson's disease. However the dosing and dosage interval should be individualized based on the need. Even improper dosing and dosage interval may produce poor control. Also there are many other drugs available which could helpful if added to Syndopa judiciously. There is a surgical procedure called DBS is available which can give better result in carefully selected patients.

Ok, please try for the laser therapy 6 to 10 session may do some change for your back pain please don't discontinue your spinal extension exercises planks, core strengthening exercises, hamstring stretching, standing balance on stability disc or you may try for seragem for one month.

Sports Taping- stretch the tape from both ends and apply on the affected area Contrast Fomentation (Hot and Cold). Contrast bath---It is a form of treatment knee is applied warm water followed by the immediate application of the knee with ice water. This procedure is repeated several times, alternating hot and cold. Note that the treatment should always end in the ice water, as heat will induce the body's inflammatory response, while cold helps to decrease inflammation.

In a bike accident on 8/7/2016 my right lower femur fractured. There was no other injuries. I have been operated on 22/7/2016 with internal fixation plate. I was discharged on 6.8.2016 from hospital after cutting of my stich. In first review x-ray there was remarkable improvement in healing of the bone and callus were noticed. Surgeon advised my for non weight bearing for 02 more months. He also advised me for quadriceps drill and knee bending exercise. But I am facing problem in bending my knee. I feared in case of bending the bone get non union again. Is there any chance of non union during knee bending? Can physiotherapy help be taken for knee bending exercise. As I still in bed rest rest and using bed pan and urine pot for toilet. Can I use walker for going to toilet, bath and slow moving in the home at this stage? How time is require for complete healing of the bone? WHEN I COULD RETURN TO MY JOB AS MY JOB IS CLERICAL NATURE RAILWAY JOB.

As your fracture is already fixed ,you need not worry about displacement of fracture or of non union at all. After any fracture fixation ,It is advised to start active range of motion exercises as early as possible. This prevents the joints from getting stiff, increases blood supply to the fractured bone and thus helps in better healing of bone. This movement exercises do not hamper callus formation in any way but in fact they encourage callus formation and early recovery. If the fixation is good you need not worry about the bending of bone on exercising. Taking physiotherapy helps, but ask you physio to be gentle You should use walker for your daily activities without putting weight on the operated limb. You can take a physio help for that.

I had a hair line facture on my left elbow. I happened on march 31st. I have gained some weight which is excess for my height. From when I get back to gym i'm very much because i'll be getting married by this year end I am really worried about it which is making me feel stressful.

You can restart your gym. Avoid those exercises which cause pain in injured elbow. Go for marriage with out any tension.

Heel Pain: Know its Causes and Treatments

Oh my aching Heel!! The combination of foot and ankle consists of 26 bones, 33 joints and an excess of 100 tendons. The longest bone present in the foot is the heel. Over usage or injury to the heel would lead you to experience pain in the heels. The severity of the heal pain can range from mild to excruciating. 

The cause behind heel pain is typically associated with excess usage of the heel bone (learn more about to maintain bone and joint health). Straining of your heel occurs if you excessively pound your feet on surfaces that are hard, or you are overweight or even if you wear shoes that do not fit your feet properly. These strains cause irritation in the muscles, bones or tendons that are present in the heels. Some of the other common causes include: 

  1. Heel Spurs: Development of heel spurs occurs due to the continuous stretching of the lining, which covers the heel. Upon continuous stretching, pieces of the lining tend to break off. Development of heel spurs is common in athletes who frequently jog or run. People who are obese also tend to experience pain due to heel spurs.
  2. Plantar Fasciitis: Development of Plantar Fasciitis occurs due to the inflammation of the tendinous tissue that connects the heel to the ball of the foot. Development of Plantar Fasciitis is common in athletes who frequently jog or run as well as people who wear shoes that don't fit the feet properly. It may occasionally be associated with Vitamin B12 deficiency and raised uric acid in blood 
  3. Achilles Tendinitis: This is the inflammation of the Achilles tendon. Achilles Tendinitis occurs commonly in people who have active lifestyles, such as people who are joggers, runners or even dancers. 

Treatment-

Physical therapy, medication or alternative treatments can be used to treat this form of pain. Medications for heel pain can only be in the form of anti-inflammatory drugs. Although these oral medications cannot significantly relieve heel pain, some injections containing substances such as corticosteroids, as well as Arthrocentesis (removal of fluid present in joints using hollow needles) might be able to provide relief. Recently PRP (Platelet Rich Plasma) injection have been found very useful in treating recalcitrant Plantar fasciitis and heel pains. 

Common practices that can be used to relieve discomfort include: 

  1. Application of ice to the heel for a duration of 15 minutes daily. 
  2. Utilize over-the-counter pain medications .
  3. Have adequate amounts of rest .
  4. Wear shoes that fit properly .
  5. Walking on tip toes to stretch the plantar fascia helps to relieve heel pain.

Advice…. Hot Fomentation x twice daily. Avoid bending in front. Postural Correction. Sit Tall, Walk Tall. Extension Exercises x twice daily. Bhujang Asana x twice daily. Back Stretching.

Core strengthening exercise- straight leg raised with toes turned outward, repeat 10 times, twice a day. Hams stretching- lie straight, take the leg up, pull the feet towards yourself, with a elastic tube or normal belt. Repeat 10 times, twice a day. Quadriceps exercises- lie straight, make a towel role and put it under the knee, press the keen against the role, hold it for 20 secs. Repeat 20 times twice a day. This will help relieve some pain. Keep your leg raised while sitting or lying quadriceps strengthening exercises- quad clenches: lie flat on your back or sit upright on a chair with leg kept horizontally on another surface. Now, tighten the muscle on the front of the thigh by pushing your knee down. You should feel your thigh muscles clench, hold for 3 secs. Repeat 10 times twice a day. Short arcs: lie flat on your back or sit upright with your leg placed horizontally on a flat surface like a chair or bed. Place a rolled up towel under the knee. Pull your toes towards you and clench you thigh muscles. Slowly lift your foot up off the bed until your knee is straight (keep your knee resting on the towel). Hold for 3 secs and slowly lower them on the chair. Repeat 10 times twice a day. Straight leg raise: lie flat on your back. One leg and knee will be straight and other leg should be bent. Pull your toes towards you and tighten/clench the muscle on the front of the thigh, locking your knee straight. Lift your foot up in the air, about 6 inches off the bed. Hold for 3 secs and slowly lower the leg. The knee must remain straight the whole time you are doing this exercise.
