Hello, According to your height and weight your BMI is overweight (29.4) so, for normal BMI reduce your weight approximately 15 to 20 kg. Some tips for weight loss :- 1) Take atleast 3 to 4 meals in a day. 2) Don't skip meals and especially breakfast. 3) Maintain your meal timing and amount. 4) Use smaller plates and healthy foods. 5) Drink unsweetened green tea. 6) Avoid processed foods, packaging foods, bakery items, processed foods. 7) Drink 2 to 3 litres water every day. 8) Eat raw fruits with skin. 9) Chew more slowly. 10) Take proper sleep. 11) Do some exercise for minimum 30 minutes. 12) Cut back on added sugar and salt. Feeling Needy - Is There A Way To Reduce That Feeling?

Man is different from the other species in that we are social beings. One of the main differentiators is our constant need to 'connect' with our fellow beings. Be it family or friends or society at large, we constantly need to be connected to our near and dear ones and want attention, affection, and assurance.

We need to respect this basic desire and appreciate that there is nothing wrong in feeling needed. Problems arise when this feeling to be needed gets excessive. There is a significant constant demand from the needy person, especially from those who are close to them.

Some cases where neediness is demonstrated are in new relationships. In the beginning, there is a great urge to please and therefore a greater focus on interacting with them. There is a fear of losing attention, there is insatiable excessive demand for attention - be it pleas, accusations, or demands - so as to not feel abandoned and deserted. Neediness is also quite evident in people with depression. Again, an attempt to not be abandoned by their near and dear ones.

By making a conscious effort, the neediness can be reduced, thereby making the relationships smoother and less painful to both the parties involved. As the saying goes - 'acknowledging the problem is half the problem solved'.

Needy people should realize or accept if told that there is an issue and make a conscious effort to work on reducing neediness.

1. Divert attention - As simple as it might sound, it is equally effective. Indulge in whatever interests you - exercise, meditation, dance, sing, etc. The idea is to divert attention so that the urge to feel needy and act on that instinct is reduced. This reduces the fear of being lonely and works magic. The busier you get, the less bored you are. As they say, be alone but not lonely.

2. Clarity in communication - One of the intricacies of neediness is making unclear demands. Sadly, quite often, the person does not know what he wants, and so are the demands or requests which are equally unclear. Once you are aware that your neediness is making you do this, make an effort to clearly communicate what you actually want. This reduces the stress on both the parties involved and leads to a happier engagement.

3. Clarity in relationships - Take a step back and see if your neediness is affecting any major relationships. If yes, revisit and clarify. Make an attempt and reestablish the relationships. If it is your truly near and dear ones, they will be very forthcoming and understanding.

4. Move on - In cases where your neediness has affected another person, be wise to accept their wishes and not be more 'needy' or clingy. This acceptance will make way for a lot of healthier relationships as you learn to deal with accepting your issues.

5. Socialize - Use your hobbies to make more friends and socialize more. Having more people to interact with reduces your need to be accepted by the same people. You have more people to rely on and might make more friends for keeps.

These are some simple ways to reduce neediness on others, but as mentioned, the first step is to accept and then work on ways to reduce it.

There are simple precautions that you can take to protect your spine from accidents and injuries while doing heavy lifts in the gym. Not only will these precautions ensure that your spine is protected from any harm, but they also yield a range of health benefits in the long run.

Following are a few precautions that can be taken from spinal protection:

  1. Check the object before lifting: It is important that you check the weight of the object before performing the lift. Test it out by pushing it with your hands or your feet very lightly to see how heavy it is and determine whether or not you should be lifting it.
  2. Perform a warm up exercise: This is one of the most essential factors for preventing exercise-related injuries. Pace around and stretch all your muscles before you start lifting. This will keep your muscles from suddenly stiffening or locking while you do the lift.
  3. Ensure that the weight is well balanced: Unbalanced weights greatly increase the risk of injury. You should always make sure that the weight you are lifting is evenly balanced on all sides and that the packing is proper so that it does not move around when you lift or have pieces falling off it.
  4. Maintain proper postureOne of the most important aspects of lifting in the right manner is to maintain proper body posture. You must always avoid arching your back or reaching out while you lift as that will surely lead to injury.
  5. Move slowly: Sudden jerks and movements are sure to lead to injury when lifting weights in the gym. Slow and smooth movements are recommended as they keep you from straining your muscles. There is no need to rush through your workout. Always avoid twisting while performing a lift.
  6. Create enough space: There has to be enough room all around you while you are lifting. This will ensure your safety while you are working out.

Obesity is a common nutritional disorder that has resulted into stress among people of all age groups. lybrate-userji, Thanks for the query. Weight loss in a diabetes patient occurs when blood glucose is uncontrolled and there is excess glucose lost through urine. Hence, primary need is to achieve a strict blood glucose control with fasting < 100 mg, PP 150 to 160 mg & HbA1c% <6.8. Once having achieved this then the efforts to add some weight can be successful. Since there are no details given like weight, height, fasting, PP glucose levels, dietary pattern and extent of daily exercise and drugs with dosage, no specific suggestions can be made. Thanks.

Before going in for details of health, disease and cure, few points are to be understood in proper perspective as we are dealing with living system. Every living system is comprised of one or more units - called cell. Every cell contains living substance. The essence of the living substance is that it differs chemically from the surrounding medium and yet maintains a DYNAMIC EQUILIBRIUM (HOMEOSTASIS) with its environment. 

Homeostasis occurs in all living cells and in the fluids and organ systems of the multicellular organisms. Referring to mammals, the 19th century French physiologist Claude Bernard coined the expression "constancy of the internal environment is the condition of life. The American physiologist W B Cannon, who first used the word "HOMEOSTASIS" referred to " the system of checks and balances which maintain internal constancy as the wisdom of the body." 

HOMEOSTASIS IN HEALTH 

Living systems may regulate it's internal constancy despite an altered environment. And such regulation is achieved by hierarchies of controls, checks & balances, alternate mechanisms, negative and positive feedbacks etc as environmental stress is applied. 

HEALTH IS EASE MAINTAINED 
In other words, when homeostasis is maintained at its optimal best i.e. integral harmonious performance of the vital functions tend to the preservation and normal development of the organism and there are no abnormal sensations and functions, it is termed health. 

WORLD HEALTH ORGANISATION DEFINES HEALTH as "HEALTH is a state of complete physical, mental and social well-being and not merely the absence of disease or infirmity." 

HOMEOSTASIS IN DISEASE 
Living system may alter a given property with reference to the environment ; the property conforms to the medium and homeostasis consists in cellular adjustments, such that metabolism continues in the altered state. So disease is an altered state of homeostasis and is attended by abnormal sensations and functions in the form of signs and symptoms. 

DISEASE IS EASE DISTURBED i.e. DIS – EASE On one extreme, even a single named disease may vary in degree of intensity, frequency and pathology according to relative abnormality of homeostasis; while on other extreme a single individual can suffer from many diseases also. "So health and disease are interconvertible and are two faces of the same coin." 

HOMEOSTASIS IN CURE 
Cure is reversal of the altered state of the homeostasis to its normal pattern. It is not mere mitigating the signs and symptoms but to reach the optimal best state of homoeostasis i.e. health. As march from health to disease is relative, the restoration of disease to health also occurs in percentages or degrees. HEALTH IS EASE RESTORED!

Getting surgery for weight loss may seem to be an easy way out, but this is no miracle cure. Bariatric surgery is of many types and to benefit from this, it is important for you to make the right choice. Some types of bariatric surgery merely restrict the amount of food that can be consumed in one sitting while others reduce the amount of fats that can be absorbed from the food as well. Duodenal switch surgery is one of the latter techniques.

For this surgery, a part of the stomach is removed. This restricts the amount of food that can be consumed. The remaining part of the stomach is fashioned into a tube. This preserves the natural inlet and outlet for food and allows the patient to have a more normal diet as compared to other bariatric surgeries such as a gastric sleeve surgery. Thus, if you do not want to be restricted to a special diet, this may be the better choice of bariatric procedures for you. Removing part of the stomach also reduces the amount of hormones such as ghrelin being produced. This hormone is responsible for stimulating the appetite.

The small intestines are then addressed and separated into two channels which merge later as one. Food flows through the shorter channel. Bile and pancreatic juices flow through the longer channel. The two channels merge shortly before food passes into the large intestines. The aim of doing this is to reduce the amount of time your food spends in the small intestines. As a result of this, lesser fat is absorbed by the body. In most cases, the amount of fat absorbed by the small intestines after this procedure is reduced to 20%. This part of the procedure is reversible. What this implies is that once the patient has lost sufficient weight, the small intestines may be brought back to their original state thus allowing nutrients to be properly absorbed once again.

If you do decide to undergo this procedure, you must first fulfill certain conditions. Bariatric surgery is usually considered a last resort when other forms of weight loss have not had significant results. However, you should have lost a certain amount of weight by dieting and exercising prior to the surgery. This is necessary as this surgery will not be effective unless the person has self-control and a will to lose weight. Thus, this is seen as a commitment towards a healthier lifestyle.

Dear Lybrateuser, - Whey protein is generally safe & does not have side effects if taken in appropriate dose - higher doses can cause side effects like increased bowel movements, thirst, cramps, weakness, tiredness, bloating, nausea & loss of appetite - consumption will depend on your other sources of protein intake, level of activity & the protein content of the brand per gram of powder, normal protein requirement of the body per day is about 1gm/ kg of body weight.

What are birthmarks?

Birthmarks are areas of discolored and/or raised skin that are apparent at birth or within a few weeks of birth. Birthmarks are made up of malformed pigment cells or blood vessels.

Although the cause of birthmarks is not known, most of them are benign (noncancerous) and do not require treatment. Babies with birthmarks should be examined and diagnosed by a health care provider.

What are the most common types of vascular birthmarks?

The following are the most common types of vascular birthmarks:

  • Macular stains or salmon patches. These are characterized by pink to red marks that may appear anywhere on the body. Angel kisses and stork bites are the most common type of vascular birthmark:

    • Angel's kisses. Marks located on the forehead, nose, upper lip, and eyelids that usually disappear with age.

    • Stork bites. Marks on the back of the neck that usually disappear with age.

  • Hemangioma. A common vascular birthmark. Hemangiomas become visible within the first few weeks or months of life and continue to grow rapidly for about 6 to 9 months. Then, they gradually lose this red color and also shrink. They are called strawberry patch hemangiomas. By age 5, 50% resolve and 90% resolve by age 9 without any treatment. Hemangiomas that grow into other organs or structures or become ulcerated should be evaluated by your health care provider.

  • Port-wine stain (also called nevus flammeus). A port-wine stain is a flat, pink, red, or purple mark that appears at birth, often on the face, arms, and legs, and continues to grow as the child grows. Port-wine stains do not go away and often need treatment if located on the eyelid or forehead. Port-wine stains involving the face may cause eye problems and be associated with other developmental disorders.

What are the most common types of pigmented birthmarks?

The following are the most common types of pigmented birthmarks:

  • Moles (also known as congenital nevi). These can be skin-colored, brown, or black, flat or raised and small or large. They can happen anywhere on the body. Moles can also happen in adulthood, but only moles that are present at birth are considered birthmarks. Other nevi that behave like congenital nevi can appear within the first 2 years of life. Congenital nevi can develop into cancer later in life, with larger nevi having a higher risk of becoming cancerous.

  • Cafe-au-lait spots. This is French for coffee with milk. These are usually oval-shaped and light brown or black. Typically these fade with age and are not a problem. However, many of them grouped together can be a sign of other health issues and should be examined by a health care provider. 

  • Mongolian spots. These are blue or blue-gray spots on the lower back or buttocks. They are most common in babies with darker skin, like African-American or Asian babies. They can be mistaken for bruises and they usually fade with age.

If you are in the habit of consuming alcohol excessively and notice redness on the palms of the hands or pale stools, then it might point to liver disease. These are one of the many symptoms of alcoholic liver diseases which is the collective terms for liver diseases occurring due to alcohol overconsumption which includes fatty liver, hepatitis, and cirrhosis. Read on to know about this condition in brief.

Causes and The Risk Factors
The main cause of the disease is overconsumption of alcohol though not all of the heavy drinkers develop this disease and the cause is not clearly understood. But it is found that a toxic chemical called acetaldehyde produced by alcohol damages the liver leading to its scarring and ultimately resulting in cirrhosis or an end state disease of the liver.

It is also noted that the people who consume liquor and beer or spirits are at greater risk of developing the alcoholic liver disease as compared to the ones who consume different alcoholic beverages like wine. Further, studies have revealed that women metabolize alcohol slower as compared to men and hence they are at a greater risk of developing the disease. And women who consume alcohol more and at the same time are on the heavier side have a high chance of developing a chronic liver disease which might even lead to death. Genetic factors increase the chances of developing a disease of the liver too.

Diagnosis
If the alcoholic liver disease is suspected, tests such as complete blood count, liver biopsy, liver function tests and coagulation studies might be conducted to confirm the diagnosis and proceed with the treatment.

Prevention and Treatment
For the prevention of alcoholic liver disease, the most critical step is to stop the intake alcohol completely. If you stop consuming alcohol, the risk for the rapid acceleration of the liver disease stops and then you can proceed with the further treatment procedures. If you cannot give up alcohol altogether, it is recommended to limit the intake of alcohol to only one or two drinks per day but is best to stop completely.

The causes and the risk factors of the alcoholic liver disease vary from one person to another, and similarly, the procedures of treatment also vary depending on the condition of the liver of the affected person.

Medications are used to treat the liver disease which depends on the condition of the liver. The healthcare professional decides the course of medication as per the requirements. In some severe cases, the doctors are left with the only option of liver transplantation which is the only definitive therapy in modern medical science, but that involves a lot of risks and other complications. So when it comes to alcoholic liver disease prevention is better than cure.

Spondyloarthritis (also known as spondyloarthropathy) is the term which describes the connective tissue diseases. It is the group of inflammatory diseases, including arthritis of the peripheral joints and spine (sacroiliitis or spondylitis); including the area where ligaments and tendons attach to bones (enthesitis or enthesopathy). These diseases cause pain or stiffness in the leg or arm joints, spine, ligaments and tendons become inflamed. Skin rashes, eye, and intestinal problems may also be able to occur.

Epidemiology

  • Spondyloarthritis is a pathology that specifically strikes young people. The symptoms most frequently start before the age of 45. It affects more males than females.
  • Predisposition to spondyloarthritis, especially spa, is determined largely by genetic factors.
  • The incidence rate is higher in populations with a higher prevalence of hla-b27.
  • Psoriatic skin lesions and colitis due to inflammatory bowel disease (ibd) have been considered as both basic, subtype-defining entities with their own genetic background (distinct from hla-b27 genotype), and as manifestations of spondyloarthritis.
  • There is a strong need to diagnose patients with spa in an earlier stage; currently, there is a delay of 5–10 years between onset of the first symptoms and diagnosis.

Types

There are five types of spondyloarthritis

Ankylosing spondylitis or bechterew disease: it is the type of arthritis that affects the spine. Symptoms include pain and stiffness in the lower back and in the neck. Spine bones fuse together and it will become rigid spine and it leads to stoope over posture.

Psoriatic arthritis: psoriatic arthritis is the form of arthritis that affects the people who have psoriasis. Most people diagnosed with psoriasis and later diagnosed with psoriatic arthritis. It may affect any part of the body such as fingertips and spine. The main symptoms are joint pain, stiffness, and swelling.

Psoriatic arthritis

Reactive arthritis: it occurs due to the infection by certain bacteria. Often the bacteria’s from genitals (chlamydia trachomatis) or bowel (campylobacter, salmonella, shigella, and yersinia). It usually targets your knees and joints of ankles or feet.

Reactive arthritis

Undifferentiated arthritis: undifferentiated inflammatory arthritis does not conform to any of the recognized inflammatory arthritis types. It may include monoarthritis of a single joint; oligoarthritis, which is usually when four or fewer joints are involved; or polyarthritis, which usually involves many small joints.

Risk factors

Risk factors may be higher due to following reasons:

  • Family member having the gene which causes spondyloarthritis
  • Positive results for the hla-b27 gene
  • Recurring bacterial infection in your gut
  • Have other inflammatory conditions such as psoriasis or inflammatory bowel disease

Causes

  • The main cause is not known, but researchers pointed out that the hereditary genetics playing an important role in this disease. Since this disease occurs often in family members of patients who have spondyloarthritis.
  • Many genes cause it. Up to 30 genes have been found. But the major gene that causes spondyloarthritis is hla-b27.
  • Bacterial infections can also cause reactive spondyloarthritis known to be triggered by a bacteria chlamydia or food borne infections.

Symptoms and complications of spondyloarthritis

The first and major symptoms of all types of spondyloarthritis are severe low back pain, swelling of arms, stiffness, and fatigue. The different types of spondyloarthritis can be manifested by the following signs and symptoms.

  • Alternative or unilateral buttock pain
  • Arthritis in small joints
  • Arthritis in large joints such as knee joint, ankle joint and elbow joint (peripheral arthritis)
  • Numbness and tingling sensation in arms and legs
  • Spinal fusion may occur in ankylosing spondylitis
  • Inflammation at the interfaces between bone and tendon or ligament of limbs (enthesitis)
  • Inflammation of intestine, urinary tract and aortic heart valves. These can lead to spinal joints inflammation.
  • Inflammation of joints in pelvis and spine (sacroiliitis)
  • Enteropathic arthritis shows symptoms such as diarrhea and abdominal pain
  • Digestive problems
  • Skin disease like psoriasis
  • Osteoporosis
  • Redness of eye (iritis)
  • Swollen fingers and
  • Pain in the heels cause difficult to walk, stand even for small duration

Diagnosis and testing

First a doctor may ask for patient’s medical history, physical exam and may suspects for symptoms of spondyloarthritis. To confirm doctor may perform the following diagnosis and tests.

  • X-rays: x-ray images shows the change of the sacroiliac joints in the pelvis and fusion of spine. If these changes not observed, but symptoms shows spondyloarthritis doctor will suggest mri scan. Mri shows these changes more promptly than the x-ray.
  • Mri scans: it can show a very clear image of enthesitis and also the changes of sacroiliac joints.
  • Ct scans: ct also used for the imaging of bone joints.
  • Hla-b27: most patients with spondyloarthritis have hla-b27 gene, but some people don’t have this gene. So it is necessary to go for a hla-b27 test if the symptoms suspects spondyloarthritis.
  • Complete blood count (cbc): cbc can be useful to find the inflammation.
  • Musculoskeletal ultrasound scanning: ultrasound imaging uses sound waves to produce the images of inflammatory conditions of muscles, ligaments, and tendons.
  • Dual-energy x-ray absorptiometry (dexa) scans: used for measuring the bone mineral density usually the bones of the lower spine and hips. Bone loss is measured through this technique.

Treatment of spondyloarthritis.

  • There is no way to cure this disease condition, but there are treatments to counterparts the symptoms.
  • Nonsteroidal anti-inflammatory drugs: certain nsaid’s like naproxen, ibuprofen, meloxicam, or indomethacin are prescribed by doctors to relief from the pain due to symptoms.
  • Corticosteroid medication: inflammation around the tendons and in the joints can be overcome by taking some corticosteroid drugs. This helps to reduce the swelling in the joints.
  • Antibiotics: antibiotics can be used in the case of reactive spondyloarthritis in which bacteria cause the major problems.
  • Disease-modifying antirheumatic drugs (dmard’s): sulfasalazine (azulfidine) and methotrexate are the best dmard’s to be used for the joint damages in limbs affected by arthritis.
  • Tumor necrosis alpha (tnf-alpha) blockers: fda approved tnab are adalimumab (humira), adalimumab-atto (amjevita), a biosimilar to humira, certolizumab pegol (cimzia), etanercept (enbrel), etanercept-szzs (ereizi), a biosimilar to enbrel, golimumab (simponi), infliximab (remicade), infliximab-dyyb (inflectra), a biosimilar to remicade, and secukinimab (cosentyx). These are biosimilars used to treat arthritis in spine joints.
  • Surgery: patients with over inflammation of pelvis joints needs hip replacement. In the case of heavy damage in the cartilage of joints needs surgery to repair. In some rare cases, spinal surgery also needed.

Prevention and control measures

  • Prevention from spondyloarthritis needed some lifestyle changes as follows.
  • Quit smoking: smoking speeds up the spinal fusion in addition to other bad health effects.
  • Follow good posture: practicing a proper sitting and posture makes your spine to escape from growing into slumped-over position and strain.
  • Daily physical exercise: frequent home exercise and physiotherapy is essential to reduce the stiffening of spine and other joint problems. It also helps to have your heart healthy.
  • Good diet: follow the diets that are rich in calcium. Dairy products are preferred foe calcium rich foods.
