I have a back pain. How minimize it.

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist,
I have a back pain. How minimize it.
Hi, thanks for your query and welcome to lybrate. You are very young to actually have persistent back pains. If you have localized back pain with no other symptoms like radiating leg pain, neurological symptoms like numbness, weakness, paresthesias etc and with no other associated constitutional symptoms like evening rise in temperature, loss of weight, excessive sweating at night etc, then you can start with following recommendations for initial period of 2-3 weeks: - to maintain proper posture of your back while working and sleeping - if pain is more then you can take a short course of an anti-inflammatory medication like tab etoshine or paracetamol or one which suits you - physical therapy initially under supervision of a trained physiotherapist and then to continue at home - adequate calcium & vitamin d intake if levels are low in body - ice packs can applied if your pain is acute, then hot fomentation can be done at home - analgesic spray for local application can be used and is easily available. - avoid lifting heavyweight/ acute forward bending in mornings etc we will observe you for next 2-3 weeks how you respond to this conservative management protocol. If you are not feeling better, then we will have to get some investigations like, dynamic x rays of lower back and few blood tests for evaluation. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

I am 28 years male had a fall 6 months ago and sustained acl injury what should I do.

I am 28 years male had a fall 6 months ago and sustained acl injury what should I do.
There are two ways to treat ACL injury. Treatment depends upon severity of injury and resultant laxity. Physiotherapy is done for muscle strengthening and if there is improvement to such an extent that patient doesn't feel any instability then well. if patient doesn't feel he had stable knee, treatment then becomes Arthroscopic reconstruction of ACL. there is no way to repair ACL.

Hi I have read this and it means that there must be some subchondral sclerosis over the lateral tibial condyle region, which means that there could be some early degeneration there, but I want to ask whether this x ray taken was a weight bearing one or not, and also can you kindly send me the films so that I can see and let you know what needs to be done. Do not hesitate to contact me if you need any further assistance.

Dip. SICOT (Belgium), MNAMS, DNB (Orthopedics), MBBS
Orthopedist, Delhi
Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. There are many causes of pain in lower back region. It can be due to faulty posture, related to weak muscles, issues with intervertebral disc, narrowing of spinal canal, any bony instability besides many other, so accurate diagnosis is the key to proper management of the condition. You will have to elaborate your problem as to a detailed discussion of your back pain which means duration of pain and mode of onset. I would also like to ask you if it is associated with any leg pain or any other neurological symptoms like numbness, paraesthesias (electrical shock like abnormal sensations) or any weakness in limbs. Please also tell me about any associated symptoms like fever, weight loss etc. What medications you generally take and what has been your treatment history till now for existing problem. Do not hesitate to contact me if you need any further assistance. You can also discuss your case and treatment plans with me in a greater detail in a private consultation.

Heel pain as in your situation is most likely due to plantar fascitis. It is due to inflammation in the tough fibrous tissue called plantar fascia in sole extending from heel to base of toes. The treatment mostly is conservative and lifestyle modifications to achieve maximum functionality with minimum pain. Follow these lifestyle changes religiously to get benefit. 1. Reduce body weight as obesity will aggravate the stress on heel. 2. Avoid exercises like running / walking on hard surfaces. Stop such activities when you start developing pain. 3. Roll your feet on bottle of cold water for 20 mins 4 times a day or can apply ice pack. 4. Use shoes with silicone sole inserts. 5. Do calf muscle and heel stretches daily. 6. Avoid prolonged standing or sitting. 7. Use night splint to keep foot flexed at night. Stop using it once pain is relieved. 8. Use analgesics like Ibuprofen for decreasing pain and inflammation. With proper lifestyle changes as above 90 % patients have relief and functionality. Injections in plantar fascia are done only after conservative treatment as above has failed.Only 10% patients would require surgical intervention like plantar fascia release or gastrocnemius recession. For further queries consult again
