Hernia - How To Deal With It?

A hernia occurs when any organ of the body pushes through an opening in the muscle that holds it in place. Hernias are one of the most common problems that occur in the abdomen. Hernias can also appear in groin areas, upper thighs and belly buttons. They are not life-threatening, but they don’t go away on their own.

Types of Hernia

These are some of the common types of Hernias:

● Groin hernias: They are Inguinal, Femoral or Obturator hernias. These are one of the most common types of hernia. They constitute about 70 percent of total hernias. They are more commonly found in men than in women. Femoral hernia is common in females. Inguinal hernias occur when intestine is moved through a weak spot in the lower abdominal wall.

● Umbilical hernia: This occurs in children and babies under 6 months old. This happens when their intestines bulge through their abdominal wall near the belly button. This is the only kind that goes away on its own.

● Incisional hernia: It can occur after having abdominal surgery. Hernia occurs through the defect in the suture line.

● Hiatal hernia: It occurs when part of the stomach extends up through the diaphragm into the chest cavity. These are most common in people above 50 years of age.

Causes of Hernia

The problem of Hernia occurs due to the combination of straining and muscle weakness in the body. Depending on its cause, it can grow over a short period or a long period of time. Common causes of Hernia are listed below:

● Sudden weight gain.

● Hernia may occur through the defect in the incision of previous surgery.

● It is caused when there is increased intra-abdominal pressure like in straining for urination or for passing stool.

● It may occur in pregnancy, as abdominal muscles are stretched.

● If a person is lifting heavy weight during exercise.

Symptoms of Hernia

The most common symptom is a bulge or lump in that area. Other common symptoms are listed below:

● Feel discomfort or dragging pain in the abdomen.

● Feeling of heaviness in the groin.

● Feel an aching sensation at the site of the bulge.

Swelling increases on coughing.

A person suffering from hernia might experience one or more of the above symptoms.

Treatments for Hernia

It is usually diagnosed through a physical examination. Whether or not a person needs treatment depends on the symptoms and size of the hernia. Some of the treatment modalities of hernia are listed below:

● A change in diet can effectively treat many symptoms of hiatal hernia.

● Some medications can lessen the amount of pain and reduce stomach acid, in case of hiatus hernia.

● If the hernia grows in size, then one has to go through surgery.

One can have either open or laparoscopic surgery. During surgery, after reduction of hernia surgeon needs to strengthen the weak abdominal wall. It is done by using Polypropelene Mesh. It may be a single dimensional or 3 dimensional Mesh. Let your treating surgeon decide about the treatment modality best suited for you.

Postmenopausal Bleeding - It Is Normal?

Most women attain menopause between the ages of late 40s and early 60s, the average age being about 51. This is an important milestone in a women's gynecological history. One major change is altered female hormone levels, and this leads to a lot of physiological changes. From hot flashes to mood swings, there is also increased predisposition to osteoporosis and uterine cancer.

If you have not had your menstrual cycles for close to 12 months, chances are you are into menopause. So, that means absolutely no vaginal bleeding anymore whatsoever. However, if you experience bleeding, even spotting, be on the alert. It is not normal and needs to be examined, and if required, diagnosed and treated.

Postmenopausal bleeding or PMB, as it is popularly called, can be due to a variety of reasons. While it could be something as trivial as inflammation of the uterine or vaginal lining, it could also be an indication of more severe issues like cancer.

  • Atrophic vaginitis: Decreasing hormonal levels lead to increased dryness and therefore inflammation of the vaginal and uterine tissue. This is one of the common causes of bleeding after menopause.
  • Endometrial atrophy: Also caused by lower hormone levels, the lining of the body of the uterus gradually thins down and can get inflamed.
  • Polyps: Noncancerous growths in the uterus, cervix, vulva, or vagina can also lead to bleeding.
  • Infections: General infection of any area along the uterine tract could lead to occasional bleeding.
  • Cancers: Though only 1 in 10 PMB cases turn out to be cancers, the prognosis improves with early diagnosis and intervention.

Diagnosis: As repeated above, reach out to your doctor if you notice postmenopausal bleeding. Diagnostic methods could include the following:

  1. Physical examination
  2. Transvaginal ultrasound
  3. Endometrial biopsy
  4. Hysteroscopy
  5. Dilatation and Curettage

Treatment: Needless to say, this would depend on the diagnosis. For very minor cases with diagnosis like altered hormone levels, no treatment may be required other than modification of the hormone replacement therapy. For endometrial atrophy and atrophic vaginitis, use of estrogen creams and pessaries would be sufficient. Polyps would require removal followed by cauterization (application of slight heat) to stop the bleeding.

Cancer: This would depend on the type and location and require a combination of chemotherapy and surgery.  Removal of the uterus also may be required in some cases. So, if you have had bleeding of any sort after a year of menopause, do not ignore it.

  4. Elevate the head: Elevating a child's head can help improve sinus drainage. When it comes to children, instead of using a pillow below their head, place the pillow below the mattress to create a gradual slope. 
  5. Onion: Onions can be used in many different ways to treat earaches. You could either juice an onion and put a few drops of the same in the ear or slice an onion in half and heat it before placing it over the ear like an ear muff. Onion contains anti inflammatory and antibacterial properties that help soothe earaches effectively and quickly.

It's highly unlikely that pregnancy would occur if sperm had to travel through water into a woman's body. In the hot tub scenario, the temperature of the water or chemicals would kill the sperm in seconds. In a bathtub filled with plain warm water, the sperm may live up to a few minutes.
