I have constant heartburn. What could be the reason for this?

AUTLS, CCEDM, MD - Internal Medicine, MBBS
General Physician,
Here are tips to prevent acidity/heartburn/gerd. -relax and eat food slowly take time to eat- don't rush. Avoid skipping breakfast or lunch. - eat smaller meals. Large meals fill the stomach and causes reflux. Don't overeat -limit fluids intake during meals. As it increases stomach distension. -avoid foods and drinks that trigger acidity (e. G. Fatty, spicy foods, onions, garlic, peppermint, chocolate, coffee, tea, citrus fruits or juices, tomatoes, cola or alcohol) -stop smoking. -avoid alcohol. Try meditation, stretching or deep breathing instead of drinking alcohol. Strive for a less stressful life. Stress may increase stomach acids -wear loose-fitting clothes. Don't wear tight clothes or belts that constrict your stomach. -lose weight. Extra fat places pressure on abdomen, pushing gastric juices up -incorporate moderate exercise. Such as walking. It keeps you upright, allowing gravity to aid digestion -don't lie down after eating. Wait at least 2-3 hours after meal before lying down or exercising. Avoid late night snacks -elevating the head end of bed by 6-8 inches keeps gastric acid down in your stomach. Sleep in left lateral position. -review your medications. There are certain medications that can increase risk of gerd: like pain relievers/ antibiotics/iron tablets home remedies: -chewing gum after meals. Stimulates saliva production, which neutralize stomach acid. But choose non-mint flavors, since peppermint can exacerbate symptoms. Herbal tea improves digestion and soothe gas and nausea. Take low fat milk. -dried ginger, dried fennel or ripened guava can speed up digestion of food -non-citrus juices-citrus drinks such as orange, tomato juice, grapefruit, and pineapple juice are very acidic and can cause acid reflux. Other juices like apple, carrot, aloe vera, and cabbage - are less acidic and thus less likely to trigger gerd symptoms. -gerd medications: antacids: for mild symptoms, like sucramal/digene/mucaine gel, may be enough to give you relief. Other antacids, such as gaviscon, create a protective foam barrier that coats the stomach proton pump inhibitors when heartburn become more severe these reduce acid in the stomach. Ie: omeprozole lansoprazole esomeprazole pantoprazole rabeprazole. These are taken once/twice daily before meals.

Sir mainly red chilli is the reason of acidity and consequently farts. Try to reduce red chilli in diet. Instead go for green chilli and black pepper. You may face the problem of piles in future so please take precaution. If you have spicy food at times, take lime water after 1 hour mixed with honey.

Sir, my age is 28 years. I had been diagnosed with Gilbert's syndrome in 2004. Earlier, I used to be sharper & attentive. But since 2009, I have started forgetting things. I don't find myself focused on a particular thing. I can't perform multiple tasks at a time (that much effectively). I searched the net and found out that GS patients may develop forgetfulness. Is GS the primary cause of these things? In that case, what should I do to rectify my GS? Some examples of problems faced by me (not always) : 1. I want to copy files from x folder to y. I go to x, select files (takes some time) and moves to paste it but where ? I forgot. Then after 2-3 seconds, I remember and paste it to y folder. 2. I had given the room key to x person while working on something (sitting with z person). After finishing work, we (myself & Z) returned to room to see room is locked. I started searching key (which I usually keep in my left pocket). Didn?t find it. Started searching other pockets. Not getting it. Panicked. Went back to the place I was sitting. Z person waited there itself. Didn?t find there too. On the way back to room, y person handed over me the key (in front of Z). I wonder how come it reached to him. He explained the key has been given to him by x person. I am still clueless how did it reach to x person. Z person (who was till then enjoying my fiasco) reminded me of giving the key to x. Then only I remembered. Quite embarrassing it was! 3. Locked the door. Came downstairs. Started thinking I haven?t. Needed to go back to check it again. (Had I been attentive enough while locking itself, I wouldn't have required to go back & check).

I am 38 year old whenever I eat something spicy or hard food I got loose motion and gastric problems. .pls suggest.
The intestinal lining do not tolerate chemical compounds available in certain spices. Some spices passes from the stomach to intestine and causes irritation. The food also moves fastly from inflamed lining. The intestines senses this and draw more water from the circulation and thus cause diarrhea. If you intend to eat spicy food please add yougurt, butter milk with your food it will minimize the diarrhea. Drink one glass of lemon juice after meal to fasten metabolism. Drink water more. Try to identify the spicy food allergic to you avoid that.

Dear , fissure is the result of passage of hard toilet or severe diarrhoea. Hard faeces is due to constipation and it is not a disease but a lifestyle problem and unless you do that sort of adjustment away from sedentary lifestyle it will remain there. Increase your fluid intake, take high fiber and high residue diet, increase physical activity like walking, games or exercises. Take emollient laxative like cremaffin plus 1 tsf at night with isabgol powder 3-4 tsf with 300 ml. Of water at night. Apply zincal ointment inside anus twice a day and sitz bath-sit in tubful of warm water for 15 minutes daily.

Irritable Bowel Syndrome Treatment

The medical community has not yet determined the cause for Irritable Bowel Syndrome. A combination of treatments may help a sufferer cope with the symptoms; however given the diverse symptoms of IBS sufferers; diarrhea, constipation, pain, bloating, etc, and it's chronic nature, none of these treatments at present will ultimately cure IBS.
IBS is a non-life threatening illness. It does not progress or increase your risk of developing Inflammatory Bowel Disease or Cancer. Treatment focuses on the relief of symptoms so you can live your life as normally as possible.

There is much anecdotal information on the internet as to what may relieve IBS symptoms. We recommend that before starting any new dietary, medical food or over the counter (OTC) remedy, that you consult with your physian. Physician's rely on risk and benefit decisions based on reproducible research data when they suggest a treatment for someone suffering from IBS.

Based on your physician's diagnosis after the appropriate medical tests, treatment, in our opinion, may be different for those suffering from mild, moderate or severe symptoms; however, they may be summarized as follows.

Mild: manage stress and make changes to your diet and lifestyle
Moderate: As per Mild + fiber supplements, over-the-counter (OTC), anticholinergic, or like, medications.
Severe: As per Moderate + tricyclic or SSRI antidepressants, or 5-HT3 or 5-HT4 or Chloride channel activator or guanylate cyclase-C (GC-C) agonist medication.

Your physician may recommend one or more or none of these treatments. See your physician for all recommendations for treatment. It is very important to follow-up with your physician. Refer to the medication page for an overview of medications for IBS.

Treatments:
Role of stress and visceral hypersensitivity
Role of diet
Prevention
Self Care
Coping Skills
Alternative Medicine
Prescription Medical Food

Specific Treatments Now and the Future:
Cognitive Behavioral Therapy (CBT) and Hypnosis: There is research evidence that IBS symptoms respond favorably to other therapeutic approaches such as cognitive behavioral therapy (CBT) and hypnosis. As IBS is a chronic condition, with an ongoing fluctuating course, these treatments may help the individual to develop skills for managing the condition over the long haul. Many people with IBS also have a co-existing anxiety or depressive disorder, the symptoms of which also benefit from these types of treatments.
Probiotics: Probiotics (good bacteria) which contain the strain bifidobacterium infantis 35624 have been shown to relieve abdominal pain, bloating, urgency, constipation or diarrhea. Align and VSL#3 are marketed over-the-counter (OTC) with this strain.
5-HT3 antagonist: Lotronex (alosetron), is used for the control of pain and diarrhea associated with IBS in women (IBS-D). It was removed from the market at the recommendation of the United States FDA in Nov. 2000 and re-introduced in June 2002.
5-HT4 receptor agonist: These medications help relieve the symptoms of constipation and pain associated with IBS and constipation (IBS-C), and chronic idiopathic constipation in women. Zelnorm/Zelmac (tegaserod) was approved by the FDA in the United States. Effective April 2, 2008 Zelnorm is only available to patients in the U.S. Under emergency situations.
Chloride channel activator: These medications help relieve the symptoms of constipation and pain. Amitiza (lubiprostone) was approved by the FDA in the United States for IBS with constipation and chronic idiopathic constipation.
Guanylate cyclase-C (GC-C) agonist: These medications help relieve the symptons of constipation and abdominal pain. Linzess (linaclotide) was approved by the FDA in the United States for IBS with constipation and chronic idiopathic constipation.
Antibiotics: There is some research evidence suggesting that IBS symptoms may be caused in part by an abnormal growth of bacteria in the small intestine, referred to as Small Intestinal Bacterial Overgrowth (SIBO). A specific regiment of antibiotics, namely Rifaximin (xifaxan), have been investigated to treat this condition.
Prescription Medical Food: These products are regulated by the FDA under the Orphan Drug Act and are meant to be used under physician supervision. One prescription medical food is indicated for the clinical dietary management of IBS-D under physician supervision as part of ongoing medical care for a specific condition or disease. Some patients due to chronic conditions, diseases or specific drug therapies have an impaired ability to ingest, digest, absorb, or metabolize food and certain nutrients. This medical food product is prescribed to help people manage their bowel problem.

Liver abscesses form usually in young males who frequently have alcohol history . Treatment depends on the extent of the liver abscess , location and symptoms of the patient . It can be treatment by oral medication , may require percutaneous drainage or may even require surgery in more advanced disease . After drainage of the abscess , the patient recovers very quickly on clinical basis but lesion may take time to resolve on Ultrasound
