Dear lybrate-user. U need to consult an endocrinologist immediately for adjustment of his medicines so that he achieves a good glycemic control. Lipid profile, urine albumin creatinine ratio, KFT, eye check up should be done annually. HbA1c 3 monthly. FBS PPBS atleast monthly.

Sugar problem and blood pressure also low in body and eye lenc are low so I have more problems please solve my problem and I want to live happily.

The thyroid gland within the body plays an important role in regulating your metabolism among many other functions. This is a butterfly-shaped gland located in your neck and plays an important role within your body. However, a common condition that may afflict it is hypothyroidism or an underactive thyroid gland. It is even more of a problem if you are pregnant as it may affect your baby’s development in the womb. It has also been noticed that pregnancy in itself may cause hypothyroidism.

Symptoms of Hypothyroidism-

Pregnant women may also exhibit symptoms common to other patients who are suffering from hypothyroidism. These may include:

  1. Fatigue along with lethargy.

  2. Mild to significant weight gain.

  3. Constipation.

  4. Feeling cold with severe chills

The thyroid gland produces the T4 hormone responsible for regulating many mechanisms within the body such as metabolism. With hypothyroidism, the production of this hormone decreases or stops altogether. This is important as disruption in the production of the hormone may disrupt the normal development of your baby.

Effects of Hypothyroidism on your baby-

Hypothyroidism during pregnancy can affect your child’s developmental abilities. Many studies in this field have shown that such children tend to have learning difficulties and may even display lower IQ scores during tests. It was noticed that sometimes, hypothyroidism during pregnancy wasn’t diagnosed correctly as the pregnant mother did not show much apparent symptoms. This is also a major issue in the early diagnosis of the problem. Hence, it is absolutely imperative to test for thyroid problems early during pregnancy.

Prevention is the Best Cure-

It is thus very important that mothers be screened properly for thyroid issues even before they are pregnant and while they are pregnant. Some of the medically recommended steps that should be taken are –

  1. Screening before pregnancy as this can help to devise a solution for this condition.

  2. Women with goiter or enlarged thyroid should most certainly be screened.

  3. Women with a family history of hypothyroidism should be screened.

Treatments for Hypothyroidism During Pregnancy-

The doctors may suggest thyroid hormone replacement therapy wherein artificial T4 hormone is introduced into the body. This helps to maintain a constant rate of the thyroid hormone within the body. This can be done even before pregnancy as the developing fetus is completely dependent on the mother for its thyroid hormone until at least 12 weeks when the baby’s body can start producing it on its own. Also, the levels of this hormone should be regularly checked within the body through the TSH or the thyroid stimulating hormone tests to ensure that the levels are at a safe minimum.

Mr. lybrate-user, If blood glucose levels go hay wire, then repeat infections tend o occur. Therefore, it is absolutely essential to maintain blood glucose levels in good control. Fasting should be closer to 100 mg, PP 160 to 180 mg & HbA1c% <7. If the control is not please se that it is regained with appropriate changes in the treatment. Thanks.

The reason for advising that is, banana contains high concentration of sugar, gets rapidly converted to glucose and raises blood glucose fairly rapidly. Eating a small banana once a while is ok. But regular consumption is not advised. Fruits like mango, banana, grapes and chikkoo as those are too sweet, hence better to be avoided. Other fruits like papaya, pomegranate, pear, guava, apple, water melon, orange, sweet lime and other melons are allowed. But only about 100 g a day should be taken with good control of blood glucose.

Hello, You cannot escape diabetes, as it is not a disease but it is a disorder. By following a good diet and maintaining ideal body weight and exercising regularly, you can reduced the chances, of getting it. Unexplained thirst, frequent urination, unexplained weight loss, excessive, hunger, lethargyness are some, underlying symptoms, but sometimes it can, just be asymptomatic.

I have had 2 bypass operations 1993 and 2004. As I was margin diabetes Dr. advised me take precautions and medication. I take Hb1AC test once in 3 months and reading is 5.8 or below. Is it necessary to take fasting and post prandial readings. I do not take them.

HbA1C of 5.8 suggest borderline diabetes. Instesd of Fasting and post prandial readings, you should go for Oral Glucose Tolerance Test.

Mr. lybrate-user, Unless hypothyroidism is well controlled with TSH level brought around 3 to 4 mU/L, all the symptoms will not be in check. So you will have to check TSH levels after 6 weeks and if still high, then dose of thyroxine will have to be increased. It I appears that starting dose given is too low. It should have been over 100 mcg. Thanks.

Miscarriage refers to the spontaneous loss of the fetus before one completes the 20th week of pregnancy, taking into account the period from the conception to gestation.  It usually happens in the first trimester of the pregnancy; that is within the seventh and the twelfth week of conception. Recurrent miscarriage is when one suffers from multiple miscarriages in a row.

Possible Causes:

  1. Abnormally-shaped Uterus: Some miscarriages, particularly late ones, are thought to happen because the uterus (womb) has an abnormal shape.
  2. Polycystic Ovary Syndrome (PCOS): Women with this condition have many small cysts in their ovaries.They also tend to have hormonal problems, including high levels of insulin and male hormone in the blood, which can lead to recurrent miscarriage.
  3. Infection: Some serious infections can cause or increase the risk of single miscarriages. These include toxoplasmosis, rubella, listeria and genital infection. But it is not clear whether infection plays a role in recurrent miscarriage.
  4. Diabetes and Thyroid Problems: Uncontrolled diabetes and untreated thyroid problems can cause miscarriage. But well-controlled diabetes and treated thyroid problems do not cause recurrent miscarriage.

Risk Factors:
Your risk of recurrent miscarriage is higher if:

  • you and your partner are older; the risk is highest if you are over 35 and your partner over 40; 
  • you are very overweight. Being very underweight may also increase your risk.

Each new pregnancy loss increases the risk of a further miscarriage. But even after three miscarriages, most couples will have a live baby next time.

Testing After Recurrent Miscarriage:

If you have had three miscarriages in a row, you should be offered tests to try to find the cause. This should happen whether or not you already have one or more children. Testing is not usually offered after one or two early miscarriages (up to 14 weeks) because these are often due to chance. But you might be offered tests after two early miscarriages if you are in your late 30s or 40s or if it has taken you a long time to conceive.
If you had a late (second trimester) miscarriage, where your baby died after 14 weeks of pregnancy, you should be offered tests after this loss.

  1. You can opt for blood tests to check for sticky blood syndrome or APS. Tests would look for antibodies that would help treat the condition. Antibodies are chemicals produced by the body to combat infections.
  2. Get an examination done should the doctor suspect chromosomal abnormalities and in case it is diagnosed, both of you can consult a clinical genetics specialist for genetics counseling.
  3. Your doctor will recommend an ultrasound scan to trace any type of abnormality that may make a pregnancy futile, for instance, a short or a fragile cervix.

Your hopes:

It is natural to pin your hopes on testing as the answer to your problems. But there are three reasons why it may not be the answer you’re looking for:

  • A cause may not be found; when this happens your miscarriages are called ‘unexplained’ 
  • Even if a cause is found, it may not be treatable;
  • Treatment may not lead to a successful pregnancy. This can happen if a pregnancy miscarries for a different reason than the one being treated.

While it is well known that smoking causes lung cancer, heavy smokers with diabetes are also at increased risk of death from causes other than lung cancer, according to a study being presented next week at the annual meeting of the radiological society of north america (rsna).

Diabetes is a chronic illness in which there are high levels of glucose in the blood. More than 29 million people in the u. S. Have diabetes, up from the previous estimate of 26 million in 2010, according to a report released by the centers for disease control and prevention. One in four people with diabetes doesn't know he or she has it. Having diabetes can also put people at risk for numerous other health complications.

To determine the extent to which diabetes is associated with deaths from lung cancer, other cancers, and other causes among heavy smokers, researchers examined the risk for all-cause mortality among people with and without diabetes within the national lung screening trial (nlst), a massive, multicenter trial that compared low-dose helical ct with chest x-ray for early detection of lung cancer in current and former heavy smokers.

" in our study, we found a statistically significant link between diabetes and all-cause deaths, non-lung cancer deaths and lung cancer deaths in women" said kavita garg, m. D, professor of radiology from the university of colorado -- denver.

For the study, Dr. Garg and colleagues looked at data from 53, 454 participants in the nlst and identified 5, 174 participants who reported having diabetes at screening.

They conducted an analysis of the relative risk for overall mortality, lung cancer mortality, and non-lung cancer mortality associated with diabetes, adjusting for age, gender, body mass index (bmi), and pack-years of smoking. Over the course of the study, there were 3, 936 total deaths, including 1, 021 from lung cancer and 826 from cancers not of the lung.

Participants with diabetes tended to be older, reported more pack-years of smoking, and had a higher bmi than those without diabetes. There were 650 deaths (12.6 percent of patients) among participants with diabetes and 3, 286 deaths (6.8 percent of patients) among participants without diabetes.

" we found that diabetes doubles the risk for all-cause mortality and non-lung cancer mortality among heavy smokers" Dr. Garg said" we also found that women with diabetes have an increased risk of lung-cancer mortality, but did not find the same effect in men"

The researchers continue to analyze data in an effort to better understand the underlying cause. In the meantime, Dr. Garg emphasizes the importance of taking control of diabetes and undergoing lung cancer screening if you're a smoker.

Diabetes mellitus is a group of diseases where the body does not adequately produce insulin, use insulin properly, or both. Insulin plays a crucial role in allowing blood sugar into the cells to be used for energy. There are two main types: type 1 and type 2 diabetes.

People with diabetes have abnormally high levels of blood sugar. This can damage many organs in the body if left untreated. The national institute of diabetes and digestive and kidney diseases recommend the following steps to manage diabetes:

 Make healthy choices in eating
 Engage in regular physical activity or exercise
 Take medications, if required.

Healthy eating is important in keeping blood sugar levels at a healthy level. The healthy range is 80 to 130 mg/dl before meals or below 180 mg/dl after meals, according to the american diabetes association.

People with type 1 diabetes require insulin. Various insulin delivery systems and protocols are used to manage blood sugar both between and at meal times.

People with type 2 diabetes often manage their condition with diet and exercise, and with medications as needed to keep blood sugar within the target range. These medications vary in how they work.

People with diabetes will have different treatment plans, and they will respond to food, exercise, and medication differently.

It is important to consult with a doctor to get individualized recommendations on target blood sugar levels, medications, diet, and exercise.
How do carbohydrates affect diabetes?

Carbohydrates are an important source of energy for the body. Carbohydrates are found in foods that have starches and natural or added sugars. Examples are grains, vegetables and legumes, fruit, dairy products, and sweets.

Carbohydrates are broken down by the digestive system into sugar. When the digested sugar enters the blood, the body produces a hormone called insulin. Insulin helps the sugar enter cells. Once the cells absorb the sugar, blood sugar levels fall.

People with diabetes have an impaired ability to produce insulin, use insulin, or both.

People with type 1 diabetes cannot produce insulin, so they take insulin to make sure the cells can get the sugar they need for energy.

People with type 2 diabetes are often insulin resistant. They also often have difficulty producing enough insulin to keep their blood sugar in the normal range.
Counting carbohydrates

Carbohydrate counting is a way of keeping track of the carbohydrates in the daily diet. A person with diabetes who uses carbohydrate counting to manage their diet sets an amount of carbohydrate to eat for meals and snacks.
Foods containing carbohydrates
Bread, pasta, rice, and potatoes are all sources of carbohydrates.

The american diabetes association suggest a target of about 45 to 60 grams of carbohydrate per meal. This recommendation may vary depending on other factors such as gender, weight goals, and blood sugar target goals.

The three different types of carbohydrates are starch, sugar, and fiber.

Starches are complex carbohydrates found in starchy vegetables such as peas, potatoes, and corn. Beans and whole grains are also complex carbohydrates.

Fiber comes from plants and cannot be digested. Fiber is found in foods such as vegetables, fruit, whole grains, legumes, and nuts.

Unlike other carbohydrates, fiber does not raise blood sugar, and it can help to slow the digestion of meals. This helps to minimize spikes in blood sugar. It is recommended to eat between 20 to 35 grams of fiber per day.

Sugar is a carbohydrate. It is generally absorbed into the body more quickly. There are natural sugars found in milk and fruit. There may also be added sugars in canned fruits, baked goods, and processed foods.

There are also carbohydrates in non-starchy vegetables such as lettuce, peppers, cucumber, mushrooms, and many others. There are fewer carbohydrates in these foods because they have a high water content. For example, a half cup of cucumber has around 2 grams of carbohydrate.

The type and amount of carbohydrate will affect post-meal blood sugar levels.

Foods that digest more slowly, such as those with a lot of fiber, and those eaten as a mixed meal, digest more slowly. They can help to prevent post-meal spikes in blood sugar. Large amounts of carbohydrates eaten at one time will raise blood sugar more than smaller amounts.
Is eating rice healthy with diabetes?

High-carbohydrate foods like grains, cereals, pasta, rice, and starchy vegetables are not forbidden, but they should be eaten in moderation.

Rice is a high-carbohydrate grain, but it can be incorporated into meals in appropriate amounts.

One-third of a cup of rice has 15 grams of carbohydrate. That accounts for one-fourth to one-third of the amount of carbohydrate recommended for a single meal, if the target is 45-60 grams of carbohydrate per meal.

Meals that also include healthy proteins and fats can help to slow the impact of the rice on blood sugar levels.
Are some types of rice healthier than others?

Some grains are better than others for managing diabetes.

A scale called the" glycemic index" measures how quickly food is digested into sugar and absorbed in the blood. High glycemic foods raise blood sugar faster and should be eaten in limited portions, or eaten with lower glycemic index foods.

White rice is more processed and it has a higher glycemic index than brown rice, although the index of brown rice can vary with type and brand.

Different varieties of rice have different glycemic indexes. Some long grain rice varieties, converted rice, and basmati rice varieties are lower on the gi scale than white rice.

Puffed rice cereal and rice cakes are sometimes thought of as diet foods, but they have a high glycemic index and they are not ideal for healthy meals.

Foods that are high in fiber offer many health benefits. They help with blood sugar control, they promote bowel health, and they may lower cholesterol.

Whole grains have more fiber than other grains. It is important to check the label to check the fiber content.
Tips for preparing rice

Some brown rice varieties are unprocessed and have more fiber. They can be part of a balanced meal when eaten in proper portions. Mixing brown rice with other foods can help to balance blood sugar levels. Examples include legumes, such as red beans, or protein and healthy fats.

Brown rice may have a lower glycemic index than white rice.

Brown rice takes longer to cook than white rice, but the cooking process is simple. People can cook brown rice in a pot or rice cooker at a ratio of 1.5 cups of water per 1 cup of rice.

The instructions are as follows:

 Bring rice and water to a boil in an uncovered pot
 Cover the pot and simmer for about 20 minutes
 Turn off heat and let the covered pot sit for at least 10 minutes.

Rice can be mixed with seasonings, herbs, vegetables, and nuts such as slivered almonds.

Brown rice can be stored in a refrigerator and used for leftovers. People can reheat brown rice on the stove or microwave and serve with beans and salsa for a quick meal.

Care must be taken with storage, because cooked rice left at room temperature can develop toxins that lead to food poisoning.
Nutritious and delicious alternatives to rice

Because rice is high in carbohydrates, it should be accompanied by other foods.

Vegetables are high in fiber, vitamins, and other nutrients. Vegetables are made of carbohydrates, but at a much lower level than grains.

Eating foods that are lower in carbohydrates and higher in fiber can make meals more satisfying. For example, one-half cup of rice has 22 grams of carbohydrate. In contrast, one cup of squash only has 8 grams of carbohydrate.

Many foods can serve as substitutes for rice.

Examples include cauliflower, mushrooms, and eggplant. Quinoa contains the same amount of carbohydrates as rice, but it has more protein, and some types have more fiber.
Recipes for rice substitutes

A number of recipes are available for rice substitutes. Here are two examples:
1. Cauliflower" rice"

Ingredients:

 Oil
 Onions
 Cauliflower
 Lemon
 Seasoning.

Pulse the florets of a cauliflower in the food processor. Then heat the cauliflower in a pan with oil and onions. Saut until the onions are golden brown and the cauliflower is soft for about 3 to 5 minutes.

Season to taste with salt, pepper, lemon juice, and herbs.

2. Cilantro lime quinoa

Ingredients:

 Canola oil
 Onion
 Garlic
 Quinoa
 Low-sodium chicken broth
 Juice of limes
 Fresh cilantro.

Saut the onion and garlic with oil in a skillet. Reduce the heat and stir in the quinoa. Stir quinoa and cook for 2 minutes. Add chicken broth and lime juice and bring to a boil. Then reduce heat and simmer for 15 minutes. Stir in more lime juice and add chopped cilantro.

High blood sugar occurs when the insulin in the body fails to metabolize the sugar from the food, which raises the amount of glucose in your blood. It is a metabolic disorder which can be managed effectively with the help of lifestyle modifications such as exercise and food. Cinnamon: cinnamon is a herb which is very effective in getting your blood sugar down to manageable levels. You can simply sprinkle cinnamon on your food before eating it.

Fenugreek (methi): fenugreek is a common spice which is used in almost all indian cuisines. It has a number of health benefits, one of which is reducing blood sugar levels. Soaking fenugreek in water and then drinking it the next day is a very effective remedy in combating sugar problems.

Garlic: not just for sending away the vampires, it is also very effective in blood sugar control. Extracts of garlic have been shown to increase insulin in the body which is required to metabolize the sugar in the body.

Heart healthy fats: heart healthy fat based foods such as nuts and seeds are good for increasing the good cholesterol levels (hdl) in the body. Avocadoes contain nutrients which lower the risks of various diseases such as diabetes. In a nutshell, these fats make the cells of the body less resistant to insulin.

Blueberries: blueberries contain anthocyanin (water soluble pigments) which is very good for your health. They also contain fiber in large amounts which make you feel fuller faster. This fruit has also been shown to improve insulin sensitivity in the body.

Vinegar: vinegar is an age old remedy to control blood sugar levels; it contains acetic acid which causes a reduction in certain harmful enzymes in the stomach. Vinegar is also known to prevent blood sugar spikes when consumed with food.

Complex carbohydrates: complex carbohydrates raise your blood sugar but not like its cousin, simple carbohydrates. Simple carbohydrates cause an immediate spike in your blood sugar levels where as complex carbs provide a steady stream of energy. So drop that packet of refined flour and get that whole wheat variant.

Cherries: cherries, especially tart cherries and dark sweet cherries, are full of anthrocyanins. Insulin resistance prevents insulin from working properly to control blood sugar in normal ranges. Eating a diet high in anthrocyanins can mean improved use of insulin and lower blood sugars.

Nuts: people who eat nuts regularly have lower rates of heart disease than people who don't eat them.

I have hyperhidrosis problem palm soul sweeting problem I met wth doc he said endoscopic thoracic sympathetic surgery is good for your hyperhidrosis I have done all test like 24 urinary vma and t3 t4 tsh thyroid but report is normal so I am searching ets surgeon in pune.

Use baking soda, honey,lemon juice alternatively massage and wash after 15 mts .when tests are normal .why not go for some natural treatment orally like Minersls/ antioxidants / vitamins Call for advice.

My fasting sugar is around 106-110. I love fruits. Which are the fruits which I can consume without fear of my blood sugar increasing. Thank you.

ALL fruits and vegetables contain glucose. Hence, you cannot find a fruit that will not increase your blood sugar. However, take fruits in moderation, if you are a diabetic. By the way, what eaxctly are your 2 hour PPBS and HbA1c levels?

I am 44 years old. My name akshay shah. I have diabetes last 4 years. Last 2 years I have take one medicine name glimisec 1 mf. My fasting is 115. Ppbs @ 180 to 200. Please suggest some other way without medicine dose hike I have control my diabetes.

Diabetes, often referred to by doctors as diabetes mellitus, describes a group of metabolic diseases in which the person has high blood glucose (blood sugar), either because insulin production is inadequate, or because the body's cells do not respond properly to insulin, or both. It is a difficult task to control blood sugar in the desired normal range. Whatever we eat is metabolised to sugar in the body to provide energy for our daily activities. For the absolute control, one has to monitor his / her blood sugar at regular intervals (Self-Monitoring of Blood glucose – SMBG). Though difficult, this can be done by a glucometer and keep a record. Get an idea of Fasting sugar (FBS) and 2 hours after food (PPBS), & HbA1C (inform me) and check before dinner and after every major food. The basic idea is to reduce the food intake to 1/3 of what you take now. The dictum of sugar control is diet, exercise, diet + medicine and at later stage medicine+ Insulin will be needed. There are different diet plans of varying calories for each person depending on his activity & life style. For diabetes, hypertension (high BP) and many long-term illness control there is the need for regular advice and follow up. Just one time consultation or advice is not enough to guide a diabetic in scientifically & medically correct manner. Type 2 diabetes is a form of diabetes in which the body does not use insulin properly and develops resistance to insulin.

A patient with type 2 diabetes must make several changes to his/her regular diet and food patterns. The aim is to manage the blood-sugar level of the body. Hence, a type 2 diabetes patients should follow a regular diet, which comprises of:

  1. All types of foods from different food groups.

  2. Lesser calories.

  3. Lessened and limited intake of carbohydrates.

  4. Healthy fats.

Here is a list of food items you should include in your diet in case you have type 2 diabetes.

  1. Vegetables: You must have fresh or frozen vegetables and avoid using sauces, dips, fats or salts. Intake non-starchy vegetables like cucumber, spinach, broccoli, cabbage, lettuce and vegetables, which are dark green or yellow in color. Also have starchy vegetables such as corn, green peas, beans, carrots and yams. Avoid potatoes as they are pure starch.

  2. Fresh Fruits: You should include fresh fruits in your regular diet. Have apples, bananas, berries, papaya, cherries, grapes, melons, oranges, peaches, pineapples and raisins. Have whole fruits regularly, which do not contain added chemical sweeteners.

  3. Grains: Include grains in your diet and consume whole grains more. Whole grains are not processed and consist of the whole-grain kernel. Grains contain starch, which is a source of carbohydrates. Carbohydrates are bad for blood sugar. Hence, whole grains, which are rich in fiber are recommended. Fiber controls the blood-sugar level from shooting up. Fiber should be included in the form of bran of oat and wheat.

  4. Proteins: Coming to protein food items, you should consume fish and poultry items often. Always remove the skin from chicken or turkey properly before cooking. Buy lean meat and trim all the fat from meat. Avoid frying and use methods such as baking, roasting, grilling or boiling. Also use healthy oils for frying purpose.

  5. Dairy Products: In case of type 2 diabetes, always use dairy products, which are low in fat. Do not add sugar to milk, curd or yoghurt as they contain natural sugar. Do not consume more than three cups of milk a day.

  6. Oils and Fats: Cut down the intake of excessive oily and fatty foods. Avoid food items that contain high saturated fats such as bacon, hams, sausages and butter. Consume food with polyunsaturated or monounsaturated fats. Oil raises blood-sugar level and is high in calorie and should be used in lesser amounts.

A type 2 diabetes patient needs to undergo several changes in his/her diet to keep a check on the rising blood sugar levels and for living as healthy as possible.
