Hi thanks for your query and welcome to lybrate. I am Dr. Akshay from fortis hospital, new delhi. Normally these sensations are due to underlying causes related to lower back where there could be some nerve impingement, or they can be secondary to a systemic disease. You can start with physical therapy (tens) as a modality and start with low dosages of neuropathic medications like gabapentin and pregabalin and we will se how it goes in 2-3 weeks time. Also take calcium and vitamin d in regular doses as is required. Do not hesitate to contact me if you need any further assistance.

Hi, I attached here MRI reports. Please advise how could I come out from back pain without surgery. MRI - LUMBAR spine 19,10.2016: Findings: loss of normal lumbar lordosis seen? Due to muscle spasm. Lumbar vertebrae appears normal in alignment .No listhesis. Small Schmorl's nodes noted involving the endplates of multiple lumbar vertebra. Degeneration ofL1-L2, L2-L3 and L4-L5 lumbar intervertebral disc Posterior annular tear noted at L1-L2, L2-L3 and L4-LSlevels. At Ll-L2 level there is right paramidline protrusion, No foramina narrowing. At L2-L3, L3-L4 diffuse posterior disc bulge noted ,causing effacement of ventral thecal sac and inferior recess of bilateral neural foramina narrowing at L3-L4 level. At L4-L5 level, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting L4 and traversing LS nerve roots at this level. At LS-Sllevel, no disc bulge or herniation. Conus medullaris appear unremarkable. Impression: - Lumbar spondylosis degenerative changes as mentioned ,predominant at L4-L5 level. - At L4-LSlevel, posterocentral and left forminal protrusion causing, significant canal stenosis and left neural foramina narrowing ,resulting in compression of exiting left L4 and traversing Left LSnerve roots at this level. Fist think you lose your wait you can complicated bad rest 20 days you can also physiotherapy at your home but you about this ask your orthopedic doc. Then home physiotherapy but physiotherapy at list 2 months. U fill mostly released.

Do proper rest & do hot fomentation for 15-20 min thrice in a day. Learn strengthening & stretching exercises from physiotherapist. This will help in reducing the pain. Thanks.

Sleep on a hard bed with soft bedding on it. Use no pillow under the head. Kindly take BioD3 Max 1 tab dailyx10 Paracetamol 250mg OD & SOS x5days Do back(spine)/shoulder/knee exercises Make sure you are not allergic to any of the medicines you are going to take Do not ignore it. It may have to be further investigated. It could be beginning of a serious problem. If no relief in 4_5 days,then contact me again. Contact your family doctor or nearest hospital for emergency help.

Do avoid taking pain killers do take ift and vacuum therapy physiotherapy treatment for pain relief for 6 days followed by strengthening exercise from neuro physiotherapist apply hot and cold water fomentation for thrice a day for 7 days regularly do revert for further assistance best wishes.

During recent days 3 times I suffered a epileptic type condition where there were no seizures but some slight muscle stiffness combined with short 2 minutes unconscious period is seen where I can see people around but couldn't respond . One doctor suggested to take eeg and MRI scan to rule out the issue but I took the tests twice nothing negative came out everything was normal even then the doctor suggested levipil 500 mg . Is this right way to take medication ?

Muscle stiffness associated with short unconsciousness would still be interpreted as a seizure/fit by a consultant in neurology. The mere fact that concerned investigations (mri/eeg) are negative does not exclude the diagnosis of epilepsy. I also feel you must be on medication as advised. Finally please do not question the consultant's opinion as mentioned in the last sentence of your note - is this the right way to take medication.

There are many reasons of pain in soles of foot and in your case it seems like either it is due to overweight or VITAMIN-D deficiency.So I will advise you toirst undergo bloodtest for VITAMIN-D and then consult me.

You are too young to suffer from cervical spondilosys. Confirm by x-rays of cervical spine ap & lat. Views no pillow under the head during sleep. Do neck & shoulder exercises. Any way take paracetamol 250mg od & sos x 5days contact me again if need be you will need other supportive medicines also. Make sure you are not allergic to any of the medicines you are going to take.

Hello lybrate user, surgical scars are difficult to eliminate especially after four years of treatment. I would suggest you to leave it as it is. Any treatment would be very expensive and treatment complicated but the chances of success are less. Hope this helps.

Do hot fomentation twice daily. Postural correction- always sit with a small pillow at your low back. Sit tall, walk tall. Extension exercises like bridging and bhujang asana x twice daily. Sleep on your back with a pillow under your knees or on your sides with a pillow between your knees. And get vit d levels checked. Go for regular walks follow 30-30 rule when at work. After every 30 min of work. Do 30 sec of stretching or stand and walk a bit.

My tailbone pains a lot while I sit. This started when I sat continuously for 6 hrs on a chair.
You might be suffering from coccydynia.In this condition there is inflammation of tailbone (coccyx) making it difficult to sit on butt and sleeping on back. Generally coccydynia will resolve with conservative treatment as mentioned below. In severe cases an injection called Ganglion Impar block might be needed. This is done by a Pain specialist in operation theatre under X Ray guidance to locate the accurate pain transmitting nerve. 1. Take anti inflammatory drugs like Ibuprofen round the clock for pain relief and decreasing the inflammation for few days. 2. Sitting on specially designed gel-filled cushions can reduce pain when you are sitting down. 3.You can also lean forward in your seat and rest your arms in front of you on a flat surface. 4. Clothing such as tight jeans or trousers, may make your coccydynia worse. 5. It can also help to wear flat, comfortable shoes. 6. Patients with coccydynia find sleeping on their sides most comfortable. If you have severe pain, you may sleep on your front. You could also try sleeping with a pillow or cushion between your knees. 7. Warm and cold packs – applying either a warm or cold pack to the base of your spine.

Hello Doc is there any way to rejoin avulsion of PCL tibia attachment without surgery. I dnt wanna do it. As per MRI report it showing PCL avulsion of tibia attachment. I can walk, can use upstare, only the difficulties I am facing is that not able to fold my leg completely if I do so the complete area ok knee pain. Kindly suggest me something best.

The course of rehabilitation for a pcl injury is dependent on the degree of injury. An isolated injury of grade I or grade ii may be treated with physical therapy. A grade I or grade ii injury is usually treated with a brief period of knee splinting in extension, followed by early rom and a quadriceps and hamstring strengthening program. Recovery is quick, and many patients are able to return to normal function in about 4 weeks. Closed kinetic chain exercises and open kinetic chain exercises are recommended. The use of continuous passive motion (cpm) machines for early knee mobilization is an option. A good outcome is correlated with the maintenance of good quadriceps strength. Please consult a physiotherapist for detail assessment and management of your pcl avulsion. Best wishes.
