Hi. Apply whitfeld ointment in that area. You are suffering from fungal infection. Avoid wearing tight innerwear. Dry the area well after taking bath. Apply calamine lotion in that area.

Ways To Treat Gender Identity Disorder!

Born as a male but feel like a female OR born as a female but feel like a male. Some persons are unable to feel comfortable in the physical gender that they are born in; their mind, brain, feeling and thoughts are all of the opposite sex. This uncomfortableness in their mind about their physical gender is termed as Gender Identity Disorder.

What's the cause?

We do not know the exact cause of this disorder, but it is hypothesized that developmental changes in the brain and the body do not sync, and the brain/mind is left with a different gender than the body. This is mostly an unidentifiable and unpredictable change.

What are the symptoms?

  1. Children might express a desire to dress up as the opposite sex.
  2. They might express replusion at one's own sex organs/genitals.
  3. As adolescence begins, they do not develop feelings of attraction towards members of the opposite sex. Some complain of revulsion about their pubertal changes.
  4. Males want to pee while sitting down, while females want to do it standing up.
  5. Their behavior is often more like that of the opposite sex, e.g. boys might display feminine gestures, while girls might indulge in risk taking and reckless behavior.
  6. As they become adult, a realisation that they are stuck in a body that they do not like causes depression and anxiety.

Diagnosis: A diagnosis of Gender Identity Disorder (GID) is made based on a detailed assessment by a Psychiatrist, who is specially trained in this assessment. WPATH standards mandate that this is an ongoing assessment, and involves facilitating the process of Hormonal Treatment and eventual Sex Reassignment Surgery. The whole process can take 2-3 years.

Treatment: For those who have severe GID, eventual sex reassignment surgery is the treatment and outcome. However, support of family, friends and a Good Psychiatrist can enable a smooth transition. Psychiatrists will often use antidepressants and anxiolytics to make it easier for the person to cope with societal and family pressures.

Duration of sexual act or performance vary from person to person depending upon the psyche to control it. Few people complain of premature ejaculation and few complain that they are not able to discharge in time. Some techniques as per sexologist kinsey can be applied where root of the penis is pressed for few seconds before expected emmision and thereby emmision is delayed for some time. An estimated of 10 percent or more children snore for more than 1 night. Snoring results from the obstruction of breath when the air makes its way through the mouth. The air passage of the throat through which the air goes in and out causes the tissue to vibrate causing snore. The sound of the snore totally depends on the amount of air passing through the mouth and the frequency at which the throat is vibrating. Children above the age of three years tend to snore when they are deep in sleep.

Primary and secondary snoring:
Children who are otherwise healthy should not have a snoring problem unless there is some health disorder. However, if loud snoring is observed in otherwise a perfectly healthy kid, seeking medical help is a wise thing to do. In general, primary snoring is defined as a condition which is not associated with other sleep-related disorders such as lung disorder, OASS, inability to sleep for a longer duration etc.

Other possibilities:
It is estimated that close to three percent of the children suffers from breathing problem while sleeping. OSAS is a condition which is characterised by pauses in breathing and gasps along with loud snores. There could be instances where a kids’ muscle relaxes way too much obstructing the air to pass through. The body and the brain are both alerted at this stage. As soon as the brain sends a signal, the body wakes up. Thus, most kids suffering from this disorder are not able to sleep well and tend to feel tired throughout the day.

Common causes of sleep apnea:
Sleep apena is a major cause of snoring. It can be caused because of allergies, obesity, GERD etc. In children sleep, apnea can also be caused by tonsil. Since the tonsil size of children less than 7 years of age tends to be bigger than adult individuals, it causes snoring related problem. Swollen tonsils not only obstruct air but creates other health related problems as well.

Symptoms of sleep apnea during the day and night:

  1. Loud snore
  2. Snorts, gasps, pauses during sleep
  3. Heavy sweat during sleep
  4. Social and behavioural problems
  5. Failure to get up in the morning
  6. A nasal voice
  7. Easily irritated, cranky and aggressive

Relief measure:
The first thing a that a doctor does is a physical exam followed by observing the symptoms of a kid. Primary snorting can be addressed by physical exercise, counselling and breathing exercise. Problems related to apnea are addressed through medicine, counselling, and exercise. Certain dietary restrictions also needed to be followed during this time. What is Palpitation?
Palpitation is a feeling of awareness of your own heart beat. 
It is usually described as heart rate being either too fast (racing), too slow or a sensation of missing a beat.

Types of palpitations?

Normal (Benign) 
These palpitations occur as a response to physical or mental stress like exercise, fever, pain, fear, anxiety etc. 
They are harmless and settle on their own once the precipitating factors disappear. They do not require any medical treatment.

Abnormal 
These palpitations occur due to some underlying abnormality in either the structure of heart or the beating of heart. 
These can be dangerous and at times life threatening. They may or may not settle on their own and require some medical treatment.

Warning signs that suggest palpitations are abnormal?
Palpitations are abnormal if they are associated with chest heaviness, chest pain, uneasiness, sweating, weakness, giddiness, feeling of black out, fainting, nausea, vomiting, seizures.
Palpitations are more likely to be abnormal In people who have some existing heart disease and these patients should report to their doctor at the earliest.

What to do during palpitations?
Call for help. (If u are alone avoid driving, use a taxi or auto) Go to your nearest hospital/doctor and try to get an ECG during the palpitations. If you are helping someone having palpitations, if they faint, call for help, start CPR if they remain unresponsive and take them to the nearest hospita

Which doctor can treat palpitations?
Doctors who specialise in the treatment of palpitations are called Electrophysiologists. 
They specialise in heart rhythm and are capable of performing a variety of complex tests to identify and treat different types of palpitations.

Which investigations are used to diagnose palpitations?
ECG - Taken during palpitations and when the patient is normal is a very helpful tool. 
Holter - Externally applied recorder which continuously records heart rhythm for 24 hrs. 
ELR - Extended looper recorder, is like holter, but it records rhythm for longer durations. 
ILR - Internal loop recorde is attached within the body for recording rhythm for long duration. 
EPS - Electrophysiology study, is the most sure shot test to diagnose, identify and treat 
palpitations.

What is EPS?
EPS stands for Electrophysiology study. By this test a trained electro-physiologist studies the conduction and formation system of heart beat, to understand the source, cause and type of palpitation. 
It is a simple and safe procedure of 2-3 hours and can be done as a day care procedure (by admitting the patient for a few hours in hospital, with discharge on same day).
It requires fasting for 4 hrs, some standard blood investigations and is done with local anaesthesia and if required it can be combined with treatment like ablation in the same sitting.

In the procedure electro-physiologist take catheters into your heart to study and stimulate the palpitations and understand them. Once found they can use various techniques to stop the palpitations, the techniques are called Ablation.

What are the treatment options available for palpitations?
There are many options depending upon the nature of palpitations and condition of the patient - 
Drug therapy 
Cardioversion - where either a drug or electric shock is given to stop palpitations immediately. 

Ablations using many sophisticated computer softwares 

Device Therapy like ICD (Internal cardiac defibrillator) 

Combination of all the above therapies.

Your Electrophysiologist and you as a team can make a choice about the options that will be best for you.

My mri report shows 2 subserosa fibroids below 4 cms (both) are benign. What r the causes of fibroids and any medication for them not to grow further to avoid surgery in future? Upto which size are they harmless. No symptoms of discomfort like excess bleeding etc. Periods r regular. Do they shrink for me after menopause?

1) For obesity the only way is to reduce your weight by eating a healthy diet and exercising to burn out calories you are eating. Take a detailed diet consult for a diet plan to reduce weight. Start exercising begin gradually and increase to 40 mins per day. 2) For chest pain please get a ECG, chest X-Ray and Echo done and consult a local cardiologist. 3) For recurrent cold consult an ENT.

Hello Lybrate user, dengue causes weight loss due to dehydration and the patient loses immunity and also there is loss of appetite. My mother is 68 years old Type 2 diabetic / hypertension patient. Latest results are: HbA1c is 8.2. Hb is 10.4, Creatine is 3.7. She is currently on H.Mixtard 30 (26-0-10 ), telmikind (0-0-1), Thyronorm 12.5 (1-0-0). Thyronorm was started just 2 days back as we discovered she has TSH of 11. She is currently very weak, mentally confused, unable to walk without assistance. We believe that the weakness could be because of high TSH, and anaemia. We have started treating her for TSH, any ideas on how to treat anaemia considering she has CKD? Also, any opinions on what could be causing muscle weakness/ mental confusion etc? Thanks in advance.

Mr. lybrate-user, Thanks for the query. For a person with diabetes and hypertension, if both conditions are not well controlled, general weakness is always possible. Plus presence of hypothyroidism will add to it. With HbA1c% of 8.2, her diabetes is not in good control. It also has affected her kidney functions (high creatinine). No mention of her BP readings, so can not comment. TSH 11 mU/L is high, hence the dose of 12.5 mcg is bit too low. Hemoglobin 10.4 is unlikely to cause such a severe weakness. It is the effect of uncontrolled diabetes, hypothyroidism, hypertension and anemia. Hence first thing is to control her blood glucose and BP. Plus TSH should also should be brought to around 5 mU/L. It is also necessary to bring down creatinine level. This kind of a case has to be handled by the competent local physician. Thanks.
