Is Surgery The Best Option For Treating Breast Cancer?

MBBS, MS- General Surgery
General Surgeon,
Is Surgery The Best Option For Treating Breast Cancer?

Breast cancer is the most common cancer that affects women. It is also deemed to be one of the major reason of death among women, though lung cancer is the leading cause of this. The chances that a woman may die of breast cancer is about 2.7% of the women who are suffering. Breast cancer is known to affect men as well, in certain cases. Breast cancer may be genetic but, in certain cases, it is caused due to excess intake of alcohol. Surgery is an option, but treatments also include therapies like radiation and chemo.

Treatment for breast cancer
Breast cancer can be treated in several ways; however, the treatment depends upon many factors. These factors include the type of breast cancer, its stage, sensitivity of a person to the hormones, age of the patient, health of the patient and most importantly, preferences of the patient. The most common treatment options for breast cancer are radiation therapy, biological therapy, chemotherapy, hormone therapy, and surgery.

Chemotherapy
Cytotoxic drugs are important medications that are used to eliminate cancer cells. This medication is used if the chances of the cancer cells spreading are high. This process is known as the adjuvant chemotherapy.
Many a time, the tumor increases in size and surgery becomes essential. This is when the chemotherapy is administered to reduce the tumor size and make it easier for the surgeons to remove the tumor. This process is referred to as neoadjuvant chemotherapy. Chemotherapy can treat metastasized cancer or cancer that has affected other body parts, thereby reducing the symptoms, mostly in the advanced stages.

Radiation Therapy
Radiation is targeted towards the tumor in controlled doses so that the cancer cells are destroyed. Radiation therapy is mostly advised to the patients after they go through surgery, to eliminate the cancer cells that may still be there.

Hormone blocking therapy
Most people go through breast cancer that is hormone sensitive. The hormone blocking therapy helps in preventing the recurrence in this type of breast cancer. The hormone blocking therapy is also used to reduce the size of the tumor, but this method is also suggested after a person goes through surgery.

Biological treatment
There are some drugs that fall under this category that is used for destroying the various types of the breast cancer.

Surgery
If at all the patient requires surgery, it depends on his or her diagnosis reports and his or her will to undergo surgery. The types of surgery include Lumpectomy, Sentinel node biopsy, Mastectomy, Axillary lymph node dissection, and Reconstruction. Reconstruction is the method which the doctors make use of to make both the breasts look similar after the surgery is complete. There are several treatment options available to treat breast cancer, and it depends on the choice of the patient to opt for surgery. Surgery helps in eliminating the risk of the spreading of the cancer cells, and it is one of the most effective treatments for breast cancer.

Spinal Cord Injury: Know How To Prevent It!

Spinal Cord Injury: Know How To Prevent It!

The spinal cord is like a cable consisting of millions of nerves that transmit messages in the form of electrochemical signals from the brain to the rest of the body, and also sensations from the body back to the brain. We are able to perceive pain and move our limbs because of messages sent through the spinal cord.

The spinal cord is soft and vulnerable to injury. It is protected by the bony structure of the vertebral column. A spinal cord injury can happen if there is a fracture of the spine. Sometimes, even if the vertebral column is intact, violent shaking can cause cord contusions. It’s an extremely serious type of injury that is likely to have a lasting and significant impact on most aspects of daily life.

If the spinal cord sustains an injury, some or all of these impulses may get blocked. The result is a loss of sensation and mobility below the level of injury. A spinal cord injury closer to the neck will typically cause paralysis throughout the body, while one in the lower back may affect the legs but spare the hands.

Causes:

A spinal cord injury is often the result of violent trauma. Events like spontaneous hemorrhage, infection, tumors or autoimmune diseases can also cause spinal cord damage. Some causes of traumatic injury to the spinal cord are :

  • trauma during a car accident (specifically, trauma to the face, head and neck region, back, or chest area)
  • falling from a significant height
  • head or spinal injuries during sporting events
  • electrical accidents
  • a violent attack such as a stabbing or a gunshot
  • Diving head first into water that’s too shallow and hitting the bottom

Symptoms-

Some symptoms of a spinal cord injury include:

  • Neck or back pain
  • Pain radiating along limbs, or numbness/ pins and needles sensation along the arms or legs.
  • Weakness of particular muscle groups in focal injury, to complete paralysis in severe injury.
  • Clumsiness during finer actions using the hands.
  • Unsteadiness or loss of balance while walking.
  • Loss of control of the bladder or bowels You must take immediate precautions

If there is the slightest suspicion that someone has a back or neck injury:

  • Call 911 or your local emergency medical assistance number
  • Do not move the injured person – permanent paralysis and other serious complications may result.
  • Place heavy towels on both sides of the neck or hold the head and neck to prevent them from moving until emergency care arrives •    Provide basic first aid, such as stopping any bleeding and making the person comfortable, without moving the head or neck
  • If movement is absolutely necessary, arrange a cervical collar, then log-roll onto a stiff spine board or flat surface, taking care that all parts of the body move together and that any relative movement between one part of the spine to the next is avoided.

Prevention-
Because spinal cord injuries are often due to unpredictable events, the best you can do is reduce your risk. Some risk-reducing measures include:

  • always wearing a seatbelt while in a car
  • wearing proper protective gear while playing sports
  • never diving into water unless you’ve examined it first to make sure it’s deep enough and free of rocks
  • increase protective measures to avoid falls from height

Treatment:
Seek the opinion of a Neurosurgeon or an Orthopedic spine surgeon as soon as possible. X-rays, CT scan or MRI scans may be advised for assessing bony or soft tissue injuries of spine. Minor injuries require only immobilization and rest. Severe injuries, however, may require steroid injections and surgical intervention. Time is of essence, and an early surgery in certain situations may save a limb that would otherwise be paralyzed for life.

Prolapsed Uterus - 7 Conditions That Can Cause It!

DGO, MBBS
Gynaecologist, Faridabad
Prolapsed Uterus - 7 Conditions That Can Cause It!

The uterus is set up inside the pelvis with different muscles, tissue, and tendons. Due to pregnancy, labour or complicated delivery, in a few ladies, these muscles tend to weaken. Additionally, as a woman ages and with a loss of the hormone oestrogen, her uterus can drop into the vaginal canal, bringing about the condition known as a uterine prolapse.

The following conditions can bring about a prolapsed uterus:

  1. Pregnancy with complicated or even ordinary delivery through the vagina.
  2. Weakness in the pelvic muscles with the increase in age.
  3. Weakening and loss of tissue after menopause and loss of normal oestrogen.
  4. Conditions leading to increased weight in the abdomen, for example, constant coughing (with bronchitis and asthma), straining along with constipation, pelvic tumours (uncommon), or clogging of liquid in the abdomen.
  5. Being overweight puts extra strain on pelvic muscles.
  6. Previous surgery in the pelvic region leading to loss of outer support
  7. Smoking

However, this condition can be treated effectively:

  1. Self care at Home: You can strengthen your pelvic muscles by performing Kegel workouts. You do these by contracting your pelvic muscles, as though attempting to stop the stream of urine. This exercise makes the pelvic muscle strong and gives some support. Have your specialist train you on the best possible approaches to exercise your muscles.
  2. Medications: Oestrogen (a hormone) cream or suppository ovules or rings embedded into the vagina help in strength and endurance building of the tissues in the vagina. However, oestrogen is just for use in some of the postmenopausal ladies.
  3. Surgery: Based on your age and whether you wish to conceive a child naturally, surgery can repair the uterus or remove it. The uterus as in many cases can be expelled with a hysterectomy. During the surgery, the specialist can repair the hanging or saggy vaginal dividers, urethra, bladder, or rectum. The surgery might be performed by an open abdomen procedure, through the vagina, or through little cuts in the abdomen or vagina with particular instruments.
  4. Other Therapy: In case that you do not need surgery or have a poor possibility for surgery, you may choose to wear a steady gadget, called a pessary. It is worn in the vaginal tract to strengthen the falling uterus. It can be utilised briefly or can be long-lasting. They come in different shapes and sizes and should be fitted according to the person. In case that the prolapse is extreme, a pessary may not work. Prolapse surgery is always preferred by vaginal route. Likewise, pessaries can disturb the insides of the vagina and may bring about a noxious release.

It can be prevented in the following manner:

  1. Decrease your weight.
  2. Try to avoid constipation by consuming a high-fiber diet.
  3. Do Kegel exercises to strengthen the pelvic muscles.
  4. Stay away from heavy lifting or straining.

Hello kya neck k 2 ct scan se breast k cancer ka risk increase hota h kya. Please provide accurate answer. thanks.

MBBS, MS - General Surgery, FBD (Fellowship in Breast Diseases), UICC Fellowship
Oncologist, Pune
Radiation induced cancer won't come with doing 1 or 2 ct. It is basically related to some kind of radiation treatment in the past like treatment for Hodgkin's lymphoma or long term exposure to radioactive compound for years. So unlikely to get Breast cancer with 2 ct scans of neck. Don't worry.

Diseased Uterus - Can it be Replaced?

Diseased Uterus - Can it be Replaced?

In case you have a diseased uterus, which makes you infertile, you can undergo a procedure known as uterus transplant or uterine transplant to get pregnant. In the process of sexual reproduction, a diseased uterus does not allow embryonic implantation. This factor is referred to as uterine factor infertility or UFI. As a result, you will not be able to get pregnant. Learn more about having healthy pregnancy.

Who requires a uterus transplant?
This procedure involves women who have UFI and women who had their uterus removed by hysterectomy. Women who have a damaged uterus on account of an injury or infection, which does not function anymore, can also undergo a uterus transplant procedure. Women from the age of 21 to 45 are eligible for this procedure. Many women are born without having a uterus. This condition is called Mayer-Rokitansky-Küster-Hauser syndrome.

Procedure
Uterus transplantation begins with undertaking a uterus retrieval surgery on the uterus donor. The uterus, which is recovered has to be stored and transported to the location of the patient undergoing the transplant. An ischemic tolerance may last over 24 hours. Three major surgeries have to be carried out with the recipient. Firstly, a transplantation surgery is required in which the donor’s uterus gets transplanted. In case pregnancy develops, a caesarean section surgery has to be performed. The patient is given immune suppressive therapy. After childbirth, a hysterectomy is done in order to terminate the immune suppressive therapy.

Will the women be able to get pregnant after having sex?
Women receiving a uterus transplant will not be capable of becoming pregnant without undergoing fertility treatments. The transplanted uterus is not connected with the fallopian tubes, which is the location of the normal fertilisation process. The women will require to carry out IVF or in vitro fertilisation to become pregnant after a uterus transplant. IVF is a process in which the eggs are removed from the ovaries and get fertilised in a laboratory. Then, they are implanted in the uterus. After undergoing the uterus transplant procedure, a woman has to wait for a period of one year. The uterus requires time for healing and after recovery, the embryo may be implanted for pregnancy. After giving birth successfully, a woman will be able to keep the transplanted uterus. This is a reason why IVF is a popular procedure.

She has the option to get pregnant again. However, after giving birth twice, a hysterectomy must be carried out for the removal of the uterus. This is done so that the woman can stop using the immune suppressant drugs, which are associated with major risks. Uterus transplant is a relatively new technology. In October 2014, the first healthy baby was born to a woman who had undergone a uterus transplant. This surgery is kind of experimental in nature and is usually the last option for getting pregnant.

Lung Cancer and Outdoor Air Pollution - Understanding The Risk!

Lung Cancer and Outdoor Air Pollution - Understanding The Risk!

Lung cancer is the cancer killer of both men and women in the U.S. When you think of risk factors for lung cancer, what comes to mind? Most of us think about the risk associated with smoking cigarettes, but did you know that air pollution can also cause lung cancer? Overwhelming evidence shows that particle pollution in the outdoor air we breathe—like that coming from vehicle exhaust, coal-fired power plants and other industrial sources can cause lung cancer. Particle pollution increases the risk of dying early, heart disease and asthma attacks, and it can also interfere with the growth and function of the lungs.

What is particle pollution?

Particle pollution is a mix of tiny solid and liquid particles in the air and can be made up of a number of components, such as acids, organic chemicals, metals, soil and dust particles. It can be emitted directly from wood stoves, forest fires, vehicles and other sources, and it can also form from other types of pollution that come from sources like power plants.

Why does particle pollution harm our bodies?

While breathing in larger sizes of particle pollution can be harmful to our health, smaller particles are more dangerous. Bigger particles can irritate your eyes, nose and throat, but our natural defenses help us to cough or sneeze them out of our bodies. Unfortunately, those defenses don't keep out smaller particles, which get trapped deep in the lungs and can even get into the bloodstream, causing damage to our health.

Who is most at risk?

As for who is at risk for health problems from breathing in particle pollution. Those who live where particle pollution levels are high is at risk. Some people face higher risk, including children, the elderly, people with lung and heart disease and diabetes, people with low incomes, and people who work or exercise outdoors.

How do we protect ourselves from particle pollution?

Check the air quality index forecast for the day and limit your activity if pollution levels are high. Avoid exercising along heavily traveled highways regardless of the overall forecast.

As individuals, we can play our part in reducing air pollution levels by trying to avoid creating more of it. Choosing ‘active travel’ options where possible, like walking and cycling, can help reduce pollution levels from transport and is also a great way to be more active, which is linked to a reduced risk of cancer and other diseases.

5 Super Brain Drinks That Boost Your Memory

Beet juice: apart from being a nutritional powerhouse, beet juice is a great source of nitric acid. The latter helps with blood flow in the body. A smooth blood flow, on the other hand, helps the brain in functioning properly. A study has found that people who consume beet juice have an increased circulation in the white matter resulting from an increased brain activity. This juice is sweet in taste and can be consumed every alternate day.

Berry smoothies: acai is a fruit which has a south african origin and hits the top of the list when it comes to brain assisting food. It increases the flow of the blood and dilates the blood vessels. However, this is extremely expensive. Berries also have a similar effect on the brain, but they are less powerful in nature. What can be done is to mix a little ingredient of acai in frozen berries juice and drink the same every day. Frozen berries retain the freshness and the nutritional benefits. This drink will improvise your brain and increase blood flow in the body.

Pomegranate juice: pomegranate is a great antioxidant and it very aptly limits the free radicals from damaging the brain cells. Antioxidants are also credited with increased blood flow and protection of the heart. A healthy cardiovascular system is synonymous to an increased brain function. What is surprising is the fact that pomegranate has more antioxidant content compared to red wine and green tea. Pomegranate juice can be consumed daily during the morning or afternoon.

Carrot juice: carrot is known for its healthy components that are beneficial for the eyes. Recent studies have revealed that they are great for the brain too. Like other colored vegetables, they are a great source of antioxidant and have a high content of beta-carotene. Carrot juice can reduce the inflammation of the brain cells and improve memory. Carrot juice is not at all bitter and the same can be consumed every alternate day after your meal.

Cocoa: a harvard study revealed that two cups of cocoa on a daily basis greatly helps a person in improving memory. Cocoa contains flavanols that keep the blood pressure normal and relaxes the blood vessel linings. Cocoa is also known to be a great antioxidant agent that limits the free radical to affect the brain cells. Cocoa should not be overloaded with sugar and should be consumed hot before starting your day.

Uterine Prolapse - 6 Most Common Risk Factors!

MBBS, DGO, Ceritification in Minimal Invasive Surgery
Gynaecologist, Ghaziabad
Uterine Prolapse - 6 Most Common Risk Factors!

Uterine prolapse is a condition which occurs due to a weakness in the muscles and ligaments of the pelvic floor which are responsible for supporting the uterus. This condition results in the bulging out of the uterus from the vagina. The continuous stretching in the muscles of the pelvic floor due to the lifting of heavy things might initiate uterine prolapse. If you suffer from severe uterine prolapse, the doctors might suggest laparoscopic surgery to get rid of the pain. Lack of diagnosis might form infected ulcers in the pelvic regions.

Risk Factors
The risk factors that can trigger a uterine prolapse are as follows:

  1. You are more prone to suffering from uterine prolapse as you age because aging causes a significant reduction in the estrogen (the hormone responsible for the overall development of the pelvic muscles) levels of the body.
  2. If your pelvic muscles and the adjoined tissues have suffered sustained damage during pregnancy or childbirth, you have a higher risk of suffering from uterine prolapse.
  3. Obesity, sustained episodes of constipation or coughing make you more vulnerable to uterine prolapse.
  4. In women, estrogen levels reduce significantly after menopause.
  5. A woman who has undergone vaginal childbirth multiple times can suffer from uterine prolapse.
  6. Having undergone surgery in the pelvic region and experiencing severe pain during excretion raises the risk levels even more.

Symptoms
The symptoms of uterine prolapse vary with the severity of the condition. If the prolapse is a minor one, there will be no symptoms. But severe uterine prolapse might bring with it the following symptoms:

  1. Severe pain in the lower back region along with significant problems during urination such as leakage of urine.
  2. Bulging out of tissues from the vagina.
  3. Recurrent instances of constipation and abnormal discharges from the vagina.
  4. Infection in the bladder and experiencing severe problems during sex intercourse.
