As your age is not mentioned, you neck and shoulder pain can be due to Mechanical neck pain or due to Spondylosis which happens usually after 35 years. Chakkar Jo aate hain Woh unke wajah se bhi hosakta hai ya fir less water intake ki wajah se bhi hosakta hai, low sugar (Hypoglycemia) k wajah se bhi, Low haemoglobin k wajah se bhi chakkar aasakta hai. Hernia Surgery - What To Know About It?

Hernia may be defined as the protrusion of an organ into an adjacent connective tissue or muscle. The protrusion usually occurs through a tear or a weakened area in the muscle (Fascia). The hernia may be Inguinal Hernia, Umbilical Hernia, Hiatal Hernia or Femoral Hernia. The hernia can also originate from an incision (Incisional Hernia). The Inguinal Hernia is commonly observed in males. Here, the intestine squeezes through a tear in the abdominal wall into the groin (inguinal canal). At times, the small intestine bulges through the abdominal wall (weak spot) near the bellybutton. This results in Umbilical Hernia. In Hiatal Hernia, the upper part of the stomach passes into the chest through the diaphragm. Of all the hernia types, the Inguinal Hernia is the most common. 

Severe strain and muscle tear or weakness can result in hernia. Obesity, chronic constipation, chronic coughing and sneezing, ageing, damage caused by injury, medical conditions like Cystic Fibrosis, contribute to increased incidences of hernia. In case of hernia, the affected area tends to protrude or bulge out. If left untreated; hernia can prove to be detrimental. One needs to consult a physician for proper diagnosis and treatment. Physical examination can help in the diagnosis of Inguinal Hernia. An ultrasound is needed to diagnose Umbilical Hernia. A Barium X -ray or endoscopy can be of great help in the diagnosis of Hiatal Hernia.  

The severity of the hernia depends upon its size. In case the hernia is rapidly increasing in size, a surgery is needed for the repair. The surgery performed can be Open or Laparoscopic. In open surgery, hernia is identified through an incision. Once located, the hernia is removed from the adjacent tissues. In Laparoscopic Repair, small incisions are made in the affected region. Through these incisions, specialized instruments are inserted. It is through these instruments that the surgeon visualizes and performs the surgery.  In such repair, a mesh, held in place by sutures is used as a scaffold. This facilitates the growth of new tissues in the affected person. This technique significantly lowers the chances of a recurrence.

Certain factors needs to be well addressed before performing hernia operation:

  1. Hernia operations, Laparoscopic in particular, should be performed by experienced surgeons. Inexperienced surgeons will do you more harm than good.
  2. Laparoscopic hernia operation should be avoided in case a patient has adhesions from previous surgery.
  3. Extreme care should be taken while performing a hernia operation in infants and children.
  4. In some cases, hernia surgery might affect or injure the vas deferens in men. This in turn can affect fertility in men. If you wish to discuss about any specific problem, you can consult a General Surgeon.

I have been detected Inguinal Hernia my age is 46 years doctor advised to take ice shake 2 - 3 time a day and light break fast lunch, dinner i.e. 3-4 time a day. Please advice any effective remedy including home/Ayurveda homeopathy for early relief and permanent solution to get out from inguinal hernia within short time span. Thanks

Generally inguinal hernia takes long time to get total relief if it’s not extended till testies. To Do conservative treatment try following medicines for next 6 months 1.apply cotton swab soaked with castor oil on hernia part and tried it up in night for whole night 2.Take vrudhibadhika vati-2tabs before lunch and dinner 3.take tapyadi loha -2 tablets before lunch and dinner.

Hi there you can avoid knee replacement by physiotherapy treatment and following some home instructions. I would suggest you to go to nearby physiotherapist. At home you can. Apply ice pack for 15 minutes which will help. To reduce pain and swelling, avoid bending of knees more than 90 degrees, use western toilet, use knee caps when walking out, do gentle knee exercises and isometric exercises of quadriceps and hamstrings, do gentle patela glides in all directions.

Hernia: Types and Treatment

When an organ residing in a cavity such as the abdomen tries to push through the muscular layer it resides in, it is called as hernia. 

Though said to be genetic, hernias can be caused by things such as improper heavy lifting, incorrect posture, or chronic constipation and as a result of surgical complication or injury. Factors like obesity, pregnancy, smoking, chronic lung disease aggravate the severity of the hernia. It is believed that about 27% of all males and 3% of females can have a hernia during their lifetime.

Types of hernias:

  1. Inguinal hernia: The groin is the most common area, where the abdomen pushes through a weak spot in the lower abdominal wall, causing a protrusion into the inguinal canal. This is more common in men than women.
  2. Hiatal hernia: The abdomen has the diaphragm separating it from the thoracic cavity in the upper border. When it pushes through the diaphragm, a hernia is caused and there is almost always associated food reflux in these cases. Though the most common cause is associated old age, due to muscle weakness, there also are cases of congenital hiatal hernias.
  3. Umbilical hernia: The abdomen finds a weak layer along its length and protrudes through the skin on the stomach. Most commonly seen in babies around the bellybutton, it gradually corrects itself on its own. Quiet rare in adults, seen during pregnancy and in chronic obese people.
  4. Incisional: These are post-surgical, and happen when the organ protrudes through the weakened wall due to surgery. The abdomen is again the most common area and the hernia can happen either onto the external surface or internally, when they are called ventral hernias.

These are the most frequent types, though hernia affects other organs like the spine, brain, appendix, etc.

Treatment:

This includes a combination of constant monitoring followed by a decision to do surgical treatment. Hiatal hernias and umbilical hernias can be monitored for a while before deciding on surgery. Inguinal hernias may require surgery earlier in the stage. Post-surgery, a mesh is placed to hold back the tissue in its corrected place. The umbilical hernia in children could be self-limiting. If it does not get auto-corrected in the first year of life, that also would qualify for a surgical treatment. Hernias need to be managed under medical supervision. If you wish to discuss about any specific problem, you can consult a General Surgeon.

I am 30 year old, and I have ACL injury on 17 june 2017 during dancing. And I do essential exercise daily for the recovery of ACL injury, is there surgery is necessary for ACL injury. Hi there There can be many factors responsible for non union of fracture like too large space for bony remodelling to bridge, Interposition of periosteum, muscle or cartilage, Bony site with limited or less blood supply and more. If fracture won't unite after 6 months it my be considered in as delayed or non union of fracture. These approaches can be effective after non union: Non-surgical approaches Early weight bearing and casting may be helpful for delayed union and non-union. Bone stimulation can sometimes be used. This delivers pulsed ultrasonic or electromagnetic waves to stimulate new bone formation. It needs to be used for up to an hour every day, and may take several weeks to be effective. Medical treatments such as teriparatide have also been used to promote fracture healing, particularly in patients with osteoporosis. Surgical approaches: Debridement to establish a healthy infection-free vascularity at the fracture site. Bone grafting to stimulate new callus formation. Bone may be taken from the patient or may be cadaveric. Bone graft substitutes/osteobiologics. Internal fixation to reduce and stabilise the fracture. Depending on the type of non-union, any combination of the above procedures.

Hot Fomentation. Gentle Shoulder Stretching. Core Strengthening Exercises. Gentle Neck Exercises. Avoid sleeping on affected side.. Warm up and cool down is a must.

Gentle massage over the affected part with lukewarm herbal Pain relieving oil for 20 minutes followed by hot fomentation. it will relax the muscles and the veins of ur affected part improving the blood supply to the organ and giving strength to it. Please avoid over-use of the part affected i.e. lifting weight - pushing or pulling heavy objects. Give proper rest to the part - let it heal with the time. This will greatly help. Human bones are continuously being removed and formed, which is necessary for its proper maintenance. However, in certain cases, especially with aging, some bones and joints will be worn down to an extent where the body will not be able to reform them. This is especially true in the case of hips, which may become worn due to age or other problems within the body. 

Hip replacement surgery
When this happens, a hip replacement surgery can be performed to replace the original joints with a prosthesis or artificial hip, which is usually made out of metal or a specially formed composite material or even a combination of the two. Along with other parts, it replaces the natural hip with an artificial hip to restore mobility and other related functions.

Benefits of hip replacement surgery:

  1. Lessening hip pain: This is one of the major factors as well as symptoms of requiring hip replacement surgery. Due to the grinding down of the bones, even the slightest movement causes pain and after the surgery and the recovery period, the pain is lessened.
  2. Correction of bone deformity: When the bones grind down and are abnormally reformed, deformities may occur reducing mobility. Artificial hips will ensure that deformities are corrected and resulting problems are thus eliminated.
  3. Return to normal activities: This is possibly one of the primary advantages of this surgery, although it entails a long recovery period. However, most patients can get back to their normal activities and are able to resume a pain free life.
  4. Ability to have painless sleep: Many people who suffer from hip displacements or bone deformities are not able to sleep properly due to pain. After the surgery and the recovery period, they can return to normal functions and habits, thus are able to sleep properly.
  5. Leg strength increase: Many patients tend to have an increase in their leg strength as one side of the hip tends to become very weak due to bone problems before surgery. After surgery and recovery, the same side tends to have an increased strength due to the implant.
  6. In most cases a lifelong solution to hip problems: As the hip replacement prosthesis is made out of metal and if the operation is successful, it usually tends to last till the end of a person's life and this area usually doesn't tend to have problems again.
