It may be piles. There are two types of piles 1. Bleeding piles 2. Non bleeding. You should avoid tea, coffee and non vegetarian food and contact homoeopathic doctor for medicines.

A fistula is an abnormal, tube-like connection, a passage or hole that forms between two organs or an organ in your body and your skin. There are various types of fistulas that can develop in different parts of your body but the most common types are-

1. Obstetric Fistulas: This is the name given to a hole between the vagina and rectum or bladder. A vaginal fistula that opens into the urinary tract is called a vesicovaginal fistula and one that opens into the rectum is called a rectovaginal fistula. 

A woman with obstructed labour or labour that is not attended by a doctor or a midwife, can suffer labor pain for up to six or seven days. And as the labour produces contractions that push the baby’s head against the mother’s pelvic, the soft tissues between the baby’s head and the pelvic bone are compressed and do not receive adequate blood flow between the mother’s bladder and vagina and/or between the rectum and vagina.
This means that a hole forms on the vaginal wall between the urinary tract and her rectum. The urine and faeces thus start to leak from the vaginal opening causing soiling and foul smell.

Apart from labour, hysterectomies that damage the vaginal wall can also cause such types of fistulas.

Enterovaginal – A woman may have gas and foul smelling fluids leaking out of her vagina due to an enterovaginal fistula which is a hole between the vagina and the digestive tract. It’s usually caused due to inflammatory bowel disease or IBD and ulcerative colitis.

2. Anal fistula

These are fistulas that form between the end of the bowel and skin near the anus.

An anal fistula usually develops due to a previous or current anal abscess. An anal abscess is an infected cavity filled with pus found near the anus due to an acute infection in the internal glands of the anus.

Common symptoms are:

  • Pain in the anorectal area
  • Swelling
  • Redness
  • Fever
  • Rectal bleeding
  • Urinary symptoms, such as trouble initiating urination or painful urination

Causes:

  • Inflammatory bowel disease – a chronic condition of the GI track which causes inflammation
  • Diverticulitis – An infection in the large intestine
  • Tuberculosis, HIV
  • Surgery near the anus

Anal abscess and likewise anal fistula are treated by surgical drainage in which an incision is made in the skin near the anus to drain the infection causing an anal abscess. Hospitalization may be required for patients who have impaired immunity like diabetics.

Anal fistula though has no medical treatment and surgery is almost always necessary to cure it.

  • If the fistula is straightforward which means one that involves minimal sphincter muscle, a fistulotomy may be performed. This involves cutting open the whole length of the fistula so it heals.
  • Seton is another procedure in which a surgical thread called a seton is placed in the fistula and left there to help it heal. If you wish to discuss any specific problem, you can consult a general surgeon.

She may be having fissure or pile so she needs to be examined by a general surgeon and proctoscopy to be done to find out what exactly it is, keep her stool soft by taking plenty of fluids orally and high fibre diet even she can take syp cremaffin 3 to 6 tsf hs daily, use ointment lox anally before defecation, proper treatment follows according to examination.

If the so called lump is present show it to an endocrinologist to check if it is related to thyroid gland and if he suggests any investigations like ultra sono graphy of the region and thyroid test get those done. Hello, Thanks for the query. Since you are already on treatment with Thyroxine tablets (50 mcg/d), you will have to look at the TSH level, it should be maintained <3 mU/L to be free from all the symptoms. Sir. I am hyperthyroid and having thyronorm 50 mcg. I am suffering from severe shoulder pain on my left hand. Is it a side effect? Please reply me. Its hurting very badly. I consulted an ortho surgeon and medicated for that. When I stop the medicine, the pain re appears. Should I change the tablet? Thank you.

lybrate-user, Thanks for the query. Since you are taking Levothyroxine 50 mcg, you have Hypothyroidism. It is essential to maintain TSH level <3 mU/L to be free from all the symptoms hypothyroidism. Please check the level and come back. Also check your blood glucose level both fasting and Pp and come back. Thanks.

lybrate-user, Thanks for the query. When on treatment with thyroxine, TSH should be maintained arond 3 mU/L. To be free from all the symptoms of hypothyroidism. One of the symptoms is hair fall. So please see that TSH level is brought to <3 mU/L. Since no TSH value is mentioned it is difficult comment. Thanks.

Mr. lybrate-user, Thanks for the query. TSH 6.1 mU/L is a subclinical hypothyroidism. Usually no treatment is advocated. Please also get anti TPO antibodies done, to see if there is Hashimoto's Thyroiditis leading to hypothyroidism. Then please come back with results. Thanks.

The thyroid gland within the body plays an important role in regulating your metabolism among many other functions. This is a butterfly-shaped gland located in your neck and plays an important role within your body. However, a common condition that may afflict it is hypothyroidism or an underactive thyroid gland. It is even more of a problem if you are pregnant as it may affect your baby’s development in the womb. It has also been noticed that pregnancy in itself may cause hypothyroidism.

Symptoms of Hypothyroidism-

Pregnant women may also exhibit symptoms common to other patients who are suffering from hypothyroidism. These may include:

  1. Fatigue along with lethargy.

  2. Mild to significant weight gain.

  3. Constipation.

  4. Feeling cold with severe chills

The thyroid gland produces the T4 hormone responsible for regulating many mechanisms within the body such as metabolism. With hypothyroidism, the production of this hormone decreases or stops altogether. This is important as disruption in the production of the hormone may disrupt the normal development of your baby.

Effects of Hypothyroidism on your baby-

Hypothyroidism during pregnancy can affect your child’s developmental abilities. Many studies in this field have shown that such children tend to have learning difficulties and may even display lower IQ scores during tests. It was noticed that sometimes, hypothyroidism during pregnancy wasn’t diagnosed correctly as the pregnant mother did not show much apparent symptoms. This is also a major issue in the early diagnosis of the problem. Hence, it is absolutely imperative to test for thyroid problems early during pregnancy.

Prevention is the Best Cure-

It is thus very important that mothers be screened properly for thyroid issues even before they are pregnant and while they are pregnant. Some of the medically recommended steps that should be taken are –

  1. Screening before pregnancy as this can help to devise a solution for this condition.

  2. Women with goiter or enlarged thyroid should most certainly be screened.

  3. Women with a family history of hypothyroidism should be screened.

Treatments for Hypothyroidism During Pregnancy-

The doctors may suggest thyroid hormone replacement therapy wherein artificial T4 hormone is introduced into the body. This helps to maintain a constant rate of the thyroid hormone within the body. This can be done even before pregnancy as the developing fetus is completely dependent on the mother for its thyroid hormone until at least 12 weeks when the baby’s body can start producing it on its own. Also, the levels of this hormone should be regularly checked within the body through the TSH or the thyroid stimulating hormone tests to ensure that the levels are at a safe minimum.

No harm in consuming saffron along with milk. But consult an ophthmologist for your reduced vision. Hypothyroidism can have many ocular manifestations (dryness, optic neuropathy, thyroid eye disease) which may be responsible for your blurred vision.
